Den foreliggende opfindelse angår nye farmaceutiske formuleringer og nærmere angivet formuleringer med reguleret frigivelse, indeholdende acrivastin som som den aktive bestanddel.
5 Acrivastin er det vedtagne navn for forbindel sen (E)-3-[6-[(E)-3-(l-pyrrolidinyl)-l-(p-tolyl)-l-pro-penyl]-2-pyridyl]acrylsyre, der har strukturformlen:

Denne forbindelse er beskrevet i beskrivelsen til europæisk patentansøgning nr. 85959.
Acrivastin har kraftig antihistaminvirkning, medens det er i det væsentlige fri for. de sedative 20 virkninger, der sædvanligvis er forbundet med antihistaminer, såsom brompheniramin , chlorpheniramin og triprolidin . Det kan derfor fordelagtigt anvendes til behandling af flere forskellige lidelser som beskrevet i EPA-beskrivelse nr. 85959, herunder lindring af symp-25 tomerne af næsetilstoppelse som følge af forkølelse og vasomotorisk rhinitis, og til symptomatisk kontrol af allergiske lidelser.
Til anvendelse i medicinen kan acrivastin om ønsket administreres i form af et farmakologisk og far-30 maceutisk acceptabelt salt. Sådanne salte indbefatter, men er ikke begrænset til syreadditionssalte, såsom salte dannet med saltsyre, svovlsyre, salpetersyre, phosphonsyre, maleinsyre, salicylsyre, toluen-p-sulfon-syre, vinsyre, citronsyre, methansulfonsyre, myresyre, 35 malonsyre, isethionsyre, ravsyre, naphthalen-2-sulfon-syre og benzensulfonsyre, og alikametal- eller jord-

Acrivastin kan administreres ad flere forskel- I
lige veje, men det administreres bekvemt ved oral ad- I
10 ministrering. Det har således vist sig, at oral admini- I
strering af en 8 mg dosis af acrivastin , formuleret på I
I konventionel måde, til voksne frivillige personer ty- I
pisk vil give terapeutiske koncentrationer af acriva- I
I stin i blodet i op til ca. 8 timer. Følgelig vil kon- I
15 ventionelle formuleringer af acrivastin fortrinsvis I
blive administreret i tre daglige doser til opnåelse af I
vedvarende lindring af de pågældende symptomer. I
I Selvom acrivastin udviser en nyttig virknings- I
I , varighed, vil det ikke desto mindre være ønskeligt at I
I 20 administrere acrivastin som en formulering med regule- I
I ret frigivelse til forlængelse af virknihgsvarigheden I
I for medikamentet og følgelig reducering af doserings- I
I hyppigheden, uden forøgelse af den totale daglige do- I
I sis. Det vil også være ønskeligt, at en sådan formule- I
I 25 ring med reguleret frigivelse giver forholdsvis ensar- I
I tede koncentrationer af acrivastin. i legemet. Med hen- I
I blik på maksimering af biotilgængeligheden har det I
imidlertid vist sig, at frigivelseshastigheden ikke I
skal være alt for langsom. Acrivastinet skal fortrins- I
I 30 vis frigives indenfor 3-4 timer efter administrering. I
Det er almindeligt kendt indenfor den farmaceu- I
tiske formuleringskunst, at frigivelsen af aktive stof- I
I fer fra farmaceutiske præparater kan forsinkes ved for- I
mulering af den aktive bestanddel i en matrix, eller I
I 35 ved filmo vertrækning af den aktive bestanddel, og der I
I kendes mange forskellige frigivelsesforsinkende stoffer I

Det er imidlertid også kendt, at frigivelseshastigheden er afhængig af ikke blot arten af filmovertrækket eller det matrixdannende middel, men også af arten af den ak-! 5 tive bestanddel og hjælpestofferne, og af indbyrdes reaktioner mellem dem. Det er således almindeligvis ikke muligt at forudsige frigivelsesprofilen for en bestemt formulering.
I GB patentansøgning nr. 2087235A er der beskre-10 vet en granulær formulering med forsinket frigivelse, indeholdende granulerede eller krystallinske aktive stoffer overtrukket med en homogen blanding af et poly-acrylat, som er uopløseligi, men dispergerbart i vand (såsom Eudragit ® E30D), og en celluloseether, der er 15 uopløselig, men dispergerbar i vand (såsom Aquacoat ® ECD30). De to komponenter i overtræksblandingen angives fortrinsvis at anvendes i et forhold på fra 2,5:1 til 5:1 (polyacrylat:celluloseether). De eneste aktive stoffer, der er specielt angivet i denne beskrivelse, 20 er kaliumchlorid, lithiumsalte, dichofenac-natrium og pirprofen.
I GB patentansøgning nr. 2086725A og 2157170A er der beskrevet henholdsvis hurtigt desintegrerende pressede former og lagerstabile, hurtigt desintegre-i 25 rende pressede former indeholdende de ovennævnte over- trukne granuler. I hvert tilfælde anvendes komponenterne i overtræksblandingen i et forhold på fra 2,5:1 til 5:1 (polyacrylat:celluloseether).
I GB patentansøgning nr. 2178313A er der beskre-30 vet en fremgangsmåde til fremstilling af filmovertruk-ne granulære former med forsinket frigivelse og omtrent magen til dem, der er beskrevet i de ovennævnte Beskrivelser. Ifølge denne beskrivelse kan forholdet polyacrylat :ethylcellulose i overtræksblandingen imidlertid 35 ligge i området fra 20:1 til 1:5. molekylvægt på 800.000. Komponent (b) i overtrækket anvendes fortrinsvis i form af en vandig latexdispersion.
Egnede latexdispersioner af ethylcellulose indbefatter sådanne, som fås under handelsbetegnelserne Aquacoat ® 10' ECD-30 fra FMC Corporation (Philadelphia, USA) og Sure-lease fra Colorcon (West Point, Pennsylvania).
Komponenterne (a) og (b) vil almindeligvis være , til stede i overtræksblandingen i et vægtforhold i om rådet fra 2:1 til 1:2, fortrinsvis fra 1,5:1 til 1:1,5, 15 mest foretrukket på 1:1.
Foruden de ovenfor beskrevne komponenter (a) og (b) kan overtrækket med reguleret frigivelse indeholde andre passende hjælpestoffer, såsom antiagglomererings-midler (f.eks. talkum, kaolin eller kolloidt silica,
20 f.eks. Aerosil til forhindring af sammenklæbning af I
de overtrukne kerner og/eller midler til modificering af overtrækkets permeabilitet eller porøsitet, såsom elektrolytter (f.eks. natriumchlorid), polyethylengly-coler, lactose, sucrose eller mannitol. Sådanne hjælpe-25 stoffer udgør typisk mellem 5 og 40 vægt% af overtrækket. Et særlig fortrukket hjælpestof er mannitol.
De ovennævnte særskilte enheder har almindeligvis form af kerner, som indeholder acrivastin , eventuelt i blanding med et eller flere farmaceutiske bære-30 stoffer eller hjælpestoffer. Eksempler på sådanne bærestoffer eller hjælpestoffer er velkendte på det farmaceutiske område til formulering af tabletter og granuler og indbefatter f.eks. bindemidler, smøremidler, indifferente fortyndingsmidler, overfladeaktive midler 35 og dispergeringsmidler. Sådanne kerner kan f.eks. fremstilles ved blanding af acrivastin og vilkårlige eg-

nede bærestoffer eller hjælpestoffer og komprimering el- I
ler formning af den opnåede blanding. Alternativt kan I
acrivastin påføres på en indifferent kerne, f.eks. en I
nonpareil eller prill, eventuelt i blanding med et el- I
5 ler flere egnede hjælpestoffer, f.eks. i en opløsning I
af et bindemiddel, såsom polyvinylpyrrolidon, efter- I
H fulgt af tørring. Den på kernen påførte opløsning af I
acrivastine kan hensigtsmæssigt være syrnet, f.eks. med I
saltsyre. Opløsningen kan også indeholde et eller flere I
10 egnede opløsningsmidler, f.eks. en vandig alkohol, så- I
som ethanol eller isopropylalkohol. I
En anden alternativ form for kernen er en formu- I
lering af acrivastin med et eller flere egnede hjælpe- I
I stoffer til dannelse af såkaldte sfæroider, dvs. sfæ- I
15 riske partikler, med en diameter på 0,5 til 2 mm. Så- I
danne sfæroider er kendte på det omhandlede område, I
I f.eks. som beskrevet af A.D. Reynolds, Manufacturing I
I Chemists Aerosol News, juni 1970, side 40-43. Sådanne I
I sfæroider kan fremstilles ved at blande den aktive be- I
20 standdel med vand og vilkårlige egnede hjælpestoffer I
til dannelse af en ekstruderbar masse, som derpå eks- I
truderes som aflange partikler og omdannes til sfæroi- I
der, f.eks. ved anvendelse af en sfæronisator indehol- I
dende en roterende plade, hvorpå de aflange partikler I
25 omdannes af pladens roterende bevægelse til sfæriske I
partikler, efterfulgt af sigtning til fjernelse af par- I
! tikler, som er alt for fine eller alt for grove. Sfæ- I
roiderne kan emballeres som sådanne, f.eks. i en sa- I
chet, til suspendering i vand før administrering eller I
30 til strøning på fødevarer, eller inkorporeres i en I

Overtrækkene kan påføres på de særskilte enhe- I
der af acrivastin på konventionel måde. Således kan I
f.eks. polymeren og vilkårlige andre overtrækskomponen- I
35 ter suspenderes i et egnet flydende medium, f.eks. I
vand, til dannelse af en dispersion af polymeren. Dis- I
7 DK 175660 B1 persionen kan derefter påføres på de særskilte enheder ved sprøjteovertrækning, f.eks. i et fluid bed, til dannelse af et overtræk af den ønskede tykkelse. Af andre metoder til påføring af overtrækkene kan der næv-5 nes pandeovertrækning. Efter påføring af overtrækket kan de overtrukne enheder opvarmes til "hærdning" af filmen. De overtrukne enheder opvarmes fordelagtigt til en temperatur i området fra 30-100°C, fortrinsvis fra 50-80°C, f.eks. til 70°C. Opvarmningen foretages almin-10 deligvis i mellem 0,25 og 3 timer, f.eks. i 0,5 til 2 timer.
Om ønsket kan der påføres yderligere acrivastin på ydersiden af de overtrukne enheder. Dette kan f.eks. gennemføres på tilsvarende måde som påføringen af acri-15 vastin på en indifferent kerne som beskrevet ovenfor.
De overtrukne enheder indeholder almindeligvis mellem 5 og 25 vægt% acrivastin . Overtrækket udgør typisk 1 til 15 vægt% af den totale formulering.
De overtrukne særskilte enheder af acrivastin 20 ifølge den foreliggende opfindelse præsenteres fortrinsvis i enhedsdosisform, f.eks. som en tablet eller en kapsel. Sådanne enhedsdosisformer kan om hensigtsmæssigt omfatte overtrukne særskilte enheder af acriva-stine i blanding med farmaceutisk acceptable bærestof-25 fer eller hjælpestoffer, såsom de ovenfor beskrevne. De overtrukne enheder kan alternativt eller yderligere være blandet med andre terapeutiske midler, f.eks.. sym-pathomimetiske midler, såsom dekongestanten pseudoephe-drin, et antitussivt middel, såsom codein, en analgeti-30 kum, et antiinflammatorisk middel, et antipyretikum eller et ekspektorans. Sådanne yderligere terapeutiske midler kan være i en form, der er beregnet til øjeblikkelig frigivelse, eller de kan selv være formuleret til reguleret frigivelse.

Metode
Formuleringen fremstilledes efter følgende generelle metode: 25 Acrivastinet og polyvinylpyrrolidonet omrørtes i ethanol. Saltsyren fortyndedes med vand til opnåelse af en 7 vægt/vægt% opløsning og sattes langsomt til den alkoholiske suspension under konstant omrøring. Blande-operationen fortsattes, indtil alt fast materiale var 30 opløst. Medikamentopløsningen sprøjtedes på de nævnte nonpareils i et fluid-bed-system. Mannitolet opløstes i vand, og Aquacoat ®- og Eudragit suspens ionerne til sattes under omblanding. Polymersuspensionen sprøjtedes på de medikamentovertrukne nonpareils i et fluid-35 bed-system. Pellet'erne hærdedes ved 70eC i 1 time. Talkummet fordeltes gennem hele pellet-lejet og flui-diseredes i et passende tidsrum.

Claims (12)
1. Farmaceutisk formulering til oral administre- I ring og med reguleret frigivelse, kendeteg- 4 I net ved, at den omfatter særskilte enheder inde-· I 5 holdende acrivastin eller et salt heraf og overtruk- I ket med en blanding indeholdende: I a) en polymer eller copolymer indeholdende mono- I I mer-gentagelsesenheder valgt blandt alkylestere I af acryl- og methacrylsyre og I I 10 b) ethylcellulose, I I hvor komponenterne (a) og (b) er til stede i over- I I trækket i et vægtforhold på fra 2:1 til 1:2.
