Un aparato (100) de apósito para heridas que comprende: un apósito para heridas de material compuesto con múltiples capas dispuestas en una relación superpuesta, en el que las múltiples capas incluyen una capa (106) porosa y no adherente, una capa (108) absorbente y una capa (110) superior adherente no porosa, en el que la capa superior incluye además un puerto (112) de vacío en comunicación de fluido con el interior del apósito para heridas; un sistema (104) de bombeo que comprende una fuente (122) de alimentación o batería interna autónoma, un sensor de presión o transductor (124) para supervisar la presión dentro del apósito para heridas, y unos medios (126) de regulación o control autónomos; una fuente (114) de vacío; un tubo (116) de entrada que conecta un lado de entrada de la fuente de vacío al puerto de vacío; y un recipiente (118) de recogida, en el que el recipiente de recogida está conectado a un lado de salida de la fuente de vacío mediante un tubo (120) de salida o está dispuesto entre la fuente de vacío y el apósito para heridas; y en el que el sensor de presión está dispuesto dentro del interior del apósito para heridas y está en conexión eléctrica con los medios de control a través de un cableado (128) eléctrico.

1. Campo tecnico
La presente descripcion se refiere a un aparato para el tratamiento de una herida abierta y, mas espedficamente, se refiere a un aposito para heridas autonomo con un sistema de bombeo portatil externo que extrae los fluidos de la herida a un recipiente de recogida soportado por el paciente.
2. Descripcion de la tecnica relacionada
El cierre de la herida implica la migracion del tejido epitelial y subcutaneo contiguo a la herida hacia el centro de la herida hasta que la herida se cierra. Desafortunadamente, el cierre es diffcil con heridas grandes o heridas infectadas. En dichas heridas, se forma una zona de estasis (es decir, un area en la que un hinchazon localizado del tejido restringe el flujo de sangre a los tejidos) cerca de la superficie de la herida. Sin suficiente flujo de sangre, los tejidos epiteliales y subcutaneos que rodean la herida no solamente reciben una menor cantidad de oxfgeno y nutrientes, sino que tambien tienen una menor capacidad para luchar con exito contra infecciones microbianas y, de esta manera, tienen una menor capacidad para cerrar la herida de forma natural. Dichas heridas han presentado dificultades al personal medico durante muchos anos.
Los apositos para heridas se han usado en la industria medica para proteger y/o facilitar la cicatrizacion de heridas abiertas. Una tecnica popular ha sido el uso de terapia de presion negativa, conocida tambien como terapia de aspiracion o de vado. Se han desarrollado una diversidad de dispositivos de presion negativa para permitir la retirada de los fluidos sobrantes de la herida, es decir, los exudados, mientras que al mismo tiempo se afsla la herida para proteger la herida y, por consiguiente, modificar el tiempo de recuperacion. Se han modificado diversos apositos para heridas para promover la cicatrizacion de las heridas abiertas.
Las cuestiones que deben ser tenidas en cuenta continuamente cuando se usa un aposito para heridas incluyen la facilidad de uso, la eficiencia de la cicatrizacion de una herida, la portabilidad y la capacidad de control de la presion negativa. De esta manera, sigue existiendo una necesidad de mejorar constantemente los apositos para heridas de presion negativa para heridas abiertas.
Los ejemplos de sistemas de tratamiento de heridas de la tecnica anterior se describen en las solicitudes de patente US Nos. US2003/040687, US2005/090787, US2004/054338 y la solicitud de patente del Reino Unido N° 9623743.3 publicada como GB2307180.
Sumario
Segun la presente invencion, se proporciona un aparato de aposito para heridas segun la reivindicacion 1.
Preferiblemente, la fuente de vado incluye una bomba de vado peristaltica y esta adaptada para producir una presion sub-atmosferica comprendida entre aproximadamente 2.666,45 Pas y aproximadamente 66.661,2 Pas (entre aproximadamente 20 mmHg y aproximadamente 500 mmHg), mas preferiblemente, entre aproximadamente 9.999,18 Pas y aproximadamente 16.665,3 Pas (entre aproximadamente 75 mmHg y aproximadamente 125 mmHg). Preferiblemente, el aparato incluye tambien una bolsa de soporte de cuerpo que esta adaptada para su montaje en el paciente. La bolsa de soporte de cuerpo puede tener una bolsa para retener al menos uno de entre el recipiente de recogida y la bomba de vado.
La bomba de vado incluye preferiblemente medios de control para controlar su funcionamiento. Un sensor de presion puede estar incorporado con los medios de control para detectar la presion en una ubicacion predeterminada con relacion al elemento de aposito para heridas, y enviar una senal correspondiente a los medios de control. Los medios de control pueden incluir un controlador adaptado para controlar o variar la salida de la fuente de vado. La bomba de vado incluye preferiblemente una batena autonoma.
El elemento de aposito para heridas preferido incluye un elemento inferior que puede ser posicionado contiguo al lecho de la herida, un elemento superior absorbente que puede ser posicionado contiguo al elemento inferior, y un elemento superior. El elemento superior absorbente comprende un material seleccionado de entre el grupo que consiste en espumas, telas compuestas no tejidas, telas celulosicas, polfmeros super absorbentes y sus combinaciones. El elemento superior absorbente puede incluir tambien al menos uno de entre un medicamento, un agente anti-infeccioso, un antimicrobiano, polihexametileno biguanida (en adelante, en la presente memoria, "PHMB"), antibioticos, agentes de desbridamiento, analgesicos, factores de cicatrizacion, vitaminas, factores de crecimiento y nutrientes.

El aparato de aposito para heridas puede incluir un elemento de aposito para heridas dimensionado para su posicionamiento con relacion a un lecho de una herida, una bomba de vado portatil en comunicacion de fluido con el elemento de aposito para heridas para aplicar presion sub-atmosferica para facilitar la retirada de fluido desde el elemento de aposito para heridas, y que tiene una batena autonoma para hacer funcionar la bomba de vado, un recipiente de recogida en comunicacion de fluido con la bomba de vado para recoger los fluidos retirados desde el elemento de aposito para heridas y una bolsa de soporte de cuerpo que puede ser fijada al cuerpo del paciente y que tiene una bolsa para retener al menos uno de entre el recipiente de recogida y la bomba de vado.

El aparato de aposito para heridas de material compuesto de la presente descripcion promueve la cicatrizacion de una herida mediante el uso de una bomba de vado peristaltica externa. La bomba peristaltica externa aplica una presion de vado a la herida para extraer eficazmente el fluido o exudado de la herida desde el lecho de la herida. La bomba peristaltica externa esta fijada al aposito para heridas y es portatil, preferiblemente, transportada por el paciente, lo cual permite la movilidad del paciente. Ademas, no es necesario imponer restricciones al paciente durante ningun penodo de tiempo durante la terapia y mientras el exudado esta siendo eliminado de la herida.
Con referencia ahora a la Figura 1, en la misma se ilustra el aparato 100 de aposito para heridas segun una realizacion preferida de la presente descripcion. El aparato 100 de aposito para heridas incluye un aposito 102 para heridas de material compuesto y un sistema 104 de bombeo fijado al aposito 102 para heridas. El aposito 102 para heridas tiene la forma de un artfculo con multiples capas dispuestas en relacion superpuesta. Las multiples capas incluyen, pero no se limitan a, una capa 106 base, una capa 108 de empaquetado/absorbente y una capa 110 superior adherente, no porosa.
La capa 106 base esta en contacto directo con el lecho "w" de la herida. Tfpicamente, la capa 106 base es porosa y no adherente. Tal como se usa en la presente memoria, "no adherente" se refiere a un material que no se adhiere a los tejidos en y alrededor del lecho de la herida. "Poroso", tal como se usa en la presente memoria, se refiere a un material que contiene numerosas perforaciones pequenas o poros que permiten que todo tipo de fluidos de la herida pasen a traves del material a las capas de aposito superiores. Preferiblemente, el paso del fluido de la herida a traves del material poroso es unidireccional de manera que el exudado de la herida no fluya de vuelta al lecho de la herida. Esta caractenstica de direccion de flujo podna tener la forma de aberturas direccionales impartidas en la capa de material, una laminacion de materiales de diferente absorcion a la capa 106 base o una seleccion espedfica de material que estimula el flujo direccional. Los materiales ejemplares usados como la capa 106 base incluyen una capa de contacto comercializada bajo la marca comercial XEROFLO® por Kendall Corp., una division de TycoHealthcare.
Ademas, agentes tales como hidrogeles y medicamentos podnan ser unidos a o revestidos sobre la capa 106 base para reducir la biocarga en la herida, promover la cicatrizacion y reducir el dolor asociado con los cambios o retiradas de apositos. Los medicamentos incluyen, por ejemplo, agentes antimicrobianos, factores de crecimiento, antibioticos, analgesicos y similares. Ademas, cuando se usa un analgesico, el analgesico podna incluir un mecanismo que permitiera la liberacion de ese agente antes de la retirada o cambio del aposito.
La capa proximal a la capa 106 base es la capa 108 de empaquetado/absorbente. La capa 108 de empaquetado/absorbente esta destinada a absorber y capturar los fluidos y exudados de la herida. Los materiales

ejemplares usados como la capa 108 de empaquetado/absorbente incluyen el aposito antimicrobiano comercializado bajo la marca KERLIX® por Kendall Corp., una division de TycoHealthcare. Las personas con conocimientos en la materia reconoceran que la capa 108 de empaquetado/absorbente puede ser formada en cualquier forma adecuada. El unico requisito en cuanto a la forma es que la capa 108 de empaquetado/absorbente sea adecuada para ajustarse a una forma particular de la herida.
Un uso adicional para la capa 108 de empaquetado/absorbente es el de disminuir la incidencia de una infeccion en el lecho de la herida. Por lo tanto, la capa 108 de empaquetado/absorbente puede ser tratada con medicamentos. Los medicamentos incluyen, por ejemplo, un agente anti-infeccioso tal como un agente antiseptico u otro agente antimicrobiano adecuado o combinacion de agentes antimicrobianos, polihexametileno biguanida (en adelante, en la presente memoria, "PHMB"), antibioticos, analgesicos, factores de cicatrizacion tales como vitaminas, factores de crecimiento, nutrientes y similares, asf como un lavado sencillo con una solucion salina isotonica.
Con referencia continuada todavfa a la Figura 1, la capa 110 superior abarca el penmetro del aposito 102 para heridas para rodear el lecho "w" de la herida para proporcionar un sello oclusivo alrededor del penmetro del lecho “w” de la herida. La capa 110 superior puede incluir un adhesivo unido a un area que rodea el lecho “w” de la herida o puede incorporar una capa adicional que tiene el adhesivo. El adhesivo debe proporcionar una adhesion aceptable al tejido "t" que rodea la piel del lecho “w” de la herida, por ejemplo, el area alrededor de la herida, y debe ser aceptable para uso en la piel sin deterioro del contacto (por ejemplo, preferiblemente el adhesivo debena ser no irritante y no sensibilizante). El adhesivo puede ser semi-permeable para permitir que la piel contactada respire y transmita la humedad. De manera alternativa, el adhesivo puede ser impermeable. Ademas, el adhesivo podna ser activado o desactivado por un estfmulo externo tal como calor o una solucion fluida o una reaccion qmmica determinada. Los adhesivos incluyen, por ejemplo, el aposito hidrocoloide ULTEC, de Kendall Corp., una division de TycoHealthcare.
Preferiblemente, la capa 110 superior tiene forma de una lamina montada proximal a la capa 108 de empaquetado/absorbente. En una realizacion preferida, las partes HOP perifericas de la capa 110 superior incluyen un adhesivo y estan aseguradas al tejido "t" alrededor del lecho “w” de la herida. Si se desea, las partes HOP perifericas pueden ser aseguradas a la periferia de la capa 102 base. Se preve que puedan usarse tambien revestimientos desprendibles para proteger la superficie adhesiva de la capa 110 superior antes de su uso.
La capa 110 superior puede incorporar un material flexible, por ejemplo, elastico o elastomerico, que sella la parte superior del aposito 102 para heridas. En una realizacion, la capa 110 superior incluye el aposito transparente fabricado bajo la marca comercial Polyskin® II por Kendall Corp., una division de TycoHealthcare. POLYSKIN® II es un material transparente, semi-permeable que permite el intercambio de humedad y oxfgeno con el sitio de la herida, y proporciona una barrera contra los microbios y contencion del fluido. Como alternativa, la capa 110 superior puede ser impermeable. La transparencia de la capa 110 superior proporciona una indicacion visual del estado del aposito para heridas y, mas particularmente, el estado del nivel de saturacion de las capas del aposito para heridas. La capa 110 superior incluye ademas un puerto de vacfo o conector 112 en comunicacion de fluido con el interior del aposito 102 para heridas. El puerto 112 de vacfo puede ser un componente separado fijado a la capa 110 superior y conectado a la misma mediante medios convencionales o formada integralmente con la capa 110 superior. El puerto 112 de vacfo puede tener una valvula construida en el mismo, por ejemplo, una valvula unidireccional, para permitir que los exudados fluyan solo en una direccion, es decir, alejandose del aposito 102 para heridas hacia el sistema 104 de bombeo. El puerto 112 de vacfo esta adaptado para ser conectado, de manera liberable, al sistema 104 de bombeo tal como se describira y puede incluir o no una estructura para la conexion liberable al sistema de bombeo.
Todavfa con referencia a la Figura 1, se describira el sistema 104 de bombeo. El sistema 104 de bombeo incluye una fuente 114 de vacfo, un tubo 116 de entrada que conecta el lado de entrada de la fuente 114 de vacfo al puerto 112 de vacfo del aposito 102 para heridas y un recipiente 118 de recogida conectado al lado de salida de la fuente 114 de vacfo mediante un tubo 120 de salida. Como alternativa, el recipiente 118 de recogida puede disponerse "en lmea" entre la fuente 114 de vacfo y el aposito 102 para heridas. La fuente 114 de vacfo puede ser cualquier tipo de bomba que sea biocompatible y mantenga o extraiga niveles de vacfo adecuados y terapeuticos. Preferiblemente, el nivel de vacfo a conseguir esta comprendido en un intervalo entre aproximadamente 2.666,45 Pas y aproximadamente 66.661,2 Pas (entre aproximadamente 20 mmHg y aproximadamente 500 mmHg), mas preferiblemente, entre aproximadamente 9.999,18 Pas y aproximadamente 16.665,3 Pas (entre aproximadamente 75 mmHg y aproximadamente 125 mmHg). La bomba 114 puede ser desmontable, reutilizable y/o recargable. Tfpicamente, la bomba 114 es una bomba de tipo diafragma o peristaltico, o similar, en la que la parte o las partes moviles extraen el exudado desde el lecho de la herida al aposito para heridas mediante la creacion de areas o zonas de presion reducida, por ejemplo, zonas de vacfo con el aposito 102 para heridas. Preferiblemente, esta area de presion reducida se comunica con el lecho “w” de la herida para facilitar la retirada de los fluidos del mismo y al interior de la capa 108 de empaquetado/absorbente. La bomba 114 puede ser accionada por cualquier medio conocido por las personas con conocimientos en la materia. En una realizacion preferida de la presente

descripcion, la bomba 114 es una bomba peristaltica. Una bomba peristaltica adecuada es la bomba Kangaroo PET Enternal Feeding Pump fabricada por Kendall Corp., una division de TycoHealthcare. Otra bomba peristaltica adecuada puede ser el modelo 101 V/R pmy MK2 fabricado por Watson Marlow LTD de Inglaterra. Preferiblemente, la bomba peristaltica produce una presion sub-atmosferica comprendida entre aproximadamente 2.666,45 Pas y aproximadamente 66.661,2 Pas (entre aproximadamente 20 mmHg y aproximadamente 500 mmHg). Una bomba de diafragma adecuada incluye el modelo NMP 850 KNDC fabricado por KNF Neuberger de Alemania.
Los tubos 116, 120 de entrada y salida pueden ser cualquier tubo flexible adecuado fabricado en materiales elastomericos y/o polimericos. Preferiblemente, el tubo 116 de entrada se conecta, de manera liberable, al puerto 112 de vado a traves de medios convencionales, incluyendo un ajuste de friccion, un acoplamiento de tipo bayoneta, un ajuste a presion o similar. El recipiente 118 de recogida puede ser cualquier bolsa desechable flexible o similar. El recipiente 118 de recogida puede incluir un material super absorbente, tal como polfmeros super absorbentes (SAP), gel de sflice, poliacrilato de sodio, poliacrilamida de potasio y compuestos relacionados para consolidar o contener el drenaje o los residuos de la herida. Preferiblemente, el recipiente 118 de recogida es transparente para permitir observar el interior del recipiente 118 para ayudar al paciente a determinar la capacidad restante del recipiente 118 de recogida y la calidad de los exudados.
Con referencia a la Figura 2, el sistema 104 de bombeo incluye una fuente 122 de alimentacion o batena autonoma, un sensor de presion o transductor 124 para supervisar la presion dentro del aposito para heridas y unos medios 126 de regulacion o control autonomos. Preferiblemente, el sensor 124 de presion esta dispuesto en el interior del aposito 102 para heridas y esta en conexion electrica con los medios 126 de control a traves de un cableado 128 electrico (Figura 1). Un sensor de presion adecuado es el sensor de presion Dynamic ICP de la division de presion de PCB Piezotronics, Inc. El sensor 124 de presion proporcionana tambien informacion para ayudar en la deteccion de una fuga en el aposito 102 para heridas. El cableado 128 electrico puede ser pasado a traves del puerto 112 de vado y a traves del tubo 116 de entrada o sobre la superficie exterior del tubo 116 de entrada. De manera alternativa, el cableado 128 electrico puede pasar a traves de un conducto o canal en la pared del puerto 112 de vado y/o el tubo 116 de entrada. La Figura 1A ilustra las diversas disposiciones en las que el cableado 128 electrico pasa a traves del canal del tubo 116 o el lumen central del tubo 116. Preferiblemente, los medios 126 de control se incorporan dentro de la carcasa de bomba de la bomba 114. Los medios 126 de control pueden incorporar un controlador 130 de motor que incluye software o circuitena de procesamiento o accionamiento para controlar o variar el voltaje de accionamiento al motor de la bomba 114 en respuesta a la presion detectada por el sensor 124 de presion y/u otros parametros operacionales, incluyendo las limitaciones de tiempo de funcionamiento, etc. Por ejemplo, el controlador de motor puede estar programado para funcionar solo durante un penodo de tiempo predeterminado despues del arranque. La salida del motor de la bomba 114 puede ser aumentada o disminuida, o iniciada o interrumpida, segun el control de los medios 126 de control. Los medios 126 de regulacion o control pueden tener tambien una alarma 132, tal como una alarma sensorial visual, de audio o tactil (por ejemplo, vibratoria, etc.) para indicar al paciente cuando se han cumplido unas condiciones espedficas (por ejemplo, el nivel de vado deseado, perdida o fugas de vado). Puede incorporarse tambien un interruptor 130 de anulacion dentro del sistema de bombeo para permitir que el paciente inicie o termine, opcionalmente, el funcionamiento de la bomba segun lo desee sin ninguna entrada desde los medios 126 de control. Se preven tambien medios inalambricos para hacer funcionar la bomba 114 a traves de los medios 126 de control.
Con referencia ahora a la Figura 3, se ilustra una bolsa 134 de soporte de cuerpo para soportar al menos el recipiente y/o la bomba 114. Tal como se ha descrito, el sistema 104 de bombeo esta adaptado para su montaje en el cuerpo del paciente para ser una unidad portatil autonoma. En este sentido, el sistema 104 de bombeo puede ser transportado o soportado, al menos parcialmente, por la bolsa 134 de soporte de cuerpo. La bolsa 134 de soporte de cuerpo incluye generalmente una bolsa 136 y al menos una correa 138, preferiblemente dos correas, para asegurar la bolsa 136 al cuerpo del paciente. La bolsa 134 de soporte de cuerpo esta destinada a recibir y almacenar al menos el recipiente 118 de recogida y/o la bomba 114. La bolsa 134 de soporte de cuerpo puede ser dispuesta alrededor de la cintura del paciente. Esto es deseable en el sentido de que puede reducir la longitud necesaria del tubo dependiendo de la ubicacion de la herida. Ademas, la bolsa 136 puede estar situada contigua al abdomen del paciente, lo cual puede presentar una capacidad mejorada significativamente para ocultar el sistema. Los tubos 116, 120 de entrada y salida pueden ser asegurados al cuerpo con cinta adhesiva o similar, u, opcionalmente, pueden no ser asegurados y pueden ser dispuestos debajo de la ropa del paciente. De esta manera, la bolsa 134 de soporte de cuerpo permite que el paciente se mueva sin restricciones o limitaciones, y proporciona una capacidad completamente portatil al paciente durante el drenaje y la cicatrizacion de la herida.
La Figura 4 ilustra una realizacion alternativa de la bolsa de soporte de cuerpo. Segun esta realizacion, la bolsa 140 de soporte de cuerpo esta adaptada para su montaje en el hombro del paciente y tiene una bolsa 142. En otros aspectos, la bolsa 140 de soporte de cuerpo funciona de manera similar a la bolsa de soporte de cuerpo de la Figura 3.

Durante el uso, el aparato 100 de aposito para heridas es posicionado dentro del lecho “w” de herida, tal como se muestra en la Figura 1 y es asegurado sobre la herida "w" de la manera descrita anteriormente en la presente memoria. La bomba 114 y/o el recipiente 118 de recogida se posicionan dentro de la bolsa 136 de la bolsa 134 de soporte de cuerpo (o bolsa 140 de soporte). La bolsa 134, 140 de soporte de cuerpo es montada en el paciente, alrededor de la cintura, el hombro, la pierna, etc., con las correas 134. Los tubos 114, 120 de entrada y salida pueden ser asegurados al cuerpo del paciente con cinta adhesiva o similar. A continuacion, la bomba 114 es iniciada para crear una zona de presion sub-atmosferica (es decir, un estado de vado) dentro del aposito 100 para heridas. La bomba 114 puede ser iniciada mediante el interruptor 130 de accionamiento manual, o puede ser iniciada mediante el sensor 124 de presion que detecta la falta de presion sub-atmosferica dentro del aparato 100 de aposito para heridas y envfa una senal correspondiente a los medios 126 de control. Los medios 126 de control, a su vez, activan la bomba 114. A medida que la presion sub-atmosferica dentro del aposito 102 para heridas aumenta, la capa 110 superior y la capa 108 de empaquetado/absorbente puede colapsarse. Los fluidos son extrafdos del lecho “w” de la herida y son pasados a la capa 106 de empaquetado/absorbente del aposito 102 para heridas. Estos fluidos y/o exudados pueden ser retirados de la capa 106 de empaquetado/absorbente en virtud de la energfa de bombeo negativo de la bomba 114. Los fluidos son suministrados a traves de los tubos 116, 120 de entrada y salida para ser recogidos dentro del recipiente 118 de recogida. Una vez conseguido el nivel deseado de presion sub-atmosferica detectado, por ejemplo, por el sensor 124 de presion, el sensor 124 de presion envfa una senal a los medios 126 de control. Los medios 126 de control pueden terminar el funcionamiento de la bomba 114 o, de manera alternativa, pueden variar (por ejemplo, reducir) la velocidad o la salida de la bomba 114. En este estado de vado, los fluidos y los exudados de la herida son extrafdos continuamente y son pasados a la capa 106 de empaquetado/absorbente. Despues de un penodo de tiempo, el aposito 102 para heridas puede perder su estado de vado detectado por el sensor 124 de presion o detectado visualmente por el paciente. Cuando se consigue la perdida de un nivel de vado deseado, el sensor 124 de presion envfa una senal a los medios 126 de control para activar o aumentar la salida de la bomba 114. La bomba 114 retira el fluido de la capa 108 de empaquetado/absorbente y restablece el estado de vado dentro del aposito 102 para heridas. Tal como se ha indicado anteriormente, de manera alternativa, la bomba 114 puede ser iniciada mediante el interruptor 130 de anulacion manual, por ejemplo, cuando el paciente observa que el recipiente 118 de recogida esta lleno. Este procedimiento puede continuar varias veces durante la cicatrizacion de la herida durante una aplicacion o una serie de aplicaciones.
Una vez cicatrizada completamente la herida, el aparato de cierre de heridas y el sistema de bombeo pueden ser desechados. La bolsa de soporte de cuerpo tambien puede ser desechada o limpiada para un uso posterior. La bomba tambien puede ser esterilizada y reutilizada.
La Figura 5 ilustra otra realizacion de la presente descripcion. Segun esta realizacion, el aparato 200 de aposito para heridas incluye una microbomba 202 que esta adaptada para su montaje dentro de la capa 204 absorbente. La microbomba 202 tiene una longitud comprendida entre aproximadamente 2,54 y 7,62 cm (entre aproximadamente 1 y 3 pulgadas) y un diametro relativamente pequeno, preferiblemente, no mayor de aproximadamente 2,54 cm (1 pulgada). La microbomba 202 puede ser cualquier tipo de bomba que sea biocompatible y mantenga o extraiga niveles de vado adecuados y terapeuticos. La microbomba 202 puede ser desmontable, reutilizable y/o recargable. La microbomba 202 puede ser una bomba de tipo diafragmatico, peristaltico o rotatorio, o similar, en el que la parte movil o las partes moviles extraen los exudados desde el lecho de la herida al aposito para heridas mediante la creacion de areas o zonas de presion reducida, por ejemplo, zonas de vado con el aparato 200 de aposito para heridas. Preferiblemente, esta area de presion reducida se comunica con el lecho “w” de la herida para facilitar la retirada de los fluidos desde el mismo y al interior de la capa 204 absorbente. La microbomba 202 puede ser accionada mediante cualquier medio conocido por las personas con conocimientos en la materia. En una realizacion preferida de la presente descripcion, la microbomba 202 es una bomba peristaltica. Una microbomba adecuada es fabricada por Piab Vacuum Products en Hingham, MA. Preferiblemente, la bomba peristaltica produce una presion sub-atmosferica comprendida entre aproximadamente 2.666,45 Pas y aproximadamente 66.661,2 Pas (entre aproximadamente 20 mmHg y aproximadamente 500 mmHg).
Preferiblemente, la capa 204 absorbente es un aposito relleno de espuma que es transparente. La espuma puede ser una espuma basada en polfmero, elastica, absorbente de lfquidos, porosa. La espuma puede ser un lfquido dispensable que se solidifica al menos parcialmente en una disposicion similar al cristal que define tubos huecos para permitir el drenaje de los exudados. La espuma es dispensada dentro del lecho de la herida y es potencialmente colapsable para expulsar el aire desde los canales de espuma. La espuma puede ser una espuma hidrofila expandible que es capaz de absorber el fluido desde una herida y mantener la humedad del lecho de la herida. Los tubos huecos o vados definidos por la espuma proporcionan tambien unos medios para conducir la electricidad, el calor, el frio y los ultrasonidos. Los tubos huecos o vados proporcionan tambien un andamiaje bioactivo para el crecimiento tisular. Una capa 205 superior transparente de pelfcula delgada es asegurada sobre el area de la herida para encerrar la herida.

La fuente de alimentacion o batena autonoma y la circuitena de control pueden ser montadas dentro de la carcasa 206 que esta conectada a la correa 208. La correa 208 esta adaptada para su montaje en el cuerpo de un paciente, por ejemplo, alrededor de la zona de la cintura y puede incluir medios de VELCRO® para su fijacion sobre el paciente. La correa 208 puede soportar ademas el recipiente 210 que recibe los exudados de fluido desde la bomba 202 a traves del tubo 212. El tubo 214 se extiende desde la carcasa 206 a la bomba 202 y puede incorporar cables electronicos, etc., para hacer funcionar la bomba. De manera alternativa, el tubo 212 puede incorporar los cables electricos, por ejemplo, dentro de un lumen, y se extiende desde la carcasa a la bomba 202 y el recipiente 210.
Se contempla ademas que el aparato de aposito para heridas pueda incorporar medios o aplicaciones externas para estimular el crecimiento y/o la cicatrizacion tisular. Por ejemplo, un aparato de tratamiento auxiliar puede ser incorporado en el aparato de aposito para heridas para impartir energfa electrica o mecanica para el tratamiento del tejido, por ejemplo, dirigiendo energfa electrica, termica o vibratoria sobre el area de la herida y/o introduciendo diversos farmacos en el cuerpo humano a traves de la piel. El aparato de tratamiento auxiliar puede ser incorporado a la carcasa de la bomba 202, tal como se muestra esquematicamente como el numero de referencia 216 y puede ser operado a traves de unos medios de control descritos anteriormente. Un aparato de tratamiento auxiliar adecuado puede incluir un transductor ultrasonico. Se contemplan tambien otros tipos de sensores para su incorporacion en el aparato de aposito para heridas incluyendo sensores de oxfgeno, qmmicos, microbianos y/o de temperatura. La deteccion del oxfgeno contiguo a la zona de la herida ayudana al clmico a determinar el estado de la cicatrizacion de la herida. La presencia de una temperatura elevada puede ser indicativa de una infeccion. El sistema de bombeo puede incorporar circuitena para comunicarse con un ordenador, por ejemplo, un dispositivo PALM® de mano mediante medios inalambricos.
El aparato de aposito para heridas puede comprender:
un elemento aposito para heridas dimensionado para su posicionamiento con relacion a un lecho de una herida;
una bomba de vado portatil en comunicacion de fluido con el elemento de aposito para heridas para aplicar una presion sub-atmosferica para facilitar la retirada de fluido desde el elemento de aposito para heridas, en el que la bomba de vado esta adaptada para ser transportada por el cuerpo de un paciente, y
un recipiente de recogida transportado por el cuerpo del paciente y en comunicacion de fluido con la bomba de vado para recoger los fluidos retirados desde el elemento de aposito para heridas.
La fuente de vado puede incluir una bomba de vado peristaltica.
La bomba de vado puede estar adaptada para producir una presion sub-atmosferica comprendida entre aproximadamente 2.666,45 Pas y aproximadamente 66.661,2 Pas (entre aproximadamente 20 mmHg y aproximadamente 500 mmHg).
El recipiente de recogida puede incluir un material absorbente para contener el fluido retirado desde el elemento de aposito para heridas.
El aparato de aposito para heridas puede incluir una bolsa de soporte de cuerpo, en el que la bolsa de soporte de cuerpo esta adaptada para su montaje en el paciente y tiene una bolsa para retener al menos uno de entre el recipiente de recogida y la bomba de vado.
La bomba de vado puede incluir medios de control para controlar su funcionamiento.
El aparato de aposito para heridas puede incluir un sensor de presion adaptado para detectar la presion en una ubicacion predeterminada con relacion al elemento de aposito para heridas, y enviar una senal correspondiente a los medios de control.
Los medios de control pueden incluir un controlador adaptado para controlar o variar la salida de la fuente de vado.
La bomba de vado puede incluir una batena autonoma.
El elemento de aposito para heridas puede incluir un elemento inferior que puede ser posicionado contiguo al lecho de una herida, un elemento superior absorbente que puede ser posicionado contiguo al elemento inferior y un elemento superior.
El elemento superior absorbente puede comprender un material seleccionado de entre el grupo que consiste en espumas, telas compuestas no tejidas, telas celulosicas, polfmeros super absorbentes y sus combinaciones.
El elemento superior absorbente puede incluir al menos uno de entre un medicamento, un agente anti-infeccioso, un antimicrobiano, polihexametileno biguanida (en adelante, en la presente memoria, "PHMB"), antibioticos, analgesicos, factores de cicatrizacion, vitaminas, factores de crecimiento y nutrientes.
El aparato de aposito para heridas puede comprender:
5 un elemento de aposito para heridas dimensionado para su posicionamiento con relacion a un lecho de una
herida;
una bomba de vacfo portatil en comunicacion de fluido con el elemento de aposito para heridas para aplicar presion sub-atmosferica para facilitar la retirada de fluido desde el elemento de aposito para heridas, en el que la bomba de vacfo tiene una batena autonoma para hacer funcionar la bomba de vacfo;
10 un recipiente de recogida en comunicacion de fluido con la bomba de vacfo para recoger los fluidos retirados
desde el elemento de aposito para heridas; y
una bolsa de soporte de cuerpo que puede ser fijada al cuerpo del paciente, en el que la bolsa de soporte de cuerpo incluye una bolsa para retener al menos uno de entre el recipiente de recogida y la bomba de vacfo.

    REIVINDICACIONES
    1. Un aparato (100) de aposito para heridas que comprende:
    un aposito para heridas de material compuesto con multiples capas dispuestas en una relacion superpuesta, en el que las multiples capas incluyen una capa (106) porosa y no adherente, una capa (108) absorbente y una capa (110) superior adherente no porosa, en el que la capa superior incluye ademas un puerto (112) de vado en comunicacion de fluido con el interior del aposito para heridas;
    un sistema (104) de bombeo que comprende una fuente (122) de alimentacion o batena interna autonoma, un sensor de presion o transductor (124) para supervisar la presion dentro del aposito para heridas, y unos medios (126) de regulacion o control autonomos; una fuente (114) de vado;
    un tubo (116) de entrada que conecta un lado de entrada de la fuente de vado al puerto de vado; y un recipiente (118) de recogida, en el que el recipiente de recogida esta conectado a un lado de salida de la fuente de vado mediante un tubo (120) de salida o esta dispuesto entre la fuente de vado y el aposito para heridas; y
    en el que el sensor de presion esta dispuesto dentro del interior del aposito para heridas y esta en conexion electrica con los medios de control a traves de un cableado (128) electrico.
  2. 2. Aparato segun la reivindicacion 1, en el que el cableado electrico pasa a traves del puerto (112) de vado y a traves del tubo (116) de entrada o sobre una superficie exterior del tubo (116) de entrada.
  3. 3. Aparato segun la reivindicacion 1, en el que los medios (126) de control estan configurados para controlar la fuente de vado en respuesta a una presion detectada por el sensor de presion.
  4. 4. Aparato segun la reivindicacion 2, en el que el tubo (116) de entrada comprende un canal en una pared del tubo (116) de entrada, en el que el cableado (128) electrico pasa a traves del canal.
  5. 5. Aparato segun la reivindicacion 2, en el que el tubo (116) de entrada comprende un lumen central, en el que el cableado (128) electrico pasa a traves del lumen central.
  6. 6. Aparato segun la reivindicacion 1, en el que el puerto (112) de vado comprende un canal en una pared del puerto (112) de vado, en el que el cableado (128) electrico pasa a traves del canal.
  7. 7. Aparato segun la reivindicacion 1, en el que la capa (106) porosa y no adherente esta configurada para estar en contacto directo con una herida.
  8. 8. Aparato segun la reivindicacion 1, en el que la capa (106) porosa y no adherente incluye una pluralidad de poros configurados para permitir el flujo unidireccional del fluido de la herida hacia la capa (108) absorbente.
  9. 9. Aparato segun la reivindicacion 1, en el que la capa (106) porosa y no adherente incluye un medicamento revestido sobre la misma.
  10. 10. Aparato segun la reivindicacion 1, en el que la capa (110) superior es transparente.
  11. 11. Aparato segun la reivindicacion 1, en el que el sistema (104) de bombeo comprende un interruptor (130) de accionamiento configurado para iniciar o terminar la operacion de la fuente (114) de vado sin entrada desde los medios (126) de control.
  12. 12. Aparato segun la reivindicacion 1, en el que el puerto (112) de vado incluye una valvula unidireccional configurada para permitir que el fluido de la herida fluya en una unica direccion alejandose del aposito para heridas y para prevenir el flujo hacia el aposito para heridas.
  13. 13. Aparato segun la reivindicacion 1, en el que el puerto (112) de vado esta formado integralmente con la capa (110) superior del aposito para heridas.
  14. 14. Aparato segun la reivindicacion 1, en el que la fuente (114) de vado esta configurada para ser posicionada externa al aposito para heridas.
