Antecedentes de la invención Campo técnico
La presente invención está relacionada con un implante derivado del hueso fabricado la menos en parte a partir de un hueso cortical de impartición de resistencia y que tiene por objeto para su utilización en la reparación, reemplazo y/o ampliación de varias partes de sistemas del esqueleto animal o humano. Más en particular, esta invención está relacionada con un implante derivado de hueso, el cual está compuesto por dos o más capas, en el que al menos una de las mismas es un hueso cortical totalmente mineralizado o parcialmente desmineralizado, y opcionalmente una o más capas fabricadas a partir de otro material. Las capas individuales que constituyen el implante están ensambladas en una estructura unitaria capaz de soportar cargas.
Descripción del arte relacionado
Es bien conocido el uso de hueso de autotransplante, hueso de transplante alogénico, o hueso de transplante heterólogo tanto en la medicina humana como en la medicina de veterinaria. Véase el documento de Stenvenson y otros, Ortopedia clínica e investigaciones relacionadas, 323, págs. 66-74 (1996). En particular, el hueso transplantado es conocido para proporcionar un soporte, promocionar la curación, rellenar cavidades de los huesos, separar elementos óseos tales como cuerpos vertebrales, promocionar la fusión y estabilidad los lugares de las fracturas. Más recientemente, se ha desarrollado el hueso procesado en formas para su utilización en nuevas aplicaciones quirúrgicas, o como nuevos materiales para implantes que históricamente fueron hechos con materiales derivados en forma no biológica.
La patente de los EE.UU. número 4678470 describe un material de transplante de hueso no formado por capas, fabricado a partir de hueso mediante un proceso que incluye el curtido con glutaraldeido. El hueso puede ser pulverizado, utilizado como un bloque grande o mecanizado para obtener una forma precisa. El curtido estabiliza el material y lo convierte también en no-antigénico. El material del hueso puede ser también desmineralizado.
El uso de una plancha continua de hueso desmineralizado o parcialmente desmineralizado se encuentra descrito en la patente de los EE.UU. número 5556430; no obstante, la plancha tiene que ser suficientemente flexible, sacrificando por tanto la resistencia, con el fin de conformarla al lugar del esqueleto al cual se aplique.
La plancha quirúrgicamente implantable descrita en la patente de los EE.UU. número 5507813 está formada por partículas de hueso alargadas, desmineralizadas opcionalmente, conteniendo ingredientes biocompatibles, adhesivos, rellenadores, plastificadores, etc.
La patente de los EE.UU. número 4932973 expone una matriz de hueso orgánico artificial con agujeros o perforaciones que se extienden dentro del matriz del hueco orgánico. Estos agujeros o perforaciones están indicados para que sean los centros de inducción del cartílago o hueso a continuación de la implantación de la matriz del hueso.
La patente de los EE.UU. número 4394370 expone un material de transplante de hueso similar a una esponja de una sola pieza, fabricado totalmente a partir de polvo de hueso totalmente desmineralizado o bien de hueso de micropartículas, y colágeno reconstituido. El transplante similar a una esponja está interconectado opcionalmente con glutaraldehido.
Se describe otro implante poroso de una pieza en la patente de los EE.UU. número 5683459. El implante está hecho con una macroestructura polimérica biodegradable, la cual está estructurada como una red de una malla de interconexión abierta de células, y una macroestructura polimérica biodegradable compuesta por substancias de base quimiotactica tal como el ácido hyalurónico.
El documento GB-A-2175506 está relacionado con una prótesis para el reemplazo de un cartílago articular. La prótesis está hecha a partir de una sola pieza de hueso que comprende una parte no desmineralizada y una parte desmineralizada. La parte completamente desmineralizada tiene una textura esponjosa similar a la del cartílago natural, mientras que la parte no desmineralizada proporciona una matriz dentro de la cual puede crecer el hueso huésped adyacente.
El documento WO-A-88/03417 está relacionado con un material biocompuesto útil en las aplicaciones quirúrgicas de huesos. Los materiales compuestos comprenden al menos un componente biocerámico, y al menos un componente conteniendo un polímero. Estos dos tipos de materiales pueden estar apilados para formar una estructura por capas en la cual los dos tipos distintos de materiales pueden insertarse en forma alternativa.
Sumario de la invención
La presente invención soluciona la necesidad ampliamente sentida en el área médica, proporcionando un implante derivado del hueso capaz de soportar cargas, y en una realización preferida, a través de su actividad curadora del hueso y por su capacidad para incorporar substancias útiles médicas/quirúrgicas en un lugar quirúrgico para promocionar y/o acelerar el crecimiento de hueso nuevo.
Por tanto es un objeto de la presente invención el proporcionar un implante derivado del hueso compuesto por una pluralidad de capas superpuestas, fijadas entre sí en una estructura unitaria, e incorporando características de resistencia a la compresión que se aproximan a las del hueso natural.
Es otro objeto de la invención el proporcionar un implante de hueso que tiene una red de poros, aberturas, perforaciones, canales o espacios que permiten y activan la penetración mediante los materiales de curación del hueso endógenos y exógenos y de suministro de sangre, y proporcionando simultáneamente unos medios para incorporar una o más substancias de curación del hueso.
Es un objeto adicional de la presente invención el proporcionar un implante derivado de hueso que pueda ser conformado con distintas formas y tamaños que estén limitados por las dimensiones y/o tipos anatómicos normales de los huesos donadores que se encuentren disponibles para la construcción del implante.
Para el mantenimiento de estos y otros objetos de la invención, se proporciona un implante derivado de hueso según lo definido en reivindicación 1.
El implante derivado de hueso de la presente invención tiene una ventaja significativa sobre los implantes de hueso y derivados de hueso del arte anterior en su capacidad para aproximarse a las características de resistencia mecánica del hueso natural, y para permitir la transferencia gradual del soporte de cargas hasta el tejido de hueso nuevamente formado a través del tiempo.
Otra ventaja importante del implante derivado de hueso en este caso sobre los implantes del arte anterior reside en su capacidad para funcionar como un soporte para incorporar con efectividad una o más substancias útiles médica o quirúrgicamente que promocionen el crecimiento de hueso nuevo y/o que aceleren la curación.
El término "osteogénico" tal como se utiliza aquí se entenderá que se refiere a la capacidad de una substancia para inducir la formación de hueso nuevo a través de la participación de células vivas desde el interior de la substancia.
El término "osteoconmductor" tal como se utiliza aquí se entenderá que se refiere a la capacidad de una substancia o material de proporcionar superficies biológicamente inertes que sean receptivas para el crecimiento del hueso huésped nuevo.
El término "osteoinductivo" tal como se utiliza aquí se entenderá que se refiere a la capacidad de una substancia para reclutar células del huésped, las cuales tengan el potencial de reparar el tejido del hueso.

Descripción detallada de las realizaciones preferidas
El implante derivado de hueso de la presente invención comprende al menos dos capas superpuestas en al menos una de las capas, estando una capa para impartir resistencia a la compresión derivada de un hueso cortical no desmineralizado o de un hueso cortical que haya sido desmineralizado solo parcialmente. Al presentarse como una capa para impartir resistencia a la compresión, la capa de hueso cortical parcialmente desmineralizada deberá mostrar un porcentaje claramente alto de resistencia a la compresión de una capa comparable de hueso cortical no desmineralizado, por ejemplo, preferiblemente al menos de aproximadamente el 40%, más preferible al menos de aproximadamente el 50% y más todavía más preferiblemente al menos de aproximadamente el 60 por ciento de dicha resistencia. Las capas del hueso cortical parcialmente desmineralizado que muestren un valor significativamente inferior al 40 por ciento de la resistencia a la compresión de una capa comparable de hueso cortical totalmente mineralizado podrán utilizarse en el implante derivado de hueso, pero con una cierta capacidad distinta a la de poder soportar cargas. Dependiendo del grosor de las capas, podrán disponerse de hasta 2 a 200 capas en total aproximadamente en el implante derivado de hueso. El implante derivado de hueso puede incluir capas de grosores distintos, por ejemplo, la(s) capa(s) de impartición de resistencia a la compresión pueden ser considerablemente más gruesas que cualquier capa(s) que puedan estar presentes. Pueden utilizarse ventajosamente grosores que varíen de aproximadamente 0,5 a 20, y preferiblemente desde aproximadamente 1,5 a 15 mm. En general, el numero y grosor de las capas de impartición de resistencia a la compresión en un implante derivado de hueso será tal que proporcionen una resistencia a la compresión global para el implante desde aproximadamente 25 a 250, y preferiblemente de aproximadamente 100 a 200 MPa.
Las fuentes de hueso cortical para el implante derivado de hueso de esta invención son preferiblemente alogénicas pero incluyen también fuentes xenogénicas tal como el hueso bovino y porcino. Cuando se utilice el hueso cortical parcial o totalmente desmineralizado, dicho hueso podrá ser obtenido utilizando las técnicas de desmineralización conocidas, por ejemplo, las que utilizan ácidos fuertes tal como el ácido clorhídrico tal como lo expuesto en el documento de Reddi y otros, Proc. Nat. Acad. Sci. 69, páginas 1601-5 (1972). La amplitud de la desmineralización es una función de la resistencia de la solución ácida, de la forma del hueso y de la duración del tratamiento de desmineralización. Se hacer referencia a este respecto en el documento de Lewandrowski y otros, J. Biomed Material Res, 31, páginas 365-372 (1996). El uso de hueso desmineralizado parcial o totalmente puede ser beneficioso aquí puesto que dichas substancias muestran una actividad mayor de tipo osteogénico y/o osteinductivo que el hueso totalmente mineralizado.
La capa(s) de impartición de resistencia a la compresión del implante derivado de hueso puede ser proporcionada como secciones monolíticas de hueso o como capas multiseccionales construidas a partir de dos o más subsecciones, por ejemplo, unidas entre sí de forma de borde con borde de una forma que sea análoga a un entablado. De esta forma, las capas de impartición de resistencia a la compresión relativamente grandes podrán ser construidas a partir de secciones de hueso más pequeñas, para proporcionar un implante cuya dimensión global no esté limitada por el tamaño y/o forma del hueso cortical que esté disponible para su construcción.
El ensamblado de las capas superpuestas en una estructura unitaria fuerte puede llevarse a cabo mediante una amplia variedad de medios/procedi-
mientos, por ejemplo, con la aplicación de adhesivos biológicamente compatibles conocidos y convencionales, tales como los cianoacrilatos; compuestos basados en epoxias, selladores de resina dental, pegamentos de resina dental, pegamentos de ionomeros de vidrio, metacrilato de polimetilo, colas de gelatina-resolcinol-formaldehido, colas basadas en colágeno, agentes de unión inorgánicos, tales como el fosfato de cinz, fosfato de magnesio o bien otros pegamentos basados en el fosfato, carboxilato de cinc, etc., y aglutinantes basados en proteinas tales como las colas de fibrina y las proteinas adhesivas derivadas del mejillón; el uso de fijaciones mecánicas tal es como los pasadores, tornillos, espigas pasantes, etc., que pueden ser fabricadas a partir de materiales naturales o sintéticos y bioabsorbibles así como también de materiales no bioabsorbibles; soldadura de tejidos por láser; y uniones por ultrasonidos. Si así se desea, las capas de implante derivadas de hueso pueden ser proporcionadas con características de interacoplamiento mecánico, por ejemplo, elementos del tipo de "cola de milano", para facilitar su ensamblaje en el producto final y/o para fijar las capas entre sí de una forma más segura. Además de sus capas de hueso cortical totalmente mineralizado o parcialmente mineralizado con resistencia a la compresión, el implante derivado de hueso de esta invención puede tener opcionalmente una o más capas formadas a partir de uno o más materiales adicionales. Por ejemplo, estas capas opcionales pueden estar basadas o incluir hueso altamente o totalmente desmineralizado, grafito o carbón pirolítico, un material mineral tal como la hidroxiapatita, fosfato tricalcico, bioglass o bien otros polímeros biocerámicos, naturales o sintéticos, por ejemplo, los materiales bioabsorbibles tales como los almidones, poliglicolidos, poliactidos, copolimero de glicolida-lactida, y similares, y polímeros no bioabsorbibles tales como el metacrilato de polimetilo, politetrafluoroetileno, poliuretano y nylon.
Si se desea, el eje de resistencia a la compresión de una o más capas de resistencia a la compresión puede ser desplazado con respecto al eje de la resistencia a la compresión de una o más de las capas de resistencia a la compresión en una configuración muy similar a la de la madera conchabada. Por ejemplo, los ejes de resistencia a la compresión de las capas alternativas de resistencia a la compresión puede ser desplazadas hasta el 90% con respecto a los ejes de resistencia a la compresión de las demás capas de resistencia a la compresión en el implante, con el fin de distribuir la capacidad global de soporte de cargas del implante en direcciones mutuamente transversales.
Los implantes derivados de hueso de cualquier tamaño y/o configuración deseable pueden proporcionarse, por ejemplo, mediante la mecanización o bien otras operaciones de conformación mecánica, tales como el moldeo por prensado. El modelado por ordenador de un implante específico seguido por el control por ordenador de la conformación del implante puede utilizarse para proporcionar un implante derivado de hueso de formas intrincadas, el cual se aplicará en forma personalizada en el punto perseguido de la aplicación con una gran precisión.
El implante derivado de hueso puede tener una o más cavidades, las cuales si así se desea pueden comunicarse con la superficie del implante a través de poros, aberturas, perforaciones o canales provistos para este fin, y variando en el diámetro promedio desde algunas pocas micras a varios milímetros. Tales cavidades y sus poros, aberturas, perforaciones y canales asociados pueden ser rellenados parcial o completamente con una o más substancias médica o quirúrgicamente útiles, las cuales promocionarán o acelerarán el crecimiento de hueso nuevo o la curación del hueso debido, por ejemplo, a cierto efecto osteogénico, osteoconductivo y/o osteoconductivo. Las substancias útiles de este tipo que pueden ser incorporadas en el implante derivado de hueso de esta invención incluyen, por ejemplo, colágeno, derivados de colágeno insolubles, etc., y sólidos solubles y líquidos disueltos, por ejemplo agentes antivirales, particularmente los que son efectivos contra el HIV y la hepatitis; antimicrobianos y/o antibióticos tales como la eritromicina, bacitracina, penicilina, polimixina B, tetraciclinas, viomicina, cloromicetina y estreptomicinas, cefazolina, ampicilina, azactam, tobramicina, clindamicina y gentamicina, etc.; azúcares de biocidal/biostático tales como el dextroal, glucosa, etc.; aminoácidos, peptidos, vitaminas, elementos inorgánicos, cofactores para síntesis de proteínas; hormonas; tejido endocrino o fragmentos de tejido; sintetizadores; enzimas tales como la colagenasa, peptidasas, oxidasas, etc.; plataformas de células de polímeros con células parenquimatosas; fármacos angiogénicos y soportes poliméricos que contengan dichos fármacos; láminas de colágeno; agentes antigénicos; agentes citoesqueléticos; fragmentos de cartílagos, células vivas tales como los condrocitos, células de médula ósea, células de estirpe mesenquimal, extractos naturales, transplantes de tejido, hueso, polvo de hueso desmineralizado (o "matriz de hueso desmineralizado" tal como podría denominarse también), tejidos autógenos tales como sangre, suero, tejidos blandos, médula blanda, etc.; bioadhesivos, proteinas morfogénicas de hueso (BMP), factor de crecimiento de transformación (TGF-beta), factor de crecimiento similar a la insulina (IGF-1); hormonas de crecimiento tales como la somatotropina; digestores de hueso; agentes antitumorales; inmuno-supresores; potenciadores de permeabilidad, por ejemplo, esteres de ácidos grasos tales como el laureato, miristato y monoesteres de estereato del glicol polietileno, derivados de enamina, alfa-ceto aldehidos, etc.; y ácidos nucleicos. Estas y otras substancias medica/quirúrgicamente útiles pueden ser incorporadas en el implante derivado de hueso de esta invención o cualquiera de sus capas constituyentes durante cualquier etapa del ensamblado del implante. Los métodos adecuados de la incorporación incluyen el revestimiento, saturación por inmersión, encapsulado, etc. Las cantidades de las substancias medica/quirúrgicamente útiles pueden variar ampliamente con los niveles óptimos determinados fácilmente en un caso específico mediante experimentación rutinaria.
El implante derivado de hueso presente tiene por objeto que sea aplicado en un punto de un defecto del hueso, por ejemplo, en un punto resultante de una herida, en un defecto resultante del curso de una cirugía, infección, malignidad o malformación de desarrollo. El implante derivado de hueso, conformado adecuadamente y adaptado según se precise, puede ser utilizado como un transplante o reemplazo con una amplia variedad de procedimientos quirúrgicos de tipo ortopédico, neuroquirúrgico y oral y maxilofacial, tal como la reparación de fracturas sencillas o compuestas y reconstrucciones de juntas sin uniones externas y fijaciones internas, tal como en la artrodesis, artroplastia general, artroplastia de cótilo de cadera, reemplazos de la cabeza femoral y humeral, reemplazo de la superficie de cabeza femoral y reemplazo de la articulación total, reparaciones de la columna vertebral incluyendo la fusión espinal y la fijación interna, cirugía de tumores, por ejemplo, rellenado, discectomia, laminectomía, escisión de tumores de médula espinal, operaciones cervicales y torácicas, reparación de heridas espinales, tratamiento de escoliosis, lordosis y cifosis, fijación intermaxilar de fracturas, metoplastia, reemplazo de la articulación temporomandibular, aumento y reconstrucción del reborde alveolar, transplantes de hueso incrustado, colocación y revisión de implantes, elevaciones de senos, etc. Los huesos específicos que puede ser reparados o reemplazados con el implante derivado de hueso en este caso incluyen el etmoide, frontal, nasal, occipital, parietal, temporal, mandíbula, maxilar, zigomático, vértebras cervicales, vértebras torácicas, vértebras lumbares, sacro, costillas, esternón, clavícula, escápula, húmero, radio, cúbito, huesos carpianos, huesos metacarpianos, falanges, ilion, isquión, pubis, fémur, tibia, perone, rótula, calcáneo, y huesos tarso y metatarso.
Con referencia a los dibujos, tal como se muestra en la figura 1, la parte cortical del hueso 10 tomada de la zona diafiseal está cortada en capas de hueso corticales 11 de ancho variable mediante el rebanado del hueso en forma longitudinal. Si se desea, las capas de hueso cortical 11 pueden ser cortadas además según un tamaño y forma uniformes como en las capas de hueso 22 del implante 20 mostrado en la figura 2.
La figura 2 muestra un implante derivado de hueso 20 que comprende capas alternadas de hueso cortical 22 totalmente mineralizado, y hueso cortical 23 parcialmente desmineralizado. Alternativamente, pueden crearse una o más capas 23 a partir de un material distinto al hueso parcialmente desmineralizado, tal como un hueso totalmente desmineralizado o substancias minerales tales como la hidroxiapatita. El grosor total del implante de hueso será normalmente al menos de aproximadamente 5 mm, y preferiblemente al menos de aproximadamente 10 mm. El implante derivado de hueso 20 puede ser cortado, mecanizado y/o formado de cualquier otra forma en otro perfil o dimensión deseada para su implantación en un cuerpo. Por ejemplo, puede crearse un implante de hueso conformado cilíndricamente para su utilización como un reemplazo de un segmento de hueso largo, por ejemplo, para un fémur. Para formar un cilindro, el implante derivado de hueso substancialmente cuadrado o rectangular puede conformarse en un torno de acuerdo con el diámetro requerido. Puede formarse una cavidad mediante la extracción de material con, por ejemplo, un taladro, o alternativamente puede formarse una cavidad mediante el ensamblado apropiado de capas configuradas.
Tal como se muestra en la figura 3, el implante 30 derivado de hueso de forma cilíndrica o espiga pasante se muestra insertado en el punto del fibrocartílago intervertebral del lado anterior de la columna vertebral 31.
En la figura 4, el implante 40 derivado de hueso zigomático está dimensionado y conformado para formar la parte del arco zigomático y la parte de la órbita interior del cráneo 41.
Tal como se muestra en la vista en sección de la figura 5, el implante derivado de hueso 50 está construido a partir de las secciones alargadas 51 de hueco cortical totalmente desmineralizado o parcialmente desmineralizado de una sección transversal uniforme y dispuestas en forma de una red utilizando un adhesivo adecuado, por ejemplo, de la variedad del cianoacrilato. Debido a la estructura abierta del implante 50 resultante del patrón de canales longitudinales 52 y canales transversales 53, el implante permite la penetración vascular o el crecimiento interno de hueso huésped y/o la difusión de una o más substancias medica/quirúrgicamente útiles. Los canales verticales en la matriz similar a una red pueden, si se desea, estar parcial o completamente ocupados por secciones 54 configuradas y dimensionadas en la forma apropiada, fabricadas a partir de hueso cortical total o parcialmente desmineralizado, al igual que en las secciones de hueso cortical 51 o a partir de otras substancias aquí expuestas. El implante 50 de hueso totalmente ensamblado se muestra instalado en el espacio intervertebral 61 de la columna vertebral 60 de la figura 6.
Los ejemplos siguientes son ilustraciones adicionales del implante derivado de hueso de esta invención.
Ejemplo 1
La sección cortical de hueso de la zona diafisaria se cortó en la dirección longitudinal mientras que se mojaba continuamente con agua en capas aproximadamente de 1,5 mm de grosor utilizando una sierra de cuchilla de diamante. Las capas fueron congeladas a -70ºC y se secaron en congelador durante 48 horas. Las capas fueron entonces ensambladas con adhesivo de cianoacrilato y retenidas en una mordaza durante dos horas mientras que se endurecía el adhesivo. La estructura unitaria multicapa resultante se cortó en una sierra de cinta y conformada mediante el esmerilado y mecanizado con una herramienta de moldeado motorizada de mano, para proporcionar un implante de hueso conformado.
Ejemplo 2
Se corto mediante una sierra una sección de hueso cortical de la zona diafisaria como en el Ejemplo 1 en capas de aproximadamente 1,5 mm de grosor. La mitad de las rebanadas fueron totalmente desmineralizadas de la forma conocida utilizando ácido ClH 0,6N. Se ensamblaron todas las capas con adhesivo de cianoacrilato con capas de hueso totalmente mineralizado alternando con capas de hueso totalmente desmineralizado. El implante derivado de hueso de este tipo se muestra en la figura 2.
Ejemplo 3
Se corto con sierra un segmento de hueso de la zona diafisaria de 50 mm de longitud mientras que se mojaba continuamente con agua, para proporcionar secciones de hueso alargadas de una sección transversal uniforme y cuadrada. Las secciones de hueso alargadas fueron entonces ensambladas con adhesivo de cianoacrilato, para proporcionar una estructura similar a una red en la cual las capas de las secciones de hueso separadas entre sí fueron dispuestas transversalmente entre sí. La sección del implante derivado de hueso de este tipo es la mostrada en la figura 5.

Claims (24)

1. Un implante derivado de hueso (20) que comprende una pluralidad de capas superpuestas (22, 23), ensambladas con un adhesivo en una estructura unitaria, en el que al menos una capa en la estructura es una capa de impartición de resistencia a la compresión (23) fabricada a partir de un hueso cortical no desmineralizado o un hueso cortical parcialmente desmineralizado.
2. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, en el que la capa de impartición de resistencia a la compresión (23) está fabricada a partir de una capa de hueso parcialmente desmineralizado que muestre un valor no inferior al 40% de la resistencia a la compresión de una capa comparable de un hueso totalmente mineralizado.
3. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, el cual comprende al menos dos capas de impartición de resistencia a la compresión (23).
4. Un implante derivado de hueso (20) de acuerdo con la reivindicación 3, en el que los ejes de resistencia a la compresión de al menos dos capas de impartición de resistencia a la compresión consecutivas (23) están desplazadas entre sí.
5. Un implante derivado de hueso (20) de acuerdo con la reivindicación 3, en el que al menos dos capas consecutivas de impartición de resistencia a la compresión (23) están compuestas por secciones separadas entre sí con los ejes de resistencia a la compresión de las capas estando desplazados entre sí en una configuración similar a una red que definen una matriz de canales en todas las tres dimensiones.
6. Un implante derivado del hueso (20) de acuerdo con la reivindicación 1, el cual tiene un grosor total de al menos aproximadamente 5 mm.
7. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, el cual tiene una sección transversal de al menos una parte de su longitud, la cual es o bien se aproxima a un círculo, de tipo oval o poligonal, teniendo el implante opcionalmente una cavidad en al menos una parte de su longitud.
8. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, que comprende además al menos una capa de impartición de resistencia mecánica que esté provista como una sección de hueso monolítica.
9. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, que comprende además al menos una capa de impartición de resistencia mecánica, la cual está provista como una sección de hueso multiseccional en la que las subsecciones del hueso que constituyen la sección están unidas entre sí de forma de borde con borde.
10. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, que comprende unas superficies principales superior e inferior expuestas, siendo las superficies las correspondientes a las capas de impartición de resistencia a la compresión (23).
11. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, que comprende además al menos una capa fabricada en su totalidad o en parte a partir de la menos un material distinto al hueso cortical no desmineralizado o parcialmente desmineralizado.
12. Un implante derivado de hueso (20) de acuerdo con la reivindicación 11, en el que el material es de hueso totalmente desmineralizado, grafito, carbón pirolítico, hidroxiapatita, fosfato tricalcico, material bioabsorbible o un polímero no bioabsorbible.
13. Un implante derivado de hueso (20) de acuerdo con la reivindicación 11, en el que el implante comprende medios para facilitar la difusión de material de duración del hueso.
14. Un implante derivado de hueso (20) de acuerdo con la reivindicación 13, en el que los medios para facilitar la difusión del material de curación del hueso constituyen los canales definidos dentro de una o más capas.
15. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, en el que cada capa de impartición de resistencia a la compresión (23) tiene el mismo o distinto grosor promedio en el rango de aproximadamente 0,5 mm a 20 mm.
16. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, que tiene una resistencia a la compresión de aproximadamente 25 a 250 Mpa.
17. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, teniendo desde aproximadamente 2 a 200 capas superpuestas (22, 23).
18. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, fabricado en su totalidad o en parte a partir de hueso cortical alogénico y/o xenogénico.
19. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, en el que las capas superpuestas (22, 23) están ensambladas con fijaciones mecánicas.
20. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, que comprende una o más cavidades.
21. Un implante derivado de hueso (20) de acuerdo con la reivindicación 19, en el que los poros, aberturas, perforaciones o canales, proporcionan la comunicación entre la cavidad y la superficie del hueso.
22. Un implante derivado de hueso (20) de acuerdo con la reivindicación 19, en el que la cavidad contiene un agente antimicrobiano, un agente antibiótico, un agente angiogénico, un agente antígeno o un agente citoesquelético.
23. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, en el que las capas superpuestas (22, 23) están ensambladas con un adhesivo seleccionado a partir del cianoacrilato, compuesto basado en epoxias, sellador de resina dental, pegamento de resina dental, polimetilmetacrilato, cola de gelatina-resorcinol-formaldehido, cola basada en colágenos, fosfato de cinc, fosfato de magnesio, carboxilato de cinc y cola de fibrina.
24. Un implante derivado de hueso (20) de acuerdo con la reivindicación 1, en el que el implante derivado de hueso está configurado como un hueso etmoides, un hueso frontal, un hueso nasal, un hueso occipital, un hueso parietal, un hueso temporal, un hueso de mandíbula, un hueso maxilar, un hueso zigomático, un hueso de vértebras cervicales, un hueso de vértebras torácicas, un hueso lumbar, un hueso metacarpiano, un hueso de las falanges, un hueso ilion, un hueso isquión, un hueso del pubis, un hueso del fémur, un hueso de la tibia, un hueso del peroné, un hueso de la rótula, un hueso del calcáneo, o un hueso del tarso o del metatarso.
