Description

   <Desc/Clms Page number 1>

 <Desc/Clms Page number 2>

 <Desc/Clms Page number 3>

 <Desc/Clms Page number 4>

   Ces buts sont atteints suivant l'invention par l'uti- lisation d'un procédé et d'un dispositif caractérisés par les points suivants, considérés séparément ou en combinaison con- venable quelconque ; - le sang est enfermé au fur et à mesure de son prélève- ment au malade, directement dans une   membrane   en forme de poche ou de sac, utilement flasque et après la dialyse, le sang est réinfusé directement de cette poche ou de ce sac au malade;

   - on obtient la membrane en forme de poche ou de sac, en pliant une feuille de matière poreuse convenable, par exemple une feuille de   cellophàne   en acétate de cellulose., de forme par exemple rectangualaire, de façon, qu'après le pliage ses bords entourent, à un niveau approximativement commun, l'ouverture de la poche du sac qu'elle   forme,   et qu'on serre ensuite ce bord d'une façon étanche et - utilement amovible et dépliable autour d'un bouchon- en matière convenable, muni de trous d'entrée et de sortie;

 <Desc/Clms Page number 5>

 réinfusion, entre deux éléments supports en forme de grilles, rapprochables l'un de   l'autre;   -   1-*étalement   de la poche ou du sac rempli de sang se fait suivant un plan droit'; - l'étalement de la poche ou du sac rempli de sang se fait de manière à y former des bourrelets s'étendant de bas en haut de façon à   favoriser,     d'une   part, la répartition uni- forme du O2 qu'on fait barboter dans le sang, et, d'autre part, la circulation du liquide dialyseur au contact des surfaces ex- térieures de la poche ou du sac, des deux côtés extérieurs de ce dernier;

 <Desc/Clms Page number 6>

Figures 1, 2 et 3, une vue d'ensemble et deux,vues de détail de la poche ou du sac dialyseur;
Figures 4., 5, 6, 7 et 8, une vue d'ensemble et des vues de détail d'une grille support pour quatre poches ou sacs dialy- seurs suivant la figure 1 ;
Figures 9 et 10, une poche ou sac dialyseur serré dans la grille support, respectivement en positions dites de remplis- sage et de vidange;
Figure 11, une vue schématique explicative d'une trousse dite de transfusion;
Figure 12, une vue en coupe d'une cuve de dialyse con- tenant une grille de support de poches ou sacs dialyseurs;

 <Desc/Clms Page number 7>

 <Desc/Clms Page number 8>

 <Desc/Clms Page number 9>

 <Desc/Clms Page number 10>

     L'appareil dialyseur décrit ci-dessus est utilisé comme suit :

 <Desc/Clms Page number 11>

 <Desc/Clms Page number 12>

   <Desc / Clms Page number 1>

   It is known to rid the blood of its urea in renal disorders in which, following severe damage, the kidney momentarily loses its excretory properties. These affections are: - acute toxic nephritis caused by metallic salts (Hg, As); - renal blockages by incompatible blood transfusions; - renal blockages resulting from significant or prolonged operating shocks.

   In these three conditions, recovery of renal function does not have time to occur, because the subject in the meantime dies from his uremic intoxication.

   For a long time, attempts have been made to compensate for the temporary functional insufficiency of the kidney by means of apparatus which make it possible to eliminate the urea contained in the organism daily. me and worked out by him.

 <Desc / Clms Page number 2>

   These devices are based on the principle of the semi-permeability of certain membranes consisting, for example, of cellulose acetate in the form of cellophane, or of another suitable material. In these devices, the blood is separated by the membrane from the liquid dialysing medium outside the membrane, the latter allowing urea, metal ions, glucose and toxic substances to pass through.

  The dialysing medium consists of saline mixtures of varying composition, for example by 15 liters of distilled water, 90 g of sodium chloride, 30 g of sodium bicarbonate, 150 g of glucose, 6 g of potassium chloride.

   There are four types of dialyzers of the kind considered, namely:
1.- The Kolff and Merril type apparatus which consists of a coil of cellophane wound in a spiral on a rotating drum, immersed in a dialysing solution. The urea-laden arterial blood enters the coil at one end and leaves it purified by the other end, to be then sent via a pump into the patient's vein.

   2.- The Alwall type apparatus consists of a coil wound between two concentric and vertical meshed metal cylinders, the whole being immersed in a tank containing the dialysing liquid. This hermetically sealed tank is depressurized in relation to the pressure prevailing in the coil, - The coil cannot expand, it is carried out in addition to the diffusion through the membrane, a certain filtration due to this difference pressure. This system, like Kolff's, works continuously, with blood flowing from the artery to the patient's vein.

  3.- The Skeggs and Leonard type apparatus also works according to the continuous method, from the artery to the vein, but the coil is replaced there by membranes tightened between

 <Desc / Clms Page number 3>

 rigid frames superimposed and alternately containing blood and dialysing fluid. Blood and dialysis fluid are both circulating.
 EMI3.1

  4 ..- 1atJ.PJ.1:! 3il¯: t.YJ1ê = J2a works according to the discontinuous method. It consists of two cellophane membranes stretched over a circular plastic frame, forming between them a closed cavity that can contain blood. This receptacle filled with blood is immersed in a dialysis tank, the liquid of which is kept in motion. by a suction-pressing pump.

  The filling and emptying of the filter receptacle is done through a Baxter (vacuum flask) in which the blood taken from the patient is located. After purification, the blood is transfused from the filter receptacle into another Baxter from which the blood is transferred back to the patient.

   The disadvantages of these four types of devices and the work methods they involve can be summarized as follows:
The first three - require complicated and very expensive equipment - require extremely qualified personnel, which restricts the extension of these devices and their methods of application and reserves them to particularly qualified university centers - incurs the patient serious circulatory risks in the event of obstruction or rupture of the coil or the membrane.

   For the apparatus and the method of Bartrina, one will reproach its little output and the difficult handling of the blood which must first be taken in a jar before being put into the membrane receptacle, from where it must. be extracted again after filtration, to be reinjected into the patient's vein using the same jar.

   The aim of the invention is to purify the blood in a discontinuous manner but by ensuring that: - the blood leaves the patient's vein only to go into the filtering device and, only leaves the filtering device to return directly to the patient; - the filtering device has the largest possible surface per unit of blood volume; - the filtering device does not give rise to any manipulation when the filtration is started; - the filtering device can contain the largest possible quantity of blood that can be withdrawn from the patient without danger, ie between one and two liters; - the device can be easily sterilized and therefore be constantly ready for use, without last minute operations.

   These objects are achieved according to the invention by the use of a method and of a device characterized by the following points, considered separately or in any suitable combination; - the blood is enclosed as it is withdrawn from the patient, directly in a membrane in the form of a pocket or bag, which is usefully flaccid and after dialysis, the blood is reinfused directly from this pocket or this bag to the sick;

   - The membrane is obtained in the form of a pocket or a bag, by folding a sheet of suitable porous material, for example a sheet of cellulose made of cellulose acetate., for example rectangular in shape, so that after folding its edges surround, at an approximately common level, the opening of the pocket of the bag that it forms, and that this edge is then tightened in a sealed manner and - usefully removable and unfoldable around a stopper - of suitable material, provided with entry and exit holes;

   - the membrane in the form of a pocket or bag is spread during its use, and in particular during dialysis and

 <Desc / Clms Page number 5>

 reinfusion, between two support elements in the form of grids, which can be brought together; - 1- * spreading out of the bag or bag filled with blood is done in a straight plane '; - The spread of the bag or bag filled with blood is done so as to form therein beads extending from bottom to top so as to promote, on the one hand, the uniform distribution of the O2 that is made bubbling in the blood, and, on the other hand, the circulation of the dialyzer liquid in contact with the exterior surfaces of the pouch or bag, on the two exterior sides of the latter;

   - the blood taken from the patient is enclosed as and when it is taken, directly and successively in several membranes in the form of pockets or bags, usefully flaccid, these bags or bags filled with blood then being subjected, usefully to the spread state, on dialysis, either successively., 'after their filling, or all at once, after which the dialyzed contents of each bag or bag, usefully maintained in the spread state is reinfused directly from the bag or bag to the sick; - each pocket or bag is only used for the same one. sick for example two or three times at most and after this use the bag or the bag is destroyed.

   the device used comprises at least one pair of grids which can be brought together and which can be separated from one another so as to be able to support and clamp together at least one dialyzer pocket or bag filled with blood, this or these pair of grids being susceptible to be removably suspended and loaded with one or more bags or bags filled with blood, in a tank, as well as a tank equipped with a support device for the pair or pairs of screens and a device for circulating in the tank a dialyzer liquid in contact with the bags or without dialyzers supported by the grids.

   The dialysis bags or bags 1 forming the object of the present invention, which may have, for example, the shape shown in FIG. 1, and which are intended to each serve only for a single patient, can be easily obtained. from a sheet of cellulose acetate 2, of any suitable dimensions (for example 1 x 1 m.), which is folded on itself in three and in the middle of which is placed, in the inside the fold, a template 4, for example of cardboard, (for example 15 x 30 cm.), to then fold back on themselves and towards the template 4, the side parts 5-5, following the successive bis-sectrices 6a, 6b, 6c, etc ..., until the side parts thus folded up are in a vertical position, or approximately vertical,

   and determine a cavity

 <Desc / Clms Page number 7>

 rectangular seamless, open at the top. Then the template 4 is removed from the cavity thus formed and it is replaced by a plug 7, usefully of plastic material, for example polyvinyl chloride, of suitable size (for example 1.5 x 3.5 cm). , which one maintains at the. upper part of pocket or bag 1.

   The lateral, anterior and posterior folds of the sheet 2 are then tightened by their part situated near the upper edge of the opening of the pocket or of the bag 1, around the stopper 7 using a thicker adhesive strip, for example of cellulose acetate, and the tightening is completed by means of clamps 8, 9 made for example of stainless metal.

   The stopper 7 is provided with two orifices 10, 11, 10 of which contains a fine bubbler pipe 12, for example 1/5 mm. in polythene, through which oxygen is brought into the blood during dialysis or filtration, the excess oxygen escaping through orifice 11 which is also intended for the placement of tire 13 (fig. 10), serving to. the supply of blood to the. pocket 1.

   The pockets or bags 1 formed as described above, being flaccid in themselves, and therefore having no rigidity other than that conferred on them by the liquid they contain and, therefore, no spatial form clean, they are given, according to the invention, advantageously, a form of juxtaposed bead, extending from bottom to top, by inserting them between two grids 14, 15 (figs. 4, 5, 6) with bars vertical 16 and close to one another.

   To increase the efficiency of the device, it is useful to combine several, for example four of these pairs of grids
 EMI7.1
 14, 15 between them (fig4), 14a - 15a, 14b -15R, 14c - 15c, 14d-] i5d each of these grid pairs being intended to receive for example a pocket 1.

   This arrangement of the grids means that the set of

 <Desc / Clms Page number 8>

 the latter, as well as the filled pockets 1 which they carry, form a chimney which is usefully made capable of being placed in an easily removable manner in a circulation tank, as will be described below.

   Each fixed grid 14 and each mobile grid 15 is advantageously constituted by a rigid frame 16, respectively 17, 16 of which may be of profile of angular cross section, while 17 is usefully of round cross section, these profiles possibly being of steel. drawn chrome. These frames 16 and 17 are lined with vertical bars of usefully round section 18, respectively 19, having the same spacing between them, the two grids 14, 15 of each pair being usefully articulated to each other by the branch. lower part of their frames, by means of hinges (fig. 7) each formed by a split sleeve 20 embracing the lower branch of the frame 17 of the movable grid 15 and fixed by means of its legs 21, 22 and nut screws 23, to the lower branch of the frame 16 of the fixed complementary grill 14.

   The upper branch of the frame 16 of the fixed grids 14 is usefully fitted with a resilient fixed retaining tab 24 (Fig. 8) capable of engaging on and retaining the upper branch of the frame 17 of the complementary mobile grids 15, when that -ci is in the closed position, position in which the bars 18 and 19 of each pair of grids 14, 15, clamp and retain between them a pocket or bag 1 as shown in Figure 9.

   It is clear that the neighboring vertical branches 16 of the fixed grids 14 forming a frame, can be juxtaposed and fixed to one another, or simply be common to two consecutive grids 14 (FIG. 4).

   The frame formed by the fixed grids 14 is usefully provided at its upper edge with extensions 25 by means of which

 <Desc / Clms Page number 9>

 the whole system of screens 14, 15 and the pockets 1 on it can be suspended in a removable manner in the circulation tank shown in fig. 12 and taken out of it to be suspended in an inverted position from a hook 51, as shown in FIG. 10, when withdrawing the dialyzed blood from the patient.

   The bags 1, once placed and clamped between the grills 14, 15, and filled with blood, the whole is introduced into a tank 26, for example cylindrical (fig. 12) which can be constituted by chromed brass and which is provided at its bottom with a propeller 27 whose vertical axis 27 'crosses the bottom in a sealed manner, and with a drain valve 28. The inner side faces of the tank may be fitted with fins directors 29 extending for example parallel to the bars 18, 19 of the grids 14, 15.

   The upper part of the pan may be provided with notched supports 30 intended to receive the arms 25 of the grills 14 forming the frame.

   It should be noted that the guide fins 29 and the chimney formed by the set of grids 14, 15 and pockets 1, supported by the grids, force the dialysis liquid contained in the. tank 26 and set in motion by the propeller 27, to circulate in ascending and descending closed circuits, in contact with the interior and exterior faces of the pockets 1 totally submerged,
During the entire duration of the dialysis, the bags 1 communicate with the outside via the oxygen supply pipes 12 and the oxygen outlet pipes (not shown).

   The tank 26 can be closed by a removable cover 31, (fig.13) 'with a rim 32 clamping the tank. This cover, which can be formed by thick stainless steel sheet is provided with two handles 33,; a radial slot 34, starting from the center of the

 <Desc / Clms Page number 10>

 cover and opening on the periphery of the latter, as well as an opening 35 located at the periphery of the cover usefully at a point opposite to the open end of the slot 34. The slot 34 is intended to provide passage for the tubes 12 and 13 of the pocket 1 and to receive, at its central end, an immersion heater 36 ensuring the desired temperature of the dialyzer liquid and electrically connected at 37, to a thermostat 38.

   fixed in the opening 35 and in turn immersed in the dialysing liquid.

   The tank 26 forms, with its accessories, an assembly independent of the motor unit described below, and can therefore be easily sterilized with its contents, grids and bags, immersion heater, thermostat, by formoleic steam, or, on condition of '' remove the thermostat, by introducing it into an autoclave.

   The motor unit (fig. 14) comprises a tripod 39 furnished with a plate 40 with a flange 41, for example hexagonal, surrounding a layer of damping of trepidation and noise, for example of nylon foam 42, in which the tank 26 fits in a removable manner.

   From the plate 40 hangs an electric motor 43 of approximately 1/3 hp. and preferably at a constant speed of about 1,570 rev / min., the axis of the motor being able to be coupled to that of the propeller 27 in an easily detachable manner, for example by a canvas rubber hose 44 which ensures a free connection of vibrations.

   This assembly is completed by a switch 45 and a wire 46 putting the tank 26 and the motor 43 to the ground.

   The feet 39 are separated from the ground by an insulator 47, for example made of nylon foam.

     The dialyzer apparatus described above is used as follows:

 <Desc / Clms Page number 11>

After filling the tank 26 with a dialysing mixture (consisting for example of 15 liters of distilled water, 90 g of sodium chloride, 30 g of sodium bicarbonate, 150 g of glucose, 6 g of potassium chloride) and having brought this mixture, using the immersion heater 36 and the thermostat 38 at a temperature of 37.5 ° C., a temperature which is kept constant throughout the duration of the filtration of the dialysis, the bags 1 are filled with blood. , previously placed between the grids 14, 15 (see FIG. 9), this filling being carried out using a polythene pipe 13 connected to the patient and a pump (not shown) similar to.

   those used in transfusion centers (the patient's blood has been rendered poorly coagulable, for example by injection of heparin).

   The filled bags are disconnected from the patient and the system of screens 14, 15 and of bags 1 is introduced into the tank 26, the blood inlet openings 11 being connected to the outside air by four suitable pipes ( not shown). The four oxygen inlets 12 are connected to an oxygen distribution pipe, connected by a pressure reducer and a bubbler to a cylinder (not shown). The oxygen flow is adjusted so as to avoid the formation of foam in the pockets.

   The cover 31 is replaced, the various pipes connected to the pockets 1 passing through the slot 34. The motor 43 is started. Propeller
27 determines in the tank 26 an intense circulation of the liquid 'dialysing, in the direction of the arrows (fig. 12) in contact with the two faces of each bag 1.

   , The dialysis being completed, the motor 43 is stopped, the cover 31 is removed and the system of the screens 14, 15, 18, 19 and the pockets 1 is removed from the tank 26.

   With the screens and the pockets removed, the pipe 48, respectively 48 'of a Baxter kit 49 with double inlet 50, 50', is introduced into the opening 11 of the stopper of each pocket 1 (fig.

 <Desc / Clms Page number 12>

  II), which requires two kits for the complete grid system. The pipes 48, 48 'of the kit 49 are closed with the aid of pliers provided for this purpose (not shown), the system of screens 14, 15, 18, 19 and of pockets 1 is turned upside down, and it is turned over. suspended by the metal frame 16 from a hook 51 (Figure 10).

   The clamp of one of the pipes 48, 48 'is then opened in communication with the bags 1, it is purged of its air and it is placed in communication with the patient by means of the kit 49 and a drain 52 previously placed in a vein of the arm.

   The atmospheric pressure acting on the bags 1, drives the blood into the pipes 48, 48 ', 52 and each bag is thus successively emptied.

   During the time that the retransfusion lasts, a new blood sample and a new dialysis can be carried out.

   The results obtained so far by the inventor show that starting from blood enriched in urea at 4.20 g. per: liter, after one hour of dialysis, only 45 centigrams per liter are found.

   In another test, starting with blood enriched in urea at 3.9 g., Only 45 centigrams per liter are also found after one hour of dialysis.

   The corresponding amounts of urea are found in the dialysing liquid.

   These results can therefore be regarded as satisfactory compared to the performance of known purification apparatus and the human blood volume is considered to be about 1/13 of the total weight of the subject.

   It is obvious that modifications can be made to the method and to the device described above without departing from the scope of the invention.
