La présente invention concerne un cathéter pour le déploiement d'une endoprothèse de type stent destiné aux opérations de thrombectomie, et tel que, selon l'invention le cathéter comprend un tube avec un premier canal de réception d'un fil de guidage et un second canal de réception de l'endoprothèse, les deux canaux étant partiellement indépendant jusqu'à leurs extrémités distales permettant un déplacement libre de l'endoprothèse après retrait du fil de guidage dans le premier canal.

Description

CATHETER POUR LE DEPLOIEMENT D’UNE ENDOPROTHESE DE TYPE STENT DESTINE AUX OPERATIONS DE THROMBECTOMIE
Domaine technique
L’invention se situe dans le domaine technique de la chirurgie et plus particulièrement des opérations de thrombectomie mécanique avec pose d’une endoprothèse de type stent.
L’invention est particulièrement destinée au traitement de l’accident vasculaire cérébral ischémique. Cette application n’est toutefois pas limitative et le dispositif pourra également être utilisé pour d’autres types d’opérations impliquant la pose d’endoprothèse avec utilisation d’un fil de guidage ou microguide.
Technique antérieure
On connaît à ce jour une technique de chirurgie endovasculaire visant à traiter l’ischémie cérébrale dans laquelle on vient extraire le caillot de l’artère cérébrale occluse à l’aide d’un stent-retriever. A cet fin le chirurgien utilise un cathéter et un microguide pour naviguer dans les artères cérébrales jusqu’au site d’occlusion et déployer l’endoprothèse. L’opération consiste, pour le chirurgien, à franchir le caillot à l’aide à l’aide du microguide et du cathéter, puis à retirer le microguide du cathéter afin de déployer le stent-retriever dans le caillot.
Le cathéter et le stent-retriever sont ensuite retirés ensemble via un large cathéter placé dans l’artère cervicale. Cette opération peut être réalisée à plusieurs reprises jusqu’au succès de recanalisation. Dans ce type d’intervention, il est impératif d’opérer rapidement pour améliorer les chances de bon pronostic fonctionnel du patient.
A partir des cathéters actuels si le chirurgien veut effectuer un passage supplémentaire de stent-retriever, il est nécessaire de respecter plusieurs étapes suivantes, après retrait conjoint du cathéter et du stent-retriever : i - nettoyer le stent- retriever pour éliminer les possibles fragments de caillot, ii- rengainer le stent-retriever, iii- introduire à nouveau le microguide dans le cathéter puis iv- avancer le cathéter à nouveau vers le site d’occlusion. Ces étapes entre deux passages de stent-retriever allongent la durée de procédure et le temps entre le début des signes et la recanalisation, considéré comme un facteur pronostic majeur. Autrement dit, ces étapes de retrait et de nettoyage entraînent des pertes de temps préjudiciables pour le patient, le temps de non-vascularisation du cerveau représentant en effet un risque considérable pour l’état de ce dernier.
La présente invention concerne un perfectionnement aux cathéters utilisés dans les opérations de thrombectomie mécanique.
Résumé de l’invention
La présente invention concerne un cathéter pour le déploiement d’une endoprothèse de type stent destiné aux opérations de thrombectomie. Selon l’invention, le cathéter comprend un tube avec un premier canal de réception d’un fil de guidage et un second canal de réception de ladite endoprothèse, les deux canaux étant indépendant jusqu’à leurs extrémités distales permettant un déplacement libre de l’endoprothèse après retrait du fil de guidage dans le premier canal.
Avantages apportés
Un premier but de la présente invention est de résoudre tout ou partie des problèmes techniques liés à l’art antérieur précité.
Un autre but de la présente invention est de proposer un cathéter permettant un déplacement au moins partiellement indépendant du fil de guidage et du stent permettant un déploiement du stent sans retrait complet du fil de guidage hors du cathéter.
Un autre but de la présente invention est de proposer un cathéter permettant un déplacement au moins partiellement indépendant du microguide et du stent-retriever évitant l’étape de rengainage du stent-retriever entre 2 passages.
Un autre but de la présente invention est de proposer un cathéter permettant de raccourcir la durée d’intervention moyenne d’une opération de thrombectomie mécanique.

Définition
Par stent on entend dans la présente demande principalement un stent retriever notamment de forme cylindrique, maillé à auto-déploiement ou autoexpansible. Toutefois d’autres types de stents utilisés dans le domaine de la chirurgie notamment artérielle seront également adapté au fonctionnement du cathéter décrit ci-après.
Par cathéter on entend dans la présente demande des cathéters permettant un déplacement dans les vaisseaux cervico-céphaliques et donc de très faibles sections appelés communément microcathéters.
Description des modes de réalisation
La présente invention vise à protéger un cathéter pour le déploiement d’une endoprothèse de type stent destiné aux opérations de thrombectomie mécanique.
Comme représenté à la figure 1 le cathéter 1 comprend un tube 2 avec un premier canal de réception 3 d’un fil de guidage 4. Le cathéter 1 comprend en outre un second canal de réception 5 d’une endoprothèse 6 et plus précisément d’un stent retriever.
Selon un aspect avantageux de l’invention, les deux canaux 3 et 5 sont indépendants jusqu’à leurs extrémités distales respectivement 7 et 8 permettant un déplacement libre de l’endoprothèse 6 après retrait du fil de guidage 4 dans le premier canal 3.
En se reportant plus particulièrement à la figure 2a on voit que le premier canal 3 et le second canal 5 du cathéter 1 sont sensiblement parallèles jusqu’à leur zone de jonction 10. Le premier canal 3, accueillant le fil de guidage, a une section de l’ordre de 0.4 à 0.7 mm.
En se reportant à la figure 2b, on voit que les extrémités 7 et 8 distales se rejoignent au niveau de la terminaison du tube 2 formant un seul canal dit terminal 13 ou lumière. Avantageusement, le tube 2 a une longueur supérieure au mètre, et de préférence voisine de 1 .5m, la longueur du canal terminal représentant 5 à 15% de la longueur du tube et de préférence 15 cm.
Avantageusement, comme représenté à la figure 3, la section du canal terminal 13 correspond à la section du second canal 5 au niveau de la jonction 10 puis décroit vers son extrémité.
En se reportant à nouveau à la figure 3, on voit que l’endoprothèse 6 est disposée dans le canal 5. Le fil de guidage 4, dans le premier canal 3, et l’endoprothèse 6, en position de pré chargement dans le second canal 5, sont séparés et le déplacement de l’un ne peut gêner le déplacement de l’autre. Lors de l’opération le chirurgien peut, dès que le cathéter 1 est en place en aval du caillot, tirer le fil de guidage 4 pour le réintroduire dans son canal 3 puis déployer le stent 6 hors du second canal 5 pour réaliser le geste de thrombectomie. Après nettoyage de stent-retriever dans du sérum salé hépariné, il est tiré dans le second canal 5 dans lequel il est stocké en attente d’un nouveau déploiement. Une fois le stent 6 réintroduit dans le second canal 3, le chirurgien peut à nouveau ressortir le fil de guidage 4 hors du cathéter 1 . Le chirurgien peut à nouveau utiliser le microguide stocké dans le canal 3 pour l’étape de navigation intracrânienne.
Ces différentes étapes peuvent être renouvelées jusqu’au succès de recanalisation sans nécessité de retrait total du fil de guidage 4 ni rengainage de l’endoprothèse 6 et donc avec un gain de temps considérable et une simplification des manoeuvres par rapport à l’état de la technique.
Selon un aspect avantageux et optionnel, le cathéter 1 dispose d’un système de rinçage continu à l’aide de valves hémostatiques en Y connectées aux deux canaux d’entrées du microcathéter. Le fil de guidage 4 et l’endoprothèse 6 peuvent être directement rinçés dans le cathéter à l’aide du système de rinçage intégré. En se reportant à la figure 3 permettant de détailler la jonction entre les canaux 3 et 5, on voit que l’extrémité distale 7 du premier canal 3 débouche à l’intérieur du second canal 5 en aval de la zone de stockage 9 de l’endoprothèse 6.
De manière à faciliter le déplacement du cathéter 1 dans les vaisseaux sanguins, la section de l’extrémité du canal terminal 13 est décroissante en direction de sa terminaison. Avantageusement la section décroit à partir de la zone de jonction 10 entre le premier et le second canal 3 et 5.
Selon un aspect avantageux, l’extrémité distale 7 du premier canal 3 est sensiblement conique et présente un bord arrondi 1 1 constituant une surface de glissement empêchant le blocage de l’endoprothèse 6 lors de sa sortie et/ou de son introduction dans le second canal 5.
Selon un autre aspect avantageux, l’extrémité distale 7 du premier canal est rigidifiée permettant de conserver l’axe de sortie du fil de guidage 4 dans l’axe du second canal. Cette rigidification pourra être obtenue soit par un épaississement de matière au niveau de l’extrémité distale 7 par rapport au reste du premier canal 3 ou encore pas l’emploi d’un autre matériau plus rigide au niveau de ladite extrémité distale 7.
Tel que représenté dans les figures annexées, le cathéter 1 comprend essentiellement un tube 2, le tube 2 comportant les deux canaux 3 et 5 étant monoblocs et se terminant en un seul canal.
Cela étant, selon une variante de réalisation les canaux 3 et 5 ne sont pas liés et le cathéter 1 comporte des moyens de blocage, avantageusement amovibles, du premier canal 3 intérieur par rapport au second canal 5 au niveau de la partie proximale de ces derniers empêchant un glissement relatif entre les canaux.
Selon un autre aspect avantageux, un marqueur radio-opaque 12 permet d’identifier clairement la limite au niveau de la zone de stockage 9 de l’endoprothèse 6.
Bien entendu, d’autres caractéristiques de l’invention auraient également pu être envisagées sans pour autant sortir du cadre de l’invention définie par les revendications ci-après.

Revendications
[Revendications 1 ] athéter pour le déploiement d’une endoprothèse de type de stent destiné aux opérations de thrombectomie, caractérisé en ce que le cathéter comprend un tube (2) avec un premier canal de réception (3) d’un fil de guidage (4) et un second canal de réception (5) de ladite endoprothèse, les deux canaux (3,5) étant indépendant jusqu’à leurs extrémités distales (7,8 permettant un déplacement libre de l’endoprothèse après retrait du fil de guidage (4) dans le premier canal (3).
[Revendications 2] Cathéter selon la revendication 1 dans lequel le premier et second canal sont parallèles, l’endoprothèse étant disposé dans le second canal (5) et le fil de guidage (4) étant disposé dans le premier canal (3), les extrémités distales (7,8) du premier et second canal débouchant en aval de la zone de stockage de l’endoprothèse, au niveau d’une zone de jonction (10), dans un même canal terminal (13).
[Revendications 3] Cathéter selon la revendication 2 dans lequel la section du canal terminal (13) est décroissante en direction de sa terminaison facilitant le déplacement du cathéter dans les vaisseaux sanguins.
