Description
The invention relates to a process for the preparation of a medicament having an immunostimulating and immuno-restorative effect which, in addition to customary auxiliaries and excipients, contains a novel active substance.
Charakteristik des bekannten Standes der TechnikCharacteristic of the known state of the art
In small numbers are already known processes for the preparation of medicaments with immunostimulating and / or immunoreactive effect, which contain a low molecular weight active substance. Levamisole and isoprinosine are currently mainly used as medicines. Other agents such as NPT 15392 (erythro-9- (2-hydroxy-3-nonyl) hypoxanthine), azimexone and imuthiol also have an immunostimulating and / or immuno-restorative effect. However, medicines containing these latter drugs have not yet been introduced.
A standardization of known and novel drugs that act in terms of stimulation and / or restoration to the immune system, is currently difficult. This results once from the Komploxizität of the immune system with its diverse regulatory and counterregulation mechanisms and, on the other hand, also depends on missing, internationally binding agreed experimental conditions in laboratories or similar conditions for clinical investigations in humans. Furthermore, the active ingredients mentioned above are further investigated with regard to their immunostimulating and / or immune-restoring mechanism of action. Comprehensive comparability of immunostimulating and / or immuno-restorative compounds is also contrary to the different influence of the various immunocompetent cells by the respective active ingredients, resulting in a non-similar influence on specific and nonspecific defense mechanisms.
So wirkt z. So z. As levamisole predominantly immunorestaurierend in immunosuppressed organisms. With only a slight influence on the humoral immune response, T cells are predominantly stimulated. Non-specific defense mechanisms are activated by stimulation of phagocytosis and proliferation and bactericidal activity of macrophages.
Isoprenosine, on the other hand, above all exhibits an immunostimulating effect, i.a. by increasing the humoral immune response.
The proliferation and differentiation of the T lymphocytes are increased as well as the lymphokine induction mediated macrophage function.
Despite the progress that has been made in the development of immunostimulating and / or immuno-restorative substances, the few previously known methods for providing such drugs have disadvantages.
Thus, the optical isomer levamisole, obtained only by diastereomer separation of the racemic tetramisole, produces a bitter-metallic taste in humans after oral administration and may i.a. cause vomiting, nausea, leukopenia and agranylocytosis. Furthermore, levamisole has a remarkable dependence on the effect of genetic factors, age and sex of the patients.
Isoprenosine (50mg / kg / d) administered at relatively high doses may cause vomiting, hyperuricemia and increase in hematocrit. In azimexone, headache, vomiting, Hb and erythrocyte decline are seen as a side effect.
Reasons for the disadvantages mentioned are to be found in the type of active ingredients used so far.
Ziel dar ErfindungAim of the invention

Beschreibung der MethodenDescription of the methods
1. Rosettentest1st rosette test
According to BURNET's theory, lymphocytes carry on their surface immunoglobulin receptors which are directed against a specific antigen and are detectable by a reaction with the specific antigen. The determination of the number of rosetton-forming cells (RBC) can serve as a measure of the strength of a starting immune reaction and subsequently also reproduce effects in the sense of suppression or stimulation of cell proliferative processes.
1.1. SE-spezifische Rosetten1.1. SE-specific rosettes
Three days after the intravenous immunization with 2 × 10 5 sheep erythrocytes in 0.2 ml of physiological saline, the experimental animals were killed by overstretching, the spleens were removed and rubbed through Müller gauze with the addition of Parker medium (pH 7.4). The work to obtain the cell suspension was carried out in an ice-water bath (about 0-4 ° C). The cell density was determined in a counting chamber according to Neubauer and then the cell number of 1 × 10 7 nucleated cells (KHZ) / ml medium was set constant for all experimental batches. 0.9 ml of the hiilzzcllsuspension (1 χ 10 7 KHZ / ml) were then mixed with 0.1 ml of a 5% washed sheep erythrocyte suspension and incubated at 4 0 C for 24 h. Rosette was scored when a CHD was surrounded by at least 4 erythrocytes and the nucleated cell was identified.
1.2. THP-spezifische Rosetten1.2. THP-specific rosettes
For the study of the number of TNP-specific rosettes, the test animals were sensitized on day 0 with a 1% TNP solution. For this purpose, 25μΙ of this stock solution were dropped behind both ears of the test animal. On day 8, the study was for TNP-specific rosettes.
The recovery of the spleen cell suspension and the setting of the cell count to 1 χ 10 7 KHZ / ml was as in 1.1. described made. In contrast to the investigation of SE-specific rosettes, in this case the sheep erythrocytes had to be coupled to TNP groups. For this purpose, 8 mg of sodium trinitrobenzenesulfonate were dissolved in 2.8 ml of cacodylate buffer (pH 6.9) and added to 0.4 ml of SE sediment. After a reaction time of 10 min at room temperature, centrifugation was carried out at 60 × g for 10 minutes and washing with physiological saline three times. The TNP-SE sediment thus obtained was subsequently diluted with NaCl solution and used as a 5% solution for the rosetting test (see 1.1.).
2. Hämolyse-Plaque-Technik2. Hemolysis plaque technique
By means of this method it is possible to detect antibody-producing cells. Cells from lymphoid organs of a SE immunized animal emit antibodies during incubation with the specific antigen (SE), which diffuse into the environment and attach to the SE of close proximity. Nacr complement binding and activation, these SE are lysed. This results in hemolysis (plaques) with a central antibody-forming cell.
This direct procedure serves to determine the IgM antibody-producing cells.
The ability to complement activation and hemolysis is lower for the IgG antibodies, so that their detection must be performed using the indirect method of plaque technique. Hierboi acts after the first incubation an antiserum to the IgG, and there are formed by this complex formation with IgG also plaques after complement addition. The number of IgG-producing cells is then given as the difference between the values obtained using the indirect and direct methods.
In the CUNNINGHAM gel-free technique, immunocompetent lymphocytes, erythrocytes and complement are in a closed chamber. In liquid culture medium, a monolayer is formed. The chambers required for this method were made from slides that were rinsed with water and cleaned in absolute alcohol.
The cleaned slides were put together, pasted at the ends and in the middle by a double-adhesive tape (Scotch Double Coated Tape No.410, Minnesota Mining and Manufacturing Co.), and covered with a second row of slides exactly and at a fixed pressure. 3. Perkutaner Test zum Nachweis einer Kontaktüberempfindlichkeit3. Percutaneous test for detecting a contact hypersensitivity
The epicutaneous application of DNFB in guinea pigs and mice leads to the development of a delayed hypersensitivity, an immune status that is linked to the clonal multiplication of TNP-specific t-lymphocytes (T DTH -Ly). Renewed application of DNF after a latency period of at least 5 days results in a reaction with the ToTH-Ly of this main area, a release of lymphokines and therefore a localized inflammatory reaction.
Seven days after immunization with 1% igom DNFB, the flanks of the test animals were depilated and tested by applying one drop each of 0.5, 0.1, 0.05, 0.025 and 0.01% DNFB solution. The still positive concentration is inversely proportional to the degree of sensitization. The test results are based on the strength and number of positive skin reactions, the stimulation index (SI) from the comparison of the test and control animals.
4. Fiißballenschwellungstest4. Fluid bump swelling test
This as "dslayed footpad reaction" (FPR) designated test is capable of detecting a hypersensitivity of the delayed type against a particulate antigen in the mouse. This is a Forin of cell-mediated immunity, for which also called ToTirLy m phozyten responsible are.
The test was performed by a method of TAMURA et. al. (1985). The immunization of the mice was carried out by a l.-v.-Injection of 2 x 10 5 sheep erythrocytes, the skin test 4 days later by an injection of 5x 10 7 SE in 25μΙ in the right hind footpad. The developing thickening (consequence of a perivascular infiltration with mononuclear cells and a swelling of the skin collagen) was measured by means of a fast-acting 24 h after antigen Rekontakt. The intensity of the swelling is a measure of the strength of the immune response.
5. Bestimmung von Lymphknotenveränderungen5. Determination of lymph node changes
As a result of epicutaneous sensitization (ear) with dinitrofluorobenzene (DNFB), especially in the paracortical area of the draining auricular lymph node, increased occurrence of pyroninophilic immune wafers, increased lymphoproliferative processes as well as enlargement of the affected node. These changes were made by counting the pyroninophilic immunoblasts (pylB) on cut and smear preparations (light microscopic). The experimental findings correspond to those under 1.4.1. 1.5. Einfluß von LSL-041 auf DNFB-induzierte Veränderungen des aurikulären Lymphknotens1.5. Influence of LSL-041 on DNFB-induced changes in the auricular lymph node
Versuchstier: Albino-MeerschweinchenExperimental animal: albino guinea pig
1.5.1. Massenzunahme des Lymphknotens (LM%)1.5.1. Mass increase of lymph node (LM%)
As a result of epicutaneous sensitization with DNFB (20 μΙ of a 10% DNFB solution), the mass of the auricular lymph node increased, reaching its maximum on the 4th day (LM% = 23 + 15), then decreasing again and not after 7 days increased significantly (compare to control). Oral treatment with 2 mg LSL-041 (as lactose trituration) per day and on days -2 to +5 caused an increase of LM% = 32 ± 9 on day 4 and to a delayed decrease. The differences did not prove to be significant (student t-test).
With a single injection of 0.4 mg LSL-041 suspended in 0.1 ml PBS into the DNFB-sensitized ear (day 0), a value of 46 ± 4 and thus an LSL could be detected on day 4 for LM%. 041-related significant (p <0.001) mass increase by 100% can be achieved.
For comparison, isoprinosine was used, but at a two and a half higher dose of 1 mg. The LM% was 23 on a level with the DNFB controls.
Influence of under 1.5.1. By determination of pylB (counted 1000 lymph node lymphocytes) on days +1 to +6 after sensitization, the maximum was found to be 134 ± 14 on day 4. The oral G.ibe of LSL-041 caused only a small and insignificant elevation of pylB to 155 ± 22. The intradermally applied LSB-041 (0.4 mg) resulted in a significant (p <0.001) increase in pylB to 180 ± 9. No stimulation could be achieved with isopronosine. By means of sections, it was possible to show that the blast transformations after DNFB sensitization are preferably in the paracortical, i. in the thymus-dependent area of the lymph node.

Resultate:results:
In only immunized animals can be observed 24 hours after Rekontakt a thickening of the football by an average of 20%. She goes back quickly and is no longer measurable after 48 hours. The irradiation suppresses the formation of such DTH by about 50% (significant).
Treatment with LSL-041 (2 mg daily from day -2 to day +2) did not abrogate the suppression. However, the rapid decline was prevented. The thickening was present even after 48 hours in the same strength and thus at this time significantly above that of the control animals.
3. Untersuchungen zum Wirkmechanismus3. Studies on the mechanism of action
Proliferation and maturation processes of immunocompetent effector lymphocytes are dependent on regulatory T lymphocytes (Th) and their secretory products-the interleukins IL-2, IL-4, and IL-5. By thymectomy, irradiation and subsequent reconstruction of these animals with bone marrow cells of syngeneic mice, the influence of T H lymphocytes can be almost completely eliminated by neonatal thymectomy.
Five weeks after surgical removal of the thymus gland, the animals were immunized with SE and tested by plaque technique as previously described.

The thymectomy-related suppression was significant with ρ <0.001, the restorative effect with ρ <0.01.
Die Werte lagen bei 237 ± 74PBZ/109KHZ.The E; 'porimonto performed on the same experimental protocol on neonatal thymectomized CBA mice gave suppression of the IgM-PBZ to 186 ± 10 2 PBZ / 10 0 KHZ (controls 1131 + 172) and a negligible, non-significant treatment effect. The values were 237 ± 74PBZ / 10 9 KHZ.
The number of IgG-secreting cells was reduced to 107 + 72PBZ / 10 6 KHZ (controls 520 ± 134) and raised to 186 + 71 PBZ / 10 6 KHZ by treatment with LSL-041. Again, this difference did not prove significant.
4. Zusammenfassende Wertung4. Summary rating
Based on the animal experimental data presented, it may be considered proven that the compound LSL-041 exerts a promoting influence on immunoproliferative processes. Both B-lymphocytes capable of producing antibodies and stimulating delayed-hypersensitivity (cell-mediated immunity) ToTir lymphocytes are stimulated.
The experimental results obtained on thymectomized animals clearly indicate a strong involvement of regulatory T lymphocytes.
