  Tragbares Trockenspirometer zur Registrierung des einzelnen Ausatmungsstosses und der maximalen   Venblationsgrösse    pro Minute
Bei der medizinischen Untersuchung der mechanischen Leistungsfähigkeit der menschlichen Lunge spielen zwei Messungen eine hervorragende diagnostische Rolle. Die eine Messung erfordert eine maximale Einatmung, wonach der Patient so rasch als möglich das gesamte ausatembare Luftquantum durch einen speziellen Apparat ausstösst, der das ausgeatmete Luftquantum als Ordinate in Abhängigkeit von der Zeit als Abszisse aufschreibt. Der vertikale Gesamtausschlag entspricht der sogenannten Vitalkapazität. Das pro Sekunde ausgeatmete Luftquantum (Test von Tiffeneau) wird in Prozenten der Vitalkapazität ausgedrückt. Bei gewissen Lungenerkrankungen, wie z. B.

   Emphysem, Asthma, Bronchitis und anderen, ist dieser Prozentsatz gegenüber gesunden Lungen um so niedriger, je schwerer der Krankheitsgrad der Lunge ist.



   Die andere Messung besteht darin, dass der Patient während einer gewissen Zeit, z. B. 15 Sekunden, so stark als möglich atmet, eventuell nach vom Arzt bestimmter Frequenz, wobei das gesamte geatmete Luftquantum registriert wird und mit 4 multipliziert die maximale Atmungskapazität pro Minute ergibt. Diese maximale Atmungskapazität wird in Beziehung gesetzt zu Körperdimensionen und Alter und gibt so ebenfalls eine wertvolle Indikation für die mechanische Leistungsfähigkeit des Atmungsapparates.



   Die beiden Messungen werden gebräuchlicherweise mit Spirometern ausgeführt, bei denen man 2 Ausführungen unterscheidet. Das Wasserspirometer, bei dem eine Spirometerglocke unter Wasserabschluss steht. Infolge der Schaukelbewegung des Wassers ergeben sich falsche Messwerte. Das Trockenspirometer, welches einen Gummibalg benötigt, der sowohl für horizontale wie für vertikale Bewegungsrichtung präzise Führung des Balges und der daran befestigten Schreibvorrichtung verlangt. Daraus entstehen Reibungswiderstände, die das registrierte Resultat fälschen. Die Zwischenschaltung einer Zahnrad- oder   Zahnstangenübertragung    vom Balg auf den Schreiber fälscht ebenfalls wegen des unvermeidlichen Spieles bei der   Zahnradiibertragung.   



   Ein weiterer Fehler jedes Gummibalges ist dessen Elastizität. Entleert herrscht in seinem Inneren ein Unterdruck und gefüllt ein Überdruck, was sich auf die rasch bis zum Extrem erfolgende Lungenbewegung hemmend auswirkt.



   Die Erfindung vermeidet die Nachteile der bekannten Spirometer. Sie betrifft ein tragbares Trokkenspirometer mit einem elastischen Faltenbalg. Das wesentliche   Merkmal    der Erfindung besteht darin, dass ein beweglicher Flansch des Faltenbalges um eine feste Achse kippbar ist, wobei die Kippbewegung ohne Zwischenschaltung eines   tZbertragungsmecha-    nismus die Grösse des geatmeten Luftquantums direkt auf einen mit Teilung versehenen Aufzeichnungsträger aufschreibt.



   Bei einer Ausführungsform des Trockenspirometers können die Änderungen der elastischen Spannung des ringförmig gebogenen Faltenbalges infolge des veränderten Füllungsgrades und die änderungen der wirksamen Gewichtskomponente durch Federzug kompensiert werden, so dass sich das Trockenspirometer bei jedem Füllungsgrad im Gleichgewicht befindet.



   Zweckmässig wird mit einem einzigen Schalthebel die gewünschte Registriergeschwindigkeit eingestellt und die Einschaltung des Antriebsmotors sowie der Verschluss des Spirometers vorgenommen.



   Der Faltenbalg kann beispielsweise durch einen schwenkbaren Hebel unterstützt und seine Form  durch einen die Falten auseinanderhaltenden Ring aufrechterhalten   werdien.    Der Aufzeichnungsträger lässt sich vorzugsweise auf einer zylindrischen Auflagefläche eines mit kontinuierlicher Geschwindigkeit angetriebenen Wagens anordnen.



   In der Zeichnung ist ein Ausführungsbeispiel des erfindungsgemässen Spirometers schematisch dargestellt.



   Der Flansch 1 des Spirometers ist fest montiert.



  Er trägt das Rohr 2 zur Befestigung des zum   Mund    stück des Patienten führenden Atmungsschlauches.



  Für die Einatmung ist das elektromagnetische Ventil 3 geöffnet. Es wird für die Registrierung gleichzeitig mit dem Einschalten des Motors geschlossen.



   Der Gummibalg 4 ist auf der linken Seite durch den beweglichen Flansch 5 verschlossen, der sich mit dem Hebel 6 um die Achse 7 drehen kann.



  An dem Hebel 6 ist ein Schreibhebel 8 mit der Schreibspitze 9, z. B. einem Kugelschreiber, fest angeordnet.



   Damit der Gummibalg 4 im aufgeblasenen Zustand nicht infolge seines Gewichtes nach unten durchhängt, wird er in seiner Mitte   durch    einen die Gummirippe umgebenden Ring 10 und einen Hebel 11 gestützt, wobei letzterer sich um die Achse 7 drehen kann. Da der Flansch 5 bei Füllung des Balges 4 sich nach links unten bewegt, so wirkt sein zunehmendes Gewicht der zunehmenden elastischen Spannung des Gummibalges entgegen. Zwei Stahlfedern 12, von denen nur eine eingezeichnet ist, sind so befestigt, dass sie den Flansch 5 stark nach rechts ziehen, damit dieser in jeder Stellung ausbalanciert ist.



   Da die Schreibspitze 9 einen Kreisbogen beschreibt, so ist der Aufzeichnungsträger, z. B. Registrierpapier, auf einem Wagen 13 aufgespannt, der eine zylindrische Auflagefläche besitzt. Der Wagen 13 bewegt sich senkrecht zur Bildebene. Er wird angetrieben durch das Ritzel 14 des Elektromotors 15. Mit einem in der Zeichnung nicht dargestellten Schalthebel kann der Motor 15 nach rechts gekippt werden, so dass das Ritzel 14 in das Zahnrad 16 eingreift. Mit 16 sind auf derselben Achse fest gekoppelt das Zahnrad 17 und die Schnurrolle 18, die mittels eines dünnen Kabels die lineare Verschiebung des Wagens 13 bewirkt. Wird der Motor 15 nach links gekippt, so kommt das Ritzel 14 mit dem Zahnrad 19 in Eingriff, welches mit dem Zahnrad 20 auf der gleichen Achse verbunden ist. Vom Zahnrad 20 wird die Rotation über ein Zwischenrad 21 auf die Räder 17 und 18 übertragen.

   Dieser Übertragungsmechanismus ergibt die gewünschte kleine Vorschubgeschwindigkeit des Wagens 13.



   Die Bedienung erfolgt durch einen einzigen Drehschalter. Bei dessen Betätigung nach der einen Seite wird der Strom für den Motor 15 eingeschaltet, dessen Ritzel 14 zum Eingriff in das Zahnrad 16 bzw.



  19 gebracht und das elektromagnetische Ventil 3 geschlossen.



   In der Nullstellung des Drehschalters ist das Ritzel 14 nicht im Eingriff und der Wagen 13 kann von   Hand    in die Ausgangsstellung zurückgeführt werden.

   The two measurements are usually carried out with spirometers, which are divided into two versions. The water spirometer, in which a spirometer bell is under the exclusion of water. The dry spirometer, which requires a rubber bellows, which requires precise guidance of the bellows and the writing device attached to it for both horizontal and vertical directions of movement. This creates frictional resistance that falsifies the registered result. The interposition of a gear or rack transmission from the bellows to the recorder is also falsified because of the unavoidable play in the gear transmission.



   Another flaw of every rubber bellows is its elasticity. When it is emptied, there is a negative pressure inside it and when it is filled there is an overpressure, which has an inhibiting effect on the lung movement, which takes place quickly to the extreme.



   The invention avoids the disadvantages of the known spirometers. It relates to a portable dry spirometer with an elastic bellows. The essential feature of the invention is that a movable flange of the bellows can be tilted around a fixed axis, the tilting movement writing the size of the breathed air quantity directly onto a graduated recording medium without the interposition of a transmission mechanism.



   In one embodiment of the dry spirometer, the changes in the elastic tension of the annularly curved bellows as a result of the changed degree of filling and the changes in the effective weight component can be compensated for by means of a spring balancer, so that the dry spirometer is in equilibrium with every degree of filling.



   Appropriately, the desired recording speed is set with a single lever and the drive motor is switched on and the spirometer is closed.



   The bellows can for example be supported by a pivotable lever and its shape can be maintained by a ring that keeps the folds apart. The recording medium can preferably be arranged on a cylindrical support surface of a carriage driven at continuous speed.



   An exemplary embodiment of the spirometer according to the invention is shown schematically in the drawing.
