Abstract

【課題】拡張部の強度を良好に保つことができ、かつ、気体の排出速度を容易に調整することが可能な止血器具を提供する。 【解決手段】止血器具10は、手首Wに巻き付けるための帯体20と、帯体を手首に巻き付けた状態で固定する固定手段30と、帯体に連結され、かつ、気体を注入することにより拡張する拡張部40と、拡張部に注入された気体の排出を制御するフィルター部材70と、を備え、帯体は、帯体の内表面と外表面との間を貫通する1つ以上の孔部24を有し、拡張部は、帯体の内表面において、孔部を覆うように配置され、フィルター部材は、孔部を塞ぐように帯体に配置され、かつ、孔部からの気体の排出を抑制する。

Description

止血器具
 本発明は、穿刺した部位を圧迫して止血するための止血器具に関する。
 近年、腕や脚等の血管を穿刺し、穿刺部位にイントロデューサーシースを導入し、イントロデューサーシースの内腔を介してカテーテル等の医療器具を病変部に送達する、経皮的な治療・検査等が行われている。このような治療・検査等を行った場合、術者は、イントロデューサーシースを抜去した後の穿刺部位を止血する必要がある。この止血を行うために、腕や脚等の肢体に巻き付けるための帯体と、帯体を肢体に巻き付けた状態で固定する固定手段と、帯体に連結されており、流体を注入することにより拡張して、穿刺部位を圧迫する拡張部と、を備えた止血器具が知られている。
 このような止血器具では、拡張した拡張部が、長時間にわたって穿刺部位およびその周辺の血管や神経を強く圧迫し続けると、しびれや痛みを引き起こしたり、血管が閉塞したりする可能性がある。血管閉塞等を防ぐため、一般的に、医師や看護師は、拡張部を拡張させた後、定期的にシリンジ等の専用の器具を止血器具に接続し、拡張部内の流体を排出し、拡張部の内圧を減圧する減圧操作を行うことで、穿刺部位に作用する圧迫力を経時的に低減させている。
 これに対し、下記特許文献1に係る止血器具では、拡張部を経時的に伸長する材料によって構成している。このため、拡張部に流体を注入して拡張させた後、拡張部は、拡張部内の流体からの圧力によって徐々に拡張変形していく。拡張部内の流体の量は大きく減少しないのに対し、拡張部の内部空間の容積は徐々に大きくなるため、拡張部の内圧を経時的に低減することができる。これによって、穿刺部位に作用する圧迫力を経時的に低減させることができる。

 上記特許文献1に係る止血器具によれば、医師や看護師が減圧操作を行う手間を省くことができる。しかしながら、拡張部を経時的に伸長する材料によって構成すると、拡張部は経時的に拡張変形していくため、それに伴い拡張部の厚みは薄くなる。拡張部の強度を良好に保つ観点からすれば、拡張部の厚みはある程度維持されることが好ましいと考えられる。
 上記のような拡張部の構成材料の性質を利用した減圧方法に代替する方法として、例えば、拡張部の内部空間(拡張空間)と連通する孔を帯体に形成し、当該孔を介して気体を排出させる方法が考えられる。しかしながら、帯体に形成した孔から気体を排出させるように構成した場合、気体の排出速度(排出量)は、孔の内径や個数といった孔の構造に依存したものとなる。
 拡張部の減圧は、穿刺部位Pに圧迫力を付与しつつ血管閉塞を防止できる程度に経時的に進行させる必要があるため、気体の排出速度は精密に制御することが望まれる。上記のように、帯体に形成した孔から気体を排出するように止血器具を構成した場合、孔の構造のみで気体の排出速度を制御しなければならないため、気体の排出速度を精密に制御するのが難しくなる。これにより、止血器具は、所望の減圧プロトコルを実現し難いものとなる。
 本発明は、上記課題を解決するためになされたものであり、拡張部の強度を良好に保つことができ、かつ、気体の排出速度を容易かつ精密に調整することが可能な止血器具を提供することを目的とする。
 上記目的を達成する止血器具は、肢体の止血すべき部位に巻き付けるための帯体と、前記帯体を前記肢体に巻き付けた状態で固定する固定手段と、前記帯体に連結され、かつ、気体を注入することにより拡張する拡張部と、前記拡張部に注入された気体の排出を制御するフィルター部材と、を備え、前記帯体は、前記帯体の内表面と外表面との間を貫通する1つ以上の孔部を有し、前記拡張部は、前記帯体の内表面において、前記孔部を覆うように配置され、前記フィルター部材は、前記孔部を塞ぐように前記帯体に配置され、かつ、前記孔部からの気体の排出を抑制する。
 上記のように、孔部は、帯体の拡張部が配置されている領域に設けられており、フィルター部材は、孔部を塞いで、拡張部内に注入された気体の排出量を制御する。このため、気体透過量を増やすために拡張部の厚みを薄く作製する必要がなく、拡張部の強度を良好に保つことができる。また、拡張部に注入された気体の排出速度はフィルター部材によって制御されるため、帯体に設けた孔の構造のみで気体の排出速度を制御する場合と比較して、気体の排出速度を容易かつ精密に制御することができる。さらに、フィルター部材は、孔部を塞ぐように帯体に配置されているため、帯体によりフィルター部材がサポートされ、フィルター部材の変形が抑制される。これにより、気体の排出速度はフィルター部材の変形による影響を受けて変化し難くなるため、止血器具は、所望の減圧プロトコルを実現可能なものとなる。

 以下、添付した図面を参照しながら、本発明の実施形態およびその変形例を説明する。なお、以下の記載は特許請求の範囲に記載される技術的範囲や用語の意義を限定するものではない。また、図面の寸法比率は説明の都合上誇張されており、実際の比率とは異なる場合がある。
 図1~図5を参照して、本実施形態に係る止血器具10を説明する。図1~図3は、止血器具10の各部の説明に供する図である。図4および図5は、止血器具10の使用例の説明に供する図である。
 実施形態に係る止血器具10は、図4および図5(A)に示すように、治療・検査等を行うカテーテル等を血管内に挿入する目的で、手首W(「肢体」に相当)の橈骨動脈Rに形成された穿刺部位P(「止血すべき部位」に相当)に留置していたイントロデューサーシースを抜去した後、その穿刺部位Pを止血するために使用するものである。
 止血器具10は、図1および図2に示すように、手首Wに巻き付けるための帯体20と、帯体20を手首Wに巻き付けた状態で固定する面ファスナー30(「固定手段(固定部材)」に相当)と、気体を注入することにより拡張し、穿刺部位Pを圧迫する拡張部40と、拡張部40を穿刺部位Pに位置合わせするためのマーカー40cと、気体を拡張部40に注入可能な注入部60と、拡張部40に注入された気体の排出を制御するフィルター部材70と、を有している。
 本明細書中では、帯体20を手首Wに巻き付けた状態のとき、帯体20において手首Wの体表面に向かい合う側の面(装着面)を「内表面」と称し、その反対側の面を「外表面」と称する。
 帯体20は、拡張部40が配置される支持板21(「支持体部」に相当)と、支持板21に連結されると共に、固定手段30が配置されて支持板21を手首Wに対して保持するベルト22(「保持部」に相当)と、を備えている。
 支持板21は、手首Wの周方向に沿って配置される。図2に示すように、支持板21の長手方向における中央部21aは、ほとんど湾曲せずに平板状になっている。この中央部21aの両側には、それぞれ、内表面側に向かって、かつ、支持板21の長手方向(手首Wの周方向)に沿って湾曲した第1湾曲部21b(図2の左側)および第2湾曲部21c(図2の右側)が形成されている。支持板21は、ベルト22よりも硬質な材料で構成されており、ほぼ一定の形状を保つようになっている。
 支持板21の第1湾曲部21bが設けられている側の端部には、支持板21を厚み方向に貫通する貫通穴23bが設けられている。支持板21の第2湾曲部21cが設けられている側の端部には、支持板21を厚み方向に貫通する貫通穴23cが設けられている。各貫通穴23b、23cは、図1に示すように、支持板21の幅方向(支持板21の長手方向と交差する方向)に沿って延在している。ベルト22は、第2湾曲部21c側の貫通穴23cを挿通した状態で、支持板21の端部と連結されている。止血器具10を手首Wに装着する際は、図5(A)に示すように、第1湾曲部21b側の貫通穴23bには、ベルト22を挿通する。第1湾曲部21b側の貫通穴23bを挿通したベルト22は、折り返すようにして、手首Wの外周の一部に沿って巻き付けられる。
 支持板21の拡張部40が配置される領域には、図3および図5(A)に示すように、支持板21の内表面と外表面との間を貫通する(すなわち、支持板21を厚み方向に貫通する)複数の孔部24が設けられている。なお、図面の都合上、孔部24の個数を省略して図示しているが、図3に示すように支持板21には多数の孔部24を形成している。
 孔部24の形状、大きさ、個数等は、例えば、支持板21の強度、気体の透過量、および、支持板21と当接するフィルター部材70への損傷リスクを考慮して設定することができる。支持板21の強度を高く保つためには、孔部24は、その個数が比較的少なく、かつ、図3に示すように、支持板21の内表面側から平面視した際の大きさが比較的小さい(すなわち、孔部24とフィルター部材70との接触面積が小さい)ほうが好ましい。一方で、気体の透過量を高く保つためには、孔部24の数は多く、支持板21の内表面側から平面視した際の大きさが比較的大きい(すなわち、孔部24とフィルター部材70との接触面積が大きい)ほうが好ましい。また、フィルター部材70が損傷しないようにするためには、孔部24は、支持板21の内表面側からの平面視において、丸形状に近い形状を備えることが好ましい。例えば、孔部24が四角形で形成されていると、孔部24の周縁の角部(エッジ)とフィルター部材70とが接触する部分で、フィルター部材70の一部に局所的に応力が掛かり易くなるため、フィルター部材70が損傷してしまう可能性がある。
 以上の点を考慮して、本実施形態では、孔部24は、図3に示すように、支持板21の内表面側からの平面視における形状が正六角形となるように形成している。平面視における孔部24の形状を正六角形で形成することにより、隣り合う孔部24との間に大きな隙間を形成させることなく、密集させることができる。また、平面視における形状が角部のない丸形に近い形状となるため、角部によるフィルター部材70の損傷を防止することができる。
 孔部24は、支持板21を厚み方向に貫通しているため、支持板21の拡張部40が配置される領域には、正六角形で形成された複数の孔部24からなるハニカム構造が構成されている。複数の孔部24がなすハニカム構造は、単純な円形等の孔が複数形成される場合に比べて、支持板21の強度を高く保つ。また、ハニカム構造とすることにより、各孔部24は、隙間なく配列されるため、気体の透過量も好適に維持することができる。
 なお、孔部24の平面視における形状は、正六角形のみに限定されることはなく、例えば、正六角形以外の多角形の形状、円形、楕円形等であってもよい。また、孔部24の支持板21の厚み方向に沿う断面は図示省略するが、本実施形態においては、断面形状が厚み方向に沿って変化することなく、一定の形状となるように形成している。
 支持板21の構成材料は、ベルト22および拡張部40よりも弾性率の高い材料によって構成することができる。そのような材料としては、例えば、アクリル樹脂、ポリ塩化ビニル(特に硬質ポリ塩化ビニル)、ポリエチレン、ポリプロピレン、ポリブタジエンのようなポリオレフィン、ポリスチレン、ポリ-(4-メチルペンテン-1)、ポリカーボネート、ABS樹脂、ポリメチルメタクリレート(PMMA)、ポリアセタール、ポリアクリレート、ポリアクリロニトリル、ポリフッ化ビニリデン、アイオノマー、アクリロニトリル-ブタジエン-スチレン共重合体、ポリエチレンテレフタレート(PET)、ポリブチレンテレフタレート(PBT)のようなポリエステル、ブタジエン-スチレン共重合体、芳香族または脂肪族ポリアミド、ポリテトラフルオロエチレン等のフッ素系樹脂等が挙げられる。
 支持板21は、拡張部40と重なる部分が実質的に透明であることが好ましいが、透明に限定されず、半透明または有色透明であってもよい。これにより、穿刺部位Pを外表面側から確実に視認することができ、後述するマーカー40cを穿刺部位Pに容易に位置合わせすることができる。なお、支持板21は、中央部21aのような平板状の部分を有さず、全長にわたって湾曲しているものであってもよい。
 ベルト22の支持板21に連結されている側(図1および図2の左側)には、面ファスナー30の雌側31a、31bが配置されている。ベルト22の支持板21に連結されていない側(図1および図2の右側)には、面ファスナー30の雄側32が配置されている。面ファスナー30は、例えば、日本でVELCRO(登録商標)又はマジックテープ(登録商標)のような一般的な製品として知られるhook and loop fastenerである。
 止血器具10を手首Wに装着する際は、図5(A)に示すように、ベルト22を支持板21の貫通穴23bで折り返しつつ手首Wの外周の一部に沿うように取付ける。これによって、各雌側31a、31bと対向するように雄側32を配置することができる。この際、装着者の手首Wの外周長(太さ)に合わせて帯体20を折り返す長さを調整することにより、装着者の手首Wの外周長に依らずに、止血器具10を適切に装着させることが可能になる。止血器具10は、各雌側31a、31bと雄側32が接合されることにより、手首Wに装着される。一般的に、雌側31a、31bは、雄側32よりも柔軟であるため、手首Wの外表面に近接する位置に雌側31a、31bを配置することにより、装着者に与える不快感を抑えることができる。
 なお、面ファスナー30は、例えば、一つの雌側と一つの雄側とで構成したり、雌側の位置と雄側の位置を入れ替えて構成したりすることも可能である。また、帯体20を手首Wに巻き付けた状態で固定する手段は、面ファスナー30に限定されず、例えば、スナップ、ボタン、クリップなどの固定部材を用いてもよい。
 ベルト22の構成材料は、可撓性を備える限り特に限定されない。そのような材料としては、例えば、ポリ塩化ビニル、ポリエチレン、ポリプロピレン、ポリブタジエン、エチレン-酢酸ビニル共重合体(EVA)のようなポリオレフィン、ポリエチレンテレフタレート(PET)、ポリブチレンテレフタレート(PBT)のようなポリエステル、ポリ塩化ビニリデン、シリコーン、ポリウレタン、ポリアミドエラストマー、ポリウレタンエラストマー、ポリエステルエラストマー等の各種熱可塑性エラストマー、あるいはこれらを任意に組み合わせたもの(ブレンド樹脂、ポリマーアロイ、積層体等)が挙げられる。
 拡張部40は、気体を注入することにより拡張し、穿刺部位Pに圧迫力を付与する機能を備えている。なお、拡張部40に注入する気体は、拡張部40を拡張可能である限り、特に限定されないが、例えば、空気等を用いることができる。
 拡張部40は、略矩形状のシートによって構成している。拡張部40は、図3に示すように、支持板21の内表面側において、孔部24が設けられた領域を覆うように配置されている。拡張部40の外周縁部は、支持板21側に折り込まれた状態で、支持板21と、後述するように支持板21に設けられた孔部24を塞ぐように配置されているフィルター部材70の外周縁部と、に接合されている。このため、拡張部40によってフィルター部材70の外周縁を押さえつけることができ、フィルター部材70は支持板21から比較的剥がれにくくなる。なお、接合は、例えば、融着、接着剤による接着等の方法で行うことができる。また、拡張部40の外形形状は、矩形状に限定されず、丸形状、多角形形状であってもよい。
 拡張部40と、フィルター部材70との間には、気体が注入される拡張空間40aが形成されている。
 図5に示すように、拡張部40は、帯体20の内表面側に配置されている。このため、拡張部40を拡張させると、拡張部40の圧迫力が手首W側に集中し、穿刺部位Pに好適に圧迫力を付与することができる。また、拡張部40は、帯体20により手首Wに押付けられて内圧が高まるため、後述するように、支持板21の孔部24およびフィルター部材70を介して、拡張部40内の気体を好適に外部に排出することができる。
 拡張部40の構成材料は、支持板21よりも弾性率が低く、可撓性を備える限り特に限定されないが、例えば、前述したベルト22の構成材料と同様のものを用いることができる。
 拡張部40は、実質的に透明であることが好ましいが、透明に限定されず、半透明または有色透明であってもよい。これにより、穿刺部位Pを外表面側から視認することができ、後述するマーカー40cを穿刺部位Pに容易に位置合わせすることができる。
 マーカー40cは、図2に示すように、拡張部40の支持板21に臨む面の略中央に設けられている。このため、拡張部40を穿刺部位Pに対して容易に位置合わせすることができ、拡張部40の位置ズレが抑制される。また、マーカー40cが穿刺部位Pと直接接触しないようにすることができる。なお、マーカー40cを設ける位置は、拡張部40を穿刺部位Pに位置合わせ可能である限りにおいて特に限定されない。例えば、マーカー40cは、孔部24による気体の排出を妨げることのない限りにおいてフィルター部材70や支持板21に設けられていてもよい。
 マーカー40cの形状は、特に限定されず、例えば、円、三角形、四角形等が挙げられ、本実施形態では、四角形をなしている。
 マーカー40cの大きさは、特に限定されないが、例えば、マーカー40cの形状が四角形をなしている場合、その一辺の長さが1~4mmの範囲であることが好ましい。一辺の長さが5mm以上であると、穿刺部位Pの大きさに対してマーカー40cの大きさが大きくなるため、拡張部40の中心部を穿刺部位Pに位置合わせし難くなる。
 マーカー40cの材質は、特に限定されず、例えば、インキ等の油性着色料、色素を混練した樹脂等が挙げられる。
 マーカー40cの色は、拡張部40を穿刺部位Pに位置合わせすることができる色であれば特に限定されないが、緑色系が好ましい。緑色系にすることにより、マーカー40cを血液や皮膚上で容易に視認することができるため、拡張部40を穿刺部位Pに位置合わせすることがより容易となる。
 また、マーカー40cは半透明または有色透明であることが好ましい。これにより、穿刺部位Pを帯体20の外表面側から視認することができる。
 拡張部40にマーカー40cを設ける方法は特に限定されないが、例えば、マーカー40cを拡張部40に印刷する方法、マーカー40cの片面に接着剤を塗布して拡張部40に貼り付ける方法等が挙げられる。
 注入部60は、拡張部40に気体を注入するための部位であり、図1に示すように、拡張部40に接続されている。
 注入部60は、その基端部が拡張部40とフィルター部材70との間に配置され、その内腔が拡張部40の拡張空間40aに連通する可撓性を有するチューブ61と、チューブ61の内腔と連通するようにチューブ61の先端部に配置された袋体62と、袋体62に接続された逆止弁(図示せず)を内蔵する管状のコネクタ63と、を備えている。
 拡張部40を拡張(膨張)させる際には、コネクタ63にシリンジ(図示せず)の先筒部を挿入して逆止弁を開き、このシリンジの押し子を押して、シリンジ内の気体を注入部60を介して拡張部40内に注入する。拡張部40が拡張すると、チューブ61を介して拡張部40と連通している袋体62も膨張し、気体が漏れずに、拡張部40を加圧できていることを目視で確認できる。拡張部40内に気体を注入した後、コネクタ63からシリンジの先筒部を抜去すると、コネクタ63に内蔵された逆止弁が閉じて気体の漏出が防止される。
 フィルター部材70は、拡張部40内に注入された気体が拡張部40の外部に排出される排出速度を制御する機能を備えている。
 フィルター部材70は、図1に示すように、略矩形状のシートによって構成している。フィルター部材70は、図2に示すように、孔部24を塞ぐように支持板21の内表面に接合されている。したがって、フィルター部材70は、支持板21と拡張部40との間に配置される。このため、フィルター部材70が、外部に露出して、装着者や周辺の医療器具等がフィルター部材に接触してフィルター部材70が損傷するのを防止することができる。また、フィルター部材70が、装着者や周辺の医療器具等に接触して、フィルター部材70の気体排出機能が阻害されるのを好適に防止できる。なお、フィルター部材70の外形形状は、孔部24を塞ぐことができる限り特に限定されず、例えば、丸形状、四角形以外の多角形形状等であってもよい。
 本実施形態では、フィルター部材70は、図5(B)に示すように、多孔質層71と、多孔質層71よりも単位面積当たりの気体透過量が多いスキン層72と、を備えた非対称膜によって構成している。
 多孔質層71は、本実施形態では、スキン層72よりも厚みが大きくなるように構成している。多孔質層71は、薄層のスキン層72を支持することにより、フィルター部材70全体の強度を良好に保つ役割を持つ。また、多孔質層71は、多数の孔を備えている。本実施形態では、多孔質層71は、1nm以上の大きさの孔を備えている。
 スキン層72は、多孔質層71と比較して、単位面積当たりの孔の数が少ない。これにより、スキン層72は、多孔質層71と比較して、単位面積当たりの気体透過量が少ない。なお、スキン層72と多孔質層71の気体透過量の大小関係は、両者に設けられる孔の数、孔の径、孔の形状等により調整することが可能である。
 本実施形態では、スキン層72は、1nm以上の大きさの孔を備えていない、このため、拡張部40に注入された気体は、溶解・拡散現象によってスキン層72を透過する。また、本実施形態では、拡張部40とフィルター部材70とは同一の材料で形成されており、スキン層72の厚みは、拡張部40の厚みよりも小さい。したがって、拡張部40内に注入された気体は主に、フィルター部材70を介して外部に排出され、排出速度は、主に、スキン層72の気体の透過量に依存したものとなる。
 このように、拡張部40内に注入された気体の排出は、主に帯体20に設けられた孔部24およびフィルター部材70を介して行われる。このため、気体透過量を増やすために拡張部40の厚みを薄く作製する必要がなく、拡張部40の強度を良好に保つことができる。また、フィルター部材70は、拡張部40に注入された気体の排出を制御するため、帯体20に設けた孔部24から気体の排出を行う場合(孔部24がフィルター部材70に塞がれていない場合)と比較し、気体の排出を抑制することができる。すなわち、拡張部40内に注入された気体の排出速度は、フィルター部材70により容易かつ精密に調整することができる。
 なお、フィルター部材70は、比較的薄層であって損傷しやすいスキン層72を備えており、支持板21および拡張部40よりも弾性率が小さい(柔軟性が高い)。このため、フィルター部材70は、変形等により比較的損傷しやすいという性質がある。ただし、本実施形態では、フィルター部材70は、弾性率の比較的高い支持板21に配置されており、かつ、支持板21により支持されているため、当該フィルター部材70が変形してスキン層72が損傷するのを好適に防止することができる。
 フィルター部材70全体としての気体透過速度は、以下の条件1、2を満たす減圧プロトコルを実現可能に形成されていることが好ましい。
(条件1)帯体20を手首Wに巻き付けた状態で、拡張後4時間にわたってフィルター部材70および孔部24を介して気体を拡張部40の外部に排出することで、1時間経過するごとの拡張部40の内圧が、その1時間前の拡張部40の内圧の70~97%(好ましくは、75~94%)となる;
(条件2)帯体20を手首Wに巻き付けた状態で、拡張後4時間経過した後の拡張部40内の内圧が初期内圧の30~80%(好ましくは、40~71%)となる。
 上記の条件を満たすために、フィルター部材70の厚みおよび支持板21の孔部24の大きさを適時設定する必要はあるが、拡張部40の拡張空間40aの内圧が40kPa~50kPa(300mmHg~375mmHg)になるように拡張部40に18mlの気体を注入した場合、フィルター部材70は、1時間当たりの気体透過量が0.5~3.0mlであることが好ましい。また、多孔質層71の厚みは、例えば、10μm~1mmとし、スキン層72の厚みは、例えば、100nm~数μmと設定することができる。
 なお、フィルター部材70は、拡張部40内に注入された気体が拡張部40の外部に排出される排出速度を制御する機能を備えていればよい。そのため、フィルター部材70は、拡張部40の材料よりも単位面積当たりの気体透過量が多い材料を使用することで代用することが可能である。
 次に、本実施形態に係る止血器具10の使用方法について説明する。
 止血器具10を手首Wに装着する前は、図2に示すように、拡張部40は、拡張していない状態となっている。図4および図5(A)に示すように、右手の手首Wの橈骨動脈Rに穿刺を行う場合、穿刺部位Pは、親指側へ片寄った位置にある。通常、穿刺部位Pにはイントロデューサーシースが留置されている。このイントロデューサーシースが留置されたままの状態の手首Wに帯体20を巻き付け、拡張部40に設けられたマーカー40cが穿刺部位P上に重なるように拡張部40および帯体20を位置合わせして、面ファスナー30の雌側31a、31bおよび雄側32を接触させて接合し、帯体20を手首Wに装着する。
 この際、止血器具10は、注入部60が、橈骨動脈Rの血流の下流側(掌側)に向くように、手首Wに対して装着される。これにより、手首よりも上流側での手技や、上流側に位置する器具(例えば、血圧計等)に干渉することなしに、注入部60の操作が可能となる。また、止血器具10を、注入部60が下流側に向くように右手の手首Wに装着することで、拡張部40は、手首Wの親指側へ片寄って位置する橈骨動脈Rに位置する。なお、動脈の場合、血管の上流側とは、血管の心臓に近づく方向をいう。また、血管の下流側とは、血管の心臓から遠ざかる方向をいう。
 なお、止血器具10は、左手の手首の橈骨動脈に穿刺を行う場合に使用してもよい。この場合、注入部60は、橈骨動脈の血流の上流側に向くように、左手の手首に対して装着される。
 止血器具10を手首Wに装着した後、注入部60のコネクタ63にシリンジ(図示せず)を接続し、前述したようにして気体を拡張部40内に注入し、拡張部40を拡張させる。
 この際、気体の注入量により、症例に応じて、拡張部40の拡張度合、すなわち、穿刺部位Pに作用する圧迫力を容易に調整することができる。例えば、仮に、拡張部40に気体を注入しすぎて拡張部40が過拡張した場合は、シリンジを用いて拡張部40内から注入しすぎた分の気体を排出すればよい。拡張部40を拡張させた後、コネクタ63からシリンジを離脱させる。そして穿刺部位Pからイントロデューサーシースを抜去する。
 拡張部40を拡張させた後は、主にフィルター部材70および支持板21の孔部24を介して、穿刺部位Pに圧迫力を付与しつつ血管閉塞を防止できる程度に、図5(A)、(B)に破線矢印で示すように、拡張部40内の気体が経時的に拡張部40の外部に排出される。
 なお、仮に、拡張部40の拡張後、止血が十分に行われていない場合は、拡張部40に気体を注入して、拡張部40の内圧を上昇させてもよい。例えば、拡張部40の内圧を、拡張部40に気体を注入した時の内圧に戻したい場合は、拡張部40から排出された分の気体を注入すればよい。
 所定の時間が経過して、穿刺部位Pの止血が完了したら、止血器具10を手首Wから取り外す。止血器具10は、面ファスナー30の雌側31a、31bおよび雄側32を剥がすことによって手首Wから取り外される。
 以上のように、本実施形態に係る止血器具10は、穿刺部位Pに巻き付けるための帯体20と、帯体20を手首Wに巻き付けた状態で固定する固定手段30と、帯体20に連結され、かつ、気体を注入することにより拡張する拡張部40と、拡張部40に注入された気体の排出を制御するフィルター部材70と、を備えている。帯体20は、帯体20の内表面と外表面との間を貫通する1つ以上の孔部24を有している。拡張部40は、帯体20の内表面において、孔部24を覆うように配置されている。フィルター部材70は、孔部24を塞ぐように帯体20に配置され、かつ、孔部24からの気体の排出を抑制する。
 上記のように、孔部24は、帯体20の拡張部40が配置されている領域に設けられており、フィルター部材70は、孔部24を塞いで、拡張部40内に注入された気体の排出量を制御する。このため、気体透過量を増やすために拡張部40の厚みを薄く作製する必要がなく、拡張部40の強度を良好に保つことができる。また、拡張部40に注入された気体の排出速度はフィルター部材70によって制御されるため、帯体20に設けた孔の構造のみで気体の排出速度を制御する場合と比較して、気体の排出速度を容易かつ精密に調整することができる。さらに、フィルター部材70は、孔部24を塞ぐように帯体20に配置されているため、帯体20によりフィルター部材70がサポートされ、フィルター部材70の変形が抑制される。これにより、気体の排出速度はフィルター部材70の変形による影響を受けて変化し難くなるため、止血器具10は、所望の減圧プロトコルを実現可能なものとなる。
 また、帯体20は、拡張部40が配置される支持板21と、固定手段30が配置されるベルト22と、を有し、支持板21は、拡張部40およびベルト22よりも弾性率が高い。このため、拡張部40を好適に拡張変形させつつ、支持板21に形成された孔部24を塞ぐように配置されたフィルター部材70が変形するのを好適に防止できる。すなわち、拡張部40を拡張させた場合も、フィルター部材70の気体透過機能を好適に維持することができる。
 また、フィルター部材70は、拡張部40と帯体20との間に配置されている。このため、フィルター部材70が、装着者や周辺の医療器具等に接触して損傷するのを好適に防止できる。また、フィルター部材70が、装着者や周辺の医療器具等に接触して、フィルター部材70の気体排出機能が阻害されるのを好適に防止できる。
 また、フィルター部材70の外周縁部には、拡張部40が接合されている。このため、フィルター部材70が帯体20から不用意に分離や脱落するのを好適に防止できる。
 また、フィルター部材70の材料は、拡張部40の材料よりも単位面積当たりの気体透過量が多くなるように構成されている。このため、フィルター部材70は、フィルター部材70によって、拡張部40に注入された気体の排出速度を制御することができる。
 また、フィルター部材70と拡張部40は同じ材料で形成されており、フィルター部材70は、多孔質層71と、多孔質層71よりも単位面積当たりの気体透過量が少ないスキン層72と、から構成されている。このため、フィルター部材70は、スキン層72によって、拡張部40に注入された気体の排出速度を制御しつつ、多孔質層71によってスキン層72を支持し、フィルター部材70の強度を良好に保つことができる。また、フィルター部材70と拡張部40が同じ材料で形成されているため、フィルター部材70と拡張部40とを接合する際、フィルター部材70は、拡張部40に容易に融着することができる。
 次に、上述した実施形態の変形例を説明する。なお、各変形例の説明において前述した実施形態と同様の構成については、同一の符号を付し、その説明を省略する。
 (変形例1)
 図6は、変形例1に係る止血器具100の説明に供する図である。
 変形例1に係る止血器具100は、フィルター部材170の配置位置が前述した実施形態と相違している。
 具体的には、図6(A)に示すように、フィルター部材170は、支持板21の孔部24が設けられている領域において、支持板21の外表面側に配置されている。なお、フィルター部材170は、前述した実施形態と同様に、多孔質層71とスキン層72とを備える非対称膜によって構成している。
 支持板21のフィルター部材170が配置される領域には、外表面側から内表面側に向かって窪んだ凹部121dが形成されている。そして、フィルター部材170は、図6(B)に示すように、スキン層72が凹部121dに入り込むようにして支持板21の外表面上に配置されている。このため、薄層で損傷しやすいスキン層72が外部に露出して、装着者や周辺の医療器具等と接触し、破損するのを好適に防止することができる。また、フィルター部材170の一部が凹部121d内に配置されることにより、フィルター部材170の位置ずれが生じ難くなる。
 以上、変形例1に係る止血器具100によれば、フィルター部材70は、帯体20の外表面側に配置されている。このため、孔部24が形成された帯体20の外表面側から孔部24を覆うようにフィルター部材70を配置することにより、気体の排出速度を調整する機能を止血器具100に付加することが可能になる。したがって、変形例1によれば、止血器具100の製造作業が容易なものとなる。
 なお、上記変形例においては、フィルター部材170を支持板121に形成した凹部121d内に配置する例を示したが、例えば、支持板121に凹部121dを形成せずに、支持板121の外表面にフィルター部材170を配置するようにしてもよい。
 (変形例2)
 図7は、変形例2に係る止血器具200の説明に供する図である。
 変形例2に係る止血器具200は、拡張部240の構成において、前述した実施形態と相違する。
 拡張部240は、穿刺部位Pを圧迫する第1圧迫部241と、第1圧迫部241と支持板21との間に配置され、第1圧迫部241の圧迫力が付与する圧迫力の方向を、穿刺部位Pに向かう方向に調整する第2圧迫部242と、を備えている。
 第1圧迫部241は、略矩形状の2枚のシートを重ね合わせて袋状にすることによって形成している。第1圧迫部241には、前述した拡張部40と同様に注入部60が接続されている(図示せず)。なお、第1圧迫部241の構成は、気体を注入することによって拡張可能であれば特に限定されない。例えば、第1圧迫部241は、1枚のシートを折り曲げ、縁部を接合した袋状の部材によって構成してもよいし、縁部を備えない風船状の部材によって構成してもよい。また、第1圧迫部241の外形形状は特に限定されない。第1圧迫部241は、例えば、平面視において、円形、楕円形、多角形等の外形形状を備えていてもよい。
 第2圧迫部242は、略矩形状のシートによって構成している。第2圧迫部242は、支持板21の内表面側において、孔部24が設けられた領域を覆うように配置されている。第2圧迫部242の外周縁部は、支持板21側に折り込まれた状態で、支持板21と、フィルター部材70の外周縁部と、に接合されている。第2圧迫部242とフィルター部材70との間に気体が注入される空間が形成されている。第2圧迫部242の略中央は、第1圧迫部241に連結されており、第1圧迫部241の内部空間と第2圧迫部242の内部空間とは連通している。
 マーカー40cは、第1圧迫部241の内表面の略中央に設けられている。
 以上、変形例2に係る止血器具200によれば、第2圧迫部242によって、第1圧迫部241が付与する圧迫力の方向を、図7に実線の矢印で示すように穿刺部位Pに向かう方向に調整することができる。
 (変形例3)
 図8は、変形例3に係る止血器具300の説明に供する図である。
 変形例3に係る止血器具300は、支持板21に設けられた孔部324の形状が前述した実施形態と相違する。
 孔部324は、図8(B)に示すように、支持板21の厚み方向に沿う断面において、支持板21の内表面側から外表面側に向かって、先細った形状を備えている。このため、フィルター部材70と孔部324の接触面積を比較的大きく保つことで気体透過性を良好に維持しつつ、外表面側から異物が孔部324に侵入して、フィルター部材70を損傷するのを好適に抑制することができる。
 また、孔部324を形成している支持板21の内表面側の角部324rは、丸みを帯びた形状を備えている。このため、フィルター部材70が角部324rに接触した際に、フィルター部材70が損傷するのを好適に防止することができる。
 なお、支持板21の内表面側からの平面視における孔部324の形状は、特に限定されず、例えば、正六角形等の多角形の形状、円形、楕円形等に形成することができる。
 (変形例4)
 図9~図15は、変形例4に係る止血器具400の説明に供する図である。図9~図11は、止血器具400を装着した状態を示す断面図であり、図12~図14は、注入部450および逆流防止機構492を示す図、図15は、止血器具400が備える支持板21(支持体部)を示す図である。
 変形例4に係る止血器具400は、図9に示すように、拡張部40に気体(空気)を注入可能な注入部450が当該止血器具400に一体的に設けられている。
 注入部450は、図12に示すように、気体(空気)を収容可能な収容空間(内腔)450aを備える立体形状の部材によって構成されている。注入部450は弾性変形が可能に構成されており、弾性変形することにより、拡張部40へ空気を注入する。
 注入部450は、図9および図12に示すように、当該注入部450を貫通し、かつ、収容空間450aと連通した孔部450bを有している。孔部450bは、後述する保持板480の注入部450が配置されている領域への垂線X1上に配置されている。
 注入部450は、図12に示すように、保持板480に固定される底部451と、底部451から孔部450bに向かって立ち上がる縦壁部452と、孔部450bで構成された上部453と、を有している。底部451には、保持板480において注入部450と固定される固定部451aが設けられている。注入部450は、例えば、接着剤等により保持板480に固定することができる。
 本実施形態においては、注入部450の底部451は、保持板480の外面に臨む開口部で形成されている。このため、底部451は、注入部450の底面を構成しておらず、保持板480の外面の一部が注入部450の底面を構成している。注入部450の収容空間450aは、保持板480の外面の一部および縦壁部452に囲まれた空間に相当する。
 縦壁部452の外周は、底部451側から孔部450b側(底部451から離間する方向)に向かって小さくなるように形成している。また、図9および図12に示すように、縦壁部452は、保持板480の注入部450が配置されている領域への垂線X1と平行な断面において、テーパー形状に形成している。ここでいうテーパー形状とは、底部451から孔部450bに向けて徐々に外周長が小さくなる形状を意味する。
 なお、注入部450の外形形状は、特に限定されず、例えば、四角柱等の多角柱であってもよいし、底部、縦壁部および上部の区別がない球、断面視がテーパー形状以外の形状等であってもよい。また、底部451および上部453が開口部(孔部)で構成された注入部450を図示しているが、注入部450は、底部に底面が形成され、上部に上面が形成された構造のものであってもよい。また、孔部450bの数は1つ以上であれば特に限定されず、孔部450bの形状も図示した形状に限定されることはない。
 注入部450の収容空間450aの容積は、拡張部40の拡張空間40aの容積の1/4~1/3程度であることが好ましい。これにより、注入部450を適度な大きさに形成し、注入部450が止血器具10の周辺で行われる手技等を妨げるのを防止するとともに、後述する拡張部40に空気を注入する注入動作を行う回数を低減することができる。
 注入部450は、収縮可能であって、かつ、収縮後に元の形状に復元可能となるように、例えば、シリコーンゴム、ラテックスゴム等のエラストマー素材やポリプロピレン、ポリエチレン等の熱可塑性プラスチック素材、またこれら両方の性質を併せ持つ各種熱可塑性エラストマー素材によって構成していることが好ましい。なお、注入部450は、後述するように保持板480を支持板21に固定する際(図11を参照)、上下方向(図12中の上下方向)に折り畳みが可能となるように、肉厚が比較的薄く形成されていることが好ましい。
 図9に示すように、注入部450の収容空間450aは、チューブ491を介して拡張部40の拡張空間40aと連通している。
 注入部450は、孔部450bから収容空間450a内へ空気を取り込むことにより、図9に示す初期形状となる。注入部450を利用した拡張部40の拡張操作を行う際は、注入部450が初期形状となっている状態で、図10に示すように、孔部450bを手指で塞ぎつつ注入部450を押し潰すように変形させる。
 上記のように注入部450を押し潰す操作を行うと、チューブ491を経由して空気が拡張部40へ送られる。拡張部40へ空気を送り込む操作を再度行う場合、孔部450bから手指を移動させて、収容空間450aを外部と連通した状態にする。注入部450は、孔部450bから収容空間450aに取り込まれた空気により、図9に示す初期形状に復元するように変形する。収容空間450a内に空気が取りこまれた状態で注入部450を再度押し潰すと、拡張部40へ空気を送り込むことができる。
 図9および図12に示すように、注入部450は、保持板480に配置されている。保持板480は、支持板21に連結している。
 保持板480は、支持板21の第1連結部178と連結可能な第2連結部481を有している。
 支持板21の第1連結部178は、略円形の断面形状を有する軸状の部材で構成している。保持板480の第2連結部481は、支持板21の第1連結部178に嵌め込み可能な凹状の溝で構成している。図11に示すように、保持板480の第2連結部481は、支持板21の第1連結部178に嵌め込まれた状態で、第1連結部178の外周面に沿って回転可能となる。
 支持板21の第1連結部178は、図11に示すように、支持板21と一体的に形成されており、支持板21の外面から離間する方向へ突出している。第1連結部178の周辺部分には、隙間部を設けている。隙間部は、保持板480が回転する際、保持板480と支持板21とが干渉するのを防止して、保持板480の円滑な回転を可能にする。このように、保持板480は、帯体20の長手方向に対して交差する第1連結部178(回転軸)を中心に回転可能に構成している。なお、保持板480は、帯体20の長手方向に対して直交する第1連結部178(回転軸)を中心に回転可能に構成していることが好ましい。これにより、注入部450および保持板480は、保持板480が回転する際、支持板21よりも手首W側に突出しにくくなる。
 保持板480は、例えば、支持板21と同様の材質で構成することができる。なお、保持板480は、注入部450よりも硬い材料で構成されることが好ましい。これにより、術者は、後述する拡張部40へ空気を送り込む際、注入部450を把持しやすくなる。
 図12に示すように、保持板480の幅W1は、注入部450の幅W2よりも長く形成している。なお、上記の幅は、帯体20の長手方向と直交する方向に沿う寸法(図1を参照)を意味する。また、注入部450の幅とは、当該注入部450において最も幅寸法が大きくなる部分の幅寸法を意味し、本変形例では固定部451aの幅を指す。
 上記のように保持板480の幅W1に対して注入部450の幅W2が小さく形成されることにより、注入部450が保持板480の幅方向に突出しない。このため、止血器具400を手首Wに装着した状態で(図4を参照)、手首W側に注入部450が突出しなくなる。これにより、注入部450が装着者の手首Wと接触しにくくなるため、装着者が感じる不快感を低減することができる。
 また、保持板480の幅W1は、例えば、支持板21の幅よりも小さく形成することができる。保持板480の幅W1が支持板21の幅よりも小さく形成される場合、注入部450の幅W2も支持板21よりも小さく形成される。これにより、保持板480および注入部450は、支持板21よりも手首W側により確実に突出しなくなるため、装着者が感じる不快感を好適に低減することができる。
 なお、保持板480は、注入部450が配置可能であればその構成は特に限定されず、例えば、各部の寸法や断面形状等は適宜変更することが可能である。
 支持板21および保持板480には、保持板480を固定するロック機構を設けている。ロック機構は、支持板21に設けた第1部材485aと保持板480に設けた第2部材485bとにより構成している。
 第1部材485aは、第2部材485bが嵌合可能な凹部を有している。図11に示すように、保持板480が回転した際、第2部材485bは第1部材485aの凹部と嵌合することにより保持板480を支持板21に対して固定する。これにより、保持板480の回転が規制される。例えば、注入部450を利用して拡張部40を拡張させた後、保持板480を固定することにより、保持板480に設けた注入部450が不用意に回転するのを防止することができる。このため、拡張部40を拡張させた後、止血器具400を操作等する際に注入部450により円滑な作業が妨げられるのを防止できる。また、保持板480を支持板21に対して固定することにより、注入部450は、保持板480と支持板21との間に挟まれる。このため、止血器具400は、注入部450が不意に押し潰され、拡張部40に空気を送り込まれることを防止することができる。
 なお、ロック機構は、保持板480の回転を規制可能であればその構成は特に限定されない。ロック機構は、部材同士の機械的な接続(嵌合)により保持板480の回転を規制する構造に限定されず、例えば、磁力等で保持板480の回転を規制する構造であってもよい。また、機械的な接続で保持板480を固定する構造を有する場合においても、ロック機構(第1部材485a、第2部材485b)の具体的な形状、位置、大きさ、個数等は、図示したものに特に限定されない。
 図12に示すように、注入部450は、保持板480の外面から離間する方向に延在しており、孔部450bは、保持板480の注入部450が配置されている領域への垂線X1上に配置されている。このため、拡張部40へ空気を注入する際に注入部450を押圧する方向は、保持板480の注入部450が配置されている領域への垂線X1に沿う方向(縦方向)となる(図10を参照)。このため、手指から注入部450に対して効率良く力を付与することができ、拡張部40へ円滑に空気を注入することができる。なお、注入部450を押し潰す際、図10に示すように、保持板480の外表面側および内表面側から二つの手指で注入部450を挟み込みことにより、注入部450を効率良く押し潰すことができる。
 注入部450は、縦壁部452の外周が底部451側から孔部450b側に向かって小さくなるため、注入部450を押し潰す操作がなされる際、注入部450の底部451側で座屈しにくく、折れ等の破損が生じるのを防止することができる。さらに、注入部450は、縦壁部452の外周が底部451側から孔部450b側に向かって小さくなるため、注入部450を押し潰す操作がなされる際、注入部450の孔部450b側が注入部450の収容空間50a内側に折り畳まれるように変形する。このため、注入部450を孔部450b側から底部451側に向けて小さい力で押し潰すことができ、収容空間450aの空気を拡張部40へ効率的に送り込むことができる。
 注入部450は、比較的硬質な保持板480に固定されているため、注入部450を押圧する際に、保持板480が変形するのを防止できる。また、保持板480は、支持板21に対して回転可能に連結されているため、注入部450を押圧した際に保持板480に伝達される押圧力は、保持板480と支持板21の連結部分で吸収される。その結果、注入部450を押圧した際に生じる押圧力は、支持板21まで伝達され難くなり、穿刺部位Pへも伝達され難くなる。このため、装着者は、拡張部40を拡張させる操作が行われている最中に、拡張部40から穿刺部位Pに付与される圧迫力のみを適切に把握することができる。したがって、装着者が感じる圧迫力に基づいて注入部450を操作することにより、拡張部40に穿刺部位Pの止血に最適な量の空気を注入することができる。
 図12および図13に示すように、チューブ491の基端部は、注入部450の底部451側に取り付けられており、チューブ491の先端部は、拡張部40の拡張空間40aに配置されている。なお、注入部450においてチューブ491を取付ける位置は、注入部450の収容空間450aと拡張部40の拡張空間40aとを連通可能である限り特に限定されない。例えば、チューブ491の基端部は、注入部450の縦壁部452に取り付けてもよい。ただし、本実施形態のように孔部450bが上部453側に配置される場合、チューブ491の基端部は、収容空間450aの空気を拡張部40へ効率的に送り込むため、注入部450の底部451側に取り付けられることが好ましい。
 図15には、図9に示す矢印15A方向から視た支持板21の平面図(支持板21の内表面側から見た平面図)を簡略化して示している。
 支持板21には、複数の孔部24の周囲を囲む凸部175が形成されている。
 拡張部40は、図10に示すように、凸部175に固定されている。拡張部40は、凸部175に固定されることにより、凸部175と拡張部40に囲まれた空間に気密性を備える拡張空間40aを形成している。凸部175は、複数の孔部24の周囲を囲むように、かつ、支持板21の内表面から突出して形成されている。なお、凸部175は、複数の孔部24の全周を囲むように形成されることが好ましい。これにより、拡張部40を凸部175に簡単に固定することができ、支持板21に拡張部40を簡単に取り付けることができる。また、拡張部40は、凸部175の間に拡張空間40aを形成するように固定されていればよく、例えば、凸部175の突出方向の端部以外の部分に固定してもよい。また、凸部175は、複数の孔部24の全周を囲むように形成されていなくてもよい。その場合、拡張空間40aは、支持板21、拡張部40、及び凸部175に囲まれた空間により形成される。
 支持板21に形成された凸部175は、止血器具10を手首Wに装着し、かつ、拡張部40を拡張させる前の状態(図9に示す状態)において、穿刺部位Pと支持板21との間の距離を調整する機能を有している。
 帯体20を手首Wに巻き付ける際に支持板21を手首W上に配置すると、凸部175は手首Wに接触する。穿刺部位Pと支持板21との間の距離は、凸部175の高さ寸法(突出方向の寸法)に応じて所定の大きさに調整される。このため、例えば、装着者ごとの手首Wの外周長の違いに依らずに、拡張開始前の拡張部40と穿刺部位Pとの間の距離を所定の距離に保つことができる。そして、拡張開始前の拡張部40と穿刺部位Pとの間の距離が所定の距離に調整された状態で拡張を行うことにより、装着者の手首Wの外周長に依らずに、一定の圧迫力を付与することが可能になる。
 図9に示すように、凸部175は、例えば、突出方向の先端部が丸みを帯びた断面形状で形成することが可能である。凸部175は、このような断面形状を有することにより、手首Wに接触した際に装着者に対して過剰な押圧力が掛かるのを防止する。
 凸部175の高さ寸法(突出方向の寸法)、断面形状、支持板21上において配置される位置について特に制限はなく、適宜変更することが可能である。また、凸部175は支持板21と一体的に構成してもよいし、支持板21とは別部材で構成してもよい。また、凸部175には、皮膚への接触時の押圧力を緩和する緩衝部材を設けてもよい。
 図9に示すように、保持板480には貫通孔が設けられており、チューブ491の基端部は、当該貫通孔を挿通するようにして配置されている。また、図15に示すように、支持板21の凸部175には、チューブ491の一部を挿通させるための貫通孔176を設けている。
 チューブ491は、図9および図15に示すように、例えば、保持板480の貫通孔、支持板21の第1連結部178の内面側、支持板21において帯体20が連結される連結端部25bの外面側、および支持板21に形成された貫通孔176を通って拡張部40と連通することができる。
 チューブ491は、保持板480の回転前には所定の長さのたわみ部を有している(図9を参照)。チューブ491のたわみ部は、図11に示すように、保持板480を回転させた際、たわんだ状態を解除するように延ばされる。たわみ部が延びることにより、保持板480の回転がチューブ491によって阻害されるのを防止できる。また、保持板480の回転に伴ってチューブ491に位置ズレが生じることのないように、チューブ491において支持板21の貫通孔176に挿通した部分は、支持板21に対して固定されている。固定は、例えば、接着剤により行うことが可能である。
 図9に示すように、チューブ491の拡張空間40a内に配置された先端部には、拡張空間40a内から注入部450側への逆流を防止する逆流防止機構492を設けている。
 逆流防止機構492は、拡張部40内に配置している。逆流防止機構492は、図13、および図14(A)、(B)に示すように、チューブ491の先端部に接続される芯材493と、芯材493を覆う被覆部材494と、を備えている。
 芯材493は、芯材493の基端部をチューブ491の内腔491aの先端側に挿入して固定することで、チューブ491と連結している。
 芯材493は、略円柱状の外形形状を備えている。芯材493は、チューブ491の内腔491a内において開口した基端開口部493bと、芯材493において被覆部材494が設けられている面において開口した先端開口部493cと、基端開口部493bと先端開口部493cに連通する内腔493aと、を有している。
 なお、芯材493とチューブ491は、芯材493の基端部をチューブ491の内腔491aに挿入して固定する方法以外の形態で連結してもよく、例えば、芯材493の基端面とチューブ491の先端面とを突き合せた状態で各々を固定し、芯材493の内腔493aとチューブ491の内腔491aとを気密に連通させるようにしてもよい。
 芯材493の構成材料は、被覆部材494よりも硬度の高い材料であることが好ましい。そのような材料としては、例えば、公知の金属材料、プラスチック材料等が挙げられる。
 被覆部材494は、円筒状の外形形状を備えている。芯材493は、被覆部材494を挿通している。
 被覆部材494の構成材料は、弾性部材であることが好ましく、そのような材料としては、例えば、ブチルゴム、多硫化ゴム、エピクロロヒドリンゴム、高ニトリルゴム、フッ素ゴム、シリコーンゴム等のエラストマー素材、および各種熱可塑性エラストマー素材等が挙げられる。
 図14(A)、(B)を参照して、逆流防止機構492の動作を説明する。図14において、点線の矢印は空気の流れを示し、実線の矢印は空気が被覆部材494に付与する圧力の方向を示している。
 図14(A)に示すように、拡張部40が十分に拡張していない状態において注入部450からチューブ491に空気が注入されると、空気は、芯材493の内腔493aを通り抜けて、芯材493から離れる方向の圧力を被覆部材494に付与する。
 被覆部材494は、注入部450から送り込まれた空気から受ける圧力が所定の大きさ以上になると、芯材493の外表面から離れて、先端開口部493cと拡張空間40aを連通させる。例えば、注入部450を十分に押し潰す操作がなされず、注入部450側から送り込まれた空気量が少ないと、被覆部材494に付与される圧力も低下するため、先端開口部493cと拡張空間40aを連通させることができない。これに対して、比較的時間を掛けてゆっくりと注入部450を押圧し、注入部450を十分に押し潰す操作が行われると、被覆部材494は芯材493の外表面から離間する。物品等で注入部450の孔部450bが誤って塞がれてしまった場合においても、注入部450を十分に押し潰す操作がなされないと、拡張部40内へ不用意に空気が送り込まれないため、穿刺部位Pが必要以上に圧迫されるのを好適に防止できる。
 図14(B)に示すように、拡張部40が十分に拡張している状態においては、拡張部40内の空気は、芯材493と接触する方向の圧力を被覆部材494に付与する。これにより、先端開口部493cが被覆部材494によって塞がれるため、拡張部40内の空気は、芯材493側、ひいては注入部450側に逆流しない。また、拡張部40が十分に拡張している状態においては、拡張部40内の空気が先端開口部493cを塞ぐように被覆部材494に圧力を付与する。当該圧力は、空気の注入圧力よりも高くなる。このため、拡張部40が十分に拡張し、拡張部40の内圧が所定値になると、注入部450から拡張部40に空気を注入することができなくなる。これにより、拡張部40が十分に拡張している状態においては、必要以上に拡張部40に空気が注入され、拡張部40が過拡張して、穿刺部位Pを必要以上に圧迫するのを好適に防止することができる。
 本変形例に係る止血器具400を使用して止血処置を行う場合、図11に示すように、拡張部40内に注入された気体の排出は、帯体20に設けられた孔部24およびフィルター部材70を介して主に行われる。このため、気体透過量を増やすために拡張部40の厚みを薄く作製する必要がなく、拡張部40の強度を良好に保つことができる。また、フィルター部材70は、拡張部40に注入された気体の排出を制御するため、帯体20に設けた孔部24から気体の排出を行う場合(孔部24がフィルター部材70に塞がれていない場合)と比較し、気体の排出を抑制することができる。すなわち、拡張部40内に注入された気体の排出速度は、フィルター部材70により容易かつ精密に調整することができる。
 また、止血器具400は、当該止血器具400と一体的に設けられた注入部450を有している。このため、止血器具400とは別体の専用の器具を使用することなく、簡単な操作で拡張部40の拡張を行うことが可能になる。
 なお、変形例4の止血器具400において、フィルター部材70を配置する位置は、支持板21の内表面側に限定されず、例えば、支持板21の外表面側であってもよい。また、止血器具400に変形例2に示した第2圧迫部242(図7を参照)を設けることも可能である。また、注入部450は、支持板21に連結された保持板480に配置せずに、支持板21に配置してもよい。また、保持板480は、支持板21の一部により構成することも可能である。この場合、保持板480を回転可能にするために、例えば、一部が薄肉に構成されたヒンジ部を支持板21に設けることができる。
 以上、実施形態および変形例を通じて本発明に係る止血器具を説明したが、本発明は説明した各構成のみに限定されるものでなく、特許請求の範囲の記載に基づいて適宜変更することが可能である。
 例えば、止血器具を構成する各部は、同様の機能を発揮し得る任意の構成のものと置換することができる。また、任意の構成物が付加されていてもよい。
 また、本発明は、手首に装着して使用する止血器具に限らず、脚等に装着して使用する止血器具にも適用することができる。
 また、前述した実施形態では、拡張部とフィルター部材が同一の材料によって構成されている場合を説明したが、拡張部とフィルター部材の材料は異なっていてもよい。
 また、本発明において、フィルター部材は、拡張部に注入された気体の排出を制御可能であればよく、必ずしも非対称膜によって構成する必要はない。フィルター部材は、例えば、気体透過性の高いシリコーンや天然ゴム等の熱硬化性エラストマーを用いることができる。また、フィルター部材は、フィルター部材の厚みを薄くし、かつ、単位面積当たりの気体透過量を高めたポリ塩化ビニル、ポリエチレン、ポリプロピレン、ポリ塩化ビニリデン等の熱可塑性樹脂又はオレフィン系熱可塑性エラストマー、スチレン系熱可塑性エラストマー等の各種熱可塑性エラストマーを用いることができる。これらの場合も、フィルター部材の単位面積当たりの気体透過量を拡張部の単位面積当たりの気体透過量よりも大きくすることができ、フィルター部材によって拡張部内に注入された気体の排出が制御されるようにすることができる。
 また、前述した実施形態では、拡張部は、支持板およびフィルター部材の両方に接合されている形態を説明したが、拡張部は支持板にのみ接合されていてもよい。

Claims (7)

  1.  肢体の止血すべき部位に巻き付けるための帯体と、
     前記帯体を前記肢体に巻き付けた状態で固定する固定手段と、
     前記帯体に連結され、かつ、気体を注入することにより拡張する拡張部と、
     前記拡張部に注入された気体の排出を制御するフィルター部材と、を備え、
     前記帯体は、前記帯体の内表面と外表面との間を貫通する1つ以上の孔部を有し、
     前記拡張部は、前記帯体の内表面において、前記孔部を覆うように配置され、
     前記フィルター部材は、前記孔部を塞ぐように前記帯体に配置され、かつ、前記孔部からの気体の排出を抑制する、止血器具。
  2.  前記帯体は、前記拡張部が配置される支持体部と、前記固定手段が配置される保持部と、を有し、
     前記支持体部は、前記拡張部および前記保持部よりも弾性率が高い、請求項1に記載の止血器具。
  3.  前記フィルター部材は、前記拡張部と前記帯体との間に配置される、請求項1または請求項2に記載の止血器具。
  4.  前記フィルター部材の外周縁部は、前記拡張部に接合されている、請求項3に記載の止血器具。
  5.  前記フィルター部材は、前記帯体の外表面側に配置される、請求項1または請求項2に記載の止血器具。
  6.  前記フィルター部材の材料は、前記拡張部の材料よりも単位面積当たりの気体透過量が多い、請求項1~5のいずれか1項に記載の止血器具。
  7.  前記フィルター部材と前記拡張部は同じ材料で形成されており、
     前記フィルター部材は、多孔質層と、前記多孔質層よりも単位面積当たりの気体透過量が少ないスキン層と、から構成される、請求項1~6のいずれか1項に記載の止血器具。
