Opfindelsen angår et apparat til brug ved anbringelse af et vævsklæbestof på basis af menneskelige eller dyriske proteiner til sømløs eller sømunderstøttende forbinding af menneskelige eller dyriske vævs- eller organdele, til sårforsegling, blod-5 stilning eller lignende processer, hvilket vævsklæbestof bliver dannet på stedet ved sammenbringning af opløsninger af proteinet og af blodkoaguleringsfremmende koaguleringsmidler, hvilket apparat har en holdeindretning for et flertal af i konusser udmundende sprøjtelegemer, især standardiserede én-10 gangssprøjtelegemer af plast, hvor konussen for det enkelte sprøjtelegeme gennem et samlehoved er forbundet med en transportkanal for den fra konussen udtrædende komponent af vævsklæbestoffet, hvilken transportkanal er adskilt fra transportkanalerne, som de øvrige konusser er forbundet med.

Ved anvendelsen af et vævsklæbestof af den nævnte art bliver en opløsning, der indeholder "Faktor-XIII" og fibrinogen blandet med en opløsning, der indeholder et thrombin og anbragt på det sårsted, der skal klæbes sammen eller beskyttes. Kendte 20 påføringsmåder består i at anbringe den ene opløsning på klæbestedet og den anden opløsning i et lag oven på eller først at blande begge opløsninger i et blandekar eller på en lille plade, trække det op i en sprøjte og så påføre det på det sted, der skal klæbes.

Begge metoder har ulemper: Ved den første metode indtræffer der på grund af forskellig viskositet i de to opløsninger en for tidlig størkning på overfladen, før en tilstrækkelig intensiv gennemblanding har fundet sted. Anvendelsen af den an-30 den metode kræver stor behændighed hos det medicinske personale, fordi størkning af klæbemidlet sker allerede kort tid efter sammenbringningen af de to opløsninger. Sammenbringningen af komponenterne, deres blanding, optrækning i en sprøjte og påføring på det sted, der skal klæbes, må ske indenfor få se-35 kunder. Denne metode er derfor begrænset til kun få anvendelsesområder .

En indretning af den i indledningen beskrevne art kendes fra US patentskrift 3.223.083. Sprøjtelegemet bliver omsluttet af en klemmeagtig holdeindretning, og sprøjtelegemets konus er indsat i en Y-formet del. I denne Y-formede del bliver vævs-5 klæbestoffets' to komponenter sammenblandet, hvorved der ved en kortvarig afbrydelse af udsprøjtningen sker en uundgåelig størkning af vævsklæbestoffet i den Y-formede del. Oer må derfor efter en sådan afbrydelse også foretages en udskiftning af den Y-formede del, hvad der er omstændeligt. Formålet med op-10 findelsen er at undgå de nævnte ulemper og vanskeligheder og at anvise et apparat af den i indledningen nævnte art, med hvilket påføringen af vævsklæbestofkomponenterne kan ske samtidigt ved betjening af apparatet med én hånd, hvorved der opnås en tilstrækkelig blanding og undgås en for tidlig størknen 15 af komponenterne, før de når sårstedet.
Dette formål opnås ved hjælp af apparatet ifølge opfindelsen, som er ejendommeligt ved, at transportkanalerne er adskilt fra hinanden helt frem til mundingerne, og at der til stemplerne 20 for alle sprøjtel egernerne findes en fælles betjeningsindret ning. Til apparatet kan anvendes standardiserede, i handelen værende éngangsapparatdele, f.eks. éngangssprøjtelegemer og -stempler såvel som standardiserede kanyler, som kan fås i sterile pakninger i forretninger, hvor man forhandler lægein-25 strumenter.
Fordelagtigt kan der på samlehovedet påsættes en blandingskanyle.
30 Med henblik på at opnå en bedre håndtering af apparatet er den fælles betjeningsindretning for sprøjtestemplerne monteret på en føringsstang, der går gennem holdeindretningen.
Formålstjenligt er mundingerne for transportkanalerne anbragt 35 tæt ved siden af hinanden på endefladen af en påsætningskonus for samlehovedet.

Fordelagtigt kan mundingerne for transportkanalerne til samle-hovedet ligge i området ved udløbsåbninger for under en vinkel med hinanden fremførte tiIføringskanaler for sterile luftarter.

Ved en yderligere fordelagtig udførelsesform er akserne for udløbsåbningerne for tiIføringskanalerne rettet i en spids vinkel i forhold til hinanden, hvor vinkelspidsen ligger i en afstand, fortrinsvis i en afstand på 10-20 cm foran mundinger-10 ne for transportkanalerne.
Opfindelsen bliver i det følgende forklaret nærmere ved hjælp af to udførelsesformer under henvisning til tegningen, hvor 15 fig. 1 viser et billede af en delvist gennemskåret første udførelsesform for apparatet ifølge opfindelsen set fra siden, fig. 2 et snit langs linien 11-11 i fig. 1, 20 fig. 3 et snit langs linien 111-111 i fig. 1, fig. 4 en anden udførelsesform for opfindelsen delvist i snit langs linien IV-IV i fig. 5, og 25 fig. 5 et snit langs linien V-V i fig. 4.
Af de to sprøjtelegemer, der er betegnet med 1 og 2, tjener det ene til optagelse af en thrombin-indeholdende opløsning, 30 og det andet til optagelse af en "Faktor-XIII"- og fibrinogen-indeholdende opløsning. Sprøjtelegemerne 1, 2 er formålstjenligt udformet som standardiserede éngangssprøjtelegemer af plast. De er begge indsat i en holdeindretning 3. Holdeindretningen 3 har to U-formede render 4, 5, som begge i deres ender 35 med det største tværsnit er udformet med overgribende nopper 6, så at sprøjtelegemerne 1, 2, som bliver anbragt i renderne ovenfra, kommer i indgreb og ved hjælp af nopperne 6 bliver

4 fastholdt i renderne. Ved den ende af holdeindretningen, der har to ved siderne anbragte fingergreb 7, 8 findes der U-for-mede udvidelser 9, 10 i renderne, som sprøjtelegemernes flangeender 11, 12 er ført ind i, så sprøjtelegemerne 1, 2 også 5 bliver fikseret i retning af deres respektive længdeakser 13 i holdeindretningen 3.
Midt imellem de to U-formede render 4, 5 findes der en udtagning 14 for en føringsstang 15, hvilken udtagning ved den ende 10 af holdeindretningen, hvor fingergrebene 7, 8 sidder, går over i et gennemgående borehul 16 i holdeindretningen. Føringsstangen 15 er fast forbundet med en betjeningsindretning 17, hvori knastopsatse 18, 29 hørende til stemplerne 20, 21 i sprøjtelegemerne 1, 2 er indsat. Knastopsatsene 18, 19 hørende til 15 stemplerne 20, 21 er begge indsat i en U-formet udtagning 22, 23 i betjeningsindretningen 17. Føringsstangen har i sin forreste ende en fortykkelse 24, så at den ikke kan glide ud af hullet 16 i holdeindretningen 3.
20 De to konusser 25, 26 på sprøjtelegemerne er ført ind i ind-stikskonusser 27, 28 i et samlehoved 29 og er via dette samle-hoved forbundet med hinanden. Inden i samlehovedet 29 fører der fra hver indstikskonus 27, 28 en særskilt transportkanal 30, 31 ind i et på samlehovedet anbragt påsætningskonushoved 25 32. Mundingerne 33, 34 hørende til transportkanalerne 30, 31 er anbragt tæt ved siden af hinanden på endefladen 35 af dette påsætningskonushoved. En blandingskanyle 36, som ved sin bageste ende er forsynet med en standardiseret indstikningskonus 37 kan skydes ind over påsætningskonushovedet 32.

Ved et tryk med tommelfingeren på betjeningsindretningen 17 træder der samtidigt ud af de to sprøjtelegemer 1, 2 forudbestemte mængder. Som følge af de ringe afstande mellem mundingerne 33, 34 hørende til transportkanalerne 30, 31 kommer op-35 løsningerne straks i berøring med hinanden, hvorved blandingseffekten er optimal. Påføringen med én sprøjtefyldning kan ske såvel intermitterende som også kontinuerligt. Brugte sprøjte-

5 legemer 1, 2 og blandingskanylen 36 kan hurtigt udskiftes, og hele apparatet kan i givet fald steriliseres uden problemer. Enhåndsbetjeningen og den automatiske dosering af de to opløsninger i et ønsket blandingsforhold udgør en væsentlig ar-5 bejdslettelse. Bliver der anvendt en blandingskanyle 36, så at blandingen er særlig intensiv, hvorved klæbninger af højeste styrke kan opnås. Bliver påføringen af vævsklæbestoffet afbrudt, så må blandingskanylen 36 aftages og erstattes af en ny. Samlehovedet 29 kan derimod anvendes igen. Det beskrevne 10 apparat muliggør en anvendelse af vævsklæbestoffer på næsten alle områder.
Den i fig. 4 og 5 viste udførelsesform muliggør påsprøjning af vævsklæbestoffet på en overflade, der skal klæbes eller for-15 segles. Ved denne udførelsesform er samlehovedet udformet som et sprøjtehoved 38. Det har med udgangspunkt i indstikningskonusserne 39, 40 to fra hinanden adskilte transportkanaler 41, 42, hvis mundinger 43, 44 er anbragt i en afstand, der svarer til afstanden 45 mellem sprøjtelegemerne 1, 2. Opløsningernes 20 udtrædningsretning 46 fra mundingerne 43, 44 er ca. vinkelret på længderetningen 13 for sprøjtelegemerne 1, 2.
Foruden transportkanalerne 41, 42 har samle- eller sprøjtehovedet 38 en ti 1før ingskanal 47 for en steril luftart, f.eks.
25 trykluft, som inden for samlehovedet deler sig i to grene 48, 49. Udtrædningsåbningerne 50, 51 for disse to ti 1 føringskanaler 48, 49 findes i nærheden af mundingerne 43, 44 hørende til transportkanalerne 41, 42. Akserne 52, 53 for udtrædningsåb- ningerne 50, 51 er rettet ca. vinkelret på udtrædningsretnin-30 gen 46 for opløsningerne, hvortil kommer, at akserne 52, 53 for udtræningsåbningerne er rettet i en spids vinkel 54 i forhold til hinanden, og at vi nkel spidsen 55 ligger i en afstand 56 fra ca. 10-20 cm foran mundingen af transportkanalerne.

1. Apparat til brug ved anbringelse af et vævsklæbestof på basis af menneskelige eller dyriske proteiner til sømløs eller sømunderstøttende forbinding af menneskelige eller dyriske 25 vævs- eller organdele, til sårforsegling, blodstilning eller lignende processer, hvilket vævsklæbestof bliver dannet på stedet ved sammenbringning af opløsninger af proteinet og af blodkoaguleringsfremmende koaguleringsmidler, hvilket apparat har en holdeindretning (3) for et flertal af i konusser (25, 30 26) udmundende sprøjtelegemer (1, 2), især standardiserede én- gangssprøjtelegemer af plast, hvor konussen (25, 26) for det enkelte sprøjtelegeme (1, 2) gennem et samlehoved (29, 38) er forbundet med en transportkanal (30, 31, 41, 42) for den fra konussen udtrædende komponent af vævsklæbestoffet, hvilken .35 transportkanal (30, 31, 41, 42) er adskilt fra transportkanalerne (31, 30, 42, 41), som de øvrige konusser (26, 25) er forbundet med, kendetegnet ved, at transportkana- 7 DK 164801 B lerne (30, 31, 41, 42) er adskilt fra hinanden helt frem til mundingerne (33, 34, 43, 44), og at der til stemplerne (20, 21. for alle sprøjtelegemerne (1, 2) findes en fmiles betjeningsindretning (17). 5
