La presente invenzione si riferisce generalmen-. te a strutture di catetere , idoneo ad essere introdoitto in un corpo vivente, e pi? specificatamente alla : progettazione di un catetere che pu? essere introdot;-to ed instradato attraverso il sistema vascolare di ; un paziente con un minimo di danno o trauma al tessuto endoteliale.
11. Discussione della tecnica precedente :
le procedure chirurgiche del giorno d'oggi permettono la introduzione di cateteri tubolari nel si-: sterna vascolare di un corpo. Ad esempio, cateteri anigiografici comunemente vengono introdotti nella arteria femorale ed instradati attraverso il sistema arteriale e alla apertura coronarica per cui contrasto r;adioopaco o medicamenti possono essere iniettati cornei parte di una procedura diagnostica c di trattamento. I cateteri di tecnica precedente, angiografici e altri catereri diagnostici nonch? conduttori per stimolazione cardiaca generalmente sono formati da polietilene, poliuretano, polipropilene o gomma al silicone o altro materiale non trcmbogenicc . Sebbene prcrji abilmente rari, vi sono stati casi in cui i catete-j ri di piccolo diametro aventi punte con bordo acuto sono stati fatti avanzare attraverso la parete dell aorta, dando come risultato la morte del paziente Sommario dell'invenzione
La presente invenzione ovvia ai problemi sopra-; descritti inerenti a certi cateteri di tecnica prece}-dente, attraverso la inclusione di un organo morbido,, deformabile, attaccato alla punta distale del cateteire. Ir una forma di realizzazione, l'organo a punta deformabile comprende un elemento a "palloncino" anujlare, gonfiabile, flessibile, non distensibile , at- : taccato alla estremit? distale della punta altrimenti relativamente rigida del catetere. In questa disposizione, lo stesso catetere dotato di un lumen che si estende sostanzialmente per la intera lunghezza del catetere e che comunica con l'organo a punta1 defermabile per cui un fluido pu? essere introdotto per controllare la dimensione ,sagoma e stabilit? della parte di punta del catetere. Gonfiando selettivamente la punta del palloncino, l'area effettiva della punta pu? essere aumentata per ridurre la forza totale per area unitaria sul punto di contatto fra la punta del catetere ed un vaso sanguigno.
In una seconda forma di realizzazione, l'organe a.punta morbido deformabile comprende un__manicotto di plastica o di gomma anulare che circonda la punt^. altrimenti rigida del catetere e che si estende normalmente oltre ad essa per_una distanza predeterminata? Quando il manicotto viene fatto schiacciare, l'area effettiva di contatto fra la punta deformabi-j?_ le ed il tessuto corporeo aumenta per ridurre di nu vo la pressione o forza per area unitaria applicata ! al tessuto.
Scopi
Di conseguenza, lo scopo principale della presente invenzione ? il realizzare una disposizione di} catetere originale e perfezionato per applicazioni ! mediche.
Un altro scopo dell'invenzione ? il fornire un catetere con una punta morbida deformabile, per ridurre in tal modo la probabilit? di danno a tessuto vascolare quando il catetere viene introdotto ed instradato attraverso il sistema vascolare.
Ancora un ulteriore scopo dell'invenzione ? il realizzare un catetere con un organo a punta gonfiaile per cui la sagoma e la stabilit? della estremi-[t? distale del caterere possono essere controllate attraversa la introduzione di un fluido sulla estre-! imita prossimale del catetere.
Ancora un altro scopo dell'Invenzione ? il rea-; ]lizzare un catetere con una punta morbida, deformabile, sotto forma di un organo a manicotto di un profi,-;lo di spessore di parete predeterminato, per cui 1' organo a punta deformabile si schiaccer? o ceder? in un modo prevedibile per aumentare effettivamente 1' :area totale di contatto fra la punta distale ed il .tessuto corporeo con cui essa viene a contatto.

Descrizione dei disegni

Descrizione delle forme di realizzazione preferite
Con riferimento dapprima alla figura 1, viene illustrata una sezione trasversale longitudinale della parte di estremit? distale di un tipico catetere angiografico di tecnica precedente. Si pu? vedere che esso comprende una lunghezza predeterminata di tubazione di plastica flessibile 10 avente un lumen. centrale 11 che si estende per la lunghezza del relativo catetere dalla sua estremit? prossimale (non; mostrata) alla sua estremit? distale 12? Il lumen 1J. ? realizzato in modo che coloranti angiograf ici o aj.tri-medicamenti possano essere ..lat-rodot-tl-nel cate? _ tere. sulla sua .estremit? prossimale disposta estej:-namente. al corpo-.e .fatti_fluixe...at.traverso??l_..lum.en fino ad un sito predeterminato nell Lint.ejcn.o _de_l__cpr po su cui ? posizionata la estremit?_distale .12.del catetere.
Si deve notare in modo speciale che la punta distale 12 del catere di tecnica precedente della figbra 1 termina con un bordo netto 13 che ? relativameinte aguzzo e, in dipendenza dal tipo di materiale usato per il corpo del catetere 10, pu? possedere rigir dit? sufficiente per provocare danno a tessuto vascolare, specialmente lo strato endoteliale (tunica} intima) che riveste i vasi sanguigni quando il catejtere viene instradato attraverso il sistema vascolare .
Per ovviare a queste problema, in conformit? arf una prima forma di realizzazione dell'invenzione ililustrata nella figura 2, il corpo 10 del catetere ?! dotato di un doppio lumen, cio? il lumen 11 per conjsentire il flusso di coloranti angiografici o altrii fluidi ed un ulteriore lumen 14 che si estende anchj' esso per l'intera lunghezza del catetere. Fissato alla estremit? distale del corpo del catetere vi ? un organo 15 a "palloncino" flessibile , non distensibi le e gonfiabile, nella sua forma espansa, ha la for-; ma generale di un toroide. Il lumen 14 ? disposto pei? comunicare con l'interno del toro comprendente la punta 15 mentre il lumen 11 comunica con la apertura centrale 17 di quel toroide. Mentre fluido introdotto nel lumen 11 sulla estremit? prossimale del catetere infine pu? essere fatto fluire all'esterno attraverso la apertura 17 della punta morbida flessibile 15,, fluido introdotto nel lumen 14 fluisce nella camera -anulare 16 facendo s? che l'organo a punta venga confiato. L'organo a punta 15 preferibilmente ? formata da una pellicola di poliolefina reticolata o orientata biassialmente e pu? avere uno spessore nell'in- . tervallo da 0,001 a 0,0025 pollici (pari rispettivamente a mm 0,00254 e mm 0,0635). Come tale, quando non gonfiato o svuotato, esso manca di rigidit? e si. avvolge attorno e si conforma approssimativamente alla sagoma della parte di estremit? del corpo 10 del catetere. Tuttavia, una volta debitamente pressurizzato, esso si espande in modo da aumentare l'area efifettiva desiderata di contatto fra le estremit? del ' catetere e qualsiasi tessuto corporeo con il quale ^ possa entrare in attestatura quando viene instradatoi? o posizionato nell'interno del corpo. LIorgano a palloncino toroidale 15.preferibil-., mente e stampato separato e discosto dal corpo 10 del catetere e quindi fissato ad esso mediante unione t r mica o.chimica, ma pu? essere una cavit? formata nelle pareti della estremit? distale del catetere che,i quando riempito con un fluido sotto pressione, fa si che la parete del catetere stessa si rigonfi.
La figura 3 illustra una forma alternativa della forma di realizzazione della figura 2 che ? stat? modificata per incorporare un manicotto elastomeric? distensibile 18 per controllare la pressione inter-! na dell'organo a punta non estensibile 15. Il mani-; cotto elastomerico distensibile 18 pu? essere posi-, zicr.atc su qualsiasi punto lungo la lunghezza del corpo 10 del catetere e, infatti, pu? essere disposto sulla estremit? prossimale estrema del cateterej e su di una ubicazione che rimane all'esterno del corpo in seguito alla sua inserzione ne11?instradamento. Il manicotto elastomerico preferibilmente e disposto nell'interno di una cabit? ad anello formajta nella superficie esterna del corpo 10 del cateteje ed ? idoneamente unito in un modo a tenuta di flpi o al corpo del catetere sui bordi di estremit? come indicato dai riferimenti numerici 19 e 20, rispettiamente. Un foro radiale 21 si estende dalla superficie della cavit? del corpo del catetere al lumen secondario 14. Il foro radiale 21, nonch? la parte del lumen 14 in modo distale alla stessa, e la camera 14 :della punta defermabile 15 vengono riempiti con un fluido non compressibile, e un idoneo tappo 22 viene disposto nel lumen 14 su di un punto prossimale rispetto al foro radiale 21.
Quando una forza viene applicata all'organo a punta 15 nella figura 3, la pressione interna di questo organo a punta viene aumentata provocando deformazione (gonfiaggio) dell'organo a manicotto elastomerico 18 ed uno sgonfiaggio ccrrispondente dell'organo di punta 15. Il grado di sfongiaggio naturalmente ? proporzionale alla forza esercitata su di esso che, a sua volta, ? una funzione della "rigidit?" del manicotto elastomerico 18. Una diminuzione del volume dell'organo di punt? d? come risultato un aumento nell'area di contatto fra esso e qualsiasi sur ?perfid e di accoppiamento contro cui esso venga forr :zato .

Con attenzione continuata dire tta all'organo ai__ punta 15 nella figura 3, ? stato riscontrato espediente incorporare un anello toroidale pi? pesante ; come su 23 per definire la apertura anulare 17. Que]-sto anello ha un diametro esterno che non ? maggio-: re del diametro interno di qualsiasi dispositivo dii introduzione attraversa il quale il catetere debba ?_ essere fatto passare durante l'impianto. Usando questa costruzione, l'organo a punta 15 si conformer? : ad un lumen pi? piccolo del diametro completamente esteso dell'organo a punta 15 poich?, dato che la forza che poggia sulla parete aumenta, la pressione interna nell'organo a punta diminuir? in seguito alla espansione del manicotto elastomerico 18.
In pratica, il lumen 14 pu? avere un diametro j nell'intervallo da 0,002 pollici a 0,004 pollici (pari rispettivamente a mm 0,05 e mm 0,10) che si j estende dall 'ergano a punta 15 attraverso ed inclu-i dente la luce di lato radiale 21. Il lumen 14 pu? ! essere continuato fino alla estremit? prossimale d^l catetere dove il manicotto elastomerico 18 e la luce laterale inclusa 21 possono essere disposti. _ Come con la forma di realizzazione della figura 2, l'organo a punta 15 preferibilmente ? formato; da una pellicola di poliolefina non estensibile e pu? avere un intervallo di spessore da 0,001 pollici a ; 0,0025 pollici (pari rispettivamente a mm 0,0254 e mm 0,0635). Queste dimensioni e questi materiali sono qui esposti soltanto a scopi illustrativi e non ; debbono essere considerati come limitanti necessariamente l'ambito dell'invenzione.

Con riferimento alla figura 5, si pu? vedere chje quando l'organo a punta cede o si schiaccia, l'area; effettiva di contatto fra esso e l'oggetto contro cui viene pressato viene aumentata per diminuire cos? la pressione effettiva esercitata dalla struttura del catetere contro quel tessuto.
Senza intendere alcuna limitazione e soltanto a scopi illustrativi, il lumen del catetere pu? aver ' un diametro di 0,045 pollici (pari a mm 1,14) ed uno spessore di parete di 0,0085 pollici (pari mm 21,59) sulla sua estremit? distale.
