Abstract

Bei Gelenk-Prothesen mit einem Gelenkpartner als Kugelkopf, insbesondere bei Hüftgelenk-Endoprothesen, wird der Kugelkopf von dem Chirurgen mit der Hand auf den Konus des Schaftes aufgesetzt und fixiert. Die Handhabung eines Kugelkopfes durch den Chirurgen mit durch Körperflüssigkeit benetzten Handschuhen ist aufgrund der polierten Oberfläche schwierig. Durch die bei der Operation benutzten Geräte und durch Rückstände auf den Handschuhen des Chirurgen kann die Oberfläche verkratzt werden. Es gibt bereits Schutzhüllen, die aber aufgrund ihrer der Kugelform angepassten Form schwierig von den Kugelköpfen zu entfernen sind. Erfindungsgemäss wird deshalb vorgeschlagen, dass auf dem Pol (12) der Schutzkappe (2) ein Aufsatz (9) in Form einer Platte (10) angeordnet ist und dass die Platte (10) eine solche Ausformung und eine solche Ausdehnung gegenüber der Schutzkappe (2) aufweist, dass sie auf der der Schutzkappe (2) zugewandten Seite (18) hintergreifbar ist.

Description

Einrichtung zur Handhabung von Kugelköpfen von Gelenk-Prothesen
Die Erfindung betrifft eine Einrichtung zur Handhabung von Kugelköpfen von Gelenk-Prothesen entsprechen dem Oberbegriff des ersten Anspruchs.
Gelenk-Prothesen, bei denen ein Gelenkpartner als Pfanne und der andere Gelenkpartner als Kugelkopf ausgebildet ist, der in der Pfanne drehbar gelagert ist, sind insbesondere als Schulter- und Hüftgelenk-Prothesen bekannt. Diese Prothesen sind in der Regel modular aufgebaut. Hüftgelenk-Endoprothesen beispielsweise bestehen aus der Pfanne, die in den Hüftknochen, und dem Schaft, der in den Oberschenkelknochen eingesetzt wird. Der Schaft hat einen Zapfen, den sogenannten Konus, auf den der Kugelkopf aufgesteckt wird. Insbesondere bei dieser modular aufgebauten Endoprothese werden zur Anpassung an den Körperbau des Patienten Implantatkomponenten unterschiedlicher Materialien und Größe miteinander verbunden. Beispielsweise werden Kugelköpfe aus einer Kobalt-Chrom-Legierung oder aus einer Aluminiumoxid-Keramik auf einen Konus aus einer Titan- oder Kobald-Chrom- Molybdän-Legierung aufgesteckt. Als Verbindungstechnik zwischen metallischen oder keramischen Kugelköpfen und dem Konus wird die Klemmsitz-Verbindung angewendet, insbesondere die konische Klemmung. Dabei wird der Kugelkopf mit einer konischen Bohrung auf den Konus gesetzt. Nach dem Aufstecken des Kugelkopfs auf den Konus erfolgt die Fixierung durch einen Schlag auf den Kugelkopf.
Wie aus der Veröffentlichung „Das Prinzip der Konus-Steckverbindung für keramische Kugelköpfe bei Hüftendoprothesen" von G. Willmann, Mat.-wiss. U. Werkstofftech. 24, Seiten 315 - 319 (1993), bekannt ist, sind alle Keramiken spröde. Bei nichtflächig eingeleiteten Belastungen sowie bei Beschädigungen der Oberfläche durch Kratzer können Spannungskonzentrationen auftreten, die zur - 2 -
Zerstörung des Kugelkopfs führen. Bei metallischen Kugelköpfen können Kratzer auftreten, die wiederum einen Abrieb in der sie umgebenden Lagerschale bewirken können.
Um die Kugelköpfe bis zum Zeitpunkt ihres Einsatzes im Operationssaal vor Beschädigungen zu schützen, ist es bekannt, sie mit einer der Kugelform angepaßten Schutzkappe zu versehen. Bei Kugelköpfen, die sich im Durchmesser nur geringfügig unterscheiden, werden Schutzkappen mit gleichem Innendurchmesser verwendet. Die Schutzkappen werden in der Regel erst im Operationssaal entfernt. Eine Schutzkappe ist in ihrem unteren Bereich in Lamellen aufgeteilt, welche den Kugelkopf bis unterhalb seines äquatorialen Durchmessers umgreifen und die Schutzkappe somit selbsttätig auf dem Kugelkopf halten. Die Kugelköpfe der Hüftgelenk-Endoprothesen werden durch den Chirurgen mit der Hand auf den Schaft aufgesetzt. Dazu muß die Schutzkappe abgezogen werden, was in dem Umfeld der Operationswunde aufgrund der durch Körperflüssigkeiten benetzten Handschuhe des Chirurgen schwierig ist. Beim Aufsetzten eines Kugelkopfs kann durch Rückstände auf den Handschuhen die polierte Oberfläche verkratzt werden. Bei metallischen Kugelköpfen kann das zu erhöhtem Abrieb in der Pfanne führen. Außerdem besteht aufgrund der Vielzahl von Instrumenten, die während einer Operation eingesetzt werden, zusätzlich die Gefahr, die Oberfläche der Kugelköpfe zu beschädigen. Eine weitere Schwierigkeit besteht bei der Operation darin, Kugelköpfe, die sich in ihrem Durchmesser nur wenig unterscheiden, sicher zu erkennen.
Es ist deshalb die Aufgabe der vorliegenden Erfindung, die Schutzkappen so zu gestalten, daß die Handhabung der Kugelköpfe während der Operation erleichtert wird, daß der Schutz der Kugelköpfe auch während des Aufsteckens auf den Konus des Schafts gewährleistet ist und daß die Möglichkeit der Unterscheidung unterschiedlicher Größen und Werkstoffe verbessert wird. - 3 -
Die Lösung der Aufgabe erfolgt mit Hilfe der kennzeichnenden Merkmale des ersten Anspruchs. Vorteilhafte Ausgestaltungen der Erfindung werden in den Unteransprüchen beansprucht.
Die erfindungsgemäße Einrichtung zur Handhabung von Kugelköpfen weist auf dem Pol der Schutzkappe, welche den Kugelkopf von seiner Herstellung bis zum Zeitpunkt des Einsetzens vor Beschädigungen schützten soll, einen Aufsatz in Form einer Platte auf. Die Platte hat eine solche Ausformung und eine solche Ausdehnung gegenüber der Schutzkappe, daß sie auf der der Schutzkappe zugewandten Seite hintergreifbar ist. Die Platte kann rund sein, mehreckig oder auch eine andere Umfangsform aufweisen.
Während eine herkömmliche Schutzkappe auf einem Kugelkopf von einem Chirurgen während der Operation mit von Körperflüssigkeiten benetzten Handschuhen nur sehr schwierigt zu entfernen ist und der Kugelkopf aufgrund seiner polierten Oberfläche schwierig zu handhaben ist, bietet die erfindungsgemäße Einrichtung zur Handhabung von Kugelköpfen die Möglichkeit, den Kugelkopf bis zu seinem Aufsetzen auf den Schaft sicher zu halten und sicher zu führen. Der Aufsatz bietet die Möglichkeit, den Kugelkopf mit seiner konischen Öffnung gezielt auf den Konus des Schaftes aufzusetzen. Die Oberfläche auf der Platte des Aufsatzes kann zusätzlich als Schlagfläche für das Fixierwerkzeug dienen, mit dem der Kugelkopf auf den Konus durch einen Schlag aufgepreßt wird. Die Platte bietet eine größere Fläche zum Auftreffen des Schlagwerkzeugs als eine gewölbte Kugeloberfläche. Weiterhin wird die empfindliche, polierte Oberfläche der Kugel vor einer Beschädigung durch das Schlagwerkzeug geschützt.
Während der gesamten Handhabung bis zum Fixieren auf dem Konus des
Schaftes ist die Oberfläche des Kugelkopfs stets vor Beschädigungen durch Operationswerkzeuge oder durch Verschmutzungen geschützt. Wenn der
Kugelkopf auf dem Konus des Schaftes fixiert ist, läßt sich mit Hilfe des - 4 -
hintergreifbaren Aufsatzes die Schutzkappe leicht von dem Kugelkopf abziehen. Während des Abziehens spreizen sich die Lamellen der Schutzkappe und geben den Kugelkopf frei. Der Werkstoff der Einrichtung kann beispielsweise ein schlagfester, zäher Kunststoff sein.
Die Platte ist in vorteilhafter Ausgestaltung der Erfindung so angeordnet, daß sie senkrecht zur verlängerten Verbindungslinie vom Mittelpunkt des Kugelkopfs zum Pol der Schutzkappe steht. Dadurch ist eine Ausrichtung der Einrichtung zur Handhabung von Kugelköpfen zum Konus des Schafts möglich. Die Oberfläche der Platte kann eben oder leicht konvex gewölbt sein. Dadurch kann sie auch als Schlagfläche für das Fixierwerkzeug dienen.
In einer weiteren vorteilhaften Ausgestaltung der Erfindung kann zwischen der Schutzkappe und der Platte des Aufsatzes ein Hals angeordnet sein. Durch diesen Hals, der beispielsweise eine Länge von etwa 1 cm haben kann, wird die Möglichkeit des Hintergreifens und damit die Handhabung noch weiter erleichtert. Außerdem bietet der Hals die Möglichkeit einer gezielten Einleitung der Fixierungskraft in die Kugel. Beim Aufsetzen des Kugelkopfs auf den Konus des Schaftes wird die durch den Fixierungsschlag aufgebrachte Kraft genau in die Längsrichtung des Konus eingeleitet, wenn der Hals genau in Längsrichtung des Konus ausgerichtet wird.
Der Hals kann auch in Form eines Stiels verlängert sein und beispielsweise eine Länge bis zu 100 mm aufweisen. Eine solcherart ausgebildete Einrichtung zur Handhabung von Kugelköpfen kann beispielsweise bei Kugelköpfen eingesetzt werden, die nicht mit einer Schutzkappe bedeckt geliefert worden sind. Zur Vorbereitung auf die Operation können diese Kugelköpfe in eine solche Einrichtung zur Handhabung eingesetzt werden. Der Stiel dient dann dem Chirurgen als Ausricht- und Einsetzhilfe. Aufgrund der Länge des Stiels bietet diese Einrichtung eine sehr gute Möglichkeit des Positionierens und Aufsetzens auf den Schaft. - 5 -
Ein Kugelkopf kann leicht auf den Konus eines Schaftes aufgesetzt werden, wenn die Mittellinie der konischen Öffnung des Kugelkopfes so ausgerichtet ist, daß sie durch den Pol der Schutzkappe der erfindungsgemäßen Einrichtung zur Handhabung verläuft. Wenn der Aufsatz einen Hals oder einen Stiel für die Platte aufweist, können diese in Richtung des Konus ausgerichtet werden. Dann fluchten der Konus und die Öffnung in dem Kugelkopf und dieser braucht nur noch auf den Konus aufgeschoben werden.
In einer weiteren Ausgestaltung der Erfindung kann der Durchmesser der Platte jeweils in einem bestimmten Verhältnis zum Durchmesser des Kugelkopfs stehen. Dadurch ist es möglich, bereits aufgrund eines zuvor in Bezug auf den Durchmesser des Kugelkopfs festgelegten Durchmessers der Platte einen Hinweis auf den Durchmesser des Kugelkopfs zu geben. Für den Chirurgen wird dadurch die Unterscheidung von Kugelköpfen unterschiedlichen Durchmessers während der Operation wesentlich erleichtert.
Die Unterscheidungsmöglichkeit der Kugelköpfe kann in einer weiteren Ausgestaltung der Erfindung noch dadurch erhöht werden, daß die Oberfläche der Platte, die der Schutzkappe abgewandt ist, Träger einer Kennzeichnung ist. Diese Kennzeichnung kann eine Beschriftung, eine Gravur oder eine farbliche Kennzeichnung sein. Die Kennzeichnung kann beispielsweise Auskunft geben über den Werkstoff des Kugelkopfs, seinen Durchmesser, die Art und die Größe seiner konischen Bohrung sowie die Chargenbezeichnung und den Hersteller. Für den Chirurgen würde damit die Unterscheidungsmöglichkeit noch besser und noch sicherer.
An Hand von Ausführungsbeispielen wird die Erfindung näher erläutert.

In Figur 1 ist mit 1 eine erfindungsgemäße Einrichtung zur Handhabung von Kugelköpfen in perspektivischer Ansicht vergrößert dargestellt. Die Schutzkappe 2, die entsprechend einer nochmals vergrößerten Darstellung in Figur 2 den Kugelkopf 4 bedeckt, ist etwa ab dem äquatorialen Durchmesser 3 des Kugelkopfs 4 in gleich breite Lamellen 5 aufgeteilt, die jeweils durch Schlitzte 6 voneinander getrennt sind. Es sind im vorliegenden Ausführungsbeispiel acht gleich breite Lamellen 5 über den Umfang der Schutzkappe 2 verteilt angeordnet, die den Kugelkopf 4 über den äquatorialen Durchmesser 3 hinaus umgreifen. Jede der Lamellen 5 wird durch einen Steg 7 verstärkt. Der Steg 7 hat außerdem die Aufgabe, daß jeweils nur eine geringe Fläche der Lamellen 5 an der Oberfläche des Kugelkopfs 4 anliegt. Dadurch ist die Auflagefläche der Schutzkappe 2 und auch die Reibung, die auf die Kugeloberfläche ausgeübt wird, gering. Die Lamellen 5 weisen an ihrem unteren Ende jeweils einen ringförmigen Ansatz 8 auf. Diese ringförmigen Ansätze 8 erleichtern das Aufstecken der Schutzkappe 2 auf einen Kugelkopf.
Die Schutzkappe 2 trägt erfindungsgemäß einen Aufsatz 9 in Form einer Platte
10. Der Umfang dieser hier kreisförmigen Platte 10 kann auch mehreckig sein oder jede andere Form aufweisen. Der Durchmesser 19 der Platte 10 kann so gewählt werden, daß er in Bezug gesetzt werden kann zum Durchmesser 3 des - 7 -
Kugelkops 4. Je kleiner der Durchmesser 19 ist, desto kleiner ist beispielsweise der Durchmesser des Kugelkopfs.
Figur 2 zeigt einen Schnitt durch die erfindungsgemäße Einrichtung 1 zur Handhabung von Kugelköpfen nach Figur 1. Die Schutzkappe 2 ist auf einen Kugelkopf 4 aufgesetzt, der geschnitten und in der linken Hälfte der Schutzkappe weggelassen ist. An Hand des Schnittbilds ist ersichtlich, daß im vorliegenden Ausführungsbeispiel die Platte 10 mit ihrer ebenen Oberfläche 14 senkrecht, im rechten Winkel 15, zur verlängerten Verbindungslinie 11 vom Mittelpunkt 13 des Kugelkopfs 4 bis zum Pol 12 der Schutzkappe 2 steht. Die Verbindungslinie 11 zwischen dem Pol 12 der Schutzkappe 2 und dem Mittelpunkt 13 des Kugelkopfs 4 ist gleichzeitig die Mittellinie der konischen Bohrung 16 im Kugelkopf 4 zur Aufnahme des Konus auf dem Schaft. Für eine ideale Voraussetzung zum Aufsetzen auf den Konus verläuft die Mittellinie der konischen Bohrung 11 durch den Pol 12 der Schutzkappe 2. Dadurch wird das Aufsetzen des Kugelkopfs auf den Konus und die Fixierung mittels eines Schlags durch ein Schlagwerkzeug auf die Platte 10 wesentlich erleichtert. Fertigungstechnisch bedingt besitzt der Aufsatz 9 eine Öffnung 17, welche in Richtung der Mittellinie 11 , der Verbindungslinie zwischen Mittelpunkt 13 des Kugelkopfs 4 sowie dem Pol 12 der Schutzkappe 2 verläuft.
Die Platte 10 weist eine solche Ausdehnung 19 gegenüber der Schutzkappe 2 auf, daß sie auf der der Schutzkappe 2 zugewandten Seite 18 hintergreifbar ist. Diese Ausdehnung kann beispielsweise etwa dem größten Außendurchmesser der Schutzkappe 2 entsprechen.
Figur 3 zeigt eine Aufsicht auf die Oberfläche 14 der Platte 10. Wie ersichtlich, ist die Oberfläche 14 Träger einer Kennzeichnung 20. Die Oberfläche 14 kann beispielsweise mit einer Beschriftung versehen sein, die Auskunft gibt über den Durchmesser, im vorliegenden Fall 32 mm, über den Werkstoff, im vorliegenden Fall Al20 , und über den Konustyp, im vorliegenden Fall ein Eurokonus 12/14. - 8 -
Die Beschriftung kann aufgedruckt oder eingeprägt sein. Es können auch farbliche Markierungen oder eine Durchfärbung des Werkstoffs, vorzugsweise eines schlagfesten Kunststoffs, vorgesehen sein.
Das Ausführungsbeispiel in Figur 4 zeigt eine Einrichtung 101 zur Handhabung von Kugelköpfen, die sich vom vorhergehenden Ausführungsbeispiel dadurch unterscheidet, daß beim Aufsatz 109 auf der Schutzkappe 2 zwischen der Schutzkappe 2 und der Platte 110 ein Hals 121 angeordnet ist. Gegenüber dem vorhergehenden Ausführungsbeispiel unverändert gebliebene Merkmale sind mit denselben Bezugsziffern bezeichnet.
Durch den größeren Abstand zwischen der Platte 110 und der Schutzkappe 2 ist die Seite 118 der Platte 110 des Aufsatzes 109 wesentlich besser hintergreifbar. Der Durchmesser 119 der Platte 110 kann deshalb beispielsweise geringer sein als im vorhergehenden Ausführungsbeispiel.
Im Ausführungsbeispiel nach Figur 5 weist die Einrichtung 201 zur Handhabung von Kugelköpfen zwischen der Schutzkappe 2 und der Platte 210 einen Hals auf, der in Form eines Stiels 221 verlängert ist und der bis zu 100 mm lang sein kann. Mit einer solchen Handhabungshilfe ist das Positionieren und Einsetzen der Kugelköpfe besonders leicht und genau möglich.

Claims

- 9 -Patentansprüche
1. Einrichtung zur Handhabung von Kugelköpfen von Gelenk-Prothesen, bei denen die Oberfläche des jeweiligen Kugelkopfs vor dem Aufsetzen auf den Konus eines Schafts bis über den äquatorialen Durchmesser hinaus von einer Schutzkappe bedeckt ist, so daß die Schutzkappe selbsttätig auf dem Kugelkopf gehalten und ohne Werkzeug abziehbar ist, dadurch gekennzeichnet, daß auf dem Pol (12) der Schutzkappe (2) ein Aufsatz (9, 109, 209) in Form einer Platte (10, 110, 210) angeordnet ist und daß die Platte (10, 110, 210) eine solche Ausformung und eine solche Ausdehnung gegenüber der Schutzkappe (2) aufweist, daß sie auf der der Schutzkappe (2) zugewandten Seite (18, 118, 218) hintergreifbar ist.
2. Einrichtung nach Anspruch 1 , dadurch gekennzeichnet, daß die Platte (10, 110, 210) senkrecht auf der verlängerten Verbindungslinie (11 ) vom
Mittelpunkt (13) des Kugelkopfs (4) zum Pol (12) der Schutzkappe (2) steht.
3. Einrichtung nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß zwischen der Schutzkappe (2) und der Platte (1 10, 210) ein Hals (121 , 221 ) angeordnet ist.
4. Einrichtung nach Anspruch 3, dadurch gekennzeichnet, daß der Hals in Form eines Stiels (221 ) verlängert ist.
5. Einrichtung nach Anspruch 4, dadurch gekennzeichnet, daß die Länge des Stiels (221) bis zu 100 mm beträgt. 10
6. Einrichtung nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, daß die Verbindungslinie (11 ) zwischen dem Mittelpunkt (13) des Kugelkopfs (4) und dem Pol (12) der Schutzkappe (2) gleichzeitig die Mittellinie der konischen Bohrung (16) im Kugelkopf (4) ist .
7. Einrichtung nach einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, daß der Durchmesser (19, 119, 219) der Platte (10, 110, 210) in einem vorgegebenen Verhältnis zum Durchmesser (3) des Kugelkopfs (4) steht.
8. Einrichtung nach einem der Ansprüche 1 bis 7, dadurch gekennzeichnet, daß die Oberfläche (14, 114, 214) der Platte (10, 110, 210), die der Schutzkappe (2) abgewandt ist, Träger einer Kennzeichnung (20) ist.
