Abstract
Un aparato de sujeción quirúrgico que comprende:un bastidor de retención de sujetador (24) que incluye una primera parte anular (24a) que tiene unapluralidad de retenedores (28) dispuestos en la misma y una segunda parte anular (24b) que tiene unapluralidad de perforaciones (26) formada en la misma;un miembro de guía de sujetador (30) que contiene una pluralidad de sujetadores quirúrgicos (32), siendo elsujetador quirúrgico acoplable con la pluralidad de retenedores para sujetar el tejido entre los mismos;un empujador anular (40) para expulsar la pluralidad de sujetadores quirúrgicos del miembro de guía desujetadores hacia la pluralidad de retenedores correspondientes; yuna hoja anular (51) para cortar la pluralidad de perforaciones después de que la pluralidad de sujetadoresquirúrgicos haya sido acoplada con la pluralidad de retenedores; caracterizado poruna estructura de soporte para el bastidor de retención de sujetador, comprendiendo la estructura desoporte un miembro de brida (16) unido fijamente a un yunque y asegurando de manera separable elbastidor de retención de sujetador, en donde el miembro de brida tiene un rebaje anular (18) y un reborde(18a), en donde el bastidor de retención de sujetador está dispuesto en el rebaje anular y asegurado en elmismo por el reborde.

Aparato de sujeción quirúrgico

- Campo Técnico
La presente invención se refiere a un aparato de sujeción quirúrgico, y más concretamente, a una aparato de sujeción quirúrgico que tiene un sistema sujetador y retenedor.
- 2.
- Antecedentes de la Técnica Referida
Son bien conocidos diversos tipos de instrumentos de grapado para realizar una anastomosis circular, en los que un operador acciona el aparato en una posición que es relativamente remota de la posición en la que tiene lugar la anastomosis circular. La anastomosis es la unión quirúrgica de secciones de órgano separadas. Típicamente un proceso de anastomosis implica cirugía en la que una sección enferma o defectuosa del tejido hueco es retirada.
Un instrumento de grapado quirúrgico convencional para realizar una anastomosis circular en un procedimiento hemorroidal u otro procedimiento quirúrgico incluye un conjunto de valvas que tiene una disposición anular de grapas y ranuras de grapa y un conjunto de yunque que tiene una disposición circular de bolsillos de formación de grapa en una superficie proximal del mismo. Un ejemplo de instrumentos de grapado quirúrgicos para realizar anastomosis circular se describe en la Patente de Estados Unidos Nº 7.168.604, 7.303.106 y la solicitud de Estados Unidos pendiente 12/550.443, presentada el 31 de Agosto de 2009. Típicamente, el conjunto de yunque se puede mover longitudinalmente desde una posición abierta a una posición cerrada que coloca el conjunto de yunque adyacente al conjunto de valva para pinzar el tejido del cuerpo entre los mismos. Después de llevar los conjuntos de yunque y valva a una posición cerrada para pinzar el tejido, un empujador anular que incluye una pluralidad de empujados que están configurados para expulsar una correspondiente grapa de la ranura de grapa a través de los tejidos y contra un bolsillo de recepción del yunque para formar las grapas, es accionado por el operador en una región relativamente remota para realizar una anastomosis circular. Después de grapar los tejidos juntas, el tejido es cortado por la hoja anular y extraído.
Se conocen instrumentos de sujeción quirúrgica que aplican dos sujetadores quirúrgicos de parte. En estos instrumentos, una pluralidad de retenedores están soportados en un conjunto de yunque y una pluralidad de sujetadores están soportados en un conjunto de sujeción. Los sujetadores son hechos avanzar a través del tejido y hasta acoplamiento con las aberturas en los respectivos retenedores. Estos dos sujetadores están compuestos típicamente por un material reabsorbible. La fuerza de disparo de los sujetadores en algunos de estos instrumentos podría mover los retenedores fuera de su posición lo que puede dar lugar a desalineación. Además, los retenedores se pueden deslizar contra el yunque y/o se pueden soltar del yunque antes del disparo lo que puede dar ligar a desalineaciones. Por consiguiente, sería ventajoso proporcionar una configuración que aumenta el acoplamiento de los sujetadores y los retenedores y que pueda ser útil para proporcionar tal configuración en un instrumento de anastomosis circular.
El documento US2009/0001121 describe una grapadora quirúrgica que tiene una arandela que está insertada en un yunque. La arandela incluye una parte anular exterior que incluye orificios, en los que son recibidas las grapas.
SUMARIO
La presente invención proporcionar un aparato quirúrgicos de acuerdo con la reivindicación 1.
La segunda parte anular que define la pluralidad de perforaciones puede estar dispuesta concéntricamente dentro de la primera parte anular que tiene una pluralidad de retenedores dispuestos circunferencialmente en la misma. En una realización, la primera parte anular tiene una pluralidad de eslabones que conectan la pluralidad de retenedores circunferencialmente dispuesta sobre la misma formando un único cuerpo de retenedores. En una realización alternativa, la pluralidad de retenedores están formados monolíticamente, En una realización, la primera parte anular está formada de manera monolítica con la segunda parte anular. La pluralidad de retenedores dispuestos circunferencialmente en la primera parte anular de bastidor de retención de sujetador puede estar dispuesta para definir dos anillos concéntricos de retenedores.
En una realización, la pluralidad de retenedores está separada de manera sustancialmente uniforme. En otra realización, la pluralidad de perforaciones dispuestas circunferencialmente en la segunda parte anular está separada de manera sustancialmente uniforme. Los retenedores pueden estar hechos de un polímero biodegradable. La pluralidad de sujetadores puede estar también hecha de un polímero biodegradable. El bastidor de retención de sujetador puede estar hecho de un polímero biodegradable.
Un lado distal del bastidor de retención de sujetador puede estar axialmente ahusado con respecto al espesor del mismo. El rebaje anular definido por el miembro de brida también puede estar axialmente ahusado con respecto al espesor del mismo correspondiente al bastidor de retención de sujetador axialmente ahusado. El bastidor de retención de sujetador en algunas realizaciones tiene una configuración de fijación por salto elástico con el miembro de brida.
En una realización, cada pluralidad de perforaciones definidas en la segunda parte anular tiene una parte radialmente ranurada configurada para facilitar la ruptura de la misma después del accionamiento de la cuchillo anular. En otra realización, cada pluralidad de perforaciones definidas en la segunda parte anular tiene un par de hendiduras opuestas configurada para facilitar la rotura de la misma después del accionamiento de la hoja anular. En una realización, la pluralidad de sujetadores quirúrgicos está configurada para fijarse por salto elástico con la pluralidad de retenedores. En algunas realizaciones, el espesor de una parte que define la pluralidad de perforaciones en la segunda parte anular del bastidor de retención de sujetador es menor que la del resto de la segunda parte anular para facilitar la rotura de la pluralidad de separaciones. En algunas realizaciones, el bastidor de retención de sujetador puede ser elástico. En algunas realizaciones, el miembro de guía de sujetador puede incluir una pluralidad de ranuras sujetadores que se extiende axialmente y dispuesta circunferencialmente para adaptarse a la pluralidad de sujetadores quirúrgicos.

DESCRIPCIÓN DETALLADA DE LAS REALIZACIONES
Se describirán con detalle varias realizaciones de la presente invención y el efector con referencia a los dibujos, en los que los números iguales identifican similares o idénticos elementos. En los dibujos y en la descripción que sigue, el término “proximal” se referirá al extremo del componente que está más cerca del operador durante el uso, mientras que el término “distal” se referirá al extremo del componente que está más lejos del operador, como es tradicional y convencional en la técnica.
Con referencia la las Figs. 1, 2 y 7, se ilustra un efector de extremo 100 adecuado para un aparato de sujeción quirúrgico 1000. Un ejemplo adecuado de un aparato de sujeción quirúrgico 100 se expone en las Patentes de Estados Unidos Nº 7.168.604, 7.303.106 y la solicitud de patente de Estados Unidos número de serie 12/550.443, presentada el 31 de Agosto de 2009. El aparato de sujeción quirúrgico 1000 incluye un conjunto de mango 200, un vástago alargado 300 que se extiende distalmente desde el mismo, y un efector 100 acoplado al extremo distal del vástago alargado 300. La longitud y la curvatura del vástago alargado 300 pueden ser adaptadas para cumplir con las necesidades específicas del procedimiento quirúrgico que se está realizando. El vástago alargado 300 alternativamente puede ser flexible para facilitar las maniobras del aparato de sujeción quirúrgicos 1000, más concretamente, el efector extremo 100, al área objetivo en el cuerpo que contiene los tejidos a unir.
El conjunto de mango 200 generalmente incluye un mango estacionario 240, un gatillo pivotable 210 y un pomo giratorio 220. Cuando el pomo giratorio 220 es girado manualmente, el conjunto de yunque 20 se trasladará longitudinalmente con relación al conjunto de valva 10 entre una posición abierta y una posición cerrada de la manera que se describirá más adelante. El accionamiento del gatillo 210 hacia el mango estacionaria 240 hace avanzar el empujador anular 40 (Fig. 3) distalmente dentro del conjunto de válvula 10 para expulsar los sujetadores del conjunto de valva 10 de la manera descrita más adelante. El accionamiento del gatillo 210 también traslada longitudinalmente un cuchillo circular 51 que corta el tejido además de separar los sujetadores formados a partir de un bastidor de retención de la manera descrita a más adelante.
Haciendo referencia ahora a las Figuras 3 y 5, una realización de la presente invención se muestra generalmente como un efector de extremo de grapas 100 que define un eje longitudinal “A-A”. El efector de extremo de grapas 100 incluye un conjunto de yunque 20 y un conjunto de valva 10. El conjunto de yunque 20 incluye un yunque 22, un bastidor de retención de sujetador 24, y un miembro de brida 16. El miembro de brida 16 está preferiblemente compuesto de material plástico tal como HDPE. Como se expondrá con más detalle a continuación, el bastidor de retención de sujetador 24 incluye una pluralidad de perforaciones 26 y una pluralidad de retenedores 20 (Fig. 5) y se está asegurado de forma separable al miembro de brida 16 que está unida fijamente al yunque 22. El conjunto de válvula 10 aloja en el mismo un miembro de guía de sujetador 30 que incluye una pluralidad de ranuras 34 de sujetador que se extiende axialmente, dispuestas circunferencialmente, para recibir en la misma una pluralidad de sujetadores quirúrgicos 32 que se extiende axialmente que tienen dientes de perforación de tejido dirigidos distalmente para acoplarse con la pluralidad de retenedores 28 en el bastidor de retención de sujetador 24 para sujetar el tejido entre los mismos. El conjunto de valva 10 aloja también en el mismo un empujador anular 40 que incluye una pluralidad de dedos empujadores 42 operativamente asociados con la pluralidad de sujetadores quirúrgicos 32. El empujador 42 está configurado para expulsar un sujetador quirúrgico asociado 32 hacia un correspondiente retenedor 28 en el bastidor de retención de sujetador 24. Un extremo distal del vástago de accionamiento 4 está operativamente conectado con el empujador anular 40 y el extremo proximal del vástago de accionamiento 4 está operativamente conectado con el gatillo 210, por lo que el accionamiento del gatillo 210 hace avanzar distalmente el vástago de accionamiento 4 que hace avanzar el empujador anular 40 dentro del alojamiento 45 del conjunto de vástago 10 para expulsar los sujetadores quirúrgicos 32 del miembro de guía de sujetador 30 de la manera que se describirá más adelante. Un cuchillo circular 51 con una hoja de corte anular es hecho avanzar por el empujador 40 para cortar el tejido de la manera descrita más adelante.
Haciendo particular referencia a las Figs 5 y 6, el bastidor de retención de sujetador 24 se describirá con detalle. El bastidor de retención de sujetador 24 incluye una primera parte anular 24a que tiene una pluralidad de retenedores 28 circunferencialmente dispuestos en la miasma y una segunda parte anular 24b que tiene una pluralidad de perforaciones dispuestas circunferencialmente 26. Las dos partes 24a y 24b están dispuestas concéntricamente, con la segunda parte anular 24b dispuesta concéntricamente dentro de (radialmente hacia dentro de) la primera parte anular 24a. La pluralidad de retenedores 28 está formada o bien monolíticamente o bien individualmente y conectada mediante estructuras adecuadas, por ejemplo eslabones, por lo que se forma un único cuerpo. Además, la primera parte anular 24a puede estar formada monolíticamente con la segunda parte anular 24b. La primera parte anular 24a y la segunda parte anular 24b pueden estar hechas de materiales diferentes; en algunas realizaciones están hechas de polímeros biodegradables (por ejemplo L4). Para mayor claridad, sólo están etiquetados unos pocos retenedores, perforaciones, etc.
Con referencia todavía a las Figs. 5 y 6, cada retenedor 28 incluye un puente 27 y un par de postes 29 formando cada uno una cavidad o abertura 29a en los mismos (nótese que para mayor claridad sólo uno de los retenedores está etiquetado en la Fig. 5). Cada retenedor 28 esta acopado con un sujetador quirúrgico asociado 32. Específicamente, cada diente 33 del sujetador quirúrgico asociado 32 está insertado en cada cavidad 29a del retenedor asociado 28. El sujetador quirúrgico 32 y el retenedor 28 pueden estar configurados para acoplarse mediante salto elástico. Además, una punta 33a de cada diente 33 puede incluir una lengüeta 34 para aumentar el acoplamiento de seguridad con el retenedor 28. El puente 35 (o “backspan”) se extiende entre los dientes 33. La pluralidad de retenedores 28 circunferencialmente dispuestos en la primera parte anular 24a del bastidor de retención de sujetador 24 están dispuestos para definir una anillo de retenedores 28. La primera parte anular 24a en la realización ilustrada incluye dos anillos de retenedores 28 en donde cada anillo tiene 16 retenedores, aunque se contempla un número diferentes de retenedores y/o un número diferente de anillos. Cuando están presentes más de un anillo de retenedores 29 en la parte anular 24a, los retenedores 28 que forman un anillo pueden estar radialmente alineados con los que forman el otro anillo. Alternativamente, los retenedores 28 que forman un anillo pueden tener un solape parcial o estar radialmente desplazados con los que forman el otro anillo para permitir la integridad uniforme del grapado circular. Los retenedores 28 pueden estar separados de forma sustancialmente uniforme; sin embargo, la separación entre retenedores 28 puede ser confeccionada para cumplir las necesidades específicas de la operación quirúrgica que se va realizar. Los sujetadores 32 y los retenedores 28 están preferiblemente hechos de una material reabsorbible.
Con referencia todavía a la Fig. 5, el bastidor de retención de sujetador 24 incluye una pluralidad de perforaciones 26 en la segunda parte anular 24b del mismo. La pluralidad de perforaciones 26 puede estar formada de forma sustancialmente uniforme (separada de forma sustancialmente uniforme) en la segura parte anular 24b. Como alternativa a la separación equidistante o separación sustancialmente equidistante, también se contemplan otras separaciones para las perforaciones incluyendo perforaciones arbitrariamente definidas o separadas de otro modo. En una realización, la pluralidad de perforaciones 26, al igual que los retenedores 28, pueden definir más de un anillo de perforaciones. El diámetro de cada pluralidad de perforaciones 26 puede ser seleccionada de manera que el espesor de la hoja anular es menor que el diámetro de cada una de las perforaciones 26. Tal configuración puede facilitar el corte o rotura de la rotura de la pluralidad de perforaciones. Además, cada perforación 26 puede incluir una parte con muesca radialmente configurada para facilitar el corte o rotura de las perforaciones 26 después del accionamiento del empujador anular 40. Alternativamente, cada perforación 26 puede incluir un par de hendiduras opuestas que sirven también para facilitar la rotura de las perforaciones 26 después del accionamiento del empujador anular. Cuando se empelan las perforaciones 26 que contienen por ejemplo el par de hendiduras, la anchura de las hendiduras se puede elegir para corresponderse con el espesor de la hoja anular para permitir que la hoja anular sea acoplada dentro de las hendiduras opuestas de la perforación asociada. En algunas realizaciones, el espesor de la segunda parte anular 24b que define la pluralidad de perforaciones 26 puede variar. Por ejemplo, la parte interna de la segunda parte anular 24b, es decir, la parte que está dentro de la pluralidad de perforaciones 26, puede tener un espesor menor que el de la `parte exterior de la segunda parte anular 24b, es decir una parte que está en contacto con la primera parte anular 24a, por lo que la parte relativamente más delgada o una parte más frágil puede tener para facilitar la rotura de las perforaciones dando lugar a una separación del bastidor de retención de sujetador 24 del miembro de brida 16 como se expondrá más adelante. En la alternativa, el espesor de sólo una parte que define la pluralidad de perforaciones 26 puede ser menor que el del resto de la segunda parte anular 24a por lo que tal configuración facilita también la rotura de la pluralidad de perforaciones 26 dando lugar a la separación del bastidor de retención de sujetador 24 del miembro de brida 16.
Volviendo a la Fig. 4, se ilustra el conjunto de yunque 20 es una posición cerrada anterior al disparo de los sujetadores quirúrgicos 32 por el accionamiento del gatillo 210. El miembro de brida 16 tiene un rebaje anular 18 e incluye un reborde 18a para formar una estructura de soporte/retención para el bastidor de retención de sujetador
24. El bastidor de retención de sujetador 24 está dispuesto en un rebaje anular 18 y está asegurado en el mismo mediante el reborde 18a. La Figura 4 ilustra el reborde 18a asegurando el bastidor de retención de seguridad 24 en la segunda parte anular 24b. Por consiguiente, el espesor de la parte interna de la segunda parte anular 34b y/o la rigidez de la segunda parte anular 24b se puede elegir, por ejemplo, seleccionado el material que tiene la rigidez requerida, para evitar el doblado del bastidor de retención de sujetador acoplado 24 en las perforaciones y/o en una posición en contacto con el reborde 18a antes de cortar la pluralidad de perforaciones 26. En una realización, un lado distal del bastidor de retención de sujetador 24 está axialmente ahusado con respecto al espesor del mismo para aumentar la unión segura del mismo al rebaje anular 18 del miembro de brida 16 también ahusado axialmente con respecto al espesor del mismo. El miembro de brida 16 que tiene un bastidor de retención de sujetador 24 asegurado de manera separable al mismo está fijamente unido al yunque 22 de manera que el bastidor de retención de sujetador 24 y el yunque 22 se mueven como una única unidad en una translación longitudinal del mismo a través de una operación manual del pomo rotaciones 220 realizada por el operador.
Haciendo todavía referencia a la Fig. 4, el conjunto de valva 10 aloja el miembro de guía de sujetador 30 que contiene una pluralidad de ranuras sujetadoras dispuestas circunferencialmente y que se extienden radialmente 34 para recibir en las mismas una pluralidad de sujetadores quirúrgicos que se extiende axialmente 32 que tiene dientes de perforación de tejido 33 para acoplarse con una pluralidad de retenedores 28 en el bastidor de retención de sujetador 24 para sujetar el cuerpo del tejido entre los mismos. La Fig. 4 ilustra un sujetador quirúrgico 32 cargado en la ranura de sujetador 34 y que tiene un lado proximal en el mismo acoplado con un empujador asociados 42 antes de que sea disparado.
En uso, el conjunto de yunque 20 está separado del conjunto de valva 10 como se muestra en la Fig. 7 de manera que el efector de extremo está en una posición abierta no aproximada. Una vez que los tejidos corporales que vana a ser sujeto son colocados en el espacio abierto entre el conjunto de yunque 20 y el conjunto de valva 10, el conjunto de yunque 20 es trasladado de manera proximal a una posición para pinzar los tejidos corporales entre los mismos de manera que el efector de extremo está en una posición cerrada o aproximada. Esto se consigue girando el pomo giratorio 220 del conjunto de mango 200.
Como se muestra en la Fig. 4, el conjunto de yunque 20 es una posición cerrada previa al disparo está acoplado con el conjunto de valva 10 (véase la Fig. 1).
La unión del bastidor de retención de sujetador 24 al miembro de brida 16, que está unido fijamente al yunque 22, reduce el deslizamiento del bastidor de retención de sujetador 24 contra el yunque 22 y retiene su posición con respecto al yunque 22 durante las translaciones axiales del mismo. Una vez que ha tenido lugar el pinzamiento de los tejidos corporales, el operador acciona el gatillo 210 que a su vez acciona distalmente el vástago anular 4 conectado al empujador anular 40. La pluralidad de empujadores 42 en el empujador anular 40 distalmente trasladado expulsa el correspondiente conjunto de sujetadores quirúrgicos 32 de las ranuras de sujetadores 34 del miembro de guía de sujetador 30. Cada conjunto expulsado de sujetadores quirúrgicos 32 perfora a través del tejido corporal y son recibidos en un correspondiente retenedor 28. Específicamente, una parte de cada uno de los dientes del sujetador 32 es recibida en una respectiva cavidad 29a formada en los dientes 29 en el retenedor 28. Nótese que la punta 33 y las lengüetas 34 de los dientes de sujetador 33 deforman las paredes de recepción 29b de los postes 29 cuando el diente de sujetador 33 es forzado a través de la cavidad 29a. Las lengüetas se acoplan a un reborde de la cavidad 29a para evitar la retracción (movimiento proximal) del sujetador 32, por lo que se bloquea el sujetador 32 con el respectivo retenedor 28. Nótese que dado que el bastidor de retención de sujetador 24 está unido de forma segura al miembro de brida 16 que está unido fijamente al yunque 22, el deslizamiento del bastidor de retención de sujetador 24 contra el yunque 22 debido a la fuerza de disparo producida por la activación del gatillo 210 que expulsa la pluralidad de sujetadores 32 de las ranuras de sujetador 34 en los tejidos corporales es reducido. De este modo, la posición del bastidor de retención de sujetador 24 se mantiene con respecto a un yunque 22 durante el disparo de los sujetadores quirúrgicos 32. Además, la pluralidad de retenedores 28 está formada o bien monolíticamente o bien está formada individualmente y conectados mediante estructuras adecuadas, por ejemplo eslabones, formado por tanto un único cuerpo. En cualquier ejemplo, la configuración de la pluralidad de retenedores 28 como un único cuerpo, en combinación con el bastidor de retención de sujetador 24 que está asegurado al miembro de brida que está unido fijamente al yunque 22, reduce la desalineación radial de los sujetadores quirúrgicos 32 con el bastidor de retención de sujetador 24.
Como se ha observado anteriormente, el bastidor de retención de sujetador 24 y el miembro de guía de sujetador 30 puede estar configurado para proporcionar una pluralidad de anillos de sujetadores quirúrgicos 32 en los tejidos, por ejemplo, dos anillos de sujetadores quirúrgicos en donde cada anillo está definido por dieciséis sujetadores, para cumplir con las necesidades del procedimiento específico que se está realizando. Después del acoplamiento de la pluralidad de sujetadores quirúrgicos 32 con el bastidor de retención de sujetador 24, se accionada una hoja anular 51 para trasladarse distalmente a través del conjunto de valva 10. Nótese que la hoja anular se puede trasladar mediante el accionamiento del gatillo 210 que hace avanzar los sujetadores, o alternativamente mediante un accionador separado accionado en una etapa separada. La hoja anular entre en contacto con el bastidor de retención de sujetador 24 y es presionada contra la pluralidad de perforaciones 26 definidas en el bastidor de retención de sujetador 24 cortando o rompiendo las perforaciones 26. Las perforaciones rotas o cortadas 26 hacen posible que el sujetador quirúrgico acoplado 32 y el bastidor de retención de sujetador 24 sea separado del yunque
22. El corte o rotura de las perforaciones 26 deja un anillo residual, es decir, la parte interna de la segunda parte anular 24b del bastidor de retención de sujetador 24, dentro del reborde 18a del rebaje anular 18 del miembro de brida 16. En algunas realizaciones, para evitar las virutas de perforación que se pueden producir durante el corte o la rotura de las perforaciones 26, cada pluralidad de perforaciones 26 se puede disponer con, por ejemplo, un par hendiduras o de partes con muesca, como se ha mencionado anteriormente, en donde el espesor de la hoja anular es seleccionado para que sea menor que, por ejemplo, la anchura del par de hendiduras opuestas. El sujetador quirúrgicos acoplado 32 y el bastidor de retención de sujetador 24 que sujeta el tejido corporal entre los menos sería entonces liberado para desplazarse son estar restringido o unido al yunque 22. El accionamiento del cuchillo corta el tejido. El operador mueve después el yunque 22 a la posición abierta (no aproximada) girando el pomo giratorio 220 del conjunto de mango porque se traslada distalmente el conjunto de yunque 20. Después de la retirada del tejido cortado, por ejemplo el tejido hemorroidal, el dispositivo de sujeción quirúrgico es retirado del cuerpo.
De lo anterior y con referencia a los distintos dibujos de figuras, los expertos en la técnica apreciarán que también se pueden hacer ciertas modificaciones a la presente invención sin que se salgan del campo de la misma. Por ejemplo, la hoja anular puede estar asegurada fijamente al yunque y configurada para romper la pluralidad de perforaciones en el bastidor de retención de sujetador después del disparo de la pluralidad de sujetadores. Tal diseño elimina la necesidad de una translación longitudinal de la hoja anular. Adicionalmente, aunque descritas como partes anulares para utilizar con un aparato de sujeción circular, el uso de partes lineales para utilizar con el aparato que aplica disposiciones lineales de sujetadores también se contempla.
Aunque varias realizaciones de la invención han sido mostradas en los dibujos y/o expuestas aquí, no está destinada a que la invención se limite a las mismas y está destinada a que la invención tenga un campo amplio ya que la técnica permitirá que las especificaciones se lean de la misma manera. Por lo tanto, la descripción anterior no se debe interpretar como limitante, sino meramente como ejemplos de realizaciones particulares. Los expertos en la técnica preverán otras modificaciones dentro del campo de las reivindicaciones adjuntas a la misma.
Claims (14)
- REIVINDICACIONES1. Un aparato de sujeción quirúrgico que comprende:un bastidor de retención de sujetador (24) que incluye una primera parte anular (24a) que tiene una pluralidad de retenedores (28) dispuestos en la misma y una segunda parte anular (24b) que tiene una pluralidad de perforaciones (26) formada en la misma;un miembro de guía de sujetador (30) que contiene una pluralidad de sujetadores quirúrgicos (32), siendo el sujetador quirúrgico acoplable con la pluralidad de retenedores para sujetar el tejido entre los mismos;un empujador anular (40) para expulsar la pluralidad de sujetadores quirúrgicos del miembro de guía de sujetadores hacia la pluralidad de retenedores correspondientes; yuna hoja anular (51) para cortar la pluralidad de perforaciones después de que la pluralidad de sujetadores quirúrgicos haya sido acoplada con la pluralidad de retenedores; caracterizado poruna estructura de soporte para el bastidor de retención de sujetador, comprendiendo la estructura de soporte un miembro de brida (16) unido fijamente a un yunque y asegurando de manera separable el bastidor de retención de sujetador, en donde el miembro de brida tiene un rebaje anular (18) y un reborde (18a), en donde el bastidor de retención de sujetador está dispuesto en el rebaje anular y asegurado en el mismo por el reborde.

- Un aparato de sujeción quirúrgico de la reivindicación 1, en el que la pluralidad de retenedores en la primera parte anular está dispuesta circunferencialmente en la misma y la pluralidad de perforaciones de la segunda parte anular está circunferencialmente dispuesta en la misma, en el que la primera y la segunda partes anulares están dispuestas concéntricamente.

- Un aparato de sujeción quirúrgico de la reivindicación 1 ó 2, en el que la segunda parte anular está dispuesta concéntricamente dentro de la primera parte anular.

- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-3, en el que la primera parte anular incluye además una pluralidad de eslabones (27) que conectan la pluralidad de retenedores.

- 5.
- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-4, en el que la pluralidad de retenedores está monolíticamente formada.

- 6.
- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-5, en el que la primera parte anular está formada monolíticamente con la segunda parte anular.

- 7.
- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-6, en el que la pluralidad de retenedores circunferencialmente dispuestos en la primera parte anular del bastidor de retención de sujetador está dispuesta para definir dos anillos concéntricos de los retenedores.

- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-7, en donde la pluralidad de retenedores está separada sustancialmente de manera uniforme y la pluralidad de perforaciones dispuestas circunferencialmente en la segunda parte anular está separada de forma sustancialmente uniforme.

- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-8, en el que la pluralidad de retenedores y sujetadores quirúrgicos están hechos de polímero biodegradable y el bastidor de retención de sujetador está hecho de polímero biodegradable.

- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-9, en el que el bastidor de retención de sujetador y un lado distal del bastidor de retención de sujetador está axialmente ahusado con respecto al espesor del mismo.

- 11.
- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-10, en el que el bastidor de retención de sujetador tiene una configuración de fijación por salto elástico con el miembro de brida.

- 12.
- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-11, en el que cada una de las pluralidades de perforaciones de la segunda parte anular tiene una parte con muesca dispuesta radialmente o tiene una par de hendiduras opuestas.

- 13.
- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-12, en el que la pluralidad de sujetadores quirúrgicos está configurada para fijarse mediante salto elástico con la pluralidad de retenedores.

- 14.
- Un aparato de sujeción quirúrgico de cualquiera de las reivindicaciones 1-13, en el que el espesor de una parte que define la pluralidad de perforaciones en la segunda parte anular es menor que el del resto de la segunda parte anular.
