Abstract
Un dispositivo de mantenimiento de espacio para la implantación en un cuerpo humano o animal, comprendiendo el dispositivo de mantenimiento de espacio: un primer cuerpo tubular (4) que tiene una superficie de camisa con una pluralidad de aberturas para el crecimiento intersticial del tejido adyacente; al menos un segundo cuerpo tubular (3) que tiene una superficie de camisa con una pluralidad de aberturas, estando el segundo cuerpo tubular dispuesto en el interior del primer cuerpo tubular, con los extremos de los cuerpos tubulares alineándose para formar un dispositivo de mantenimiento de espacio multi-pared, caracterizado por que el segundo cuerpo está dispuesto directamente o a través de uno o más elementos de conexión en el primer cuerpo por medio de un ajuste por presión o un ajuste por fuerza, en el que la circunferencia exterior del segundo cuerpo o los segundos cuerpos es mayor que la dimensión interna del primer cuerpo o de un elemento de conexión en el que el segundo cuerpo o los segundos cuerpos se reciben por medio de un ajuste por presión o un ajuste por fuerza.

segundos cuerpos tubulares 3’, 3’’ y 3’’’ difieren en lo que respecta a sus dimensiones, es decir, el diámetro de los segundos cuerpos tubulares 3’, 3’’ y 3’’’ se elige mucho más pequeño que el del cuerpo tubular externo 4. Los receptores 32 de las placas de retención 30 están dispuestos en los puntos de esquina de un triángulo imaginario, de tal manera que los segundos cuerpos tubulares 3’, 3’’ y 3’’’ se alojan uno al lado de otro en el espacio interior del cuerpo tubular externo 4. Los ejes longitudinales tubulares de los segundos cuerpos tubulares 3’, 3’’ y 3’’’, que se extienden a través del centro de la sección transversal circular de los segundos cuerpos tubulares 3’, 3’’ y 3’’’, se desplazan, por lo tanto, en paralelo al eje longitudinal tubular central del cuerpo tubular externo 4.
Los segundos cuerpos tubulares 3’, 3’’ y 3’’’ también se alojan mediante un ajuste por presión o un ajuste por fuerza en los receptores 32 de las placas de retención 30. El diámetro exterior de los segundos cuerpos tubulares 3’, 3’’ y 3’’’ se elige de nuevo, por lo tanto, un poco mayor que el diámetro de los receptores 32, de tal manera que, en la inserción de los segundos cuerpos tubulares 3’, 3’’ y 3’’’, se produce una deformación elástica de los segundos cuerpos tubulares 3’, 3’’ y 3’’’ y de las placas de retención 30, lo que influye en el ajuste por presión de los cuerpos tubulares 3’, 3’’ y 3’’’ en los receptores 32.
Mientras que la realización de la figura 1 puede usarse como un dispositivo de mantenimiento de espacio para vértebras, la variante mostrada en la figura 2, también en una representación en perspectiva, está pensada como una sustitución para los discos vertebrales. En consecuencia, el dispositivo de mantenimiento de espacio 1 de la figura 2, en el que los componentes iguales o similares están provistos de números de referencia iguales, se elige mucho más pequeño en longitud. En consecuencia, solo se proporciona una única placa de retención 30, en lugar de las dos placas de retención de la realización de la figura 1. La placa de retención 30 en la realización de la figura 2 está dispuesta aproximadamente a la mitad de la altura del dispositivo de mantenimiento de espacio, es decir, en el medio del alargamiento a lo largo del eje longitudinal tubular.
Aparte de las diferencias descritas en el presente documento, la realización de la figura 2 no difiere de la de la figura 1, de manera que es superflua una descripción más detallada.
La figura 3 muestra una vista en planta de la realización de la figura 2 en la que se muestra claramente la disposición del cuerpo tubular externo 4 y de los segundos cuerpos tubulares internos 3’, 3’’ y 3’’’. Además, se muestran las aberturas 31, que se proporcionan en las placas de retención 30 para el crecimiento intersticial y la permeación del tejido. Como se muestra, las aberturas 31 pueden tener diferentes tamaños.
En general, con las realizaciones de las figuras 1 a 3, se proporciona un implante o dispositivo de mantenimiento de espacio que, a causa de la disposición de ajuste por presión o ajuste por fuerza elegida, puede fabricarse fácilmente y cuyos componentes facilitan una disposición sencilla y variable. Además, se proporciona al cuerpo tubular externo 4 el suficiente espacio libre para el crecimiento intersticial del tejido por la disposición de los cuerpos tubulares. Al mismo tiempo, sin embargo, se proporcionan unas superficies de contacto suficientemente grandes en los extremos del dispositivo de mantenimiento de espacio 1 para alojar y disipar la carga.
Las figuras 4 a 11 muestran diferentes realizaciones en las que, sin el uso de una placa de retención, uno solo o varios segundos cuerpos tubulares 3 de diferentes formas se alojan en los cuerpos tubulares externos de diferentes formas 4, de nuevo mediante un ajuste por presión o un ajuste por fuerza.
En la realización que se muestra en las figuras 4 y 5, en una representación de vista en planta en perspectiva, el cuerpo tubular externo 4 tiene, en un plano de sección transversal perpendicular al eje longitudinal tubular, es decir, perpendicular a la superficie de camisa, una forma de riñón, mientras que los segundos cuerpos tubulares 3’, 3’’ y 3’’’ alojados en el cuerpo tubular externo 4 tienen una sección transversal circular. En consecuencia, los segundos cuerpos tubulares 3’, 3’’ y 3’’’ también se alojan en este caso uno al lado de otro en el cuerpo tubular externo 4.
En el caso del dispositivo de mantenimiento de espacio 1, que, en las figuras 6 y 7, se muestra en unas representaciones de vista en perspectiva y de vista en planta, se disponen dos cuerpos tubulares cilíndricos 3’ y 3’’, que tienen una sección transversal circular, también mediante un ajuste por presión o un ajuste por fuerza, en un cuerpo tubular externo 4 con una sección transversal en forma de óvalo, mientras que, en la realización de las figuras 8 y 9, se disponen tres segundos cuerpos 3’, 3’’ y 3’’’ con forma tubular cilíndrica, es decir, de sección transversal circular, en un cuerpo tubular externo 4 que tiene una forma tubular-cilíndrica y, por lo tanto, también una sección transversal circular.
En las figuras 10 y 11, se muestra una realización adicional de un dispositivo de mantenimiento de espacio 1 de acuerdo con la invención, en el que, de nuevo, solo se disponen dos cuerpos tubulares uno dentro de otro. En la realización mostrada en las figuras 10 y 11, el cuerpo tubular interno 3 tiene una forma triangular en una sección transversal que se extiende perpendicular al eje longitudinal tubular, mientras que el cuerpo tubular externo 4, a su vez, posee una forma tubular cilíndrica con sección transversal circular. Por lo tanto, en la realización mostrada en las figuras 10 y 11, solo se aloja un cuerpo tubular 3 mediante un ajuste por presión o un ajuste por fuerza en el cuerpo tubular 4.

Las figuras 12 a 16 muestran diferentes formas de sección transversal de los cuerpos tubulares 2, 3, 4 del tipo que pueden usarse en la presente invención. Aparte de una sección transversal circular o anular, tal como la mostrada en la figura 12, pueden concebirse formas oblongas, especialmente formas rectangulares y preferentemente cuadradas (figura 13), formas hexagonales (figura 14), formas ovaladas (figura 15) o formas de riñón (figura 16). Además, existe la posibilidad de usar otras formas, tales como formas de base octogonal o formas totalmente libres. Se prefieren, sin embargo, formas de base simple. Especialmente, también es posible combinar entre sí cuerpos tubulares cilíndricos que tienen diferentes formas de sección transversal.
Las figuras 17 a 22 muestran diferentes formas de las aberturas 9 y su disposición recíproca en las superficies de camisa 10 de los cuerpos tubulares 2 a 4. Aparte de la forma de diamante (rombo) de la figura 17, pueden concebirse formas circulares (figura 18), formas oblongas, especialmente cuadradas y rectangulares (figura 19), formas hexagonales (figura 20), formas ovales (figura 21) o formas octogonales (figura 22). Además, pueden concebirse otras formas adecuadas que facilitan un área grande para las aberturas 9 en combinación con la estabilidad simultánea del armazón intercalado.
Por lo que se refiere a la disposición recíproca de las aberturas 9, estas pueden o bien disponerse en filas, en las que las aberturas 9 están totalmente separadas en filas, tal como en las figuras 19 y 22, o las aberturas se disponen en las filas de tal manera que sobresalen en las cavidades correspondientes de las filas adyacentes, como es especialmente pronunciado en las figuras 17 y 20.
Esto también muestra que las aberturas 9 en las columnas en las que están dispuestas pueden proporcionarse directamente unas debajo de otras o, preferentemente, desplazadas unas de otras, de tal manera que se mejora, especialmente, la disipación de la carga axial. Como muestran adicionalmente las figuras 17 a 22, las columnas con las aberturas 9 dispuestas unas debajo de otras pueden cada una desplazarse entre sí esencialmente por la mitad de la anchura de una abertura.
Las figuras 23 a 24 son representaciones laterales o en sección esquemáticas de las aplicaciones de los dispositivos de mantenimiento de espacio de acuerdo con la invención, sirviendo el dispositivo de mantenimiento de espacio 1 en la figura 23 como un disco vertebral de sustitución y sirviendo el dispositivo de mantenimiento de espacio 1 en la figura 24 como una vértebra de sustitución.
Los dispositivos de mantenimiento de espacio 1 en las aplicaciones de las figuras 23 y 24 son parte de un sistema de estabilización de columna vertebral, en el que los tornillos pediculares 20, especialmente los tornillos poliaxiales, están dispuestos en las vértebras, que alojan entre las mismas una varilla de conexión 21 para alinear y estabilizar recíprocamente la columna vertebral.
Debido a la disposición en la columna vertebral, los dispositivos de mantenimiento de espacio 1 para la columna vertebral o los discos vertebrales están expuestos a tensiones específicas, especialmente tensiones dinámicas. El dispositivo de mantenimiento de espacio de acuerdo con la presente invención y, en particular, la configuración multi-pared o la formación multi-componente de la misma, proporciona una solución para enfrentarse con las tensiones observadas.
Además, el dispositivo de mantenimiento de espacio de acuerdo con la invención también puede, sin embargo, usarse para aplicaciones clínicas, tales como los huesos largos, por ejemplo tras una fractura, como se muestra en la figura 25, en el que en caso de la destrucción del hueso 22 en su área central, la disposición de un dispositivo de mantenimiento de espacio correspondiente 1 de la invención y la estabilización con un clavo 23 y un tornillo 24 pueden servir para reproducir la estructura ósea.
