Die vorliegende Erfindung betrifft einen Nagellack Zur Behandlung von Onychomykosen enthaltend als Aktivstoff die Verbindung der Formel (Terbinafine) C(CH3)3
I in Form der freien Base oder eines Säureadditionssalzes, zusammen mit einem polymeren Filmbildner und geeigneten weiteren Arzneihilfsmitteln. 75 Derartige Lacke sind aus der EP 0 381 446 A1 bekannt, welche jedoch in erster Linie auf die Behandlung der Mundhöhle zum Überzug von Zähnen abzielt und Terbinafine nur als einen von vielen fungiziden Wirkstoffen erwähnt. Diese Lacke enthalten den fungiziden Wirkstoff in ein hydrophobes Polymer eingebettet zur anhaltenden Freisetzung.
Die Verbindung der Formel I kann in Form der freien Base oder als Säureadditionssalz erfindungsge-20 mäss vorliegen bzw. eingesetzt oder verwendet werden. Ein Säureadditionssalz kann aus der freien Base in üblicher Weise hergestellt werden. Umgekehrt erhält man die freie Base aus dem Säureadditionssalz ebenfalls in Üblicher Weise. Beispiele für geeignete Formen des Säureadditionssalzes sind das Hydrochlorid, das Lactat und das Ascorbat. Die freie Base und das Hydrochlorid sind bevorzugt.
Die Verbindung der Formel I ist beispielsweise auch aus der EP 24587 A1 bekannt. Sie gehört zur 25 Klasse der Allylamin-Antimykotika. Sie ist in der Literatur unter dem generischen Namen Terbinafin bekannt und im Handel unter dem Handelsnamen LAMISIL (von der Firma Sanabo G.m.b.H.) erhältlich. Zusätzlich. zu ihrer Wirksamkeit gegen Dermatophyten nach oraler wie auch topischer Anwendung haben wir zwar gefunden, dass sie bei oraler Anwendung auch hochwirksam bei der Behandlung der Onychomykosis ist, da sie eine starke fungizide Wirksamkeit und eine hohe Affinität zum Keratin der Nägel hat, wo sie 30 angereichert wird. Daher kann eine wesentlich höhere Heilungsrate erreicht werden, als mit üblichen Therapien wie beispielsweise der oralen Behandlung mit Griseofulvin.
Obwohl die Verbindung der Formel I ein sehr sicherer Wirkstoff ist, hat jedoch die systemische Behandlung von Onychomykosis einige Nachteile. So wird beispielsweise der gesamte Organismus dem Wirkstoff ausgesetzt und es werden relativ hohe Dosen benötigt. Deshalb ist die Möglichkeit zur lokalen, 35 namentlich topischen Behandlung sehr wünschenswert und würde von vielen Patienten bevorzugt werden. Andererseits sind schon viele Versuche mit verschiedenen Wirkstoffen, beispielsweise Griseofulvin, unternommen worden, um topische Formulierungen zur Behandlung von Onychomykosis herzustellen, jedoch konnten die erhaltenen Resultate nicht überzeugen. Das mag mit der ungenügenden Penetration dieses Wirkstoffes in die tieferen Lagen der Nägel Zusammenhängen. 4o Beispielsweise sind in der EP 0 389 778 A1 sowie in der EP 0 298 271 A1 antimykotische Nagellacke beschrieben. Terbinafin wird in beiden jedoch nicht erwähnt.
Es wurde jetzt gefunden, dass die'Verbindung der Formel I, wenn sie in einem speziellen Trägermaterial formuliert wird, überraschenderweise bei der topischen Anwendung auf infizierten Nägeln bei der Behandlung der Onychomykosis hochwirksam ist. 45 Solche Formulierungen sollten idealerweise die folgenden Eigenschaften haben: - weil die Penetration in infizierte Nägel ein eher langsamer Prozess ist, sollten sie nach der ' Anwendung im Nagelmaterial angereichert werden, von welchem aus der Wirkstoff sich dann leicht im Nagelzellgewebe verteilen kann; - sie sollten die Eigenschaft haben, den Wirkstoff leicht abzugeben; so - sie sollten bequem für den Patienten sein, beispielsweise leicht aufzubringen sein, in niederer Frequenz anzuwenden sein, leicht zu entfernen sein und gut vertragen werden.
Es wurde jetzt gefunden, dass solche Formulierungen mit den genannten vorteilhatten Eigenschaften und einer hohen Wirksamkeit erhalten werden können, indem man die Verbindung der Formel I in Form der freien Base oder eines Säureadditionssalzes als Nagellack formuliert, der einen oder mehrere spezielle 55 Filmbildner und weitere übliche geeignete Arzneihilfsmittel enthält.
Die Bestandteile der erfindungsgemässen Nagellacke sind: 1. Die Verbindung der Formel I in Form der freien Base oder eines Säureadditionssalzes. 2

Ein wasserunlöslicher polymerer Filmbildner ausgewäblt aus Polyvinylacetat, Acryl- und Methacrylsäu-reester-Copolymerisaten mit quaternären Ammoniumgruppen und Methylvinylether-Maleinsäuremonoal-kylester-Copolymerisaten. Acryl- und Methacrylsäurealkylester-Copolymerisate mit quaternären Ammoniumgruppen sind beispielsweise unter den Handelsbezeichnungen EUDRAGIT RL und EUDRAGIT RS Harze (Fa. Rohm), Methylvinylether-Maleinsäuremonoalkylester-Copolymerisate, sind beispielsweise unter der Handelsbezeichnung GANTREZ ES (Fa. ISP) erhältlich.
Die Verbindung der Formel I und der polymere Filmbildner sind in den erfindungsgemässen Mitteln vorzugsweise in einem Massenverhältnis zwischen 1: 0,5 bis 1: 25 vorhanden. Weiter bevorzugt ist ein Massenverhältnis zwischen 1:1 und 1:20, insbesondere aber ein Massenverhäitnis von 1:1 bis 1:10.
Der Anteil der Verbindung der Formel I bezogen auf die Gesamtmasse des Mittels beträgt 0,5% bis 30%, vorzugsweise von 1% bis 20%.
Das erfindungsgemässe Mittel enthält ausser dem Wirkstoff und dem polymeren Filmbildner weitere übliche Arzneimittelhilfsstoffe, beispielsweise ein Lösungsmittelsystem. Dieses kann entweder wässrig oder organisch sein oder es ist eine Mischung aus organischen Lösungsmitteln und Wasser. Solche geeigneten Lösungsmittel sind physiologisch annehmbar und mit dem Wirkstoff und den anderen Bestandteilen des Mittels vereinbar. Typische organische Lösungsmittel sind Ethanol, Isopropanol, Aceton und Ethylacetat. Das bevorzugte Lösungsmittelsystem ist Ethanol,· welches einen gewissen Anteil an Wasser enthält. Die Menge des Wassers ist in den meisten Fällen geringer als die Menge der organischen Lösungsmittel. Typische Wasser-organische Lösungsmittel-Verhältnisse liegen beispielsweise unterhalb von 1 : 3. Jedoch kann in einigen Fällen der Wasseranteil auch den des Lösungsmittels übersteigen. Das Mischungsverhältnis liegt dann beispielsweise bis zu 2,5 :1.
Die enfindungsgemässen Mittel enthalten üblicherweise weitere Bestandteile, die die Formulierung stabilisieren und ihre Eigenschaften verbessern. Im einzelnen sind solche Arzneimittelhilfsstoffe: - Weichmacher wie Dialkylphthalate, beispielsweise Dibutylphthalat, Hydroxy-Fettsäureöle, beispielsweise Rizinusöl, Triglyceride und Silikonöle: · - Filmmodifizierungsmittel, welche die Eigenschaften des hauptsächlichen Filmbildners verändern, insbesondere seine Anwendungseigenschaften verbessern, beispielsweise die Härte nach dem Verdampfen des Lösungsmittels oder die Flexibilität auf dem Nagel. Diese Modifizierungsmittelkönnenbeispielsweise Acrylesterharze, Arylsulfonamid-Formaldehydharze-Cellulosederivate oder Polyamidharze sein; - oberflächenaktive Mittel, beispielsweise Polyethylenglykol-Alkylether (beispielsweise erhältlich unter der Handelsbezeichnung BRIJ, Firma ICI), welche die Löslichkeit des Wirkstoffs bessern, insbesondere in wasserhaltigen Trägern; - Penetrationsverstärker, beispielsweise Azol, Dimethylsulfoxid, ungesättigte Fettalkohole, oberflächenaktive Mittel und Propylenglykol; - Farbstoffe; - Antioxidantien, beispielsweise Tocopherol; - komplexierende Reagenzien, beispielsweise Ethylendiamintetraessigsäure (Komplexon III); und - UV-Absorber.
Die erfindungsgemässen Nagellacke können durch ein Verfahren hergestellt werden, das darin besteht, dass die Verbindung der Formel I in Form der freien Base oder eines Säureadditionssalzes mit einem geeigneten polymeren Filmbildner, ausgewählt aus Polyvinylacetat oder Acryl- und Methacrylsäurealkyle-ster-Copolymerisaten mit quaternären Ammoniumgruppen oder Methylvinylether-Maleinsäuremonoalkyle-ster-Copolymerisaten, und weiteren Arzneihilfsstoffen wie beispielsweise einem Lösungsmittelsystem gemischt werden. Das erfindungsgemässe Verfahren kann in für die Herstellen von Nagellacken üblicher Weise durchgeführt werden.
Die erfindungsgemässen Mittel dienen zur Behandlung von Onychomykosis. Eine angezeigte tägliche Dosis zur Anwendung beträgt üblicherweise zwischen 0,05 bis 5,0 mg der Verbindung der Formel l pro Quadratcentimeter des behandelten Nagelmaterials. Bevorzugte Anwendungsmassen sind zwischen 0,1 und 3,0 mg pro Quadratcentimeter. Die Konzentration der Verbindung der Formel I im Gewebe am Anwendungsort ist vorzugsweise zum Beispiel zwischen 0,001 und 1,0 mg pro Gramm, wobei die Menge abhängig ist von der Art der Nagelpilzinfektion und der Art der behandelten Nägel. Die Behandlung kann in schweren Erkrankungsfällen einmal täglich stattfinden, oder sonst auch nur einmal wöchentlich. Vorzugsweise wird die Behandlung jeden zweiten oder dritten Tag wiederholt.
Sowohl das Nagelmaterial an den Fingern wie auch an den Zehen kann behandelt werden, wenn es mit Pilzen infiziert ist, welche Onychomykosis verursachen, beispielsweise mit Dermatophyten, Hefepilzen oder Schimmelpilzen. 3

Claims (7)

EUDRAGIT RL 100) Acrylharz, hart, haltbar (z.B. GANTREZ ES 425) Ethanol 94 % 55.0 Die Wirksamkeit der erfindungsgemässen Formulierung kann in vitro oder in vivo gezeigt werden. Eine geeignete In-vitro-Methode ist das Messen der Penetration durch herausgeschnittene Nägel, wobei gezeigt werden kann, dass auch in tieferen Lagen des Nageigewebes fungizid wirksame Konzentrationen des Wirkstoffs erreicht werden können. Eine weitere In-vitro-Methode ist das Messen der Freisetzung des Wirkstoffes aus einer getrockneten Lackschicht, wobei gezeigt werden kann, dass fungizid-wirksame Mengen des Wirkstoffes in genügender Menge aus den getrockneten Lackschichten freigesetzt werden, in vivo ist die überzeugendste Testmethode die Doppelblindstudie mit Onychomykosis. Patentansprüche 1. Nagellack zur Behandlung von Onychomykosis enhaltend als aktiven Wirkstoff die Verbindung der Formel Iin Form der freien Base oder eines Säureadditionssalzes, in einer Konzentration von 0,5% bis 30% bezogen auf die Gesamtmasse des Mittels in Gegenwart eines wasserunlöslichen filmbildenden Polymeren und weiteren geeigneten Arzneihilfsmitteln, dadurch gekennzeichnet, dass es als polymeren Fiimbildner Polyvinylacetat oder ein Acryl- und Methacrylsäurealkylester-Copolymerisat mit quaternären Ammoniumgruppen oder ein Methylvinylether-Maleinsäuremonoalkylester-Copolymerisat enthält. 5 AT 400 518 B
- 2. Nagellack gemäss Anspruch 1, dadurch gekennzeichnet, dass der polymere Filmbildner in einem Mischungsverhältnis der Verbindung der Formel I und des polymeren Filmbildners von 1:0,5 bis 1:25 (M/M) enthalten ist.
- 3. Nagellack gemäss Anspruch 2, dadurch gekennzeichnet, dass der polymere Filmbildner in einem Mischungsverhältnis der Verbindung der Formel I und des polymeren Filmbildners zwischen 1:1 und 1:20 (M/M) enthalten ist.
- 4. Nagellack gemäss Anspruch 3, dadurch gekennzeichnet, dass der polymere Filmbildner in einem Mischungsverhältnis der Verbindung der Formel I und des polymeren Filmbildners zwischen 1:1 und 1:10 (M/M) enthalten ist.
- 5. Nagellack gemäss einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass die Verbindung der Formel I in einer Konzentration von 1% bis 20%, bezogen auf die Gesamtmasse des Mittels, vorliegt.
- 6. Verfahren zur Herstellung eines Nagellacks gemäss Anspruch 1, wobei man die Verbindung der Formel I gemäss Anspruch 1 in Form der freien Base oder eines Säureadditionssalzes mit einem geeigneten polymeren Filmbildner und üblichen weiteren Arzneihilfsstoffen mischt, dadurch gekennzeichnet, dass man als polymeren Filmbildner Polyvinylacetat oder ein Acryl- und Methacrylsäurealkylester-Copolymerisat mit quaternären Ammoniumgruppen oder ein Methylvinylether-Maleinsäuremonoalkylester-Copolymerisat einsetzt.
- 7. Verwendung einer Verbindung der Formel I, wie in Anspruch 1 definiert, zur Herstellung eines Nagellacks zur Behandlung von Onychomykosis, welcher als polymeren Filmbildner Polyvinylacetat oder ein Acryl- und Methacrylsäurealkylester-Copolymerisat mit quaternären Ammoniumgruppen oder ein Methylvinylether-Maleinsäuremonoalkylester-Copolymerisat neben üblichen weiteren Arzneihilfsstoffen enthält. 6
