Opfindelsen angår en fremgangsmåde til konservering af ophthalmiske opløsninger med spormængder af stabiliserede peroxyforblndelser. Specielt angår forbindelsen anvend-5 elsen af stabiliserede spormængder af hydrogenperoxid som konserveringsmiddel i pufret saltvand til opløsninger til pleje af øjet.
Hydrogenperoxid er et velkendt germicidt middel. Således er det f.eks. kendt, at hydrogenperoxid i form af en re-10 lativt fortyndet opløsning, f.eks. ca. 0,5 til 6 vægt-% i vand, er effektivt som desinfektionsmiddel til anvendelse med kontaktlinser for at dræbe eventuelle tilstedeværende mikroorganismer.
En ulempe ved ustabiliserede fortyndede hydrogenperoxid-15 opløsninger er imidlertid, at de vandige peroxidopløs-ninger uden anvendelse af en stabilisator eller en kombination af stabilisatorer karakteristisk sønderdeles i løbet af et tidsrum. Den hastighed, med hvilken sådanne fortyndede hydrogenperoxidopløsninger sønderdeles, vil 20 naturligvis afhænge af sådanne faktorer som pH og tilstedeværelsen af spormængder af forskellige metalliske urenheder, såsom kobber eller chrom, der kan tjene til katalytisk at sønderdele disse. Desuden accelereres sønderdelingshastigheden af sådanne fortyndede vandige 25 hydrogenperoxidopløsninger stærkt ved moderat forhøjede temperaturer.
Et antal forskellige stabilisatorer er foreslået anvendt sammen med hydrogenperoxid til deaktivering af katalytiske sporurenheder, herunder stannosalte, ethylendiaminte-30 traeddikesyre o.l.. Således beskriver USA-patentskrift nr. 3.860.391 f.eks. blegemidler indeholdende hydrogenperoxid og som stabilisator amino-lavere alkylenphosphater, herunder diethylentriamin-penta(methylenphosphonsyre) eller salte deraf, og/eller hydroxyalkanphosphater med 35 eller uden yderligere stabilisatorbestanddele, indstillet 2 DK 175864 B1 til en pH-værdi mellem ca. 9,0 og 12,0 med f.eks. natriumhydroxid til blegning af cellulosematerialer. Midler med en pH-værdi på 12,0 er eksemplificeret. Imidlertid er | 5 sådanne stærkt basiske opløsninger uønskelige i opløsning- 1 er i ophthalmiske sammenhæng, herunder øjendråber og ren- seopløsninger til kontaktlinser.
Desuden beskriver britisk patentskrift nr. 1.500.707 en opløsning til sterilisation af kontaktlinser, der anvender 10 hydrogenperoxid sammen med 200 til 2000 ppm phosphat-[ pyrophosphat-]stabilisator ved en pH-værdi på 4,5.
Imidlertid udføres desinfektionen ikke ved en pH-værdi, der stemmer overens med øjets miljø, da pH må hæves til ca. 7. Desuden må peroxidet fjernes for at gøre opløs-15 ningen forligelig med øjet.
USA-patentskrift nr. 4.304.762 beskriver stabilisering af vandigt hydrogenperoxid ved tilsætning af diethylentri-amin-penta(methylenphosphonsyre) eller et salt deraf ved en pH-værdi på ca. 7. Imidlertid omtales den stabilserende 20 phosphonatforbindelse som særlig effektiv til anvendelse sammen med basiske hydrogenperoxidopløsninger. Endvidere beskrives disse opløsninger som anvendt som grundlag for flydende blegemidler. Der er hverken nogen antydning om, at spormængder af hydrogenperoxid stabilisers effektivt, 25 eller nogen angivelse af, at hydrogenperoxid kunne anvendes som konserveringsmiddel til en ophthalmisk opløsning.
Nogle af de opløsninger til pleje af øjet, der nu fås i handlen, anvender benzalkoniumchlorid i stedet for hy-30 drogenperoxid som konserveringsmiddel. Således indeholder opløsninger til kontaktlinser typisk 0,9% natriurochlorid, puffere, overfladeaktive midler, befugtningsmidler og 0,002 til 0,01 % benzalkoniumchlorid. Benzalkoniumchlorid anvendes også i andre produkter, herunder isotoniske 3 DK 175864 B1 dekongestante ophthalmiske opløsninger, såsom "Visine"® øjendråber fremstillet af Leeming Division of Pfizer,Inc.

Der består imidlertid et problem i, at benzalkoniumchlo-5 rid, hvis karakter er kationisk, reagerer med proteiner, der findes i øjets omgivelser, og forårsager uønskede afsætninger på bløde kontaktlinser. Benzalkonlumchlorid og dets analoge optages også af linsematerialet og kan have en ødelæggende virkning på linsernes struktur, jf. Davis, 10 S.S. m. fl., "The Adsorption of Cationic Antimicrobial Agent Onto PolyHema", Colloids and Surfaces, 12, 203-212 (1984) desuden lnaktiveres benzalkonlumchlorid af mange forbindelser, herunder de med bomulds- og nylonfibre forbundne. Endvidere bemærkes det i K.C. Swan, "Reactivity 15 of the Ocular Tissues to Wetting Agents", Am.J.Ophthalmol., 27, 118 (1944), at gentagen anvendelse af benzal-koniumchlorid i koncentrationer på 1:5000 eller stærkere kan denaturere hornhindeproteinet og forårsage irreversibel skade. Det har også vist sig, at 0,04 til 0,05%'s 20 opløsninger af benzalkonlumchlorid kan forårsage forstyrrelse af hornhindeepithelet ved overfladisk punktur.
Andre konserveringsmidler, der for tiden anvendes, omfatter thlmerosal, der kan forårsage en følsomhedsreak-25 tion hos øjet, og sorbinsyre, der sædvanligt forårsager misfarvning af linsen. Ulemperne ved de sædvanligt anvendte konserveringsmidler, såsom thlmerosal, benzal-koniumchlorid og andre, er behandlet i følgende litteratur: M. Ikke-stabiliserende kontaktlinsesterilisationsopløsninger indeholdende en peroxidforbindelse eventuelt i kombination med thimerosal er kendt fra FR-A-2.134.666. Stabilisering af 0,5-6% H20,-kontaktlinsedesinfektionspræparater med di-5 ethylentriaminpenta(methylenphosphonsyre) eller med en kombination af specifikke diphosphonsyrealkanoler og en anden peroxidstabilisator er kendt fra EP-A-265 386 eller EP-A-289 463.
10 Et formål med opfindelsen er at tilvejebringe konserverede opthalmiske opløsninger med komponenter, som danner hyd-rogenperoxid.
Overraskende overvindes ulemperne ved de kendte konserve-15 ringsmidler med stabiliserede spormængder af peroxyforbin-delser tilvejebragt af den foreliggende opfindelse, som kan anvendes som konserveringsmiddel i opthalmiske opløsninger, såsom renseopløsninger til kontaktlinser.
20 De lave koncentrationer af peroxyforbindelser ligger under de kommercielt egnede mængder, der er nødvendige for at peroxyforbindelsen virker som desinfektionsmiddel, og er tilstrækkelig lave til direkte applikation i øjet. Endvidere er pH-værdien også forligelig med øjets omgivelser.

Opfindelsen angår derfor en konserveret, pufret, iso-tonisk, saltholdig kontaktlinseopløsning, som er forenelig med øjets omgivelser og har en pH-værdi på 5,5-8, kendetegnet ved, at opløsningen i det væsentlige består af 30 (i) natriumperborat eller natriumperoxid som en hydrogen- peroxid-kilde, der tilvejebringer hydrogenperoxid i en mænge på 0,001 og derover men mindre end 0,01 {100 ppm) vægt-%, 35 (ii) en hydrogenperoxidstabilisator, som er en phosphon-syre valgt blandt forbindelser med formel (I) 5 DK 175864 B1 (HjPOrCwalkylen ^N^aDcylen - NH^aDcyleft-POiHi) 0).

25 Alternativt eller yderligere kan der tilsættes fra 0,005 vægt-% til ca. 0,2 vægt-% 1-hydroxyethyliden-l,1-diphos-phonsyre til opløsningen.
Kilden til hydrogenperoxid, der tilvejebringer effektiv 30 resulterende mængde hydrogenperoxid, omfatter natriumper-boratdecahydrat og natriunqperoxid.
Opløsningerne ifølge den foreliggende opfindelse er fri for opthalmisk aktivt middel og kan anvendes som konser-35 veret saltopløsning, som en konserveret stabiliseringsopløsning til kontaktlinsr eller som en konserveret opløsning til behandling af kontaktlinser i tidsrum, hvor de ikke bæres.

35 Fysiologisk forligelige salte af forbindelser af formlerne I og II omfatter f.eks. vandopløselige salte med gængse farmaceutisk acceptable kationiske dele, herunder kationer af alkalimetaller, f.eks. natrium og kalium, jordalkali- DK 175864 B1 7 metaller, f.eks. calcium, ammonium og aminer. Egnede aminsalte omfatter f.eks. mono-, di- og tri-lavere alkylaminer, f.eks. methylamin, ethylamin, diethylamin, triethylamin, dimethylamin, trimethylamin, propylamin, 5 o.l., og mono-, di- og tri-lavere hydroxyalkylaminer, f.eks. ethanolamin, diethanolamin, triethanolamin, glucamin, 2-hydroxypropylamin o.l.. Ved "lavere" i sammenhæng med en alkylgruppe forstås en alkylgruppe med op til 6 carbonatomer, fortrinsvis op til 4 carbonatomer.

Fortrinsvis er koncentrationen af stabilisatoren med formlen I eller saltet deraf i det stabiliserede middel mellem ca. 0,006 og 0,02 vægt-% af midlet, især mellem ca.
0,006 og ca. 0,0120 vægt-% af midlet.

Ligeledes foretrækkes, at stabilisatoren med formlen II er til stede i en mængde på mindst ca. 0,024 mmol (50 ppm), fortrinsvis 0,039 mmol (80 ppm) op til ca. 0,34 mmol (700 2o PPm)/ især 0,049 mmol (100 ppm) op til ca. 0,29 mmol (600 ppm), mest foretrukket 0,073 mmol (150 ppm) til ca. 0,19 mmol (400 ppm) pr. 100 g oplosning. Mængderne i parentes gælder for "Dequest"® 2010, der har en molvægt på 206.
Andre stabilisatorer med formlen II bør være til stede i 25 molere ækvivalenter dertil.
Fortrinsvis ligger den stabiliserede opløsnings pH-værdi mellem ca. 5,5 og ca. 8. Mere foretrukket ligger den stabiliserede hydrogenperoxidopløsnings pH-værdi mellem

6,0 og 8,0, mest foretrukket mellem 6,5 og 7,5. pH-værdien kan indstilles som ønsket ved tilsætning af passende mængder syre eller base af en i de anvendte mængder fysiologisk acceptabel art, f.eks. saltsyre eller natriumhydroxid.
Den stabiliserede opløsnings pH-værdi frembyder en anden fordel i forhold til den kendte teknik, da pH-værdieme i de fleste eksisterende ophthalmiske opløsninger indehold-.

Desuden kan der i den stabiliserede hydrogenperoxidopløs-15 ning ifølge opfindelsen være et eller flere gængse, i det væsentlige indfferente, fysiologisk acceptable tonicitsforøgende midler til stede. Sådanne egnede midler omfatter f.eks. alkalimetalhalogenider, -phosphater, -hydrogenphosphater og -borater. Natriumchlorid, 20 natriumdihydrogenphosphat og natriummonohydrogenphophat foretrækkes. Sådanne tonicitetsforøgende midlers funktion er at sikre, at opløsningen har tilnærmelsesvis fysiologisk tonicitet, når den inddryppes i øjet, eller at hjælpe med at sikre en sådan tonicitet efter fortynding, 25 hvis fortynding er nødvendig forud for kontakt med øjet på grund af peroxidindhold som angivet ovenfor.
Fortrinsvis er der tilstrækkelige tonicitetsforøgende midler til stede i opløsningen, således at den i det 30 væsentlige er isotonisk, eller således at den efter sønderdeling eller fortynding af hydrogenperoxidet deri fremkomne opløsning er i det væsentlige isotonisk, dvs i det væsentlige med en tonicitet ækvivalent med en 0,9 vægt-% vandig natriumchloridopløsning.

Generelt karakteriseres de stabiliserede hydrogenperoxid-opløsninger ifølge opfindelsen ved deres ekstraordinære stabilitet, selv under fremskyndede betingelser, f.eks. opvarmning af opløsningerne til 100°C i 24 timer. Således 5 forøges disse midlers lagerholdbarhed. Endvidere er de foreliggende midler kendetegnet ved deres fysiologiske tolerance efter sønderdeling af hydrogenperoxidet.
En anden fordel ved anvendelse af hydrogenperoxid i 10 ophthalmiske opløsninger er, at spormængden af hydrogenperoxid, især mindre end 100 ppm, ødelægges, så snart den kommer i kontakt roed øjet. Således vil f.eks. katalase, der findes i øjevævet, forårsage nedbrydning af .
hydrogenperoxidet til vand og oxygen. Som resultat heraf 15 bliver opløsningen efter anvendelsen fri for konserveringsmiddel, hvilket i høj grad minimerer uheldige reaktioner. De med andre konserveringsmidler ·' forbundne problemer, såsom den manglende evne til nedbrydning af uskadelige forbindelser undgås.

Oplåsningerne ifølge opfindelsen kan formuleres på enhver gængs måde. Således kan f.eks. alle de andre bestanddele end peroxyforbindelse og vand anbringes i en beholder, og dertil kan sættes peroxyforbindelse under blanding. Alter-25 nativt kan de tørre bestanddele rives op med en lille portion flydende stabilisator, og derpå kan resten af stabilisatoren tilsættes, efterfulgt af peroxyforbindelse og det meste af vandet. Det viskositetsforøgende middel, dvs. fortykkelsesmidlet, kan derpå tilsættes, eller den 30 fremkomne opløsning kan sættes til fortykkelsesmidlet. En fagmand vil være klar over talrige variationer i måden at formulere opløsningerne ifølge opfindelsen.
De følgende eksempler tjener til illustration af opfind-35 elsen og skal ikke begrænse dens omfang, men skal vise stabiliteten af peroxyopløsningerne stabiliseret i overensstemmelse med den foreliggende opfindelse. Alle dele er efter vægt, med mindre andet er angivet.

Eksempel 1 0,35 g natriumchlorid, 0,35 kaliumchlorid, 0,58 g borsyre, 0,005 g natriuroboratdecahydrat og 0,006 g diethyl entr i a- i 5 min-penta( methyl enphosphonsyre) . opløses i 80 ml .; demineraliseret vand. Der tilsættes 0,0238 g natriura-perborat og vand op til 100 ml, og pH-værdien indstilles til 7 ved tildrypning af fortyndet saltsyre eller natriumhydroxid. Denne opløsnings varmestabilitet er over 10 91%.
Eksempel 2
Fremgangsmåden ifølge eksempel 1 gentages, bortset fra, 15 at 0,35 g kaliumchlorid erstattes med 0,35 g calciumchlo-rid. Denne opløsnings varmestabilitet er over 93%.
Eksempel 3 20 0,61 g natriumchlorid, 0,50 borsyre, 0,005 g natriumborat- decahydrat og 0,006 g diethylentriamin-penta(methylenphosphonsyre) opløses i 80 ml demineraliseret vand. Der tilsættes 0,0133 g natriumperoxid (fra Mallinckrodt, kat. nr. 7864) og vand 25 op til 100 ml, og pH-værdien indstilles til 7 ved tildrypning af fortyndet saltsyre eller natriumhydroxid.
Denne opløsnings varmestabilitet er over 93%.
Sammenlioninaseksempel 30
Fremgangsmåden ifølge eksempel 3 gentages, bortset fra, at 0,0133 g natriumperoxid erstattes med 0,0138 g urinstof-hydrogenperoxid (fra Aldrich, kat. nr. 28913-2).
Denne opløsnings varmestabilitet er over 53,6%.
