Description

Banda quirúrgica para tratar la incontinencia urinaria femenina.
La presente invención se refiere un instrumento quirúrgico para tratar la incontinencia urinaria femenina del tipo descrito en el documento WO-A-9003766, que comprende un vástago que tiene un mango en un extremo del mismo, un elemento curvado similar a una aguja que está construido para ser conectado con el vástago para formar una parte curvada.
El documento WO-A-9606567, que forma parte de la técnica anterior a tenor del artículo 54(3) de la CEP, da a conocer un dispositivo de incontinencia quirúrgico que permite aliviar la incontinencia urinaria femenina mientras que restaura la contingencia fijando dos agujas curvadas a una banda que está diseñada para quedar implantada permanentemente dentro del tejido, entre la pared vaginal y la pared abdominal de un paciente, reforzando de dicha forma el tejido requerido para restaurar la incontinencia urinaria. El instrumento quirúrgico según la presente solicitud es una mejora respecto a dicho instrumento, en el que la banda comprende una malla encerrada por una cubierta delgada de plástico, de forma que la inserción se facilita mientras que se evita la irritación o el daño al tejido corporal.
La invención se explicará con más detalle haciendo referencia a los dibujos adjuntos, que desvelan el instrumento quirúrgico según la invención, y en los que:
La figura 1 es una vista lateral del instrumento quirúrgico según invención.
La figura 2 es una vista en planta del instrumento quirúrgico.
La figura 3 es una vista lateral despiezada ordenadamente de una de las agujas y la banda y la manguera contraible para ser conectada con dicha aguja.
La figura 4 es una vista lateral del aguja de la figura 3 con la banda conectada entre ambas.
La figura 5 es una vista en corte axial fragmentario, aumentado de tamaño, de un acoplamiento del instrumento para conectar una aguja intercambiable del mismo.
La figura 6 es una vista lateral de dos agujas y una banda que interconecta dichas agujas.
En la descripción que sigue, se ha usado los mismos números de referencias que en el documento WO-A-9606567, para detalles correspondientes del instrumento.
El instrumento quirúrgico comprende un vástago 10 tubular cilíndrico que tiene en un extremo del mismo un mango 11. En el otro extremo del vástago se encuentra un casquillo 14. Un eje cilíndrico 15 está montado rotativamente en el vástago y se puede rotar manualmente por medio de un pomo 16 montado en un extremo del eje. El otro extremo del eje forma una parte cilíndrica 17, figura 5, de un diámetro exterior menor que el eje, que une una parte 18 que tiene unas roscas externas, una parte 19 de extremo lisa y de un diámetro reducido adicionalmente que une la parte roscada 18, formando la parte de extremo 19 un pasador guía en dicho otro extremo del eje. Unas partes 18 y 19 están recibidas en la parte de casquillo 14 que se proyecta desde el vástago. El instrumento quirúrgico descrito hasta ahora está de acuerdo con el instrumento desvelado en el documento de patente WO-A-9606567, excepto que la parte distal 14' del casquillo 14 está aplanada desde los opuestos (véanse las figuras 1 y 2), de forma que la sección transversal de dicha parte de extremo es no circular.
El instrumento quirúrgico incluye también una aguja 21 intercambiable y desechable que en un extremo de la misma está fijada al vástago por un extremo de la aguja y se extiende sobre sustancialmente un cuarto de un círculo hacia el otro extremo libre del mismo con objeto de seguir sustancialmente el perfil del pubis entre la vagina y la pared abdominal. La aguja tiene una sección transversal circular uniforme y tiene una superficie exterior lisa, preferentemente pulida. En el extremo libre de la misma, la aguja forma un punto 22 que está terminado por una parte cónica.
Para la fijación del aguja 21 al vástago 10, la aguja forma en dicho extremo de la misma una parte recta 30 que es cilíndrica pero que tiene unas caras planas 31 pulidas sobre dicha pieza de dicha aparte 30, extendidas desde el extremo adyacente del aguja, que se recibirán por la parte de casquillo 14'. La aguja debería orientarse en una posición rotacional predeterminada en relación con el vástago, y más particularmente, debería proyectarse con ángulos rectos respecto del plano del mango 11. Esta posición rotacional se asegura por la forma no circular de la parte de casquillo 14', y la parte de extremo del aguja tiene las caras planas 31 que encajan dentro del aparte de casquillo 14'. La parte de extremo de la aguja que tiene las caras planas 31 se une al cuerpo de la aguja sobre una parte cónica 32 que se ahusa hacia un reborde 33.
Un agujero ciego axial se extiende desde la superficie de extremo de la aguja, teniendo dicho agujero una parte roscada 23 y hacia dentro de la misma una parte cilíndrica 24 más estrecha. El pasador guía 19 está dimensionado para ser recibido de forma guiada por dicha última parte cuando la parte roscada 18 es atornillada, para fijación a la aguja 21 con resto del instrumento quirúrgico, dentro de la parte roscada 23 del agujero ciego rotando el eje 15 mediante la rotación manual del pomo 16, presionando las superficies de extremo del vástago y la aguja entre sí. También esta fijación está de acuerdo con lo descrito en el documento WO-A-9606567.
Cuando se pone en la práctica el procedimiento descrito en el documento WO-A-9606567, las dos agujas 21A y 21B, figura 6, de la forma de realización descrita, deberán estar conectadas en un extremo de la banda 26. Según la presente invención, la banda de la forma de realización preferente comprende una malla o red que forma unas aberturas del orden de 1 mm. Un material apropiado para la banda es PROLENE (marca registrada), una malla tejida de polipropileno que tiene un grosor de 0,7 mm fabricada por Ethicon, Inc., Sommerville, New Jersey, EE.UU. Este material está aprobado por la FDA en los EE.UU., para su implantación en el cuerpo humano. La malla (banda) preferentemente tiene una anchura de aproximadamente 10 mm y está encerrada en una cubierta 34 delgada de polietileno que en la condición aplanada tiene sustancialmente la misma anchura que la banda, aunque, con una anchura diferente se muestra en la figura 2 con objeto de hacer la provisión de la cubierta más clara. La longitud de la malla deberá ser de aproximadamente 400 mm. La malla y la cubierta están interconectadas por medio de dos filas 35 de puntadas. La parte de extremo de la cubierta está fijada a la parte cónica 32 de la aguja por medio de un pegamento resistente apropiado, y la interconexión de la aguja y la cubierta está cubierta por una manguera contraible 36 de caucho que se extiende desde el reborde 33 sobre la parte cónica 32 y parcialmente sobre la parte 30 de extremo cilíndrica de la aguja. La manguera cantraible está sustancialmente a ras con la superficie de la aguja en el reborde. Mediante dicha configuración, la malla está fijada de forma segura a la aguja.
El objeto de la cubierta 34 es por encima de todo facilitar la inserción de la malla en la forma descrita en el documento WO-A-06567, es decir cuando la malla es traccionada en los extremos de la misma desde la pared de la vagina hasta la piel abdominal y para evitar que los bordes rugosos de la malla irriten o dañen los tejidos corporales.
Cuando la banda ha sido colocada en una posición correcta como un cabestrillo alrededor de la uretra, la cubierta de polietileno se debe retirar, y con objeto de facilitar la retirada, la cubierta debería estar perforada en el centro longitudinal de la misma, en la forma indicada por una línea 37 de puntos y rayas en la figura 6, de forma que los dos mitades de la cubierta puedan ser retiradas del cuerpo haciendo tracción en los extremos exteriores respectivos de la misma, estando las mitades separadas en la perforación bajo la influencia de la fuerza de tracción.
El objeto de la cubierta de polietileno es también proteger la malla durante la fijación a las agujas y durante la manipulación antes y durante su inserción dentro del cuerpo.
El centro longitudinal de la banda y la cubierta debería estar indicado por una marca 38 de color visible, figura 6, de forma que el cirujano pueda ver fácilmente cuando la malla está situada simétricamente con referencia a la uretra durante la cirugía.

Claims (14)

1. Una banda para tratar la incontinencia urinaria femenina caracterizada porque la banda comprende una malla (26) encerrada por una cubierta (34) delgada de plástico.
2. La banda de la reivindicación 1, caracterizada porque la malla (26) está fabricado de polipropileno.
3. La banda de la reivindicación 1, caracterizada porque la malla (34) está fabricado de polietileno.
4. La banda de cualquiera de las reivindicaciones 1 a 3, caracterizada porque la cubierta (34) está perforada en el centro longitudinal de la misma.
5. La banda de cualquiera de las reivindicaciones 1 a 4, caracterizada porque la malla (26) y la cubierta están interconectadas por puntadas (35).
6. La banda de cualquiera de las reivindicaciones 1 a 5, que tiene un primer extremo que se puede fijar a un primer elemento (21A) curvado similar a una aguja en un extremo del primer elemento y un segundo extremo de la banda (26) que se puede fijar a un segundo elemento (21B) curvado similar a una aguja en un extremo del segundo elemento, estando provistos los elementos para ser pasados dentro del cuerpo vía la pared vaginal y estando dimensionados para extenderse desde la superficie interior de la pared vaginal sobre la parte posterior de hueso del pubis hasta el exterior de la pared abdominal.
7. La banda de cualquiera de las reivindicaciones 1 a 6, que comprende un primer elemento (21A) curvado similar a una aguja fijado en un extremo del primer elemento a un primer extremo de la banda (26) y un segundo elemento (21B) curvado similar a una aguja fijado en un extremo del segundo elemento a un segundo extremo de la banda (26), estando construidos los elementos para poderse conectar independientemente entre sí con el segundo extremo de un vástago que tiene un mango en un primer extremo del vástago y un segundo extremo adaptado para recibir el primer elemento (21A) curvado similar a una aguja o el segundo elemento (21B) curvado similar a una aguja, estando provistos los elementos para ser pasados dentro del cuerpo vía la pared vaginal y estando dimensionados para extenderse desde la superficie interior de la pared vaginal sobre la parte posterior de hueso del pubis asta el exterior de la pared abdominal.
8. La banda de la reivindicación 7, caracterizada porque el elemento similar a una aguja comprende una parte de extremo no circular que encaja dentro de un casquillo (14') no circular en el segundo extremo del vástago (10).
9. La banda de la reivindicación 8, caracterizada porque la parte distal del elemento (21) similar a una aguja une el resto del elemento mediante una parte cónica (32) que se ahusa hacia un reborde (33) sobre el elemento similar a una aguja.
10. La banda de la reivindicación 9, caracterizada porque la malla (26) y la cubierta (34) están conectados al elemento (21) similar a una aguja pegándolo a la parte cónica (32).
11. La banda de la reivindicación 10, caracterizada porque la malla (26) y la cubierta (34) en el lugar de fijación de las mismas están cubiertas por una manguera (36) contraible.
12. La banda de la reivindicación 11, caracterizada porque un extremo de la manguera contraible (36) se apoya en el reborde (33) y está sustancialmente a ras con la superficie del elemento similar a una aguja en el reborde.
13. La banda de la reivindicación 11 ó 12, caracterizada porque la malla (26) y la cubierta (34) se proyectan desde la manguera (36) contraible en el otro extremo de la misma.
14. La banda de cualquiera de las reivindicaciones 1 a 13, caracterizada por una marca (38) visible provista sobre la cubierta (34) en el centro longitudinal de la misma.
