技术领域
本发明涉及一种用来使人体的自然呼吸道保持畅通的呼吸治疗仪,它由一用来产生持续负压的泵和输入管组成。这种仪器用来治疗阻塞性的睡眠呼吸暂停。另一种作用是防止打鼾。
背景技术
为了呼吸治疗已知用来产生正呼吸道压力的所谓CPAP仪(CPAP=持续呼吸道正压),就象例如在US5655522中所述。这种仪器在一面罩内提供呼吸空气。该空气的压力相对于大气压持续地提高几百帕(hPa)。如果患者在这种人工气氛中呼吸,那么他的呼吸道由于正的相对压力张开足够大,以进行呼吸。因此CPAP-仪仅仅起到呼吸道的“气动上支架/支撑”的作用。呼吸过程本身还是完全建立在自身呼吸的基础上。
打鼾是呼吸道松弛和变窄的反映。在以高的流动速度通过咽喉时那里空气压力下降,因为随着速度的提高空气分子之间的距离加大。因经在那里松弛的组织萎陷(kollabieren),并使呼吸道完全阻塞。由此由流动引起的负压消失,呼吸道打开,空气重新流通,直到其达到一定的速度。这样便很快重新导致下一次阻塞。由于呼吸道阻塞和打开的快速交替便形成打鼾声。CPAP-仪将空气压力相对于正常的大气压(相对压力)提高到这么大,使得由流动引起的压力下降不再造成阻塞,由此防止打鼾声的形成。
如果作为打鼾原因的机理超过一定的严重程度,那么便失去呼吸动力。现在便出现需要治疗的疾病“阻塞性睡眠呼吸暂停”,至少1%的人患有这种疾病。这种病主要用CPAP-仪治疗。机械辅助方法或手术治疗是其它的,但是附属的治疗可能性。
已知CPAP仪的缺点是,它具有内部流动阻力,患者必须用其呼吸肌克服这种内部流动阻力。由此产生额外的负担,因为仪器的内部阻力与患者的呼吸道阻力叠加。
另一个缺点是,空气在穿过仪器时被加热,由此其相对湿度降低。其结果是,空气从粘膜中吸取水分。因为干燥的粘膜不仅不舒服,而且还失去其防护功能,在呼吸治疗的情况下需要空气加湿。因此CPAP-仪配备一内置的空气加湿器或在CPAP-仪和患者之间连接一作为辅助器械的空气加湿器。
CPAP-仪的另一些缺点是工作噪声大、受面罩的妨碍、卫生风险和高的能耗。
由EP-A-030740还已知一种人工呼吸仪,其中它也治疗睡眠呼吸暂停并由一钟罩形件和一风机组成,两者通过输入管相互连接。在该输入管上连接一用来控制风机功能的工业阀门和调节装置。钟罩形件放在人体的胸腔上,钟罩下面产生一负压和正压之间的有节奏的交替。这种节奏与人的自然呼吸节奏相吻合,从而积极支持呼吸过程。
这种呼吸仪的主要缺点是,它不适合于用来防止打鼾声。
发明内容
因此本发明的目的是,制造一种这一类型的方法和相应的呼吸治疗仪,它使得可以呼吸开放的大气。另一些目的是,取消面罩、降低工作噪声、改善卫生和减小能耗。
这个目的首先通过这样一种用来防止打鼾声的方法来实现,其中使人的上呼吸道始终保持张开,其特征为:给呼吸道提供大气压力,给呼吸道的外围区域提供人工负压,其中在内部的大气压和外部的负压之间存在一持续的压差。
所述目的还通过一种用来使人体的自然呼吸道保持畅通的呼吸治疗仪来实现,它由一用来产生持续负压的泵和输入管组成,其中,所述泵是一通过抽吸软管与一稳压中空体连接的抽气泵,所述中空体在保持自然呼吸口畅通的情况下包围人体,并至少在呼吸道的外围区域内形成一负压腔。
在所述呼吸治疗仪的一种实施形式中,稳压中空体做成一罩子,该罩子以其边缘包围从颌部出发在下颌两侧沿颈侧和锁骨区域至胸骨上端的前部颈部区域,并做成这样,使其离皮肤一定距离穹顶形地覆盖被其边缘所限定的皮肤区域。
在所述呼吸治疗仪的另一种实施形式中,所述稳压罩由这样的材料组成,即该在垂直于表面伸展的方向上是耐压的,而在表面伸展的方向上是柔性的,以允许头部的运动。
在所述呼吸治疗仪的另一种实施形式中,所述稳压罩由一最好是内置的由稳固的骨架组成的基体和一在基体表面上的弹性外皮组成。
在所述呼吸治疗仪的另一种实施形式中,所述外皮由橡胶或其它弹性体组成。
在所述呼吸治疗仪的另一种实施形式中,所述抽吸泵配备一用来产生和保持可预先调整的负压的压力调节装置。
本发明排除了现有技术的缺点。
通过人们将整个身体或最好是身体的一部分,例如正面柔软的颈部,放在一处于减小的相对压力之下的空腔内的方法,防止在喉部呼吸道的萎缩,因为这样普通的大气的较高静压力使呼吸道装上护甲。在物理意义上这种方法是按现有技术已知的CPAP-原理的颠倒。由此患者可以呼吸普通大气,并具有这样的优点,不再需要碍事的呼吸面罩。
因为呼吸空气不必流过机械构件,也不再存在额外的呼吸阻力,呼吸肌不再有额外的负担。由于呼吸普通大气不需要附加的空气加湿,取消了空气加湿器的费用。
因为按本发明的呼吸治疗仪不再提供呼吸空气,而只须在一个空腔内得到负的相对压力,还取消了呼吸软管,由此可以设置一个细的抽吸软管。但是抽气机不必带动大的空气量,而始终只需(抽去)由于负压空腔的不密封性而流入负压空腔的空气量。因此抽气机所需要的功率小,也可以由电池供给。此外整个抽气机做得非常小,因此可以很好地隔声。
在安全方面也存在优点。抽气机的停机不会自动地导致CO2回吸。在这种情况下将重新出现阻塞性睡眠呼吸暂停的征兆,但是不会发生更多的事情。存在卫生方面的优点,因为呼吸空气不再流过附属器件,如软管和面罩,因此不再形成冷凝水。因此时间上的清洗周期可以大大加长。不必顾虑不同患者使用同一台仪器。
因为该仪器结构简单,可经济地制造,其应用不带来危险,也可以不用医生的处方使用,以例如防止打鼾。对此已知的装置大多由一必须戴在口腔内或牙齿上的物体组成。这时,如果例如所用的防打鼾环脱落和被吞下的话,可能造成轻微的事故。

具体实施方式
举例的装置由一颈罩1、一抽吸软管2和一抽气泵3组成。颈罩1是一半张开的腔,它穹顶形地张紧在前面的(正面的)颈部区域上。颈罩1的边缘以一个回环限定从颌部出发沿下颌两侧至颈侧并从那里重新向前下方直至胸骨上端的区域。沿回环设一密封装置6,在使用状态它贴合在皮肤上。用这种方法使颈罩的张开侧被患者的皮肤封闭,形成一封闭的空腔7。
颈罩的表面做成这样,使它在垂直于其表面的方向是刚性的,但是在表面的伸展方向是柔性的。从而使颈罩表面在头部作俯仰运动时可以扩大和缩小,并在头部作旋转运动时可以侧向扭转。在本实施例中这个性能通过这样的方法达到,即颈罩1的外壳由一在稳固的骨架4组成的基体上的弹性表皮5组成,它例如由橡胶或其它弹性体组成。
在合适的位置,优选是颈罩1的下部区域上设置一管接头形的空气排出管8,该空气排出管从外部穿过颈罩1的外壳通入封闭的空腔7,并通过抽吸软管2与抽气泵3的吸气侧连接。为了颈罩1的贴合和正确地配合的稳定性设有一项带9,它也可以配备锁扣,尤其是搭链锁扣(Klettverschluss)。
在使用时抽气泵3通过连接在空气排出管8上的抽吸软管2在封闭空腔7内产生一恒定的几hPa的负压。这通过压力调节的抽气泵或通过将抽气机调整到一定转速达到。如果在患者皮肤和密封装置6之间产生泄漏,那么便将流入封闭空腔7内的泄漏量抽出。如果抽气泵3或所用的抽气机具有调节压力的装置,那么头部运动也不会造成封闭空腔7内负压的变化。
因为颈罩1张紧在整个正面的颈部区域上,在这个带有封闭空腔7的部位上建立一人工的负压气氛,患者的呼吸道通过正常大气的压力被撑开。在患者咽喉部位由气流引起的压力下降不再造成呼吸道的萎陷。不会再形成打鼾声。带有阻塞性睡眠呼吸暂停的患者可以自由呼吸,并重新具有健康的睡眠。

Claims (6)

1.用来使人体的自然呼吸道保持畅通的呼吸治疗仪,它由一用来产生持续负压的泵和输入管组成,其特征为:所述泵是一通过抽吸软管(2)与一稳压中空体连接的抽气泵(3),所述中空体在保持自然呼吸口畅通的情况下包围人体,并至少在呼吸道的外围区域内形成一负压腔(7)。
2.按权利要求1所述的呼吸治疗仪,其特征为:稳压中空体做成一罩子(1),该罩子以其边缘包围从颌部出发在下颌两侧沿颈侧和锁骨区域至胸骨上端的前部颈部区域,并做成这样,使其离皮肤一定距离穹顶形地覆盖被其边缘所限定的皮肤区域。
3.按权利要求2所述的呼吸治疗仪,其特征为:所述稳压罩(1)由这样的材料组成,即该在垂直于表面伸展的方向上是耐压的,而在表面伸展的方向上是柔性的,以允许头部的运动。
4.按权利要求3所述的呼吸治疗仪,其特征为:所述稳压罩(1)由一内置的由稳固的骨架(4)组成的基体和一在基体表面上的弹性外皮(5)组成。
5.按权利要求4所述的呼吸治疗仪,其特征为:所述外皮由橡胶或其它弹性体组成。
6.按权利要求1所述的呼吸治疗仪,其特征为:所述抽吸泵(3)配备一用来产生和保持可预先调整的负压的压力调节装置。
