Description
Die vorliegende Erfindung betrifft Mittel zur Herstellung von "Technetium-Radiodiagnostika zur Scintigraphie von Knochen sowie kalkeinlagernden Tumoren, die bestimmte Carboxyphosphonsäuren oder deren wasserlösliche Salze enthalten.
Seit einiger Zeit ist bekannt, dass Röntgenuntersuchungen zur Erkennung von Skeletterkrankungen und Tumoren, insbesondere im Frühstadium, wenn noch eine wirksame Behandlung möglich ist, nicht völlig zufriedenstellend sind. Daher sind neuere Methoden entwickelt worden, in denen die radioaktiven Isotope Fluor-18 sowie Strontium-85 verwendet werden, die selektiv im Skelett und insbesondere an krankhaften Stellen im Skelett adsorbiert werden. Auch in kalkhaltigen Tumoren findet eine Anreicherung dieser radioaktiven Isotope statt. Durch Radiographie lassen sich dann diese Knochenbzw. Gewebeerkrankungen erkennen und lokalisieren. Jedoch erfordert die Herstellung von 18F komplizierte Vorrichtungen, die in Krankenhäusern nicht zur Verfügung stehen, und 18F hat zudem eine äusserst kurze Halbwertzeit von nur 110 Minuten. Das Isotop 8SSr dagegen hat eine sehr lange Halbwertzeit von 65 Tagen und erfordert aufgrund der geringen Zerfallshäufigkeit sehr lange Abtastzeiten.
Aufgrund dieser Nachteile hat sich in letzter Zeit das Interesse auf das Isotop Technetium 99m gerichtet, das eine Halbwertzeit von 6 Stunden aufweist. Zu seiner Herstellung stehen gut handbare Vorrichtungen zur Verfügung, aus denen durch Eluierung mit isotonischer Kochsalzlösung das radioaktive Isotop in Form von 99mPertechnetat erhalten werden kann.
Pertechnetat 99m unterscheidet sich von 18F~ sowie auch vom 85Sr2+ darin, dass es im Körper nicht spezifisch im Skelett oder an kalkhaltigen Tumoren gebunden wird. Zu seiner Anwendung muss es daher zu einer niedrigen Oxidationsstufe reduziert werden und dann mit einem geeigneten Komplexbildner in dieser Oxidationsstufe stabilisiert werden. Der Komplexbildner muss weiterhin eine hohe Selektivität zur bevorzugten Adsorption am Skelett bzw. an kalkhaltigen Tumoren aufweisen. Erste Erfolge sind mit gewissen Polyphos-phaten erzielt worden, deren Komplexe mit niederwertigem Technetium jedoch nur mässige Stabilität haben. Weiter wurde ein brauchbares Mittel durch Mischen von "mPertechnetat-Lö-sung mit einer wässrigen Lösung von Dizinn(II)äthan-l-hy-droxy-l,l-diphosphonat hergestellt und beschrieben (J. Nucl. Med. 14, 73; J. Nucl. Med. 13, 947). Die Stabilität dieser Dizinn(II)äthan-l-hydroxy-l,l-diphosphonat-Lösung war jedoch auch mit einem Überschuss des Äthan-l-hydroxy-1,1-diphosphonats begrenzt, insbesondere infolge einer Tendenz zur Hydrolyse des Zinn(II)-Ions.
Es wurde nun gefunden, dass sich bestimmte Carboxyphosphonsäuren und deren Salze infolge der hohen Stabilität ihrer Komplexe mit niederwertigen Technetium-Ionen und der hohen Selektivität der Ablagerung von 99mTc im Knochengewebe sowie in kalkhaltigen Tumoren eignen.
Die Aufgabe, ein lösliches, stabiles Produkt herzustellen, das nach Zugabe zu einer Pertechnetat-Lösung ein wirksames und selektives Mittel zur Radiographie von Knochen und kalkhaltigen Tumoren darstellt, wird durch Präparate gelöst, die ein Gemisch sind aus a) einer oder mehrerer Carboxyphosphonsäuren der allgemeinen Formel r -

bedeuten oder der pharmazeutisch brauchbaren wasserlöslichen Salze der genannten Verbindungen sowie b) einem pharmazeutisch brauchbaren Zinn(ll)-, Eisen(II)-oder Chrom(II)-Salz in stöchiometrisch untergeordneten Mengen, bezogen auf die Komponente a).
Mit Hilfe dieser Mittel wird die einfache Herstellung hochstabiler Produkte ermöglicht, welche zum Verkauf in fester Form als Tablette, als Lösung oder als Lyophilisat, enthalten in einer Ampulle, geeignet sind. Die Tablette oder der Inhalt einer Ampulle bilden nach Zugabe zu einer Pertechnetat-Lö-sung ein sehr wirksames Mittel zur Diagnostik von Knochentumoren, lokalen Störungen des Knochenstoffwechsels sowie kalkeinlagernden Gewebetumoren.
Zur pharmazeutischen Anwendung kommen anstelle oder in Kombination mit den oben genannten Carboxyphosphonsäuren auch ihre pharmakologisch unbedenklichen Salze wie Natrium-, Kalium-, Magnesium-, Zink-, Ammonium- und substituierten Ammoniumsalze wie Mono-, Di- oder Triätha-nolammoniumsalze in Frage. Sowohl die partiellen Salze, in denen nur ein Teil der aciden Protonen durch andere Kationen ersetzt ist, als auch Vollsalze können benutzt werden, jedoch sind partielle Salze, die in wässriger Lösung annähernd neutral reagieren (pH 5-9), bevorzugt. Mischungen der vorgenannten Salze können ebenfalls angewandt werden.
Geeignete Phosphonoalkanpolycarbonsäuren sind in der nachstehenden Tabelle angeführt. Anstelle der dort genannten Säuren finden auch die wasserlöslichen Salze physiologisch unbedenklicher Kationen Verwendung.

Besonders gute Ergebnisse bezüglich der Stabilisierung von ®9mTC-l0nen im Komplex, der Vermeidung der Bildung von kolloidalen Partikeln, bezüglich der Selektivität der Ablagerung von 99mTc im Skelett oder in verkalkten Tumoren ergeben sich mit den Natriumpartialsalzen folgender bevorzugter Carboxyphosphonsäuren :
1. Phosphono-äthan-l,2-dicarbonsäure, Mischung Dinatrium-salz : Trinatriumsalz 2:1
2. l-Phosphono-propan-l,2-dicarbonsäure, Mischung Dina-triumsalz : Trinatriumsalz 2:1
3. 2-Phosphono-butan-l,2,4-tricarbonsäure, Trinatriumsalz. Diese bevorzugten Carboxyphosphonsäure-Partialsalze ergeben eine hervorragende Aufnahme durch das Skelett und eine sehr geringe Aufnahme in den weichen Geweben ausser in verkalkten Tumoren. Sie eignen sich hervorragend zur Identifizierung von Knochenmetastasen von Mamma- oder Prostata-Karzinomen.
Die Herstellung der verwendeten Phosphonoalkanpolycarbonsäuren erfolgt nach bekannten Methoden.
l-Phosphonoäthan-l,2-dicarbonsäure lässt sich durch Umsetzung von Maleinsäureester mit Diäthylphosphit in Gegenwart von Natriumalkoholat und anschliessende saure Verseifung des Esters herstellen. 2-Phosphonopropan-2,3-di-carbonsäure kann in ähnlicher Weise erhalten werden, wobei jedoch vor der Verseifung noch eine Umsetzung mit Methylchlorid erfolgt.
l-Phosphonopropan-l,2,3-tricarbonsäure kann hergestellt werden durch Umsetzung von Maleinsäureester mit Phospho-noessigsäureester in Gegenwart von Alkoholat und anschlies-

sende Verseifung des erhaltenen Esters. Die Herstellung von l-Phosphonobutan-2,3,4-tricarbonsäure kann erfolgen durch Umsetzung von Dimethylphosphit mit l-Buten-2,3,4-tricar-bonsäureester in Gegenwart von Natriumalkoholat und anschliessender Verseifung des angefallenen Esters zu der gewünschten Säure.
Durch Umsetzung von Methandiphosphonsäurealkylester mit Maleinsäurealkylester in Gegenwart von Natriumalkoholat wird ein Ester erhalten, der durch saure Hydrolyse in 1,1-Di-phosphonopropan-2,3-dicarbonsäure überführt wird.
2-Phosphonobutan-2,3,4-tricarbonsäure kann erhalten werden durch Umsetzung von a-Diäthylphosphonopropion-säure-methylester mit Maleinsäurediäthylester in Gegenwart von Alkoholat und anschliessender Verseifung des erhaltenen Esters.
Die Herstellung von 2,2-Diphosphonobutan-3,4-dicarbon-säure erfolgt durch Umsetzung von Maleinsäureester mit Äthan-l,l-diphosphonsäureester in Gegenwart von Natriumalkoholat und anschliessender saurer Verseifung des erhaltenen Produktes.
Die weiteren Phosphonoalkanpolycarbonsäuren erhält man nach analoger Methode, wobei insbesondere anstelle von Maleinsäureester Citraconsäureester verwendet wird.
Die Herstellung der entsprechenden wasserlöslichen Salze kann durch vollständige oder teilweise Neutralisation der Säuren mit anorganischen Basen wie NaOH, KOH, NH4OH oder Alkanolaminen sowie auch mit Alkalicarbonaten erfolgen.
Als reduzierendes Agens werden Zinn(II)-, Eisen(II)- und Chrom(II)-Salze mit pharmazeutisch akzeptablen Anionen zugesetzt. Infolge ihrer allgemein anerkannten Sicherheit sind die Chloride und Sulfate bevorzugt, und wegen des hohen Reduktionspotentials und auch wegen der Abwesenheit von Kristallwasser wird besonders wasserfreies Zinn(II)-Chlorid bevorzugt.
Dieser Zusatz dient dazu, das aus einem kommerziellen "mPertechnetat-Generator eluierte 99mPertechnetat zu reduzieren. Das resultierende niederwertige 99mTc-Ion kann sodann von einem der angeführten Carboxyphosphonat-Kom-plexbildner komplexiert und im Organismus zum Skelett bzw. zu kalkhaltigen Tumoren transportiert werden.
Wässrige Lösungen der zuvor genannten Carboxyphosphonsäuren und Zinn(II)-, Eisen(II)- oder Chrom(II)-Salzen in Gegenwart von Luft haben die unerwünschte Eigenschaft, über einen längeren Zeitraum zu oxidieren oder zu hydrolysie-ren. Diese Schwierigkeiten können umgangen werden, indem man entweder die Lösung unter Stickstoff in Ampullen ver-schliesst, ein unter Stickstoff oder unter Vakuum abgeschlossenes Lyophilisat herstellt, oder indem man die erfindungsge-mässe Zusammensetzung in Form einer Tablette oder eines Dragees verwendet. Die erfindungsgemässen Präparate können damit in stabiler Form gehandelt und aufbewahrt werden und werden der 99mPertechnetat-Lösung erst kurz vor der beabsichtigten Anwendung zugefügt. Dabei wird ein hervorragendes Mittel zur Knochen- und Tumorradiographie erhalten, das bei minimaler Aufnahme von Metallionen hervorragende Scintillationsbilder liefert.
Die erforderliche 99mTc-Aktivität ist mit etwa 10 bis 15 Millicurie (mCi) äusserst gering, und die bei Anwendung der Carboxyphosphonsäure erforderliche Zinn(II)-Menge ist ebenfalls äusserst gering. Die bevorzugte Menge liegt jedoch oberhalb der stöchiometrisch zur Reduktion des 99mPertechnetats erforderlichen Menge, und zusammen mit der wiederum wesentlich grösseren Menge Carboxyphosphonat bildet sich ein durch überschüssiges Carboxyphosphonat stabilisierter 99mTc-Sn-Carboxyphosphonat-Komplex, dessen genaue Struktur nicht bekannt ist.
Vorzugsweise wird das reduzierende Agens den erfindungsgemässen Präparaten in Mengen von 1 bis 5 Gew.%, bezogen auf die Komposition, zugesetzt.
Für die Handhabung und Abmessung der geringen Mengen der benötigten reduzierenden komplexbildenden Zusammensetzung ist es von Vorteil, wenn pharmazeutisch verträgliche Füllstoffe wie Glucose oder Natriumchlorid verwendet werden. Dabei wird Natriumchlorid besonders bevorzugt, da es auch bei mitunter erforderlichem Verdünnen der Pertechnetat-Lö-sung mit sterilem Wasser zur Erhaltung der Isotonie beiträgt.
Die wirksamen Komponenten der Zusammensetzung werden homogen vermischt und in fester Form in Standardglasampullen gefüllt oder unter Verwendung von Glucose und/oder Natriumchlorid als Füllstoff zu Tabletten verpresst. Bevorzugt wird jedoch die Herstellung einer Lösung der Komponenten, die unter Stickstoff in Standardampullen abgefüllt und lyophilisiert wird. Das Lyophilisat kann unter Stickstoff oder unter Vakuum aufbewahrt werden. Gegebenenfalls kann auch eine wässrige isotonische Lösung der Komponenten unter Stickstoff aufbewahrt werden.
Beispiele
A. (Zusammensetzung der Komponenten)
1. In eine Standard-Glasampulle (10-20 ml) wird eine wässrige Lösung, enthaltend 8 mg der Phosphono-äthan-1,2-dicarbonsäure (Mischung Dinatriumsalz: Trinatriumsalz 2:1) und 0,15 mg SnCl2 eingefüllt. Die Lösung wird lyophilisiert und die Ampulle unter Vakuum oder gefüllt mit Stickstoff verschlossen. Zur Anwendung wird das Substanzgemisch mit isotonischer 99mPertechnetat-Lösung aufgelöst und intravenös injiziert.
2. In eine graduierte 10 ml-Standardampulle werden 8 mg der l-Phosphono-propan-l,2-dicarbonsäure (Mischung Dinatriumsalz:Trinatriumsalz 2:1) und 0,10 mg Chrom(II)chlorid, gelöst in 5 ml steriler wässriger Lösung eingefüllt und lyophilisiert.
3. In eine graduierte 20 ml-Standardampulle werden 8 mg des 2-Phosphono-butan-l,2,4-tricarbonsäure-trinatriumsalzes und 0,15 mg Eisen(II)sulfat, gelöst in 5 ml steriler isotonischer Kochsalzlösung, eingefüllt.
4. In eine graduierte 5 ml-Standardampulle werden 8 mg des 2-Phosphono-butan-l,2,4-tricarbonsäure-trinatriumsalzes und 0,15 mg Zinn(II)chlorid, gelöst in 5 ml steriler wässriger Lösung, eingefüllt und lyophilisiert.
5. 8 mg l-Phosphono-propan-l,2-dicarbonsäure (Mischung Dinatriumsalz:Trinatriumsalz 2:1), 0,2 mg Zinn(II)chlorid, 45 mg Natriumchlorid und 26,8 mg Glucose werden zu 80 mg schweren Mini-Tabletten verpresst. Die Tabletten lösen sich rasch in 5 ml sterilem Wasser und ergeben eine isotonische Lösung.
6.4 mg l-Phosphono-propan-l,2-dicarbonsäure (Mischung Dinatriumsalz:Trinatriumsalz 2:1), 4 mg 2-Phosphono-bu-tan-l,2,4-tricarbonsäure-trinatriumsalz, 0,2 mg Eisen(II)sulfat, 45 mg Natriumchlorid und 26,8 mg Glucose werden zu 80 mg schweren Mini-Tabletten verpresst. Die Tabletten lösen sich rasch in 5 ml sterilem Wasser und ergeben eine isotonische Lösung.
7. 8 mg Phosphono-äthan-l,2-dicarbonsäure (Mischung Dinatriumsalz : Trinatriumsalz 2:1), 0,07 mg Zinn(II)chlorid, 0,08 mg Eisen(II)sulfat werden, gelöst in 5 ml steriler isotonischer Kochsalzlösung, in eine 5-ml-Standardampulle gefüllt.
B. Aus jeder dieser Zusammensetzungen in gelöster Form wird nach Zugabe von etwa 5 ml 99mPertechnetat-Lösung mit einer Aktivität von etwa 50 mCi/ml und nach sorgfältigem Schütteln ein Mittel erhalten, welches z. B. durch intravenöse Injektion an Menschen verabreicht werden kann. Bei einem Erwachsenen mit einem Körpergewicht von etwa 70 kg wird s

man zur Skelett-Scintillographie etwa 1 ml der Lösung anwenden, die langsam injiziert wird. Bei Kindern können gegebenenfalls entsprechend geringere Mengen verwendet werden. Zur Scintillographie von verkalktem Weichgewebe, z. B. von verkalkten Tumoren oder bei fortgeschrittener Verkalkungs- 5 atherosclerose, können gegebenenfalls grössere Mengen verwendet werden. Die Injektion erfolgt vorzugsweise 1-2 Stunden nach der Herstellung.
C. Mittel, die aus dem Gemisch nach Beispiel 3 hergestellt wurden, führten bei der Skelett-Scintillographie zu hervorra- 10 genden Resultaten. Sie bewährten sich insbesondere bei der Suche nach Knochenmetastasen bei Patienten mit Mammakarzinom oder Prostatakarzinom und stellen eine ideale Ergänzung zur Röntgendiagnostik dar.
Verteilungsstudien an Ratten, die ein gutes Modell für den Menschen in diesen Untersuchungen sind, mit Aktivitäten von 0,01 bis 1,0 mCi an 99mTc ergaben, dass typischerweise bei den bevorzugten Zusammensetzungen etwa 60-70% der Dosierung an das Skelett geht. Im Blut ist nach 3 Stunden noch 5 % der Aktivität zu finden, der Rest wird mit dem Urin ausgeschieden. Diese Verteilung ist als hervorragend anzusehen.
Der optimale Zeitpunkt für die scintillographische Abtastung ist etwa 3 Stunden nach der Injektion. Zur Scintillographie von verkalktem Weichgewebe wie Tumoren, Muskelgewebe oder bei fortgeschrittener Verkalkungsatherosclerose sind andere Zeitpunkte nach der Injektion optimal. Dieser Zeitpunkt hängt von der regionalen Durchblutung (blood clearance) des betreffenden Gewebes ab.

- 622 7052PATENTANSPRÜCHE 1. Mittel zur Herstellung eines "Technetium-Radiodia-gnostikums zur Scintigraphie von Knochen sowie kalkeinlagernden Tumoren, enthaltend ein Gemisch aus a) einer oder mehrerer Carboxyphosphonsäuren und/oder Carboxyphosphonaten der allgemeinen Formel hr - c-t r-11 2~C - R(I)Cp2H C02HwobeiR = H oder einen Alkylrest mit 1 bis 3 Kohlenstoffatomen,r* = po3h2,P0,R,, t 3 «-c - r,co2h p°3"2 C - R p°3H2r2 = h, einen Alkylrest mit 1 bis 3 Kohlenstoffatomen, -ch2-chrc02h, -ch3-chrp03h2 oder-ch-ch2po3h210152025co2h bedeuten und/oder die pharmazeutisch brauchbaren wasserlöslichen Salze der genannten Verbindungen sowie 30b) einem pharmazeutisch brauchbaren Zinn(II)-, Eisen(II)-oder Chrom(II)-Salz in stöchiometrisch untergeordneter Menge, bezogen auf die Komponente a).
- 2. Mittel gemäss Anspruch 1, dadurch gekennzeichnet, dass der Anteil der Komponente b) 1 bis 5 Gew.%, bezogen auf die 35 Gesamtkomposition, beträgt.
- 3. Mittel gemäss Anspruch 1 und 2, dadurch gekennzeichnet, dass der pH-Wert der Komposition im Bereich von 5 bis 9 liegt.
- 4. Mittel gemäss Anspruch 1 bis 3, gekennzeichnet durch 40 einen Gehalt an Phosphono-äthan-l,2-dicarbonsäure, vorzugsweise in Form einer Mischung des Di- und Trinatriumsalzes im Verhältnis von 2:1.
- 5. Mittel gemäss Anspruch 1 bis 3, gekennzeichnet durch einen Gehalt an l-Phosphono-propan-l,2-dicarbonsäure, 45 vorzugsweise in Form einer Mischung des Di- und Trinatriumsalzes im Verhältnis von 2:1.
- 6. Mittel gemäss Anspruch 1 bis 3, gekennzeichnet durch einen Gehalt an 2-Phosphono-butan-l,2,4-tricarbonsäure, vorzugsweise in Form des Trinatriumsalzes. 5055
