Abstract
Un método ex vivo para producir plaquetas, que comprende la etapa de someter una suspensión de megacariocitos maduros a un flujo que permite una exposición a una velocidad de corte apropiada para la generación de plaquetas y en una fase sólida recubierta con factor de von Willebrand (VWF), o con un fragmento o variante o análogo del mismo, que se une a GPIb.

obtuvieron resultados similares usando MK derivados de sangre de cordón umbilical o MK derivados de médula ósea. En contraste, en ausencia de flujo o a velocidades inferiores de corte (<1000 s-1), no se observaban proplaquetas y liberación de plaquetas durante el período de 20 min de contacto de los MK con VWF. La exposición a corte de MK inmaduros, cultivados menos de 9 días, no permitía la traslocación y fragmentación.
Especificidad de interacción de VWF con receptores de MK en formación de plaquetas
Se definieron cuatro categorías celulares para cuantificar los cambios morfológicos durante la exposición a corte: 1) MK de traslocación; 2) MK de deformación prematura con pérdida de esfericidad; 3) MK de deformación tardía con elongación de proplaquetas; y 4) fragmentación de proplaquetas y liberación de plaquetas (figura 1B, panel a). Hubo un aumento gradual en la proporción de células alargadas y proplaquetas y plaquetas, alcanzando el 27,6 ± 4,9 % de las células a los 20 min. Este aumento reflejaba la diminución en las células de traslocación del 28,2 ± 6,5 a los 5 min al 3,6 ± 1,6 % a los 20 min, y la disminución en los MK de deformación prematura (figura 1B, panel b). Se obtuvieron resultados similares a los descritos con MK de sangre de cordón umbilical cuando se medía la distribución de los MK de médula ósea expuestos a alta velocidad de corte (figura 1B, panel c). La traslocación de los MK y posteriores etapas incluyendo la formación de proplaquetas y liberación de plaquetas se anularon completamente en presencia de un anticuerpo dirigido contra la glicocalicina o el dominio de unión a GPIb de VWF (figura 1B, paneles d y e), lo que demuestra la importancia crucial de la interacción VWF-GPIb. En presencia del inhibidor de αIIbβ3 Abciximab, la proporción de células sin proplaquetas (MK sin deformación y de deformación prematura) permaneció constante en el tiempo, mientras que la elongación de proplaquetas se reducía fuertemente y la formación de plaquetas se anulaba casi completamente (figura 1B, panel f). Esta ineficaz formación de proplaquetas se debía a la pérdida de contacto con la superficie de VWF (datos no mostrados).
Caracterización de formación de plaquetas inducida por exposición de MK a una alta velocidad de corte
Las velocidades de corte aplicadas a MK maduros indujeron cambios morfológicos específicos en comparación con un control realizado incubando las células en condiciones estáticas durante 20 minutos en VWF, donde la morfología de los MK permanecía inalterada con respecto a la de las células antes del contacto con VWF (Figura 2). En contraste, la tinción post-fijación de cubreobjetos recubiertos con VWF después de 20 minutos de perfusión de MK mostró que los MK prolongaban largos filopodios en la punta y que se formaban espigas de tipo plaqueta en cuentas a lo largo del eje, así como fragmentos citoplasmáticos más grandes (Figura 2). La elongación de proplaquetas requiere el deslizamiento de microtúbulos solapantes que tiñen para tubulina α y β1 (Patel et al., 2005b). Se descubrió que se teñían extensiones de MK por anticuerpo anti-tubulina en diferentes territorios de la actina, lo que confirma que estas extensiones presentaban tinción similar identificada en MK que forman proplaquetas por el trabajo de Italiano et al. (Italiano et al., 2003; Italiano et al., 1999; Patel et al., 2005a; Patel et al., 2005b); el marcaje de tubulina seguía el eje de proplaquetas en MK alargados y presentaba un patrón anular en MK esféricos, sobre proplaquetas y sobre plaquetas (Figura 3B). La importancia de los microtúbulos en la formación de plaquetas inducidas por corte, se confirmó por pre-incubación con Nocodazol, un inhibidor del ensamblaje de microtúbulos, que evitaba completamente la elongación de MK y la formación de proplaquetas en condiciones de flujo (datos no mostrados). Para demostrar que el Nocodazol no estaba actuando sobre plaquetas preformadas en suspensiones celulares, se añadió el inhibidor de microtúbulos después de la elongación; el efecto visualizado a tiempo real mostró que Nocodazol podía revertir la elongación inducida por corte cuando se añadía después de que hubiera sucedido el proceso. La especificidad de la reversión se confirmó por tinción de tubulina. Curiosamente, después de adición de Nocodazol a los 25 minutos, la tubulina ya no estaba organizada como se observa en proplaquetas y plaquetas, indicando por tanto que los procesos elongados eran diferentes de las fijaciones (Figura 3C). De hecho, las fijaciones de membrana eran estructuras dinámicas que se extendían desde contactos de adhesión localizados pequeños bajo la influencia de flujo, que se inducen por interacción de VWF-GP Ib, pero no presentan ninguna tinción de tubulina (Dopheide et al., 2002).
La microscopia electrónica mostró ejes citoplasmáticos largos que se extienden desde el núcleo celular, que contienen haces paralelos de microtúbulos y a veces estaban hinchados con orgánulos citoplasmáticos (Figura 4, panel a). Los glóbulos nucleares con heterocromatina densa estaban localizados en un polo de la célula y se observaban ocasionalmente núcleos desnudos (Figura 4, panel b). Grandes fragmentos citoplasmáticos, desprovistos de núcleos, eran casi esféricos, con forma de mancuerna o elongados con extremidades delgadas (Figura 4, panel c y d). También eran visibles fragmentos tipo plaqueta (Figura 4, panel e).
La formación de proplaquetas se acelera por alta velocidad de corte y VWF
Recientemente se ha demostrado que las proteínas adhesivas están implicadas en la regulación de la formación de proplaquetas; en particular, la formación de proplaquetas sucedía 16 horas después de la siembra de MK en VWF en condiciones estáticas (Balduini et al., 2008). Como se muestra en la Figura 5A, se obtuvieron hallazgos similares. En contraste, en presencia de alta velocidad de corte, el proceso sucedía muy rápidamente, ya que, en 20 minutos, el 70 % de los MK de sangre de cordón umbilical y el 80 % de los MK de médula ósea formaban proplaquetas. Este efecto era dependiente del grado de velocidad de corte (600-2400 s-1) y de la concentración celular (0,5-2 x 106/ml). Las células endoteliales contienen multímeros de alto peso molecular de VWF que se liberan de forma constitutiva a una tasa baja y está aumentada en afecciones inflamatorias (Rondaij et al., 2006).
