Opfindelsen angår et sammenklappeligt filter til implantation i en patients blodkar, specielt i vena cava, og bestående af dels en af elastiske tråde, som er sammenføjet indbyrdes ved deres respektive 5 ender, dannet filterkurv, dels en serie forankrings ben, som ved den ene ende af kurven rager udefter under en vinkel med kurvens akse.
Implantationen af et sådant filter i vena cava, fortrinsvis vena cava inferior, har til formål at 10 fange thromber eller emboli, som er tilstede i det gennem venen strømmende blod, og hindre dem i at nå frem til patientens lunger og fremkalde pulmonar emboli.
Pra US-A-4.425.908 kendes et vena cava filter 15 af den angivne art og fremstillet af en legering, der går under navnet nitinol, og som har termisk "memory". I filterets udfoldede tilstand, som det indtager efter at være blevet indført i vena cava ved hjælp af et kateter, har selve filterkurven form som 20 en forholdsvis flad paraplylignende kegle, hvori krydsende sløjfer dannet af kurvens enkelte tråde afgrænser maskeåbninger til opfangning af emboli. Filteret formes fra begyndelsen i den angivne form ved forhøjet temperatur, og efter afkøling og udret-25 ning kan det indføres gennem et forholdsvis snævert kateter. Når filteret er placeret i vena cava og udsat for legemstemperaturen, folder det sig ud til sin ovenfor beskrevne virksomme facon.
Selv om indføringen af nitinol-filteret i 30 princippet er ret ukompliceret, er der dog visse problemer knyttet til filteret. Det er nødvendigt at opbevare filteret i isafkølet saltopløsning før dets indføring, og under indføringsoperationen må kateteret konstant gennemskylles med isafkølet saltopløs-35 ning. Hvis filteret ikke bliver optimalt placeret el- 151404 2 ler orienteret med det samme, er en korrektion næppe mulig, eftersom trådmaterialet ved legemstemperatur ikke har den bøjelighed, som ville kræves for at det udfoldede filter kunne rettes ud og trækkes tilbage 5 ind i kateteret.
Nærværende opfindelse tager sigte på at angive et filter, der er lige så velegnet til indføring ved den ukomplicerede perkutane teknik, men som er fri for de førnævnte problemer som optræder ved det 10 kendte nitinol-filter. Filteret ifølge opfindelsen er ejendommeligt ved, at filterkurven i udfoldet og spændingsaflastet tilstand har form som et gennembrudt, langstrakt omdrejningslegeme med spidse ender og med en længde, målt mellem de nævnte ender, som 15 mindst er lig med omdrejningslegemets største diameter.
Efter at et filter ifølge opfindelsen er blevet indsat i et blodkar, og så længe filteret ikke ved epitelvækst er blevet permanent forankret i kar-20 væggen, dvs. i praksis op til ca. en uge efter indsætningen, kan filteret uden vanskelighed flyttes til en lidt afvigende position eller orientering, hvis dette skønnes nødvendigt. Muligheden herfor beror på filterkurvens karakteristiske form, som tillader ud-25 retning af kurven fra dens udfoldede facon uden anvendelse af særlig store kræfter. Hertil kan benyttes det kateter, som blev brugt ved indføringen af filteret, idet man enten trækker filteret tilbage ind i kateteret eller skubber kateteret fremad over filter-30 kurven. Inden for den ovenfor omtalte indledende periode vil det endog være muligt at fjerne filteret fuldstændigt, hvis dette skulle vise sig ønskeligt eller nødvendigt. Filterkurven kan fremstilles af relativt billigt og let bearbejdeligt rustfrit stål af 35 sædvanlig biomedicinsk kvalitet.

En yderligere fordel ved det nye filter er, at selve filterkurven i realiteten har to filterflader i serie, henholdsvis opstrøms og nedstrøms i forhold til den rundtgående berøringslinie mellem kurven og 5 venevæggen (ved kurvens største diameter). Emboli, som bliver fanget på ydersiden af den første, opstrøms filterflade vil under påvirkning af blodstrømmen gradvis vandre ind i den ringformede "kile’1 umiddelbart før berøringslinien og samle sig på dette 10 sted. Emboli, som er kommet ind i kurvens indre ved at følge med blodstrømmen gennem den første filterflade, bliver opfanget på indersiden af den anden, nedstrøms filterflade og samler sig i midterområdet af denne flade. Det vil indses, at der kan tillades 15 opbygning af ret store masser af emboli begge steder, uden at det medfører nogen alvorlig indsnævring af det frie areal, som er til rådighed for blodstrømmen gennem filteret, og dermed heller ikke nogen generende forøgelse af trykfaldet over filteret.
20 Opfindelsen forklares i det følgende nærmere under henvisning til tegningen, på hvilken fig. 1 er et sidebillede af et vena cava filter ifølge opfindelsen vist i fuldt udfoldet tilstand, 25 fig. 2 et tilsvarende sidebillede af en let ændret udførelsesform, og fig. 3 et snit efter linien III-III i fig. 1 og i større skala.
I fig. 1 er anskueliggjort et vena cava filter 30 med en filterkurv, der som helhed er betegnet med 1, og som består af flere tynde og elastiske tråde 2 af et egnet materiale, fortrinsvis en rustfri stållegering. Som vist i fig. 1 er kurven 1 i hovedtrækkene formet som et langstrakt, gennembrudt omdrej-35 ningslegeme, som kan tænkes fremkommet ved drejning 151404 4 af en sinuslignende frembringerkurve omkring en drejningsakse, der i fig. 1 er angivet med en stiplet linie 3, og som tangerer frembringerkurven ved begge ender. Ved hver ende af selve filterkurven er trådene 5 sammenføjet indbyrdes ved hjælp af en kort muffe 4, der er fastgjort til trådene på vilkårligt egnet måde, f.eks. ved hårdlodning.
Som vist i fig. 1 er forholdet mellem kurvens længde og dens største diameter (som optræder i sym-10 metrimidterplanet, dvs. det i fig. 4 viste snit) ca.
2. Det skal dog bemærkes, at den nøjagtige værdi af dette forhold ikke er kritisk for filterkurvens funktion, og i praksis kan forholdet antage en værdi mellem 1,5 og 3 eller endnu højere, idet forbedringen 15 ved højere værdier dog sandsynligvis er marginal.
Som det vil fremgå af fig. 1 og 3 følger hver tråd 2 mellem endemufferne 4 en kurve, som ligner en skruelinie, og alle kurverne er ens med samme "stigningsretning", i eksemplet højrestigning. Fra 20 den ene ende til den anden er hver tråd 2 "vredet" ca. 90° om aksen 3.
I fig. 1 er et lille, aflangt Øje 5 af lignende trådmateriale som trådene 2 fastgjort således i hver muffe 4, at det rager aks ialt ud fra kurven 25 1. Ved hjælp af det ene eller andet øje 5 og en hertil passende krog eller anden gribeindretning på enden af en indføringstråd kan den sammenklappede filterkurv skubbes eller trækkes gennem et indføringskateter, som det kort er omtalt ovenfor.
30 I den i fig. 1 højre muffe 4 er der fast gjort flere forankringsben 6, i eksemplet fem, som fra muffen strækker sig aksialt bort fra kurven 1 og udefter i forhold til aksen 3. Den frie ende af hvert ben 5 er bukket udad til dannelse af en hage 35 7, som efter positioneringen af filterkurven i et 151404 5 blodkar trænger et lille stykke ind i karvæggen, så filterkurven holdes på plads. Benene 6 vil normalt være fremstillet af samme materiale som trådene 2 eller et lignende materiale, så de let kan klappes 5 sammen og rummes inden for indføringskateterets lysning, og når de er frigivet fra kateteret, fjedrer de tilbage i indgreb med blodkarrets væg. Den markante S-form af benene 6 i området umiddelbart inden for hagerne 7 sikrer, at det under filterkurvens bevæ-10 gelse gennem kateteret vil være benenes glatte kurveafsnit og ikke hagerne 7, som bevæger sig i kontakt med katetervæggen.
Det vil forstås, at der kan opnås en betydelig formindskelse af diameteren af filteret som helhed i 15 filterkurvens sammenklappede eller sammentrykte tilstand, når mufferne 4 er trukket fra hinanden og benene 6 klemt sammen radialt, uden at svække trådenes elasticitet og uden at skabe nogen risiko for, at benene filtrer sig sammen. Det har vist sig, at en 20 kurv med en største diameter på 25 mm i udfoldet stand let kan optages i et No. 10 French kateter, når den er klappet sammen. Som følge heraf kan der til indføring af filteret anvendes relativt simple angio-grafiske metodeskridt.
25 Det skal også bemærkes, at den specielle form af selve kurven og den relativt store afstand fra kurvens midterplan til forankringshagerne gør filteret særdeles stabilt mod uønsket kæntring, når det er placeret i et blodkar.
30 Den i fig. 2 illustrerede filterkurv, der i sin helhed er betegnet med 11, er sammensat af tråde 12, der som sådan er de samme som trådene 2, og som ligeledes mellem sig afgrænser snævre, langstrakte mellemrum i kurven 11's overflade. Ved kurven 11's 35 ender er trådene 12 sammenføjet med hinanden ved
Claims (6)
1. Sammenklappeligt filter til implantation i en patients blodkar, specielt i vena cava, og bestå-20 ende af dels en af elastiske tråde (2), som er sammenføjet indbyrdes ved deres respektive ender, dannet filterkurv (1), dels en serie forankringsben (6), som ved den ene ende af kurven (1) rager udefter under en vinkel med kurvens akse (3), kendetegnet 25 ved, at kurven (1) i udfoldet og spændingsaflastet tilstand har form som et gennembrudt, langstrakt omdrejningslegeme med spidse ender og med en længde, målt mellem de nævnte ender, som mindst er lig med omdrejningslegemets største diameter.
2. Filter ifølge krav 1, kendetegnet ved, at omdrejningslegemets længde er mellem 1,5 og 3 gange dets største diameter.
3. Filter ifølge krav 1 eller 2, kendetegnet ved, at omdrejningslegemet er symmetrisk 35 om et midterplan vinkelret på dets akse. 151404
4. Filter ifølge ethvert af kravene 1-3 # kendetegnet ved, at det gennembrudte omdrejningslegemes frembringer er en glat, quasi-sinus-formet kurve.
5. Filter ifølge ethvert af kravene 1-4, kendetegnet ved, at de elastiske tråde (2) forløber efter skruelinier på omdrejningslegemets overflade, og at alle skruelinier har samme stigningsretning.
6. Filter ifølge krav 5, kendetegnet ved, at enderne af hver tråd (2) er indbyrdes forsat ca. 90° omkring kurvens akse (3).
