Claims (31)
- 房室弁の閉鎖不全症を治療するための装置であって、
動作時には、房室弁小葉を心臓内筋肉に対して取り付けている少なくとも1つの腱索を切断し得るよう構成された切断器具と;
この切断器具を前記少なくとも1つの腱索の近傍に配置し得るよう構成された位置決めカテーテルと;
前記切断器具とは別部材であるとともに、前記カテーテルに対しての前記少なくとも1つの腱索の移動を少なくとも部分的に拘束し得るよう構成された把持手段と;
を具備し、
前記カテーテルが、開口を備え、この開口内において、前記把持手段が前記少なくとも1つの腱索の移動を少なくとも部分的に拘束し得るものとされ、
前記切断器具が、前記開口内において前記少なくとも1つの腱索を切断し得るものとされていることを特徴とする装置。 - 請求項1記載の装置において、

この第1構成を前記少なくとも1つの腱索の近傍に位置決めするための第2構成すなわち位置決め手段と;
前記第1構成に対しての前記少なくとも1つの腱索の移動を少なくとも部分的に拘束し得るよう構成された把持手段と;
を具備し、
前記第1構成が、開口を備え、この開口内において、前記把持手段が前記少なくとも1つの腱索の移動を少なくとも部分的に拘束し得るものとされ、
前記第1構成が、前記開口内において前記少なくとも1つの腱索を長尺化させ得るものとされていることを特徴とする装置。 - 請求項24記載の装置において、
前記第1構成が、
複数のノードと;
これらノードに対して連結された所定長さの少なくとも1つの人工的腱索材料と;
前記少なくとも1つの腱索に沿ったそれぞれの所定位置に各ノードを固定するための第3構成すなわち固定手段と;
固定された前記2つのノードの間において前記少なくとも1つの腱索の一部を切断するための第4構成すなわち切断手段と;
を備えていることを特徴とする装置。 - 請求項24または25記載の装置において、
さらに、前記少なくとも1つの所定長さの人工的腱索材料の長さを調節し得るものとされた調節手段を具備していることを特徴とする装置。 - 請求項25記載の装置において、
前記第4構成が、ラジオ波電極と、ブレードと、蒸発除去用のレーザーエネルギーを搬送するための光ファイバと、の中の少なくとも1つとされた第5構成を有していることを特徴とする装置。 - 請求項25記載の装置において、
前記各ノードが、第1係合部分と第2係合部分とを備え、
これら係合部分が、前記少なくとも1つの腱索の一部を受領するための凹所を有し、
前記各ノードを固定するための前記第3構成が、前記各係合部分の表面に対して適用された接着剤と、前記各係合部分上にそれぞれ対応した配置された複数の連結ペグおよび複数の孔と、各係合部分を一緒にアニールするための加熱ブレードと、からなるグループの中から選択された手段を有していることを特徴とする装置。 - 請求項24記載の装置において、
前記第1構成が、2つのノードを備え、
これら各ノードが、2つの係合部分とを備え、
これら係合部分が、前記少なくとも1つの腱索の一部を受領するための凹所を有し、
前記第1構成が、
前記2つのノードに対して連結された所定長さの少なくとも1つの人工的腱索材料と;
前記少なくとも1つの腱索に沿ったそれぞれの所定位置に各ノードを固定するための固定手段と;
固定された前記2つのノードの間において前記少なくとも1つの腱索の一部を切断するための切断手段と;
を備え、
前記第2構成が、
少なくとも2つのジョーであるとともに、各ノードの前記2つの係合部分のそれぞれ対応する係合部分に収容され、少なくとも一方のジョーが、前記所定長さの少なくとも1つの人工的腱索材料を収容しているような、少なくとも2つのジョーと;
これら少なくとも2つのジョーを回転可能とする制御可能なヒンジ機構と;
前記ジョーと前記ヒンジ機構と前記長尺化器具とを前記少なくとも1つの腱索の近傍位置にまで搬送するためのカテーテルと;
を備えていることを特徴とする装置。 - 請求項1記載の装置において、
前記開口が、前記少なくとも1つの腱索の近傍のところにおいて前記カテーテルの先端に配置されていることを特徴とする装置。 - 請求項1記載の装置において、
前記開口が、前記カテーテルの側部に配置されていることを特徴とする装置。
