Claims (32)
- 心臓弁の修復のためのデバイスであって、
患者の脈管系を通して遠隔的に心臓内の心臓弁に隣接した位置にまで搬送され得るよう構成されたカテーテルアセンブリと;
前記カテーテルアセンブリ内を通って互いに独立に並進移動可能とされた一対をなす操作可能な把持部材であるとともに、前記カテーテルアセンブリから可逆的にかつ径方向に延出可能なものとされ、これにより、2つの弁小葉のそれぞれの再配置部分を把持して近接状態とし得るような、一対をなす操作可能な把持部材と;
前記カテーテルアセンブリを通って並進移動可能とされ、さらに、前記2つの弁小葉の前記近接状態とされた部分どうしを固定し得るものとされた、固定機構と;
を具備していることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記カテーテルアセンブリが、湾曲した先端部を有した単一のカテーテルを備え、
これにより、弁に対しての大動脈からの逆行的なアクセスが可能とされていることを特徴とするデバイス。 - 請求項2記載のデバイスにおいて、
前記先端部の少なくとも一部が、形状記憶材料から構成されていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記カテーテルアセンブリが、互いにスライド可能に連結された一対をなす2つのカテーテルから構成され、
各カテーテルには、1つの前記把持部材が配置され、
各カテーテルが、少なくとも1つのサイドポートを備え、
前記固定機構が、前記サイドポートを通して延出可能とされ、さらに、前記カテーテルアセンブリに対して係合解除可能とされていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記固定機構が、径方向に延出可能とされ、さらに、前記カテーテルアセンブリの前記少なくとも1つのサイドポートを通して前記カテーテルアセンブリに対して係合解除可能とされていることを特徴とするデバイス。 - 請求項5記載のデバイスにおいて、
前記固定機構が、ステープルまたはリベットを備え、
前記ステープルまたはリベットが、小葉を変位させることなく、小葉の前記把持部分を貫通して挿入可能とされていることを特徴とするデバイス。 - 請求項5記載のデバイスにおいて、
前記固定機構の少なくとも一部が、形状記憶材料から構成されていることを特徴とするデバイス。 - 請求項5記載のデバイスにおいて、
前記カテーテルアセンブリが、前記少なくとも1つのサイドポートの近傍に、退避可能部分を備え、
この退避可能部分が、退避状態においては、ノッチを形成し、
このノッチを使用して、前記固定機構が、前記カテーテルアセンブリに対して係合解除可能とされていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記各把持部材が、逆向きに変形した状態で、前記カテーテルアセンブリの前記先端部から延出され、
これにより、弁小葉の心室側表面をクリップし得るようになっていることを特徴とするデバイス。 - 請求項9記載のデバイスにおいて、
前記カテーテルアセンブリを通って互いに独立に並進移動可能とされた一対をなす操縦可能な把持部材を具備し、
前記各把持部材が、前記カテーテルアセンブリの互いに反対側に位置した少なくとも2つのサイドポートの一方から径方向に延出可能とされ、
前記各把持部材の延出が、変形を調節可能な態様で行われ、これにより、弁小葉の心房側表面を把持し得るようになっていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記各把持部材が、前記カテーテルアセンブリの互いに反対側に位置した少なくとも2つのサイドポートの一方から延出可能とされ、
前記各把持部材の延出が、変形しつつ行われ、これにより、弁小葉の心室側表面を把持し得るようになっていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記各把持部材が、前記カテーテルアセンブリの互いに反対側に位置した少なくとも2つのサイドポートの一方から延出可能とされ、
前記各把持部材の延出が、変形を調節可能な態様で行われ、これにより、弁小葉の心房側表面を把持し得るようになっていることを特徴とするデバイス。 - 請求項12記載のデバイスにおいて、
前記把持部材の前記各先端部には、一対をなす操作可能なジョーが配置されていることを特徴とするデバイス。 - 請求項12記載のデバイスにおいて、
前記把持部材の前記各先端部には、真空吸引アセンブリが配置されていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記把持部材の少なくとも一部が、形状記憶材料から形成されていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
さらに、前記カテーテルアセンブリを前記心臓弁に向けて搬送するための導入カテーテルを具備していることを特徴とするデバイス。 - 請求項16記載のデバイスにおいて、
前記導入カテーテルが、前記カテーテルアセンブリを前記心臓弁の近傍位置にまで操作可能とするための案内アームを備えていることを特徴とするデバイス。 - 請求項16記載のデバイスにおいて、
前記導入カテーテルが、左心室内において前記導入カテーテルの位置を一時的に安定化させるための安定化手段を備えていることを特徴とするデバイス。 - 請求項18記載のデバイスにおいて、
前記安定化手段が、1つまたは複数の接触部材を有し、
この接触部材が、前記導入カテーテルから可逆的に延出可能とされ、
前記接触部材が、心臓キャビティの内表面に対して1つまたは複数のポイントにおいて接触可能とされていることを特徴とするデバイス。 - 請求項19記載のデバイスにおいて、
前記1つまたは複数の接触部材が、形状記憶材料から形成されていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
さらに、前記心臓弁の近傍領域を撮影し得るような向きとされた撮影デバイスを具備していることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
さらに、保持トリガー機構を具備し、
この保持トリガー機構が、前記把持部材に対しての弁小葉の位置を検出するための1つまたは複数のセンサを備え、
前記把持部材が、弁小葉が前記把持部材から所定距離以内に位置したことが検出されたことに応答して延出されることを特徴とするデバイス。 - 心臓弁を修復するための方法であって、
患者の脈管系を通して遠隔的に心臓内の心臓弁に隣接した位置にまでカテーテルアセンブリを搬送し;
前記カテーテルアセンブリ内を通って互いに独立に並進移動可能とされた一対をなす把持部材を使用することによって2つの弁小葉部分を個別的に保持するとともに、それら弁小葉部分を近接配置し;
1つまたは複数のポイントにおいて前記2つの弁小葉部分を固定し;
前記カテーテルアセンブリと前記弁小葉との係合を解除する;
ことを特徴とする方法。 - 請求項23記載の方法において、
前記カテーテルアセンブリの前記前進に際しては、左心室内へと連通する動脈系を逆行する経路と、静脈系を通って右心房へと入りさらに心房中隔を直接的に貫通して左心房内へと入る経路と、胸壁および心膜内の小さな切開口を通した経皮的な経路と、からなるグループの中から選択された経路を経由することを特徴とする方法。 - 請求項23記載の方法において、
前記把持ステップに際しては、
少なくとも部分的に形状記憶材料から形成された各把持部材を、心臓弁の近傍に配置し;
前記把持部材を前記カテーテルアセンブリから可逆的に延出させ、この場合、各把持部材を、前記カテーテルアセンブリから径方向に延出させ、これにより、弁小葉の一部をクリップする;
ことを特徴とする方法。 - 請求項25記載の方法において、
前記把持ステップに際しては、
各把持部材を、心臓弁の近傍に配置し;
各把持部材を、前記カテーテルアセンブリの互いに反対側に位置した少なくとも2つのサイドポートの一方から径方向に可逆的に延出させ;
各把持部材を、弁小葉の心房側表面の近傍位置にまで、操縦し;
その心房側表面を把持する;
ことを特徴とする方法。 - 請求項23記載の方法において、
さらに、心臓内において、前記カテーテルアセンブリの位置を一時的に安定化させることを特徴とする方法。 - 請求項23記載の方法において、
さらに、修復操作時に、心臓弁の近傍領域を撮影することを特徴とする方法。 - 請求項28記載の方法において、
前記撮影に際しては、心臓弁の近傍領域に対して超音波エネルギーを伝達することを特徴とする方法。 - 請求項29記載の方法において、
前記超音波エネルギーを、胸部表面からあるいは食道からあるいは心臓内から送信することを特徴とする方法。 - 請求項29記載の方法において、
前記心臓内から送信される前記超音波エネルギーを、心臓弁の近傍に配置された超音波トランスデューサから得ることを特徴とする方法。 - 請求項23記載の方法において、
さらに、弁小葉の位置を示すセンサ出力に基づいて各小葉の把持を独立的にトリガーすることを特徴とする方法。
