細胞の層を有することにより特徴づけられた。Description: BACKGROUND OF THE INVENTION The present invention relates to a wound dressing for dressing a wound on a tissue structure inside or outside a human or animal body. Currently existing wound dressings have a number of disadvantages. Traditionally, gauze dressings have been used to treat wounds such as burns, cuts, abrasions and other tissue abnormalities. However, such dressings need to be changed frequently, and can cause pain to the patient when worn and subsequently removed for replacement with a new one. Also, some bandages have the inconvenience of restricting patient movement. EP-A-0349505 (Astra Meditec AB) consists of a protein-free, bioabsorbable polymer with pores sized to allow water and salts to pass but not cells and other tissue particles It has been described that the use of porous flexible sheets can improve the healing of soft tissues in mammals, including humans. It is disclosed that this sheet causes a special effect of stinging the formation of macrophages in soft tissue and that the macrophages release growth factors that stimulate tissue healing. Preferred polymer materials for the sheets described in EP-A-0349505 are polyglycolic acid, copolymers of glycolic acid and lactic acid, copolymers of lactic acid and ε-aminocaproic acid, lactide polymers, polydesoxazon, poly (3-hydroxybutyric acid) Ester), copolymers of poly (3-hydroxybutyrate) and 3-hydroxyvalerate, polyesters of succinic acid and crosslinked hyaluronic acid. In EP-A-0349505, according to known methods, it is necessary to press-mold with fibers made by solution spinning of poly (3-hydroxybutyrate) produced according to US-A-4603070 (Steel et al.). A non-woven sheet of poly (3-hydroxybutyrate) having excellent properties was formed. Since the sheet according to EP-A-0349505 has a pore size sufficient to allow the passage of water, bacteria can enter the wound through the sheet. This may not be much of a problem when the sheet is used internally, in other words when placed inside a human or animal body to cover internal wounds, but it may be less problematic for external wounds such as skin wounds. This can be a problem in the case of a wound. GB-A-2166354 (Imperial Chemical Industries Plc) comprises a poly (3-hydroxybutyrate) polymer dissolved or swollen in a volatile solvent such as chloroform, preferably copolymerized with 3-hydroxyvalerate units. A wound dressing is disclosed. Applying this material as a solution or gel on a wound results in a thin film of polymer in intimate contact with the treated area. It is stated that poly (3-hydroxybutyrate) is hydrophilic, thus eliminating the need for an external hydrophilic layer. The dressing is stated to be particularly suitable for quickly protecting the wound site with a temporary covering. The thin layer of hydrophilic poly (3-hydroxybutyrate) disclosed in GB-A-2166354 has many disadvantages as a wound dressing. First of all, the poly (3-hydroxybutyrate) layer is not particularly absorbent, and exudate from the wound does not pass through this layer, so a solution is available for removing excess fluid. Absent. In addition, it is difficult to remove bandages from open wounds, and the volatile solvents involved tend to be cytotoxic and may irritate the wound site. Further, the pore size created in the poly (3-hydroxybutyrate) layer is such that the pore size closest to the wound is smaller than the pore size away from the wound. For this reason, there is a possibility that the invasion of bacteria into the dressing will cause the invasion of pathogenic bacteria into the wound. In summary, the prior art is easy to apply and remove if necessary, has thermal insulation to keep body temperature around the wound surface, and keeps the wound moist but removes excess fluid from the wound It did not address the problem of providing an easy-to-use wound dressing for use during periods of gradual healing, as can be done. Detailed description of the invention The present invention improves on this situation. In accordance with the present invention, a bioabsorbable material that is placed adjacent to a wound when dressing the wound with a wound dressing, has fluid permeability, eg, liquid permeability, and facilitates the wound healing process. A wound on a tissue structure of a living human or animal body, comprising a lower portion consisting of: and a lower portion that covers the lower portion when bandaging the wound with a bandage and that is permeable to vapor but impermeable to bacteria. A wound dressing for dressing is provided. Such bandages are relatively simple to manufacture. Moreover, the bandage is easy to apply and can be modified so that it can be easily removed. In addition, the bandage is capable of maintaining body temperature around the wound surface due to its insulating properties, and keeps the wound moist while removing excess fluid from the wound. The certainty that the upper part is essentially impermeable to bacteria means that there are barriers to prevent the occurrence of infection. Without the means for the exudate to collect on the upper surface of the lower portion remote from the wound, the transport of the exudate from the wound is impeded. Using an upper portion that is permeable to vapor, for example due to open micropores, alleviates this problem by allowing the exudate to dissipate slowly by evaporation. In addition, vapor emanating from tissue surrounding the wound can also penetrate the upper portion. By evaporating excess moisture after pumping exudate from the wound to the lower portion, a flow of material is created that exits the wound and into the air. This flow further impedes the passage of bacteria in the opposite direction, thereby preventing the occurrence of infection. The wound dressing of the present invention can be left on for a relatively long time. This means that the dressing needs to be changed less often than before, thereby reducing the patient's pain and preventing the healing process from being disturbed. The use of a vapor permeable upper portion ensures that the wound remains moist despite effective removal of excess fluid from the wound. Pain can be reduced by keeping the wound from drying out. In a first form of the invention, the wound dressing is an integral st ructure. In a second form of the invention, the wound dressing is formed as an integrated structure. For example, the upper and lower portions may be provided as upper and lower wound dressings, respectively, and combined with each other before dressing the wound. In a third aspect of the invention, a wound dressing is formed once the wound is dressed. For example, the upper and lower portions are provided by upper and lower wound dressings, respectively, which are applied to the wound separately. Placing the lower portion of the wound dressing as a lower wound dressing layer, such as a sheet, can be easily placed in place, facilitates transport of exudate from the wound, and separates exudate from the wound. This means that it can be used as a protective barrier to keep it in place. In embodiments according to various aspects of the invention, the wound dressing further comprises an intermediate portion between the upper and lower portions for absorbing liquid. This is particularly advantageous if a large amount of exudate occurs. In the second and third forms, the intermediate portion may be of a suitable material, such as, for example, treated cellulose fibers or polyacrylic acid. However, hydrocolloids are particularly suitable. Hydrocolloids are usually able to absorb four to six times their own volume of liquid, and new hydrocolloids have been reported that absorb twenty times their own volume. Because the hydrocolloid is also sticky to the skin, the hydrocolloid can serve two functions: an intermediate absorbent layer and a sticky edge of the dressing. The intermediate portion in the second and third aspects of the invention may take the form of an intermediate wound dressing layer or may be part of the upper or lower wound dressing layer of the wound dressing. In an embodiment of the first form of the invention, the wound dressing is composed of an upper and lower part having a lower and upper part made of a bioabsorbable material. Bioabsorbable materials can be in the form of particles or fibers. For example, a wound dressing can include particles or fibers of a bioabsorbable material in a matrix material, such as a gel matrix such as hyaluronic acid. On the other hand, the wound dressing can be a fiber sheet made of a bioabsorbable material, such as a nonwoven fiber sheet. In embodiments of the invention according to the second and third forms, the one or more wound dressing layers are in the form of one or more wound dressing sheets. In a second form of the invention, the wound dressing comprises an upper and lower wound dressing such that the lower dressing adheres to the lower surface of the upper wound dressing. Alternatively, if the wound dressing includes an intermediate wound dressing layer, the upper and lower wound dressing layers are attached to the upper and lower surfaces of the intermediate wound dressing layer, respectively. In an embodiment of the second form of the invention, the lower wound dressing layer is removably protected by the rest of the wound dressing. For example, the lower wound dressing layer is removably guarded on the lower surface of the upper wound dressing layer or on the lower surface of the intermediate wound dressing layer. In a preferred embodiment according to various aspects of the invention, the lower portion of the wound dressing is substantially free of volatile solvents. In embodiments of the second and third aspects of the invention, the lower wound dressing layer is provided as a layer of particles of a bioabsorbable material. The particles may be supported in a matrix material, such as a gel matrix composed of hyaluronic acid. In an embodiment of the second aspect of the present invention, the bioabsorbable particulate material of the lower wound dressing comprises stirring the particles in a solvent and mixing the mixture above or below the intermediate wound dressing. After being applied to the surface of the above, the solvent was evaporated to adhere to the upper surface or the lower surface of the intermediate wound dressing layer. A preferred solvent is chloroform. In an embodiment of the invention according to the third form, when dressing the wound with a wound dressing, the lower part is a layer of unfixed particles or fibers of a bioabsorbable material placed over the wound. Provided as Alternatively, the lower wound dressing layer can be a gel of bioabsorbable material applied over the wound or one or more lower wound dressing sheets placed over the wound. In embodiments of the second and third aspects of the invention, the bioabsorbable material of the lower wound dressing layer comprises a fiber supported in a matrix, such as a gel matrix formed from hyaluronic acid. It is a shape. Alternatively, the lower wound dressing layer may take the form of one or more lower wound dressing fiber sheets formed, for example, from non-woven fibers, wherein the lower surface of the lower wound dressing layer comprises fibers. Advantageously, the ends are roughened to expose them to the surface. In various embodiments of the present invention, the bioabsorbable material of the lower portion is a polymer. For example, the polymer may be protein-free, such as poly (3-hydroxybutyrate) (PHB), polylactic acid, polyglycolic acid, copolymers of glycolic acid and lactic acid, and copolymers of lactic acid and ε-aminocaproic acid. Lactide polymers, polydesoxazon, copolymers of poly (3-hydroxybutyrate) and 3-hydroxyvalerate, polyesters of succinic acid, and cross-linked hyaluronic acid. Stimulates macrophages and promotes healing by using a protein-free, bioabsorbable polymer in the lower part to create a barrier to surrounding and acting as a scaffold for cell growth during degradation It is thought that. Macrophage invasion caused by the protein-free polymer covers the wound with tissue and effectively suppresses wound pain during the first and second days. Stimulation by the polymer also causes angiogenesis and microcirculation. In addition, the degradation of certain protein-free polymers such as PHB has bacteriostatic and fungistatic effects and promotes skin wound healing. Invading macrophages also have a bactericidal effect. The wound dressing can be left on longer due to its bacteriostatic and fungistatic nature and the environment to heal the wound created under the dressing. Fewer dressing changes means less disruption of the healing process and less patient pain. PHB is a preferred material for the lower portion as applicants have found that poly (3-hydroxybutyrate) has the ability to attract macrophages to the wound site at a greater rate than the other polymers tested. PHB also appears to increase angiogenesis, probably through a macrophage effect promoted by PHB. In addition, the use of PHB allows for longer dressing periods due to the fact that the degradation of PHB increases the bacteriostatic and fungistatic properties over time. Furthermore, if the lower part is formed as a PHB fiber sheet with hydrophobic fibers, for example a non-woven fabric, due to the nature of the PHB fibers, the sheet sucks up exudate from the wound and retains it in the part between the sheet and the upper part It has a capillary capacity that helps. In addition, the nonwoven PHB fiber sheet swells due to its structure, despite the fact that the fibers are hydrophobic, with blood components and cells coming from surrounding tissues for the first 10 days or so. In conclusion, the PHB fiber sheet used as the lower part of the wound dressing allows the wound to "breathe" and allow the movement of vapor to keep the wound surface at the proper humidity. If poly (3-hydroxybutyrate) is chosen, oligo (3-hydroxybutyrate), for example those having 3 to 10 monomer units, may also be included in or constitute the lower part . A wide range of other materials can be adapted for the function of the lower part. Such materials will be apparent to those skilled in the art, but by way of non-limiting example are polysaccharides such as chitosan, collagen or proteins. In various embodiments of the present invention, the wound dressing is flexible so that the dressing can follow the outline of the wound, for example, a concave wound. In embodiments of the second and third aspects of the invention, the lower wound dressing is modified to conform to the wound outline, for example by being flexible. To assist in the wound healing process, the lower portion of various forms of wound dressings according to the present invention may support one or more growth factors. In embodiments of the second and third aspects of the invention, the upper wound dressing layer of the wound dressing comprises a polymeric material. The polymeric material may be bioabsorbable and may be protein free. By way of example, the upper part consists of poly (3-hydroxybutyrate). Alternatively, the upper portion may be made of a non-bioabsorbable, non-biodegradable material, and non-limiting examples of polymers include polyurethane and polytetrafluoroethylene. In embodiments according to various aspects of the invention, the upper portion of the wound dressing is porous. If the upper and lower portions are both porous, the pore size of the upper portion is smaller than the pore size of the lower portion. Typically, the pore size of the upper portion is less than about 0.25 μm. In embodiments according to various aspects of the present invention, the wound dressing has a tacky edge such that it can be peelably adhered to the surface of the tissue structure adjacent to the wound. This facilitates the application and removal of the bandage or its upper part. In embodiments of the second and third aspects of the invention, the upper wound dressing layer has a sticky edge, wherein the upper wound dressing layer completely surrounds the lower wound dressing layer. I have. The ideal edge of the overhang is approximately 10 to 15 mm. If the upper wound dressing layer includes a middle portion, an adhesive rim may be made in the middle portion of the upper wound dressing layer. In embodiments of the second and third aspects of the invention, the intermediate wound dressing has an adhesive edge. According to the present invention, a process of applying a fluid permeable lower layer of a bioabsorbable material to the wound, which promotes the healing process in the wound, and an upper layer that is permeable to vapor but impermeable to bacteria. There is also provided a method of treating a wound on a tissue structure of a human or animal body, comprising the step of covering the tissue. The present invention further provides a method of making a wound dressing, comprising combining a fluid permeable layer of a bioabsorbable material that facilitates the healing process in a wound with a layer that is permeable to vapor but impermeable to bacteria. I do. According to the present invention there is further provided a method of using poly (3-hydroxybutyrate) in the manufacture of a wound dressing for skin in the form of fibers or powder substantially free of volatile solvents. Hereinafter, embodiments of the present invention will be described as examples with reference to the accompanying drawings. FIG. 1 is a side sectional view of a first wound dressing according to the present invention. FIG. 2 is a side sectional view of a second wound dressing according to the present invention. FIG. 3 is a side sectional view of a third wound dressing according to the present invention. As shown in FIG. 1, a wound dressing 10 according to the present invention is applied to a skin wound 2 in the form of a non-woven fibrous sheet substantially free of volatile solvents to promote bioabsorption of the wound healing process. A layer 1 of a non-porous material (poly (3-hydroxybutyrate in this example) (hereinafter "PHB")) and an outer microporous layer 3 of a polyurethane polymer. The pores of the outer polymer layer 3 have a size of less than 0.22 μm. This allows layer 3 to be permeable to vapors and gases, but substantially impermeable to bacteria. The dressing 10 is easy to apply and remove, and has the insulating effect of maintaining body temperature around the wound surface with the outer polymer layer 3. The dressing 10 also keeps the wound 2 moist while allowing water vapor to permeate and remove excess fluid from the wound 2. This prevents drying of the nerve endings in the wound and the associated pain in the patient. The outer polymer layer 3 has a sticky rim 4 to facilitate application and removal of the bandage 10. The PHB fibers in bandage 10 exhibit hydrophobic properties. Due to the nature of the PHB fibers, the exudate is separated from the wound 2 and retained in the PHB layer 1 and in the space between the PHB layer 1 and the outer polymer layer 3. The wound dressing 10 can be left on for a relatively long period of time. This means less bandage change than previously suggested bandages. This reduces pain to the patient and can prevent interruption of the healing process. One way to change the dressing 10 is to remove only the outer polymer layer 3 and excess exudate and then apply a new outer layer 3 without disturbing the PHB layer 1. Alternatively, most of the PHB layer 1 may be removed, leaving the PHB fibers adhering to the wound 2, and replaced with a new piece of PHB. FIG. 2 shows the present invention comprising a layer 101 of PHB applied to a skin wound 102 in the form of a non-woven fiber sheet substantially free of volatile solvents and an outer microporous layer 103 of polyurethane polymer. A second wound dressing 110 is shown. The PHB layer 101 is in the form of a patch completely surrounded by the outer polymer layer 103, leaving a 10 to 15 mm edge (m) of the outer polymer layer 103 around the PHB patch 101. The size of the PHB patch 101 itself can vary depending on the size of the wound 102. Further, an intermediate absorbent layer 105 comprising a hydrocolloid material is provided. This absorbs the exudate transported by the PHB patch 101 from the wound 102 and promotes the transport of more exudate. The microporosity of the outer polyurethane layer 103 means that the exudate is slowly dissipated while keeping the wound 102 moist while simultaneously blocking bacteria. Vapor from the skin around the wound 102 can also pass through the outer polymer layer 103. The outer polymer layer 103 has a sticky rim 104 to facilitate application and removal of the bandage 110. Referring now to FIG. 3, a third wound dressing 210 according to the present invention comprises a layer 201 of PHB applied to a skin wound 202 in the form of a non-woven fiber sheet substantially free of volatile solvents, and a polyurethane polymer. Consists of an outer microporous layer 203 of Again, the PHB is in the form of a patch completely surrounded by the outer polymer layer 203, leaving a 10-15 mm polymer edge (m) around the PHB patch. Also provided is an intermediate absorbent layer 205 of a hydrocolloid material, in which case the hydrocolloid layer 205 is made to be integral with the outer polymer layer 203. Since the hydrocolloid material is sticky to the skin, it sticks to the skin by the rim 204. Thus, the hydrocolloid serves two functions: an intermediate absorbent layer and a sticky edge to the outer layer 203. Those skilled in the art will appreciate that the present invention is not limited to the specific wound dressing embodiments described above with reference to the accompanying drawings, and that it may take many forms or aspects within the scope of the appended claims. It will be easily understood. Applicants believe that the wound dressing must serve the following purposes: -Body temperature on the wound surface must be maintained by thermal insulation. -The wound must be kept moist, but the excess liquid must be removed. Dressings should be permeable to vapors but not germs. Dressings must be absorbable and breathable. -Pain must be suppressed. -The bandage should be easy to apply and remove if necessary. • Bandages should promote healing. • Bandages must be compatible with other treatments. For example, an external dressing bandage must be provided to allow effective debridement of necrotic skin. Dressings must be biocompatible and non-toxic. • Bandages must not interfere with patient movement. The wound dressing described above with reference to the accompanying drawings fulfills these objectives in a simple and inexpensive manner. In a comparative test, a wound dressing according to the invention and a wound dressing consisting of a sheet of polyurethane (Tagerderm ™ ) were used for dressing skin wounds on mammalian bodies. The dressing according to the invention consists of a lower part in the form of a nonwoven fibrous sheet made of PHB and an upper part made of a polyurethane sheet. Using the wound dressing of the present invention resulted in improved wound healing as compared to the polyurethane sheet wound dressing. This is because the wound healed covered by the wound dressing of the present invention has a thicker and better layer of epithelial cells in the regenerated tissue compared to the healed wound covered by the polyurethane sheet wound dressing. It was characterized by:
