Description

Anclaje para sujeción en un orificio.
Antecedentes de la invención
La presente invención se refiere a dispositivos de anclaje, en particular a anclajes para el aseguramiento en diámetros interiores, en primeros miembros, y lo cual permitirá la sujeción de segundos miembros a los primeros miembros. En los campos médico y quirúrgico, hay una particular necesidad para un dispositivo que pueda ser acoplado a un primer objeto, el cual es entonces insertado en un orificio en el segundo objeto, para de ese modo trabar rápidamente el primer y el segundo dispositivos juntos. Por ejemplo, hay una necesidad para un semejante dispositivo que pueda conectar prótesis a hueso, o hueso a hueso. Hay además una necesidad de un semejante dispositivo de conexión, que sea esencialmente hueco en el interior, para así permitir la colocación de partes de tejido, hueso o material artificial dentro del dispositivo, tal como varillas, pernos, válvulas, suturas, etc.
También hay necesidad de un dispositivo que pueda fijar dos objetos a la vez de un modo rápido y simple, por ejemplo, por compresión, para así fijar un componente dentro del dispositivo y simultáneamente a un medio, por ejemplo, hueso. Hay una necesidad de un tal dispositivo para fijar a la vez dos componentes, por ejemplo hueso y hueso, o tejido y hueso, o tejido y tejido, y que puede dejarse hueco para permitir que un fluido pase a través del dispositivo insertado. Hay, además, una necesidad de un dispositivo que pueda dirigir o impedir el flujo de fluido pasando a través de del dispositivo. Tal dispositivo podría ser usado en el campo médico para conectar hueso a hueso, por ejemplo para reparar fracturas o en cirugía espinal. Tal dispositivo podría ser usado para conectar otros materiales a hueso, por ejemplo, prótesis a hueso. Este podría ser también usado en prótesis de cadera, prótesis de rodilla, o como tapas de hueso e implantes dentales.
Hay, además, necesidad de un semejante dispositivo de conexión, que permita la conexión de tejido a hueso. Este podría ser usado para reparación de ligamentos, reparación de menisco y reparación de tejidos blandos. Existe también la necesidad de un dispositivo semejante que pueda proporcionar conexiones de tejido a tejido, por ejemplo, resecciones de hígado, pulmón y bazo.
Hay también una necesidad de tal dispositivo que permita conectar materiales a tejido. Semejante dispositivo podría ser usado en aplicaciones relacionadas con estenosis para proporcionar pasadizos limpios en tractos esofágicos, de próstata y coronarios a través del centro hueco del dispositivo, como filtros y válvulas para bloquear y embolizar, y como presas, por ejemplo bloqueantes pancreáticos.
Hay además una necesidad para un dispositivo de conexión rápida que pueda ser usado en las industrias de cierres y construcción, y que permita la conexión de primeros objetos, en diámetros interiores localizados en segundos objetos.
Son conocidos diversos dispositivos de anclaje, conexión rápida y para la fijación en orificios ciegos. Por ejemplo, en las patentes U.S. de números 2.143.086, 2.100.570, y 5.161.916, es asegurado un tornillo en un orificio mediante la acción de un miembro expansible, por ejemplo un elemento tubular alargado mediante un miembro cónico. En las patentes U.S. número 1.247.621 y FR 1.368.021, es asegurado un tornillo en un orificio, mediante la acción de un miembro expansible comprendiendo alas expansibles. En la patente U.S número 4.293.259, proporcionan sujeción elementos de cierre expansibles que ajustan dentro de una muesca.
En cada una de las patentes anteriores, un elemento tornillo hace, generalmente a través de la intermediación de un elemento cónico que puede formar parte del tornillo o estar separado del tornillo, que los miembros expansibles se expandan hasta ceñirse a la superficie interior del orificio, para proporcionar el acoplamiento.
En otra patente, la patente U.S. Nº 5.094.563, son proporcionados dos espaciadores interdigitados que permiten el aseguramiento de un elemento a una pared que tiene una construcción hueca. En esta patente, los dedos interdigitados son usados para proporcionar soporte entre las superficies de la pared hueca, para impedir el colapso de la pared.
La patente U.S. Nº Re. 34.293 muestra un método de acoplamiento por ligamentos que trabaja bajo un principio similar a los cierres de construcción descritos mas arriba, por ejemplo empleando un elemento expansible que es expandido mediante un elemento cónico operado por un tornillo. La patente U.S. Nº 5.037.422 revela un dispositivo de anclaje de huesos para asegurar suturas al hueso. El dispositivo está conformado cónicamente con estriaciones en la superficie externa. El dispositivo es forzado en un entrante en el hueso, y la forma cónica y las estriaciones lo mantienen asegurado en el hueso.
Sin embargo, ninguno de los dispositivos del arte previo proporciona un dispositivo de cierre simple, rápido y seguro, que sea adaptable especialmente en el campo médico, para fijar dos objetos tal como hueso, tejido u objetos extraños, juntos.
Sumario de la invención
Es un objetivo de la presente invención proporcionar un dispositivo de anclaje para aseguramiento en un orificio en un medio.
También es un objetivo de la presente invención, proporcionar un dispositivo de conexión para conectar un primer objeto a un orificio en un segundo objeto, y particularmente un semejante dispositivo de conexión que sea de uso simple y actuación rápida.
Es todavía un objetivo más de la presente invención, proporcionar un semejante dispositivo de conexión que sea adecuado para los campos médico y quirúrgico.
Es todavía un objetivo más de la presente invención, proporcionar un dispositivo que sea adecuado para usar con hueso, tejido o objetos extraños, por ejemplo materiales protésicos, o cualesquiera combinaciones de estos.
Es todavía un objetivo más de la presente invención, proporcionar un sujetador de rápida conexión para la industria de la construcción y la industria de cierres en general.
Es todavía un objetivo más de la presente invención, proporcionar un semejante dispositivo de conexión rápida que también permita la implantación de suturas en tejido, o hueso, u otras sustancias biológicas en las que las suturas no puedan ser aplicadas directamente a la sustancia.
El anterior y otros objetivos de la presente invención son logrados mediante un anclaje para sujeción en un orificio, en un medio, comprendiendo una primera, generalmente hueca, sección cilíndrica que tiene una primera pluralidad de dedos espaciados, dirigidos longitudinalmente; una segunda, generalmente hueca, sección cilíndrica que tiene una segunda pluralidad de dedos espaciados, longitudinalmente dirigidos; estando las secciones primera y segunda dispuestas en oposición la una a la otra, con los dedos de cada sección estando interdigitados con los dedos de la otra sección; estando las secciones primera y segunda adaptadas para recibir una fuerza de compresión que mueve las dos secciones relativamente una hacia la otra, mediante lo que el menos alguno de los dedos se mueve exteriormente de forma que asegura las dos secciones en el orificio, en el medio.
Otras características y ventajas de la presente invención se harán manifiestas a partir de la siguiente descripción de la invención, que remite a los dibujos anexos.

Descripción detallada de la invención
Con referencia ahora a los dibujos, la figura 1 muestra una primera realización del dispositivo de fijación de acuerdo con la presente invención. La figura 1 muestra el dispositivo previamente a su uso. El dispositivo incluye dos secciones 10 y 20. En el dispositivo mostrado en la figura 1, las dos secciones 10 y 20 son idénticas, pero no es necesario que sean idénticas. En otras realizaciones mostradas en los dibujos, las dos secciones no son necesariamente idénticas.
Las dos secciones 10 y 20 tienen cada una un grupo de dedos 15 que interdigitan con los dedos de la otra sección. Las secciones 10 y 20 pueden estar hechas de cualquier material adecuado, duro o blando, dependiendo de la aplicación. El material debe tener un grado de flexibilidad o deformabilidad requerido, puesto que debe permitirse a los dedos 15 curvarse para llevar a cabo la función de fijación, como será descrito con mas detalle mas abajo. Materiales adecuados pueden ser metales biocompatibles si son usados en el campo médico, plásticos o cualquier metal o plástico que tenga la flexibilidad o deformabilidad requerida si son usados como una sujeción general de construcción.
Para usar el dispositivo, un primer objeto, por ejemplo, un cilindro o varilla 30, según muestra la figura 14, es situado dentro de la abertura 22 de la sección 10 y fijado a la abertura 22 de la segunda sección 20. El objeto 30 puede estar unido a la sección 20 mediante cualquier medio adecuado, dependiendo de la aplicación y la composición de los elementos 20 y 30. Por ejemplo, técnicas adecuadas de fijación pueden comprender encolado, o soldadura. Preferentemente, la sección 10 está dimensionada de forma que el objeto 30 a ser sujeto ajusta de modo preciso dentro de la abertura 22. Las secciones interdigitadas 10 y 20 que rodean el objeto 30, son así situadas dentro del orificio 40, en el objeto 50 dentro del cual el objeto 30 va a ser asegurado. Preferentemente, el diámetro exterior de las secciones 10 y 20 está dimensionado de forma que ajusta de modo preciso dentro del diámetro interno 40. Un dispositivo no mostrado en detalle, pero mostrado esquemáticamente, por ejemplo, en 60 en la figura 14, y que puede rodear al objeto 30 asegurado en el interior de las secciones 10 y 20, es además usado para comprimir las dos secciones 10 y 20 juntas. Por ejemplo, la sección 10 puede ser forzada hacia dentro de la sección 20. Si el objeto 50 dentro del cual el anclaje de la invención va a ser asegurado, no es lo suficientemente resistente como para admitir una fuerza axial, es deseable que al mismo tiempo el dispositivo 60 presione la sección 10 hacia dentro de la sección 20, y sostenga al objeto 30 contra el movimiento. Esto impide que la sección 20, que ha sido asegurada al objeto 30 (por ejemplo mediante encolado o soldadura), se mueva y ejerza así una fuerza axial sobre el objeto 50. La figura 2 muestra el dispositivo después de que las dos secciones 10 y 20 han sido movidas relativamente una hacia la otra.
La fuerza de compresión tiene el efecto de forzar hacia fuera los dedos interdigitados 15 en cada una de las secciones 10 y 20. El efecto de forzar los dedos 15 hacia fuera es inmovilizar las dos secciones 10 y 20 dentro del orificio 40. Las puntas de los dedos extendiéndose en direcciones opuestas, con rozamiento, aseguran al dispositivo en el orificio contra movimientos longitudinales hacia fuera y hacia adentro. El mismo tiempo que las secciones 10 y 20 son inmovilizadas en el orificio 40, el efecto de compresión fuerza a las aberturas 22 a un diámetro disminuido, causando que el objeto 30 esté también sujeto de manera segura a la sección 10, y por lo tanto dentro del diámetro 40 en el objeto 50.
Para facilitar el movimiento hacia fuera de los dedos 15, están preferentemente hechos de forma que tienen porciones terminadas en punta 17 según se muestra mas claramente en las figuras 3, 4, y 5. Adicionalmente, entre los dedos, las secciones 19 están también terminadas en punta, según se muestra en la figura 5, para facilitar el movimiento por fuera de los dedos 15 cuando las secciones 10 y 20 son forzadas juntas.
La figura 6 muestra una segunda realización de la presente invención. En esta realización, las dos secciones 10' y 20' son diferentes. La sección 10' es similar a la sección 10 de la figura 1, con la excepción de que los ahusamientos 19' están invertidos, por ejemplo están ahusados hacia adentro, no hacia fuera. La sección 20', es diferente además, al tener dedos 15' que tienen extremos 17' que ahusan opuestos a los mostrados, por ejemplo en la figura 5. Por añadidura, la sección 10' es también diferente a la sección 10 de la figura 1, en que las secciones 19' ahusan opuestas a las mostradas, por ejemplo en las figuras 1 ó 5. La razón para esto, es permitir que los dedos 15' que interdigitan entre los dedos 15 de la sección 10' se flexionen hacia adentro, en vez de hacia fuera. Esto permite un aseguramiento mayor incluso del objeto, por ejemplo el objeto 30, en la abertura 22 a través del centro de las secciones 10' y 20'.
Aunque la realización de la figura 1 y la realización de la figura 6 son diferentes en varios aspectos, estas cumplen esencialmente el mismo propósito de asegurar, o anclar, un objeto 30 en un orificio 40 en otro objeto 50. El dispositivo 1 proporciona un grado menor de engranaje por fricción con el objeto 30, porque los dedos 15 flexionan todos hacia fuera, pero proporciona un grado mayor de rigidez torsional, porque los dedos están dispuestos de forma que sus caras son adyacentes. En el dispositivo mostrado en la figura 6, aunque los dedos están interdigitados, las caras de los dedos no encajan en toda su longitud, una vez que las dos secciones 10' y 20' han sido forzadas juntas. Esto es mostrado en la figura 8.
La figura 8 muestra la segunda realización, de acuerdo con la figura 6, después de que las dos secciones 10' y 20' han sido movidas una hacia la otra, para forzar a los dedos 15 hacia fuera y a los dedos 15' hacia adentro.
Como se muestra en las figuras 3 y 4, un manguito 70 puede además ser proporcionado en el interior 22, entre las secciones 10 y 20 y el objeto 30 localizado en la abertura 22. El manguito 70 puede tener una cola 72 provista en este, que puede usarse para conseguir el movimiento relativo de las dos secciones 10 y 20 la una hacia la otra, y por tanto la fijación del dispositivo.
La figura 9 muestra una realización adicional de la invención, teniendo un número mayor de dedos interdigitados 15 en cada sección 10'' y 20''. En todos los demás aspectos, este dispositivo es similar al mostrado en la figura 1, pero proporciona un grado mayor de fijación por rozamiento y resistencia al movimiento torsional.
La figura 10 muestra otra realización más, de acuerdo con la presente invención, que comprende tres secciones 100, 110 y 120. Cuando las tres secciones 100, 110 y 120 son comprimidas juntas colectivamente, los dedos 102, 112, 114 y 122 se mueven todos hacia fuera o hacia adentro, o en cualquier combinación, para lograr cualquier grado deseado de aseguramiento. Por ejemplo, los dedos 122 y 102 se pueden mover hacia fuera mientras que los dedos 112 y 114 pueden moverse hacia adentro. El movimiento concreto de los dedos es logrado por medio de que los extremos de los dedos y de las áreas entre los dedos son terminados en punta, según se ha discutido con respecto a la realización de la figura 1.
Las figuras 11-13 muestran una realización más, de acuerdo con la presente invención, que permite asegurar suturas a un objeto, por ejemplo tejido biológico, al cual de otro modo no podrían por lo general ser aseguradas fácilmente. El dispositivo de acuerdo con las figuras 11-13 comprende una sección 150 y una sección 160. Las secciones 150 y 160 tienen, respectivamente, dedos interdigitados 155 y 165, respectivamente. Las suturas 170 son aseguradas a un extremo inferior de la sección 160, según se muestra en 172. Las suturas son suministradas a través del centro de la sección 160, y a través del centro de la sección 150. Puede ser proporcionado un tubo de inserción 180, el cual es dispuesto a través del centro de la sección 150. Previamente a su uso, las dos secciones 150 y 160 están dispuestas como se muestra en la figura 12, con sus dedos 155 y 165 encajando estrechamente, pero no estando flexionado ninguno de los dedos fuera de los volúmenes definidos por las secciones cilíndricas 150 y 160. El tubo 180 forma un encaje ajustado en, al menos, el interior de la sección 150, aunque puede también constituir un encaje ajustado en el interior de la sección 160. No obstante, esto no es necesario porque los dedos interdigitados 155 mantendrán la sección 160 fijada a la sección 150, al menos para el propósito de la inserción de las secciones 150 y 160 en una abertura.
Las secciones 150 y 160 son después insertadas en la abertura, en el miembro al cual las suturas van a ser sujetas en la condición en la que son mostradas en la figura 12. Después de la inserción en la abertura, una fuerza de tracción adecuada puede ser ejercida en las suturas tirando de ellas contra el tirador 190. Esto causará que la sección 160 se mueva hacia la sección 150, causando que los de dedos 155 y 165 se muevan hacia fuera, como se muestra en la figura 13, fijando de este modo las secciones 10 y 20 en la abertura de la sustancia biológica. Alternativamente otro dispositivo 200, que se ajusta alrededor del diámetro exterior del tubo 180, puede ser forzado contra la sección 150 empujándolo hacia abajo, mientras se tira hacia arriba de las suturas 170, causando así que las dos secciones 150 y 160 se muevan relativamente una respecto de la otra.
Después de que las dos secciones 150 y 160 son movidas relativamente una hacia la otra, el tubo 180 es retirado de la sección 150, por ejemplo manteniendo presión sobre el dispositivo 200, de forma que la sección 150 es mantenida en posición. Las suturas 170 han sido ahora ancladas, o fijadas, en la abertura, en la sustancia biológica, por la acción de los dedos 155 y 165 de las secciones 150 y 160, fijándose ellas mismas en la abertura. Las suturas 170 pueden ahora ser usadas para asegurar otro objeto, por ejemplo otro tejido, a la materia biológica.
Aunque la presente invención ha sido descrita con relación a realizaciones particulares de esta, muchas otras variaciones y modificaciones, y otros usos, serán manifiestos para aquellos cualificados en el arte.
Por tanto, la presente invención no debe ser limitada por las revelaciones especificadas aquí, sino solo por las reivindicaciones anexas.

Claims (16)

1. Un anclaje médico para fijación en un orificio (40) en un medio (50) comprendiendo:
una primera, generalmente hueca, sección biocompatible (10) que tiene una primera pluralidad de dedos espaciados, longitudinalmente dirigidos (15) y una abertura (22);
una segunda, generalmente hueca, sección biocompatible (20) que tiene una segunda pluralidad de dedos espaciados, longitudinalmente dirigidos (15) y una abertura (22);
un primer objeto alargado (30, 180), estando situado dentro de la abertura de la primera sección, y acoplado con la segunda sección a través de la abertura de la segunda sección;
estando, las secciones primera y segunda, arregladas opuestas una a la otra, con los dedos de cada sección estando interdigitados con los dedos de la otra sección; caracterizado porque
las secciones primera y segunda están adaptadas para recibir una fuerza de compresión, moviendo las dos secciones relativamente la una hacia la otra, sin ningún movimiento rotacional, mediante lo que al menos alguno de los dedos puede moverse hacia fuera de modo que asegura las dos secciones en el orificio, en el medio (50).
2. El anclaje médico según la reivindicación 1, en donde ambas pluralidades de dedos pueden moverse hacia fuera, en respuesta a la fuerza de compresión.
3. El anclaje médico según la reivindicación 1, en donde una pluralidad de dedos puede moverse hacia fuera, y la otra pluralidad de dedos puede moverse hacia adentro, en respuesta a la fuerza de compresión.
4. El anclaje médico según la reivindicación 1, en donde los dedos de cada pluralidad de dedos tienen puntas (17), estando las puntas ahusadas, para facilitar el movimiento de los dedos hacia fuera y hacia dentro.
5. El anclaje médico según la reivindicación 4, donde cada sección tiene una porción de cerco (19) entre los dedos (15), estando la porción de cerco ahusada para facilitar el movimiento de los dedos hacia fuera y hacia dentro.
6. El anclaje médico según la reivindicación 1, comprendiendo además medios (60) para causar la fuerza de compresión.
7. El anclaje médico según la reivindicación 6, donde los medios para causar la fuerza de compresión comprenden un manguito (200) ajustado dentro de las dos secciones, teniendo un tope dirigido hacia fuera para engranar una de las dos secciones mediante lo cual, cuando una fuerza es aplicada al manguito en la dirección de las secciones, el tope fuerza las dos secciones relativamente una hacia la otra.
8. El anclaje médico según la reivindicación 1, comprendiendo además un miembro (180) insertado en el centro de las mencionadas dos secciones para fijación dentro del orificio, en el medio.
9. El anclaje médico según la reivindicación 1, en donde las dos secciones (10, 20;150, 160) comprenden un metal biocompatible.
10. El anclaje médico según la reivindicación 1, en donde las dos secciones (10, 20;150, 160) comprenden un plástico biocompatible.
11. El anclaje médico según la reivindicación 1, comprendiendo además suturas (170) conectadas a al menos una de las dos secciones.
12. El anclaje médico según la reivindicación 11, comprendiendo además un tubo hueco (180) insertado en el centro de al menos una de las secciones, estando las suturas enhebradas a través de los centros de ambas secciones y a través del interior hueco del tubo, y comprendiendo además medios (200) cooperando con el tubo y al menos una de las secciones para mover las secciones relativamente una hacia la otra para causar la fuerza de compresión.
13. El anclaje médico según la reivindicación 12, donde los medios par causar la fuerza de compresión comprenden un objeto (200) dispuesto alrededor del tubo para engranar con al menos una de las secciones, y al cual la fuerza de compresión puede ser aplicada.
14. El anclaje médico según la reivindicación 1, comprendiendo al menos una sección intermedia (110) entre las primera (100) y segunda (120) secciones, teniendo la sección intermedia pluralidades primera y segunda de dedos extendiéndose longitudinalmente (112) para engranar con las respectivas pluralidades de los dedos (122) en las primera y segunda secciones.
15. El anclaje médico según la reivindicación 8, comprendiendo además medios (180) para fijar el miembro insertado en el centro de las dos secciones, con la más distal de las dos secciones.
16. El anclaje médico según la reivindicación 15, comprendiendo además medios (200) para causar la fuerza de compresión, comprendiendo un dispositivo forzando una más próxima de las dos secciones hacia una más distal de las dos secciones.
