La Fig. 1 ilustra un ejemplo del aparato de instrumento quirúrgico actualmente descrito mostrado generalmente como 100. La primera hoja 152 está unida al extremo distal de la primera superficie 146a y el puntal vertical 146b, creando de este modo una orientación en ángulo con respecto al puntal vertical 146b. La segunda hoja 154 está unida a la primera hoja 152 y se extiende entre la primera hoja 152 y el extremo distal de la segunda superficie 146c, posicionando la segunda hoja en un ángulo de 90 grados con respecto a la primera hoja 152. Como tales, solo se analizan en detalle a continuación las diferencias.
Los extremos 350a, 350c primero y segundo de hoja 350 de cuchilla están unidos a las superficies 346a, 346c primera y segunda respectivas. El vértice 350b de la hoja 350 de cuchilla puede estar situado sustancialmente en mitad de la hoja 350 de cuchilla y dispuesto en posición distal de los extremos 350a, 350c primero y segundo de la hoja 350 de cuchilla. Esta configuración convexa crea una forma de “C” inversa que está orientada hacia el extremo proximal o en dirección opuesta a la dirección de disparo. Aunque el vértice 350b de la hoja 350 de cuchilla se sitúa sustancialmente en mitad de la hoja 350 de cuchilla en la realización ilustrativa mostrada en la Fig. 4C, se contempla que el vértice 350b pueda estar situado en cualquier lugar a lo largo de la hoja 350 de cuchilla, desviando el vértice 350b hacia las placas 113, 123 de cartucho o de yunque, cambiando de este modo la curvatura de la hoja 350 de cuchilla.
En las Figs. 5 y 6 se muestra un instrumento de grapado quirúrgico anular designado de forma general como 400. El instrumento quirúrgico 400 de grapado incluye un conjunto 403 de mango que tiene al menos un elemento 405 de mango de accionamiento pivotante y un elemento 407 de avance. Extendiéndose desde el elemento 405 de mango hay una parte 404 de cuerpo tubular que puede construirse para tener una forma curva a lo largo de su longitud. La parte 400 de cuerpo termina en un efector 402 de extremo.
El efector 402 de extremo incluye un conjunto 412 de cartucho de grapas que define una pared interior 414 y una pared exterior 416. La pared interior 414 forma una abertura 414a en el extremo distal del conjunto 412 de cartucho de grapas. La parte 404 de cuerpo tubular incluye un vástago central 412a que se extiende a través de la abertura 414a del conjunto 412 de cartucho de grapas. El conjunto 412 de cartucho de grapas incluye además al menos un conjunto anular de grapas (no mostrado) dispuesto en cada una de las ranuras 418 de recepción de grapas. Las ranuras 418 de recepción de grapas se extienden a través de una superficie 420 de contacto de tejido del extremo distal del conjunto 412 de cartucho de grapas.
El conjunto 412 de cartucho de grapas puede estar conectado de forma fija al extremo distal de la parte 404 de cuerpo tubular o puede configurarse para encajar concéntricamente dentro del extremo distal de la parte 404 de cuerpo tubular. De forma típica, el conjunto 412 de cartucho de grapas incluye un empujador 428 de grapas y una cuchilla 450, sustancialmente en forma de una copa abierta, definiendo el reborde de la misma un filo 456 de cuchilla, dispuesto dentro del conjunto 412 de cartucho de grapas. El empujador 428 de grapas ayuda a expulsar las grapas 170 del conjunto 112 de cartucho. El filo 456 de cuchilla está dispuesto radialmente hacia dentro de las ranuras 418 de recepción de grapas, las grapas 170 y expulsa las grapas 170 desde el conjunto 112 de cartucho El efector 402 de extremo incluye además un conjunto 422 de yunque colocado distalmente del conjunto 412 de cartucho de grapas. El conjunto 422 de yunque tiene un elemento 424 de yunque y un vástago 426 conectado de forma desmontable al vástago central 412a del conjunto 412 de cartucho de grapas.
Puede hacerse referencia a la US-5.915.616, concedida a Viola y col., para una explicación detallada de la construcción y el funcionamiento de un instrumento de grapado anular ilustrativo.
Volviendo ahora a la Fig. 7A, se muestra la cuchilla 450 con un filo 456 de cuchilla de corte uniforme. En funcionamiento, una parte de cuchilla 450 es empujada desde el conjunto 412 de cartucho de grapas y el filo 456 de cuchilla corta el tejido agarrado entre el conjunto 412 de cartucho de grapas y el conjunto 422 de yunque.
Con referencia a las Figs. 7B-7I, se muestran varios ejemplos de perfiles de hoja de cuchilla que pueden utilizarse en la grapadora quirúrgica 400. Las hojas 550, 650, 750, 850, 950, 1050, 1150, 1250 de cuchilla mostradas en las Figs. 7B-7I están configuradas de tal forma que cualquiera de estas hojas de cuchilla podría utilizarse con la grapadora quirúrgica 400. Como se muestra en las Figs. 7B-7J, las hojas 550, 650, 750, 850 de cuchilla pueden tener un patrón serrado o tener cualquier otro patrón de borde de cuchilla adecuado. Además, las hojas de cuchilla de las Figs. 7A-7I se muestran en forma circular pero no están limitadas a un radio de curvatura uniforme, o a una altura uniforme. Adicionalmente, las Figs. 7B, 7D, 7G y 7H representan hojas con dientes de sierra. La Fig. 7H tiene
una altura no uniforme. Con respecto a la Fig. 7C, se presenta una cuchilla festoneada. Con respecto a las Figs. 7E y 7F, se muestran hojas del tipo ondulado donde la cuchilla 950 tiene una altura no uniforme. Con referencia adicional a las Figs. 7A-7I, las hojas 450-1250 de cuchilla pueden tener una altura variable alrededor de la circunferencia.
La adición de ángulos compuestos, uno o más bordes de avance u otras características distintas que cortan el tejido desde varios ángulos o de forma progresiva mejora la capacidad de las hojas 550, 650, 750, 850, 950, 1050, 1150, 1250 de cuchilla para cortar a través del tejido grueso al reducirse el área de la cuchilla que inicialmente entra en contacto con el tejido, lo que iniciaría y propagaría más fácilmente la sección de tejido.
Con referencia ahora a la Fig. 8, es un instrumento de control de energía electroquirúrgico designado generalmente como 500. El instrumento 500 de control de energía electroquirúrgico generalmente incluye una carcasa 512 que soporta diversos accionadores en el mismo para controlar de forma remota un efector 514 de extremo a través de un vástago alargado 516. Para controlar mecánicamente el efector 514 de extremo, la carcasa 512 soporta un mango estacionario 520, un mango móvil 522, un disparador 526 y un botón 528 de rotación. El mango móvil 522 puede utilizarse para mover el efector 514 de extremo entre una configuración abierta en donde un par de elementos 530, 532 de mordaza opuestos están dispuestos en una relación espaciada entre sí, y una configuración cerrada o de sujeción en donde los elementos 530, 532 de mordaza están más cerca entre sí. La aproximación del mango móvil 522 con el mango estacionario 520 sirve para mover el efector 514 de extremo a la configuración cerrada y la separación del mango móvil 522 desde el mango estacionario 520 sirve para mover el efector 514 de extremo a la configuración abierta.
Cuando está en uso, el efector 514 de extremo del instrumento de control de energía electroquirúrgico puede moverse de una configuración abierta en donde se recibe tejido (no mostrado) entre los elementos 530, 532 de mordaza y una configuración cerrada, en donde el tejido se sujeta y se trata. Los elementos 530, 532 de mordaza pivotan alrededor de un pasador 544 de pivote para mover el efector 514 de extremo a la configuración cerrada en donde una placa 548 de sellado y una placa 551 de sellado proporcionan una presión al tejido sujetado entre ellos. Adicionalmente, el elemento 530 y 532 de mordaza superior e inferior puede configurarse para proporcionar una vía para impulsar un conjunto de cuchilla (Figs. 4A-4B) que tiene una configuración similar a la vista en la Fig. 2. Además, se describe una conexión del mango móvil 522 y el disparador 536 de cuchilla con los componentes longitudinalmente móviles del vástago alargado 516. El mango móvil 522 puede manipularse para transmitir movimiento longitudinal al elemento 560 de vástago exterior, y el disparador 536 de cuchilla puede manipularse para transmitir movimiento longitudinal a la varilla 502 de cuchilla (no mostrado). Como se ha descrito anteriormente, el movimiento longitudinal del elemento 560 de vástago exterior sirve para mover el efector 514 de extremo entre la configuración abierta y la configuración cerrada, y el movimiento longitudinal de la varilla 502 de cuchilla (no mostrado) sirve para mover una hoja de cuchilla a través del canal de cuchilla 558 (no mostrado) similar al descrito en la Fig. 1.A.
En algunos ejemplos, las placas 548 y 551 de sellado están acopladas eléctricamente a terminales opuestos, por ejemplo, terminales positivos o activos (+) y negativos o de retorno (-) asociados a un generador 541. Por lo tanto, puede proporcionarse energía bipolar a través de las placas de sellado 548 y 551 al tejido. De forma alternativa, las placas 548 y 551 de sellado pueden configurarse para suministrar energía monopolar al tejido. En una configuración monopolar, una o ambas placas 548 y 551 de sellado suministran energía electroquirúrgica desde un terminal activo, por ejemplo, (+), mientras que una almohadilla de retorno (no mostrado) se coloca generalmente sobre un paciente y proporciona una vía de retorno al terminal opuesto, por ejemplo, (-), del generador 541. Puede encontrarse una descripción más detallada del instrumento 500 de control de energía electroquirúrgico en la solicitud de patente US-61/882.097 concedida a Duffin y col., presentada el 25 de septiembre de 2013 y titulada “Wire Retención Unidad for Surgical Instrument” .
Se contempla que las características individuales de las realizaciones descritas anteriormente pueden combinarse sin abandonar el ámbito de la presente descripción.

Claims (6)
- REIVINDICACIONESi. Un instrumento quirúrgico (100), que comprende:un efector (102) de extremo que tiene un extremo proximal y un extremo distal, incluyendo el efector de extremo una primera mordaza (123) y una segunda mordaza (113);una barra (130) de accionamiento dispuesta dentro del efector de extremo y unida de forma deslizante a la primera mordaza, la barra de accionamiento incluye una muesca (146) que define una primera superficie (146a), una segunda superficie (146c), y un puntal vertical (146b); y dos hojas (152, 154) unidas a la barra de accionamiento, teniendo dichas hojas un borde recto o serrado;caracterizado por que una primera hoja (152) está unida a un extremo distal de la primera superficie (146a) definida por la muesca (146) y al puntal vertical (146b) para crear una orientación en ángulo con respecto al puntal vertical; yuna segunda hoja (154) está unida a la primera hoja (152) y se extiende hasta un extremo distal de la segunda superficie (146c) para crear una forma de V que se abre hacia el extremo distal.
- 2. El instrumento quirúrgico de la reivindicación 1, en donde un borde de la primera hoja (152) es perpendicular a un borde de la segunda hoja (154).
- 3. El instrumento quirúrgico de la reivindicación 1 o la reivindicación 2, en donde las hojas primera y segunda (152, 154) tienen unos bordes de hoja primero y segundo respectivos que están orientados uno hacia el otro.
- 4. El instrumento quirúrgico de la reivindicación 3, en donde cada borde de hoja es aserrado.
- 5. El instrumento quirúrgico de la reivindicación 3, en donde cada borde de hoja es uniforme.
- 6. El instrumento quirúrgico de cualquier reivindicación anterior, en donde la primera mordaza es un conjunto (122) de yunque y la segunda mordaza es un conjunto (112) de cartucho.
