- 【特許請求の範囲】 1.以下を有する生体人工心臓弁: 寸法的に安定で予備整列および予備組立した組織小葉サブアセンブリであって 、該サブアセンブリは、複数の小葉部を含み、ここで、各小葉部は、接合縁部と 対向する尖頭縁部、およびほぼ反対の方向で該尖頭縁部および該接合縁部の間の 接合部から伸長している2個の小葉タブを有し、該サブアセンブリ中での該小葉 部の予備整列は、該サブアセンブリから外方へ伸長している複数対の噛み合い小 葉タブを作製し、隣接する一対の小葉部あたり、一対の噛み合い小葉タブが存在 する; ほぼ円形のワイヤフォームであって、該ワイヤフォームは、該予備整列した組 織小葉サブアセンブリを一定の噛み合い連結で収容するように寸法合わせした底 面を有し、該ワイヤフォームは、上方へ伸長している交連により遮断された下部 尖頭を有し、該予備組立した組織小葉サブアセンブリ中の該小葉部の該尖頭縁部 は、該ワイヤフォームの該尖頭に沿って装着されて、小葉部/ワイヤフォームサ ブアセンブリを形成する;および ほぼ円形の支持ステントであって、該支持ステントは、該小葉部/ワイヤフォ ームサブアセンブリとの接触連結で載置され固定されるように寸法合わせした上 面を有し、該支持ステントは、上方に伸長している可撓性交連ポストを有し、該 ポストには、噛み合い小葉タブの対が装着されている。 2.前記ワイヤフォームが、布カバーを備えている、請求項1に記載の生体人 工心臓弁。 3.前記支持ステント上面が、変形可能な小葉噛み合いシートを備えている、 請求項1に記載の生体人工心臓弁。 4.前記支持ステントが、布カバーを備えている、請求項1に記載の生体人工 心臓弁。 5.さらに、前記支持ステント上に固定的に配置した適合性構造界面を有する 、請求項1に記載の生体人工心臓弁。 6.前記適合性構造界面が、縫合リングを有する、請求項5に記載の生体人工 心臓弁。 7.前記適合性構造界面が、フランジを有する、請求項5に記載の生体人工心 臓弁。 8.前記適合性構造界面が、導管を有する、請求項5に記載の生体人工心臓弁 。 9.さらに、前記ワイヤフォームの回りに固定的に配置した出口導管を有する 、請求項1に記載の生体人工心臓弁。 10.さらに、前記支持ステント上に固定的に配置した入口導管を有する、請求 項9に記載の生体人工心臓弁。 11.以下を有する生体人工心臓弁: 寸法的に安定で予備整列した組織小葉サブアセンブリであって、該サブアセン ブリは、複数の実質的に同一の組織接合小葉部から形成され、該各小葉部は、対 向末端を有するほぼ線状の接合噛み合い縁部およびその間に伸長しているほぼア ーチ形の周辺尖頭を有し、隣接小葉部の該対向末端は、互いにテンプレートに合 わせて装着されて、複数対の整列噛み合い小葉末端を規定している; ほぼ円形の布被覆ワイヤフォームであって、該ワイヤフォームは、頂面および 底面、および複数の軸方向突出交連を規定するほぼ正弦の周辺を有し、該各交連 は、該整列噛み合い小葉末端の対の1個と対応しており、そして該ワイヤフォー ムの底面は、該予備整列組織小葉サブアセンブリを受容しそれと噛み合い連結で 固定的に配置されるように寸法合わせされている;および ほぼ円形の支持ステントであって、該支持ステントは、該ワイヤフォームの該 底面と噛み合い連結で固定的に配置された該小葉部/ワイヤフォームサブアセン ブリを載置しそれと噛み合い連結で固定されるように寸法合わせした上面を有す る。 12.前記組織小葉周辺尖頭が、交連から交連へと、前記布被覆ワイヤフォーム に縫い付けられる、請求項11に記載の生体人工心臓弁。 13.前記支持ステント上面が、小葉尖頭噛み合いシートを備えている、請求項 11に記載の生体人工心臓弁。 14.前記支持ステントが、布カバーを備えている、請求項11に記載の生体人工 心臓弁。 15.前記支持ステントが、ほぼ円形の単一リング構造物であり、該リング構造 物が、前記ワイヤフォームの前記軸方向突出交連と対応する複数の軸方向突出可 撓性交連を備えている、請求項14に記載の生体人工心臓弁。 16.さらに、前記ほぼ円形の単一リング構造の回りに配置した補強材リングを 有する、請求項15に記載の生体人工心臓弁。 17.さらに、前記支持ステント上に固定的に配置した適合性構造界面を有する 、請求項11に記載の生体人工心臓弁。 18.前記適合性構造界面が、縫合リングを有する、請求項17に記載の生体人工 心臓弁。 19.前記適合性構造界面が、フランジを有する、請求項17に記載の生体人工心 臓弁。 20.前記適合性構造界面が、導管を有する、請求項17に記載の生体人工心臓弁 。 21.さらに、前記ワイヤフォームの回りに固定的に配置した出口導管を有する 、請求項11に記載の生体人工心臓弁。 22.さらに、前記支持ステント上に固定的に配置した入口導管を有する、請求 項21に記載の生体人工心臓弁。 23.適合性生体人工心臓弁を製造する方法であって、該方法は、以下の工程を 有する: サブアセンブリ周辺縁部を規定する複数の実質的に同一の組織小葉部から、寸 法的に安定で予備整列した小葉サブアセンブリを形成すること; ほぼ円形のワイヤフォームを提供することであって、該ワイヤフォームは、該 寸法的に安定で予備整列した組織小葉サブアセンブリ周辺縁部を噛み合い連結で 収容するように寸法合わせした底面を有する; ほぼ円形の支持ステントを提供することであって、該支持ステントは、該寸法 的に安定で予備整列した組織小葉サブアセンブリ周辺縁部と噛み合うように寸法 合わせした上面を有する;および 該寸法的に安定で予備整列した組織小葉サブアセンブリ周辺縁部を、該ワイヤ フォーム底面と該支持ステント頂面との間でクランプ固定すること。 24.さらに、前記寸法的に安定で予備整列した組織小葉サブアセンブリ周辺縁 部を、前記クランプ固定前に、前記ワイヤフォームの前記底面に装着する追加工 程を有する、請求項23に記載の方法。 25.前記クランプ固定工程が、以下の追加工程を有する、請求項24に記載の方 法: 前記支持ステントの前記上面を、前記装着した寸法的に安定で予備整列した組 織小葉サブアセンブリ周辺縁部と噛み合わせること;および 該支持ステントを、該寸法的に安定で予備整列した組織小葉サブアセンブリ周 辺縁部および該ワイヤフォームに装着すること。 26.前記装着工程が、縫い合わせることを有する、請求項25に記載の方法。 27.さらに、前記クランプ固定工程後、前記支持ステントに、適合性構造界面 を装着する追加工程を有する、請求項23に記載の方法。 28.心臓弁で使用するための組織-ワイヤフォーム構造アセンブリであって、 該アセンブリは、以下を有する: 複数の小葉部を有する小葉サブアセンブリであって、該各小葉部は、一対の対 向末端、該対向末端の間に伸長している実質的に線状の縁部、および該対向末端 の間に伸長しており周辺縁部を有する実質的にアーチ形の尖頭を有する; 該各小葉部の該各対向末端は、該小葉部の他の該対向末端の各個に装着されて 、複数対の小葉末端および開口部を規定し、該開口部は、該小葉部の該実質的に 線形の縁部により、規定される;および 布被覆ワイヤフォームであって、該ワイヤフォームは、複数の交連および該交 連の各個の対の間に伸長している複数のアーチ形部分を有する; 該布被覆ワイヤフォームは、該各交連が、該小葉サブアセンブリの該小葉末端 対の1個と対応して受容するように、また、該各アーチ形部分が、該小葉サブア センブリの該小葉部の1個の該アーチ形尖頭と対応するように、該小葉サブアセ ンブリと相補的に構成されている; 該小葉サブアセンブリは、該小葉末端対にて、該小葉部の該アーチ形尖頭の該 周辺縁部に沿って、該布被覆ワイヤフォームに縫い付けられている。 29.前記布被覆ワイヤフォームが、布縁部を有し; 前記小葉サブアセンブリが、該布被覆ワイヤフォームの該布縁部に縫い付けら れる、請求項28に記載の組織-ワイヤフォーム構造アセンブリ。 30.前記小葉サブアセンブリの前記各小葉部が、さらに、それらの前記各対向 末端で規定されるタブを有し; 該タブが、該小葉アセンブリを前記布被覆ワイヤフォームに縫い付けたとき、 該布被覆ワイヤフォームの前記交連から外方へ突出している、請求項28に記載の 組織-ワイヤフォーム構造アセンブリ。 31.心臓弁で使用するための組織小葉サブアセンブリを製造する方法であって 、該方法は、以下の工程を有する: 一対の対向末端を有する複数の組織小葉部、該対向末端の間に伸長している実 質的に直線状の縁部、および該対向末端の間に伸長している実質的にアーチ形の 尖頭を提供すること; 該組織小葉部の1個の該対向末端の1個を、該組織小葉部の他の1個の該対向 末端の1個に装着すること;および 該各組織小葉部の該各対向末端が、該組織小葉部の他の1個の対向末端に装着 されるまで、該装着工程を繰り返すこと。 32.前記提供工程が、以下の工程を有する、請求項31に記載の方法: 組織を提供すること; 一対の対向末端を有するテンプレート、該対向末端の間に伸長している実質的 に線状の縁部、および該対向末端の間に伸長している実質的にアーチ形の部分を 提供すること; 該テンプレートを、該組織上に配置すること;および 該テンプレートを越えて伸長する該組織の一部を切り取ること。 33.前記テンプレートが、その前記対向末端の1個に形成されたガイドスロッ トを有し、前記装着工程が、以下の工程を有する、請求項32に記載の方法: 該テンプレートを、前記組織小葉部の2個と整列させること;および 該ガイドスロットを通って該組織小葉部の該対向末端を通り、糸付き針を挿入 すること;および 該糸を固定すること。 34.心臓弁で使用するステントを製造する方法であって、該方法は、以下の工 程を有する: 複数の上方へ突出した可撓性ポストを有する実質的に円形で半径方向に安定な 支持部材を提供すること; 布被覆を提供すること;および 該布被覆が該支持部材を取り囲んで該支持部材の形状と実質的に一致するよう に、該支持部材の上に、該布被覆を縫い付けること。 35.前記支持部材が、内部支持部材および外部支持部材を有し、該外部支持部 材が、その中に該内部支持部材を受容するように構成され、前記複数のポストが 、該内部支持部材上に形成されており、さらに、以下の工程を有する、請求項34 に記載の方法: 該内部支持部材を、該外部支持部材内に配置すること;および 該支持部材を共に装着すること。 36.前記外部支持部材が、複数のポストを有し、該各ポストが、前記内部支持 部材の前記ポストに対して切り取られており、さらに、前記装着工程前に、該支 持部材の該ポストを整列させる工程を有する、請求項35に記載の方法。 37.前記支持部材が、さらに、複数の縫穴を有し、前記装着工程が、以下の工 程を有する、請求項35に記載の方法: 該支持部材の該縫穴を整列させること; 該整列縫穴の少なくとも一対に、糸付き針を挿入すること;および 該糸を固定すること。
