Dispositivo medicinal para el suministro de medicamento.
La invención se refiere a un catéter con globo para el suministro de medicamento para la terapia selectiva de determinadas secciones de tejidos o partes de órganos, y a un procedimiento para la preparación de dichos aparatos recubiertos de medicamento.
Numerosas enfermedades no afectan a todo el organismo simultáneamente, sino que se limitan a determinados tipos de tejido, frecuentemente también a zonas de tejido o partes de órganos particulares muy limitadas. Se encuentran ejemplos de ello en las enfermedades tumorales, articulares y vasculares.
La farmacoterapia de estas enfermedades se realiza en general también mediante toma oral o intravenosa de medicamentos que se distribuyen por todo el cuerpo y en muchos casos, incluso en enfermedades graves, causan efectos indeseados en tejidos y órganos sanos que limitan la aplicación terapéutica. Se ha conseguido una terapia selectiva del tejido enfermo mediante medicamentos de unión específica a tejido enfermo (por ejemplo, anticuerpos) manteniendo el modo de aplicación o mediante administración selectiva, por ejemplo, mediante inyección directa en el tejido enfermo o mediante introducción por catéter a los vasos sanguíneos que abastecen el tejido enfermo. En el caso de administración selectiva, surgen problemas por la duración generalmente corta del efecto del medicamento y los modos de administración invasiva, ya que privan de una toma repetida a voluntad. En la administración selectiva a través de la corriente sanguínea que abastece al tejido enfermo, resulta el problema adicional de una extracción insuficiente de los medicamentos por el rápido paso de la sangre o de la disolución de principio activo por los vasos sanguíneos.
Estos problemas se han tratado hasta ahora mediante distintos preparados farmacéuticos con liberación de principio activo retardada, implantes liberadores de medicamento o vías de acceso selectivas funcionales durante más tiempo, como catéteres implantados, etc.
Es ya conocido recubrir la superficie de los aparatos medicinales introducidos en el cuerpo, particularmente catéteres, con agentes para la mejora de la capacidad de deslizamiento o para la reducción de la coagulación sanguínea, pero sin efecto terapéutico.
Además, se proporcionan catéteres con dispositivos especiales para inyectar medicamentos en la pared arterial, por ejemplo, mediante agujas o inyección a alta presión a través de una perforación de la pared del catéter contigua a la pared del vaso.
Otros principios se basan en prolongar el tiempo de contacto entre la pared arterial y una preparación de principio activo aplicada sobre el catéter, deteniendo la corriente sanguínea durante un intervalo de tiempo correspondiente, por ejemplo un catéter con globo doble con cámara rellena con la disolución de medicamento que se encuentra entre los globos o con cavidades entre la pared exterior del globo provista, por ejemplo, de rebordes, en el que la corriente sanguínea puede mantenerse a una medida limitada mediante un canal que pasa por el globo.
Según el documento US 5.102.402, se disponen medicamentos en forma de microcápsulas para una liberación de principio activo retardada suelta en cavidades preconformadas de catéteres con globo. Después de la expansión del globo, las microcápsulas deben presionarse con la pared vascular, permanecer allí y liberar lentamente el o los principios activos. Numerosos autores proponen aplicar medicamentos en hidrogel incorporados a catéteres con globo, en los que la función del hidrogel como adhesivo, para mejorar la capacidad de deslizamiento o retardar la liberación del medicamento permanece abierta.
La desventaja de los productos citados es en cualquier caso la configuración compleja con los correspondientes problemas en la preparación, controles de calidad y costes, así como etapas de trabajo adicionales incómodas para médico y paciente en la aplicación. Una parte de los procedimientos citados conduce a una lesión vascular excesiva totalmente indeseada por la dilatación vascular pretendida. Por otro lado, cada medida proporcionada para la prolongación del tiempo de contacto tiene como consecuencia una falta de abastecimiento adicional de sangre y oxígeno a los tejidos conectados a continuación.
Para completar, se remite a un dispositivo descrito en el documento WO 01/24866 para la reducción de la reestenosis, que está recubierto con una sustancia ceramida lipídica derivada de membrana celular natural. Esta sustancia se usa debido a su afinidad no encontrada en los medicamentos comunes con las paredes celulares de la pared arterial. Sin embargo, en la bibliografía especializada, se argumenta además que una profilaxis farmacéutica de la reestenosis requiere una liberación del principio activo necesario durante días.
La invención se basa en el objetivo de proporcionar un catéter con globo para un suministro de medicamento limitado a determinadas zonas de tejido o partes de órgano, que ejerza un efecto terapéutico fuerte sin influencia dañina sobre tejidos sanos, que incomode sólo un poco a los pacientes y que se use y pueda prepararse a bajo coste.
Según la invención, se consigue el objetivo con un catéter con globo configurado o preparado según los rasgos de las reivindicaciones 1 y 14. A partir de las reivindicaciones subordinadas resultan rasgos adicionales y variantes ventajosas de la invención.
Mediante la invención, se proporcionan en un procedimiento de preparación sencillo catéteres con globo que portan medicamento mejorados que son utilizables versátilmente y que posibilitan una liberación de principio activo inmediata. Sorprendentemente y en contra de la teoría, no es necesaria ni provechosa ninguna liberación de principio activo duradera a partir de una matriz inerte (polímero, hidrogel, microcápsulas, etc.) o estados químicos o físicos especiales de los principios activos. Por tanto, no son precisas tampoco técnicas costosas para la producción o control de formulaciones de depósito.
El recubrimiento de globos presentes en catéteres con medicamentos según la presente invención es un beneficio especial en lo que respecta a después de la dilatación de vasos sanguíneos u otras cavidades del cuerpo con los globos, en la que existe frecuentemente la necesidad de medidas terapéuticas para evitar el estrechamiento o cierre del lumen creado con el globo a presión, para limitar el crecimiento tumoral o potenciar los procesos de curación, incluyendo la formación de circulaciones colaterales. Esto puede conseguirse mediante medicamentos que desarrollen su efecto en la inmediata vecindad de la superficie del globo. Los medicamentos se adhieren firmemente a éste en el tránsito al objetivo, principalmente por arterias intensamente perfundidas, hasta el despliegue del globo, después se liberan a dosis eficaces durante el tiempo de contacto de duración corta, a menudo de sólo segundos, del globo desplegado con el tejido y se captan por éste de un modo que evita el lavado por la corriente sanguínea reiniciada justo después del desinflado del globo.
Para el recubrimiento, se proporcionan globos de catéteres según la invención que se comprimen al menos durante un corto tiempo a presión contra tejidos enfermos. Los materiales de catéter preferidos son poliamidas, mezclas de poliamida y copolímeros, polietilentereftalato, polietileno y copolímeros, poliuretano, caucho natural y sus derivados. La longitud y el diámetro de los intervalos de catéteres o globos proporcionados para farmacoterapia no son de importancia decisiva para la aplicación, ya que la dosificación se calcula en \mug de principio activo/mm^{2} de superficie. Por ejemplo, son convencionales para los globos de dilataciones coronarias globos con intervalos de 2-4 mm de diámetro y de 1,0-4,0 cm de longitud. Para otros vasos, pueden utilizarse también globos de hasta >20 mm de diámetro y longitudes de hasta >10 cm. Las superficies a recubrir pueden proporcionarse alisadas (es decir, sin una estructura especial para la captación del principio activo), escarificadas o con estructuras de cualquier tipo, en las que las estructuras superficiales especiales no son un prerrequisito para la adhesión de los principios activos, pero no obstaculizan tampoco la adhesión. La adhesión de los principios activos a las superficies de globo se logra exclusivamente mediante la elección de disolventes adecuados y dado el caso la adhesión de aditivos influyentes. Es sorprendentemente firme incluso sobre superficies de globo exteriores totalmente lisas.
Todas las superficies pueden estar recubiertas o recubrirse adicionalmente con sustancias que mejoren la capacidad de deslizamiento de los productos, eviten la coagulación de la sangre en la superficie o mejoren otras propiedades de los productos médicos, sin que los materiales empleados para el recubrimiento deban suministrarse al entorno y sin que el recubrimiento limite esencialmente el suministro de los principios activos para el tratamiento del tejido diana, y por tanto la eficacia.
Los catéteres de globo se conforman a partir de tubos de plástico muy finos mediante ensanchamiento de un segmento de 1 a aproximadamente 10 cm de longitud. La membrana de globo de pared muy fina ensanchada se dispone a continuación en varios pliegues dispuestos a lo largo del eje del catéter y se enrolla firmemente alrededor del eje del catéter de modo que la zona ensanchada en estado plegado presente un diámetro sólo mínimamente mayor que el resto del catéter. El plegamiento estrecho de la cubierta del globo es un prerrequisito para el paso no problemático del catéter con globo por las compuertas de entrada, guía del catéter y, por ejemplo, secciones fuertemente estrechadas de vasos sanguíneos.
Los globos de catéteres se recubren en estado plegado, consiguiendo en cualquier caso un recubrimiento intacto suficientemente homogéneo de la superficie, y se adhieren también con suficiente firmeza a su superficie los principios activos durante el plegamiento de un catéter con globo recubierto en estado desplegado.
La preparación de un globo recubierto en estado desplegado se realiza según el estado de la técnica sin alteración del recubrimiento, por ejemplo, mediante el uso de cubiertas de globo con pliegues y doblamientos preconformados cuya estructura de material no se pierde con la dilatación y que después de soltar la presión del globo logra que la cubierta del globo se pliegue de nuevo correctamente lo menos suelta, sin necesidad de una fuerza externa como causa primaria. Sólo después, se comprimen los pliegues preconformados desde fuera o mediante vacío. En ningún caso, son necesarios pliegues para la fijación del principio activo. Además, el plegamiento puede lograrse mediante fuerzas mecánicas pequeñas mediante materiales muy lisos, en los que las herramientas pueden estar bañadas también, por ejemplo, con fluidos lubricantes biocompatibles en los que los principios activos no se disuelvan o en cualquier caso se disuelvan mal.
Según una variante adicional de la invención, se recubren los globos de catéteres con globo plegados preparados mediante inmersión en disoluciones de principio activo de baja viscosidad. A este respecto, disolvente y principio activo penetran entre los pliegues extremadamente estrechos y forman allí un depósito sorprendentemente homogéneo y de dosis reproducible que no se daña mediante ninguna etapa de trabajo posterior. La disolución adhesiva exterior o el depósito adhesivo exterior después del secado del disolvente pueden dejarse allí o retirarse en una etapa de trabajo posterior, de modo que permanezca sólo el principio activo encamisado en los pliegues del globo.

Después del recubrimiento, puede impulsarse en el globo plegado una prótesis endovascular por el catéter con globo y fijarse a éste. Después, es necesaria ya sólo la esterilización, por ejemplo, mediante óxido de etileno.
El proceso de trabajo así configurado es extraordinariamente sencillo, poco tendente a problemas y también puede llevarse a cabo con materiales sensibles mecánica, química y físicamente. Se ha mostrado que el recubrimiento según este procedimiento no conduce a una relajación o adherencia indeseada del plegamiento y que el principio activo así cargado se adhiere con suficiente firmeza para no eliminarse en el tránsito por la sangre, por otro lado en el inflado del globo en el tejido diana se libera en gran medida el principio activo.
Como medicamentos se tienen en cuenta medicamentos fuertemente lipófilos, en gran medida insolubles en agua altamente eficaces de unión a cualquier componente de tejido. Se caracterizan como lipófilos medicamentos cuyo coeficiente de distribución en butanol:tampón acuoso a pH 7 sea 0,5, preferiblemente 1 y de forma especialmente preferida 5, o en octanol:tampón acuoso a pH 7 sea 1, preferiblemente 10, de forma especialmente preferida >50. Como alternativa o adicionalmente, los medicamentos deben unirse reversible y/o irreversiblemente a componentes celulares en >10%, preferiblemente en >50%, de forma especialmente preferida en >80%. Se prefieren sustancias para la inhibición de la proliferación celular o también de procesos inflamatorios o antioxidantes como paclitaxel y otros taxanos, rapamicina y sustancias relacionadas, tacrolimus y sustancias relacionadas, corticoides, hormonas sexuales (estrógeno, estradiol, antiandrógenos) y sustancias relacionadas, estatinas, epotilona, probucol, prostaciclina, inductores de la angiogénesis, etc.
Las sustancias se presentan preferiblemente en forma de sustancia sólida seca o aceite sobre las superficies de los distintos productos farmacéuticos. Se prefieren partículas del menor tamaño (en su mayoría <5 \mum, preferiblemente < 1 \mum, de forma especialmente preferida < 0,1 \mum), se prefieren especialmente estructuras amorfas no cristalinas del tamaño de partícula más fino, que se disuelvan rápidamente en contacto con los tejidos debido a su gran superficie a pesar de una solubilidad acuosa básicamente baja de los medicamentos y que no funcionen como microcápsulas, es decir, que se disuelvan espontánea y rápidamente. A este respecto, es suficiente con que presenten una dosis eficaz en forma de partículas mínimas o amorfas; las partículas mayores ciertamente apenas contribuyen a la concentración de principio activo en el tejido, pero tampoco interfieren. La dosificación se ajusta según el efecto deseado y la eficacia del medicamento usado. Puede alcanzar hasta 5 \mug/mm^{2}, no representando esto ningún límite superior. Son más fáciles de efectuar dosificaciones más bajas.
Se consigue una buena adhesión a las superficies de catéter, agujas o cables para la mejora de la captación en el tejido mediante la incorporación de principios activos fuertemente lipófilos poco solubles en agua a una sustancia matriz fácilmente soluble en agua. Como sustancias matriz son adecuadas sustancias hidrófilas de bajo peso molecular (peso molecular <5.000 Da, preferiblemente < 2.000 Da) como agentes de contraste y colorantes usados in vivo para distintos procedimientos de diagnóstico en medicina, azúcares y sustancias relacionadas como alcoholes de azúcar, polietilenglicoles de bajo peso molecular, sales orgánicas e inorgánicas biocompatibles como, por ejemplo, benzoatos, sales y otros derivados de ácido salicílico, etc. Como agente de contraste, se remite también a agentes de contraste de rayos X yodados y a quelatos paramagnéticos. Son ejemplos de colorantes verde de indocianina, fluoresceína y azul de metileno. Los coadyuvantes pueden servir también para mejorar la capacidad de almacenamiento de los productos, causar efectos farmacológicos complementarios especiales o servir como controles de calidad.
En otra realización, los principios activos farmacéuticos pueden adsorberse en una partícula o aplicarse con una matriz de bajo peso molecular sobre las superficies de productos medicinales adecuados. Como partículas, son adecuadas a su vez medios de diagnóstico conocidos biocompatibles como ferrita y diversos agentes de contraste para sonografía.
Los coadyuvantes de todos los tipos pueden utilizarse en dosis menores o mayores que los principios activos.
El recubrimiento del catéter con globo se realiza mediante disoluciones, suspensiones o emulsiones de los medicamentos y coadyuvantes citados. Son medios de disolución, suspensión o emulsión adecuados, por ejemplo, etanol, isopropanol, acetato de etilo, dietiléter, acetona, dimetilsulfóxido, dimetilformamida, glicerina, agua o mezclas de los mismos. La selección de disolvente se sigue según la solubilidad de los principios activos y aditivos, así como la humectación de las superficies a recubrir y el efecto sobre la estructura del recubrimiento restante después de la evaporación del disolvente y las partículas, su adhesión sobre la superficie y la transferencia de principio activo al tejido durante tiempos de contacto muy cortos.
La aplicación puede tener lugar, por ejemplo, mediante inmersión, extensión, aplicación mediante dispositivos de medida de volumen o pulverización, respectivamente a distintas temperaturas y dado el caso saturaciones de vapor del disolvente en la atmósfera. El proceso puede repetirse varias veces, dado el caso también usando distintos disolventes y coadyuvantes.
Los globos de catéteres con globo plegados preparados pueden recubrirse mediante inmersión en disoluciones que contienen principio activo u otras medidas de forma asombrosamente homogénea y reproducible, de dosis controlable y sin alteración de la función del catéter. Con inmersiones repetidas en disoluciones de principio activo insaturadas no se llega, en contra de lo esperado, a un desprendimiento completo del principio activo anteriormente aplicado, sino a una elevación reproducible del contenido de principio activo de los globos.
La disolución en exceso o las sustancias adheridas sueltas externamente en exceso pueden retirarse de la disolución de recubrimiento con procedimientos sencillos, sin llegar a una alteración de la eficacia del recubrimiento.
Los catéteres con globo configurados y preparados según la invención de distintos tipos se ponen en contacto poco tiempo, es decir, durante segundos, minutos o pocas horas, con el tejido. En algunos casos, es deseable tratar farmacológicamente el tejido en la vecindad inmediata del producto medicinal, por ejemplo, obstaculizar el crecimiento excesivo como reacción a una lesión o reducir el crecimiento tumoral o potenciar la vascularización o reducir las reacciones inflamatorias. En todos estos casos, puede conseguirse con el procedimiento anteriormente descrito una alta concentración local de medicamento durante un tiempo asombrosamente largo. Es una ventaja esencial la extraordinaria variedad de posibilidades de uso de los productos y procedimientos descritos.
Es una aplicación preferida la disminución de la hiperproliferación de las paredes vasculares inducida mediante catéteres con globo por la dilatación vascular. Esto se consigue dado el caso en la zona de prótesis vasculares implantadas (prótesis endovasculares) también mediante el recubrimiento de las prótesis endovasculares con medicamentos, sin embargo sólo en la zona vascular cubierta directamente por la prótesis endovascular. Los catéteres con globo recubiertos tratan además las zonas necesitadas de tratamiento poco antes y poco después de la prótesis endovascular, pueden tratar sin una nueva implantación de prótesis endovascular la zona dentro de las prótesis endovasculares ya proporcionadas o vasos en los que no debe o no puede implantarse una prótesis endovascular. Es ventajoso frente a las prótesis endovasculares de liberación de medicamento durante un largo periodo de tiempo la mejor curación con una inhibición igualmente buena de la hiperproliferación y un menor riesgo de trombosis.
A continuación, se describen varias formas de realización de la invención como ejemplos del recubrimiento de catéteres con globo, así como con respecto a la adhesión del recubrimiento en la sangre, la inhibición de la reestenosis y el contenido de principio activo del catéter.
Ejemplo 1
Recubrimientos de un catéter con globo expandido con paclitaxel en acetato de etilo
Se sumergen catéteres con globo de la compañía BMT, Oberpfaffenhofen/Múnich, Alemania, con la referencia Joker Lite, tamaño de globo 2,5 mm x 20 mm, después de expansión máxima durante 1 min a lo largo de toda la longitud del globo, en acetato de etilo, 18,8 mg de paclitaxel/ml + 1% de aceite de oliva farmacéutico y se secan.
Contenido de paclitaxel: 39 \mug (después de extracción con etanol, HPLC).
Ejemplo 2
Recubrimientos de un catéter con globo plegado con paclitaxel en acetato de etilo
Se sumergen catéteres con globo de la compañía BMT, Oberpfaffenhofen/Múnich, Alemania, con la referencia Joker Lite, tamaño de globo 2,5 mm x 20 mm, en estado plegado durante 1 min a lo largo de toda la longitud del globo, en acetato de etilo, 18,8 mg de paclitaxel/ml + 1% de aceite de oliva farmacéutico y se secan.
Contenido de paclitaxel: 69 \mug.
Ejemplo 3
Recubrimientos de un catéter con globo plegado con paclitaxel en acetato de etilo

Recubrimientos de un catéter con globo con paclitaxel en acetona
Se disuelven 350 mg de paclitaxel en 9,0 ml de acetona; se sumergen catéteres con globo de la compañía BMT, Oberpfaffenhofen/Múnich, Alemania, con la referencia Joker Lite, tamaño de globo 2,5 mm x 20 mm, en estado expandido máximo durante 1 min a lo largo de toda la longitud del globo, se extraen y se seca el disolvente a temperatura ambiente durante 12 h. Después, se desinfla el globo y se pliega con una herramienta recubierta con PTFE de modo habitual. Opcionalmente, puede insertarse una prótesis endovascular de tamaño adecuado en el globo. 29 \mug de paclitaxel en el globo.
Ejemplo 5
Recubrimientos de un catéter con globo con paclitaxel en acetona
a)
Se sumergen catéteres con globo plegados de la compañía BMT con la referencia Allegro, tamaño de globo 2,5 x 20 mm en una mezcla de 0,15 ml de etanol + 4,5 \mul de Ultravist 300 (agente de contraste de rayos X de Schering AG, Berlín, Alemania) + 1,35 ml de acetona + 0,8 mg de rojo Sudán + 30,0 mg de paclitaxel. Se sumergen las secciones plegadas del globo del catéter 5 veces, la primera vez durante 1 minuto, después 3 h de tiempo de secado, después 4 veces durante 5 s cada una separadas por 1 h; después, se insertó una prótesis endovascular y se esterilizó el catéter con prótesis endovascular de modo habitual con óxido de etileno.
Contenido de paclitaxel: 172 \mug, sin productos de degradación del principio activo detectables mediante HPLC.
b)
En lugar de Ultravist 300, se añadió una disolución acuosa saturada de manita.

En lugar de Ultravist 300, se añadió una disolución acuosa saturada de salicilato de sodio, pH 7,5.

Ejemplo 6
Adhesión del principio activo en la sangre
Se usaron 12 catéteres con globo de la compañía BMT con la referencia Allegro, tamaño de globo 2,5 x 20 mm. Se sumergieron 6 piezas de secciones de globo plegadas del catéter en [0,15 ml de etanol + 4,5 \mul de Ultravist 300 + 1,35 ml de acetona + 0,8 mg de rojo Sudán + 30,0 mg de paclitaxel) o en (1,5 ml de acetato de etilo + 0,8 mg de rojo Sudán + 31,0 mg de paclitaxel) 5 veces, la primera vez durante 1 minuto, después 3 h de tiempo de secado, después 4 veces durante 5 s cada vez separadas por 1 h; después se agitaron suavemente 3 globos plegados de cada serie en 50 ml de sangre humana a 37ºC, y después se extrajeron para análisis del contenido de paclitaxel: reducción de los valores medios (n= 3 para cada procedimiento de recubrimiento) mediante 5 minutos de agitación en sangre, en comparación con 3 catéteres de control que no se habían incubado en sangre.
Acetona: 12%
Acetato de etilo: 10%.
Ejemplo 7
Análisis de la inhibición de la reestenosis después de angioplastia e implantación de prótesis endovascular en coronarias de cerdos
Se sumergieron catéteres con globo plegados de la compañía BMT de tipo Joker Lite, BMT 3,5 x 20 mm ó 3,0 x 20 mm en:
disolución A) 3,33 ml de acetato de etilo (EA) + 100,0 mg de paclitaxel o
disolución B) 0,45 ml de etanol + 100 \mul de Ultravist-370 + 4,5 ml de acetona (Ac) + 150,0 mg de paclitaxel,
durante 1 min y se secaron a temperatura ambiente durante una noche. Se llevaron a cabo un proceso de inmersión (dosis menor= L) o 4 procesos de inmersión (dosis mayor= H) adicionales respectivamente durante 5 s el día siguiente separados por 1 h.

Contenido de principio activo después de 2 inmersiones en disolución (B): de media 250 \mug, con 5 inmersiones en disolución (B), 500 \mug, en disolución (A), 400 \mug.
En total se implantaron en 22 cerdos prótesis endovasculares mediante el catéter recubierto con paclitaxel o mediante el catéter no recubierto en la pared anterior o lateral de la arteria coronaria izquierda, con lo que se dilataron ligeramente los vasos para la estimulación de la reestenosis mediante hiperplasia de tejido. Después de 5 semanas, se volvió a hacer una angiografía a los animales y se midió la extensión del estrechamiento vascular en los angiogramas con un programa informático automático.

Se separaron los globos del ejemplo 8 después de la implantación de prótesis endovascular y extracción de los animales a aproximadamente 3 cm de longitud de los catéteres con globo, y se transfirieron a 1,5 ml de etanol. Se determinó el contenido de paclitaxel mediante HPLC. Se analizaron todos los globos recubiertos disponibles y una selección de los globos no recubiertos.

A partir del ejemplo 6 se deduce que se pierde como máximo el 10% de la dosis antes de expandirse el globo y aproximadamente el 10% de la dosis permanece en el globo.

Se introduce probucol a una concentración de 100 mg/ml en acetona; se utiliza la disolución como se describe en los ejemplos anteriores para el recubrimiento del catéter con globo.
Ejemplo 10
Se disuelve rapamicina a una concentración de 10 mg/ml en dietiléter. Se realiza el recubrimiento de la parte de globo del catéter como se describe en los ejemplos anteriores; los globos deben ajustarse inmediatamente lo más horizontales posibles después de la extracción de la disolución de recubrimiento y girarse constantemente alrededor de su eje longitudinal.
Ejemplo 11
Se disuelve epotilona B a una concentración de 2 mg/ml en acetato de etilo; se utiliza la disolución como se describe en los ejemplos anteriores para el recubrimiento del catéter con globo.

Claims (22)

1. Catéter con globo, caracterizado porque se adhieren a la superficie del globo medicamentos lipófilos insolubles en gran medida en agua que se unen a cualquier componente de tejido con liberación de principio activo inmediata después del contacto con el tejido, en el que la zona cubierta por los pliegues está cubierta con medicamento secado después de la aplicación.
2. Catéter con globo según la reivindicación 1, caracterizado porque se proporciona sin o junto con prótesis endovasculares, catéteres y/o partes de estos, agujas y cables de guía así como prótesis endovasculares.
3. Catéter con globo según la reivindicación 1 ó 2, caracterizado porque contiene un globo en estado plegado preparado recubierto mediante inmersión, extensión, pulverización o aplicación mediante un dispositivo de medida de volumen en una disolución de principio activo de baja viscosidad.
4. Catéter con globo según la reivindicación 1 ó 2, caracterizado porque los medicamentos lipófilos son sustancias para la inhibición de la proliferación celular o los procesos inflamatorios o antioxidantes.
5. Catéter con globo según la reivindicación 4, caracterizado porque los medicamentos son paclitaxel y otros taxanos, rapamicina y sustancias relacionadas, tacrolimus y sustancias relacionadas, corticoides, hormonas sexuales y sustancias relacionadas, estatinas, epotilona, probucol, prostaciclina o inductores de la angiogénesis.
6. Catéter con globo según la reivindicación 4 ó 5, caracterizado porque los medicamentos lipófilos se presentan como sustancias sólidas secas o aceites sobre la superficie del globo.

Catéter con globo según la reivindicación 6, caracterizado porque la dosis eficaz de los medicamentos comprende estructuras amorfas con un tamaño de partícula entre <0,1 \mum y 5 \mum, que se disuelven rápidamente debido a su gran superficie a pesar de la baja solubilidad acuosa del principio activo.
8. Catéter con globo según la reivindicación 1, caracterizado porque los medicamentos lipófilos se incorporan a una sustancia matriz fácilmente soluble en agua para conseguir un buen efecto adherente sobre la superficie del globo y para mejorar la captación en el tejido.
9. Catéter con globo según la reivindicación 8, caracterizado porque la sustancia matriz está compuesta por una sustancia hidrófila de bajo peso molecular con un peso molecular < 5.000 Da.
10. Catéter con globo según la reivindicación 8 ó 9, caracterizado porque la sustancia matriz es un agente de contraste, particularmente un agente de contraste de rayos X yodado.
11. Catéter con globo según la reivindicación 10, caracterizado porque el principio activo es paclitaxel y el agente de contraste de rayos X es Ultravist.
12. Catéter con globo según la reivindicación 1, caracterizado porque los medicamentos lipófilos se absorben en partículas o se aplican con una matriz de bajo peso molecular sobre la superficie del dispositivo.
13. Catéter con globo según la reivindicación 1, caracterizado porque las superficies se recubren adicionalmente con sustancias para influir en la capacidad de deslizamiento del dispositivo o la reducción de la coagulación sanguínea.
14. Procedimiento para la preparación de un catéter con globo recubierto según la reivindicación 1 a 13, caracterizado porque los medicamentos lipófilos y los coadyuvantes se aplican en un medio de disolución, suspensión o emulsión mediante inmersión, extensión, pulverización o mediante dispositivo de medida de volumen sobre la superficie de un globo plegado, en el que se separan los medios en exceso y las sustancias adheridas sueltas a la superficie.
15. Procedimiento según la reivindicación 14, caracterizado porque el proceso de recubrimiento se lleva a cabo varias veces con una elevación reproducible del contenido de principio activo con los mismos o distintos medios de disolución, suspensión y emulsión y/o coadyuvantes.
16. Procedimiento según la reivindicación 14, caracterizado porque se utilizan como medios de disolución, suspensión y emulsión etanol, isopropanol, acetato de etilo, dietiléter, acetona, dimetilsulfóxido, dimetilformamida, glicerina, agua o mezclas de los mismos.
17. Procedimiento según una de las reivindicaciones 14 a 16, caracterizado porque se proporcionan como portador de principio activo globos plegados listos en gran medida para aplicación, que se recubren antes o después de la esterilización con o sin prótesis endovasculares aplicadas.
18. Procedimiento según la reivindicación 14, caracterizado porque se ensamblan prótesis endovasculares unidas a un catéter con globo antes o después de un proceso de recubrimiento.
19. Procedimiento según la reivindicación 14, caracterizado porque los catéteres recubiertos preparados se esterilizan mediante óxido de etileno.
20. Uso de los catéteres con globo recubiertos configurados y preparados según las reivindicaciones 1 a 19 para la preparación de un agente para el tratamiento de enfermedades vasculares o alteraciones circulatorias.
21. Uso de los catéteres con globo recubiertos configurados y preparados según las reivindicaciones 1 a 19 para la preparación de un agente para la formación de pasos abiertos en el cuerpo.
22. Uso de los catéteres con globo recubiertos configurados y preparados según las reivindicaciones 1 a 19 para la preparación de un agente para el tratamiento de tumores.
