本願の開示は、切開手術および／または内視鏡手術を遂行するための電気外科器具に関し、より詳しくは、対向ジョー部材間のギャップ距離を制御してシール手術中の組織の操作性およびグリップ性を高めるように設計された対向ジョー部材の一方または両方に関連する非導電性ストップ部材を備えたバイポーラ電気外科鉗子に関する。
止血鉗子または鉗子は、そのジョー間の機械的作用を用いて血管を絞窄する簡単なプライヤのような器具であり、一般に、組織を掴み、切開しおよび／またはクランプする切開手術に使用されている。電気外科鉗子は、機械的クランプ作用および電気エネルギの両方を使用して、組織および血管を加熱し、組織を凝固させ、焼灼しおよび／またはシールすることにより止血する。
この数十年間来、ますます多くの外科医が、小孔状切開部を介して器官にアクセスする内視鏡および内視鏡器具を用いて生体器官および体腔へのアクセスを行なう伝統的な切開方法を賞賛している。内視鏡器具は、トロカールを形成するカニューレまたはポートを介して患者の体内に挿入される。カニューレの一般的なサイズは３〜１２ｍｍである。一般にカニューレは小さいほど好ましいが、小型化することは、カニューレに挿通する手術器具の製造方法を見出さなくてはならない器具製造業者に設計的挑戦を課すものである。
或る外科手術は、血管または脈管組織の切断を必要とする。しかしながら、スペース的制限のため、外科医が、血管の縫合または例えば横断された血管のクランプおよび／または結紮のように出血をコントロールする他の伝統的方法を行なうことが困難である。直径２ｍｍ以下の血管は、しばしば、標準的な電気外科技術を用いて閉鎖される。より大きい血管を切断する場合には、外科医は、内視鏡手術を切開手術に変更する必要があり、従って腹腔鏡手術方法の利益を放棄しなければならない。
幾つかの雑誌論文には、電気外科方法を用いて小血管をシールする方法が開示されている。非特許文献１には、小血管のシールに使用するバイポーラ凝固器が開示されている。この論文は、２〜２．５ｍｍより大きい直径の動脈を安全に凝固させることはできないことを述べている。非特許文献２には、血管壁の焼焦を防止すべく血管への電気外科電力の供給を停止させる方法が開示されている。
電気外科鉗子の使用により、外科医は、組織に供給される電気外科エネルギの強度、周波数および供給時間を制御することにより、出血を焼灼させ、凝固／乾燥させおよび／または簡単に低減または遅延させることができる。一般に、電気外科鉗子の電気的形態は、２つの種類すなわち１）モノポーラ電気外科鉗子および２）バイポーラ電気外科鉗子にカテゴリ化できる。
モノポーラ鉗子は、クランピングエンド・エフェクタに関連する１つの活性電極と、一般に患者の体外に取り付けられる遠隔の患者戻り電極すなわちパッドとを使用する。電気外科エネルギが加えられると、エネルギは、活性電極から、患者を通って手術部位へと移動し、更に戻り電極へと移動する。
バイポーラ電気外科鉗子はほぼ対向する２つの電極を使用し、これらの電極はエンド・エフェクタの内側対向面上に配置され、かつ両電極とも電気外科エネルギ発生器に電気的に接続されている。各電極は異なる電位に荷電される。組織は電気エネルギの導体であるので、エフェクタがこれらの間に組織を掴むのに使用される場合には、電気エネルギは組織を通って選択的に伝達される。
大きい血管に適正なシールを行なうには、２つの主な機械的パラメータ、すなわち血管に加えられる圧力および電極間のギャップを正確に制御しなければならない。これらのパラメータは両方とも、シールされる血管の厚さに影響を与える。より詳しくは、圧力の正確な適用は、血管壁を対向させ、充分な電気外科エネルギが組織を通って流れるように組織のインピーダンスを充分に小さい値に低減させ、組織の加熱中の膨張力に打ち勝つようにし、かつ良好なシールの表示である端組織厚さに寄与させる上で重要である。溶融した血管壁は０．００１〜０．００５インチが最適であることが判明している。この範囲より低いと、シールが細断すなわち裂けてしまうであろうし、この範囲より高いと、管腔が適正すなわち有効にシールされないであろう。
電気外科方法は、血管壁に大きい閉鎖力を加えることができる器具に連結される適当な電気外科電力曲線を用いて、大きい血管をシールできる。小さい血管を凝固させる方法は、電気外科な血管シーリングとは基本的に異なるものと考えられる。本願での目的から、用語「凝固（coagulation）」とは、組織を乾燥（desiccating）させる方法であって、組織細胞が破裂されかつ乾燥される方法であると定義する。また、血管のシーリングとは、組織中のコラーゲンを液化させ、溶融マスとして再生させる方法であると定義する。かくして、小さい血管の凝固は、これらの血管を永久的に閉鎖するのに充分である。大きい血管は、永久閉鎖を確保するにはシールする必要がある。
種々の切開手術を行なうのに、これまで、多くのバイポーラ電気外科鉗子が提案されている。しかしながら、これらの幾つかの設計は、血管に再現可能な均一圧力を付与できず、従って得られるシールは有効でないすなわち不均一なものとなる。例えば、Willisの特許文献１、Hiltebrandtnの特許文献２および特許文献３、Boebel等の特許文献４、特許文献５および特許文献６、Lottickの特許文献７、特許文献８、特許文献９および特許文献１０、Stern等の特許文献１１、Eggers等の特許文献１２、およびRichardson等の特許文献１３の全ては、血管または組織の凝固、切断および／またはシーリングを行なう電気外科器具に関する。
これらの器具はクランプ圧力のみを用いて適正シーリング厚さを得るものであって、ギャップ公差および／または平行性および閉端部制御の条件（これらの条件は、適正に制御されるならば、一定かつ有効な組織シールを確保できるパラメータである）を考慮に入れて設計されてはいない。例えば、次の２つの理由、すなわち１）加えられる力が大き過ぎると、２つの極が接触して、組織を通ってエネルギが伝達されず、従って有効なシールが得られないこと、および２）加えられる力が小さ過ぎると、厚くて信頼性に欠けるシールが形成されることから、クランプ圧力のみを制御することによって、シールされる組織の厚さを充分に制御することは困難なことが知られている。
上記のように、大きい血管を適正かつ有効にシールするには、対向ジョー部材間に大きい閉鎖力を必要とする。両ジョー間に大きい閉鎖力を得るには、一般に、各ジョーのピボット回りの大きいモーメントを必要とする。一般にジョー部材は、各ジョー部材の枢動に関して小さいモーメントアームをもつように配置されたピンにより取り付けられるため、これは困難なことである。小さいモーメントアームと大きい力との組合せは、ピンに大きい剪断力が作用するため好ましくない。その上、特に内視鏡手術の場合には、ジョー部材および他の構成部品の物理的サイズがカニューレを通り得るものでなくてはならないため、ピンのモーメントアームを増大させることは好ましくない。
また、電極間の閉鎖力を増大させることは、他の好ましくない効果をもたらす。例えば、閉鎖力の増大により対向電極が互いに接触して短絡回路を形成する虞れがある。これに対して閉鎖力が小さいと、圧縮中および付勢前に組織の早期移動が生じる虞れがある。

かくして、対向ジョー部材間に大きい閉鎖力を発生させ、付勢中に対向ジョー間に短絡回路を形成する機会を低減させ、かつ付勢前および付勢前に組織を操作し、グリップしかつ保持することを補助する器具を提供することにより、血管組織を有効にシールしかつ上記問題を解決できるバイポーラ鉗子を開発することが要望されている。
（発明の開示）
本発明は、切開手術または内視鏡手術に使用して、組織のクランピングおよびシーリングを行なうバイポーラ鉗子に関する。本発明の鉗子は、その遠位端に対向ジョー部材を備えた少なくとも１つの細長シャフトを有している。両ジョー部材は、該両ジョー部材が互いに間隔を隔てて配置される第一位置から、両ジョー部材が協働してこれらの間に組織を掴む第二位置まで相対移動できる。鉗子は電気エネルギ源に接続され、該電気エネルギ源は各ジョー部材に接続されて、両ジョー部材がこれらの間に保持された組織を通してエネルギを導くことができるようになっている。両ジョー部材間に組織が保持されたときに両ジョー部材間のギャップ距離を制御すべく、少なくとも２つの互いに間隔を隔てた非導電性ストップ部材が、両ジョー部材の内向き面上に配置されている。
一実施形態では、ストップ部材は、ジョー部材の近位端から遠位端まで内向き面に沿って延びている一連の長手方向突出部を有している。他の実施形態では、ストップ部材は、内向き面から突出しかつジョー部材の近位端から遠位端まで延びている一連の円形状タブを有している。各ストップ部材は、ジョー部材の幅に沿って中心に配置されるか、ジョー部材の表面の長さ方向に沿って横方向に互い違いの態様で配置される。
本発明の他の実施形態では、ジョー部材の内向き面から突出しかつジョー部材の外周部に沿って延びている隆起リップが、両ジョー部材間隔を隔てたのギャップ距離を制御するストップ部材として機能する。他の実施形態では、長手方向の隆起部がジョー部材の近位端から遠位端まで延びており、両ジョー部材間のギャップ距離を制御する。
ストップ部材は、スタンピング、溶射、オーバーモールディングおよび／または接着によりジョー部材（単一または複数）に形成されるか、取付けられている。好ましくは、ストップ部材は、少なくとも１つのジョー部材の内向き面から約０．００１〜０．００５インチ、好ましくは約０．００２〜０．００３インチ突出している。ストップ部材は、パリレン、ナイロンおよび／またはセラミック等の絶縁材料で作ることを考えることができる。
本発明の他の実施形態は、第一ジョー部材が第一電位を有しかつ第二ジョー部材が第二電位を有するように両ジョー部材を電気エネルギ源に接続する駆動ロッド組立体を有するバイポーラ鉗子に関する。駆動ロッド組立体には、第一および第二ジョー部材の運動を、第一および第二位置から相互に伝達するハンドルに取り付けられている。ジョー部材（単一または複数）の内向き面には、閉じたときの両ジョー部材間の全ギャップ距離を調整するための、互いに間隔を隔てた少なくとも２のストップ部材が設けられている。
本発明の他の実施形態は、１対の細長シャフトを有し、該シャフトの遠位端にはジョー部材が設けられ、シャフトの近位端にはフィンガリングが設けられているバイポーラ鉗子に関する。フィンガリングを移動させると、両ジョー部材を、両ジョー部材が互いに間隔を隔てた関係に配置される第一位置から両ジョー部材が協働してこれらの間で組織を掴む第二位置まで相対移動させる運動が伝達される。第一シャフトは電気エネルギ源に接続されていて、第一ジョー部材に第一電位を供給し、第二シャフトは第二ジョー部材に第二電位を供給して、両ジョー部材がこれらの間に保持された組織にエネルギを通すことができるように構成されている。ジョー部材（単一または複数）の内向き面上には、両ジョー部材間のギャップ距離を制御するための少なくとも２つの互いに間隔を隔てたストップ部材が設けられている。

以下、添付図面を参照して、本発明の器具の種々の実施形態を説明する。
ここで図１〜図３を参照すると、内視鏡手術に使用するバイポーラ鉗子１０が示されており、該鉗子１０は、ハンドル組立体１８に連結される駆動ロッド組立体１１を有している。駆動ロッド組立体１１は、近位端１６および遠位端１４を備えた細長い中空シャフト部分１２を有している。添付図面および以下の説明において、伝統的に使用されているように、用語「近位」とは、使用者に近い方のバイポーラ鉗子１０の端部をいい、用語「遠位」とは、使用者から遠い方の端部をいう。また、大部分の図面すなわち図１〜図１４には内視鏡手術に使用する本発明の器具の一実施形態例えば鉗子１０が示されているが、ここに図示しかつ説明するものと同じ発明概念を、例えば図１５Ａおよび図１５Ｂに示す実施形態のような切開手術器具（open surgical instrument）１００に使用するか、該器具１００に組み込むことができる。
シャフト１２の遠位端１４にはエンド・エフェクタ組立体２２が取り付けられており、該組立体２２は対向する１対のジョー部材８０、８２を有している。好ましくは、ハンドル組立体１８はシャフト１２の近位端１６に取り付けられかつアクチベータ２０を有している。アクチベータ２０は、両ジョー部材８０、８２を、これらのジョー部材が互いに間隔を隔てて配置される開位置から、両ジョー部材がこれらの間に組織１５０（図１２）を掴むべく協働するクランプ位置すなわち閉位置へと移動させる。
図３に最も良く示すように、アクチベータ２０は、オペレータの少なくとも１本の指を受け入れるための貫通孔３４を備えた可動ハンドル２６と、オペレータの親指を受け入れるための貫通孔３２を備えた固定ハンドル２８とを有している。可動ハンドル２６は、固定ハンドル２８に対して、第一位置から、固定ハンドル２８に接近した、両ジョー部材８０、８２を閉じる第二位置まで選択的に移動できる。好ましくは、固定ハンドル２８は、可動ハンドル２６に連結されたラチェット３０を受け入れるための、近位側に延びたチャネル２７を有している。ラチェット３０は、使用者が、両ジョー部材８０、８２を、開位置から閉位置まで選択的に、漸進的にかつ増分的に移動させることを可能にする。理解されようが、ラチェット３０はまた、使用者が、付勢前および／または付勢中に、可動ハンドル２６従って両ジョー部材８０、８２を、互いに増分位置にロック係合させることを可能にする。或る場合には、固定ハンドル２８および両ジョー部材８０、８２に対する可動ハンドル２６の運動を制御および／または制限するための他の機構、例えば流体圧力システム、半流体圧力システムおよび／またはギヤシステムを設けることが好ましい。
固定ハンドル２８は、細長シャフト１２の長手方向軸線「Ａ」（図９および図１０）の回りでのエンド・エフェクタ組立体２２の回転運動を制御するための回転組立体２３を有している。好ましくは、回転組立体２３は上下のノブ組立体２４ａ、２４ｂを有し、該ノブ組立体はシャフト１２に取り付けられたギヤ６２を包囲するように互いに機械的にインターフェースしている。好ましくは、エンド・エフェクタ組立体２２に対する回転組立体２３の回転比は１：１であるが、特定目的に基いて回転比を増大または減少させるため、異なるギヤ構造例えばウォームギヤ、ギヤトレーン等を組み込むことも考えられる。
好ましくは、１対のハンドルセクション２８ａ、２８ｂが複数の機械的インターフェースにより互いに係合して固定ハンドル２８を形成するように構成する。機械的インターフェースとして、ハンドルセクション２８ｂに形成されかつハンドルセクション２８ａに取り付けられた複数の補完デテント（図示せず）を受け入れる寸法を有するソケット１３８がある。本願では用語「ソケット」を使用したが、雄型または雌型のいずれかを一方のハンドルセクション（例えばハンドルセクション２８ａ）に配置し、相手の機械的インターフェースを反対側のハンドルセクション（例えば配置ンセクション２８ｂ）に配置することができる。
図３に最も良く示すように、各ハンドルセクション２８ａ、２８ｂはほぼ中空であり、その中には、鉗子１０を形成する内部作動部品を収容するためのキャビティ５０が形成されている。例えば、キャビティ５０は、電気外科エネルギ発生器（図示せず）から伝送された電気外科エネルギを各ジョー部材８０、８２に伝送するＰＣボード５８を収容する。プラグ６２が電気外科エネルギ発生器に連結され、ハンドル組立体２８の近位端に設けられたワイヤポート２９を通って鉗子１０に供給される電気外科エネルギを、ケーブル６０を介してＰＣボード５８に伝送する。
好ましくは、両ジョー部材８０、８２の間で組織をシールする所定力または最大力を維持するためのロストモーション機構を、各ハンドルセクション２８ａ、２８ｂの間に配置する。図３に示す特定実施形態では、ロストモーション機構は、ピン４２によりハンドルセクション２８ａ、２８ｂの間で連結された弾性アーム４０を有している。より詳しくは、アーム４０は、下端部４６、上端部４５、およびこれらの間に位置するシャフト部分４７を有している。好ましくは、上端部４５は２股に分れており、それぞれ上方に延びたフランジ４９ａ、４９ｂを備えたクレビスを形成している。下端部４６は、可動ハンドル部分２６に設けられたステップ状インターフェース４８と係合できる寸法を有している。シャフト部分４７は、可動ハンドル２６内に形成された細長チャネル５６内に収容されるようにして、可動ハンドル２６の上端部の方向を向いて配置されたピボットスロット５５内に座合される。好ましくは、アーム４０を可動ハンドル２６内に固定するため、例えばスナップ嵌合形式等の慣用手段によりカバープレート３１が可動ハンドル２６に取り付けられる。
図４を参照すると、ロッド組立体１１は駆動ロッド７０を有し、該駆動ロッド７０は近位端７１および遠位端７２を備えている。駆動ロッド７０の近位端７１にはボールコンタクト３８が取り付けられ、該ボールコンタクト３８は、全体として丸い形状のヘッド部分３９と、該ヘッド部分３９とボールコンタクト３８の近位端との間に位置するノッチ４１とを有している。好ましくは、アーム４０のクレビスフランジ４９ａ、４９ｂは、アーム４０がハンドルセクション２８ａ、２８ｂの間で組立てられるときにヘッド３９を受け入れる寸法を有している（図６参照）。ハンドル２６を固定ハンドル２８の方向に移動させると、ピボットスロット５５でのアーム４０の上端部４５に枢動が生じ（図５Ａ参照）、この枢動により、ボールコンタクト３８に、該ボールコンタクト３８がエンド・エフェクタ組立体２２から遠くに位置する第一位置から、ボールコンタクト３８がエンド・エフェクタ組立体２２に近接して位置する第二位置まで移動する運動が伝達される（図５Ｂ参照）。以下に詳述するように、ボールコンタクト３８が第一位置と第二位置との間で移動されると、駆動ロッド７０に直線運動が伝達され、これにより両ジョー部材８０、８２が互いに近付く方向および離れる方向に移動される。
上記説明から理解されようが、全体として丸い形状のヘッド３９が両クレビスフランジ４９ａ、４９ｂの間に座合しているため、使用者が、ボールコンタクト３８の直線運動を妨げることなく回転組立体２３を有効に使用することが可能になる。
図４の分解図から最も良く理解されるように、エンド・エフェクタ組立体２２は、第一ジョー８０と、第二ジョー８２と、これらの間に配置される電気的な絶縁ヨーク８４とを有している。好ましくは、前述のように、ジョー部材８０、８２は、ハンドル組立体１８を移動させることにより、開位置から閉位置へと移動できる。また、ジョー部材８０、８２のいずれか一方または両方を、上記と同じまたは同様な態様で互いに移動させることを考えることもできる。第一ジョー部材８０は、該ジョー部材から延びている第一フランジ８１と、該第一フランジを貫通するカムスロット８６とを有している。同様に、第二ジョー８２も、該ジョー部材から延びている第二フランジ８１と、該第二フランジを貫通するカムスロット８８とを有している。好ましくは、各ジョー８０、８２は、ステンレス鋼または他の何らかの導電性材料で形成される。
エンド・エフェクタ組立体２２はまた、ジョー部材８２およびジョー部材８０とそれぞれ係合する外側ノーズ部分９４および内側ノーズ部分９６を有している。外側ノーズ部分９４には第一ピボット１０５が設けられ、該第一ピボット１０５は、フランジ８３に設けられた対応ピボット孔８９と係合する寸法を有している。内側ノーズ部分９６には第二ピボット１０３が設けられ、該第二ピボット１０３は、フランジ８１に設けられた対応ピボット孔８７と係合する寸法を有している。第一ジョー部材８０の回転中心は第一ピボット孔８７であり、第二ジョー部材８２の回転中心は第二ピボット孔８９である。好ましくは、各ノーズ部分９４、９６は導電性材料から作られ、かつ以下に詳述するようにそれぞれのジョー部材８２、８０に電気外科エネルギを伝送する。
図３に関連して上述したように、電気外科エネルギは電気外科発生器からＰＣボード５８に伝送され、該ＰＣボード５８はエネルギを第一極および第二極に伝送する。ＰＣボード５８には１対のターミナルクリップ６４ａ、６４ｂが接続され、該ターミナルクリップ６４ａ、６４ｂは、交流電位の第一および第二極を、それぞれ、駆動ロッド組立体１１の異なる部分に接続する。すなわち、クリップ６４ａはシャフト１２に接続されて第一極をジョー部材８２に導き、クリップ６４ｂはボールコンタクト３８に接続されて第二極をジョー部材８０に導く。駆動ロッド７０およびシャフト１２の両者は導電性材料で作られているので、駆動ロッド７０とシャフト１２との間には絶縁スリーブ７５が配置されて、鉗子１０が短絡することを防止する。
図４に最も良く示すように、内側ノーズ部分９６は駆動ロッド７０に電気的に接続され、外側ノーズ部分９４はシャフト１２に電気的に接続される。内側ノーズ部分９６および外側ノーズ部分９４は、フランジ８３，８１に沿ってヨーク８４を捕捉する。ヨーク８４は、内側ノーズ部分９６と外側ノーズ部分９４との間の空間内で、軸線「Ａ」（図７および図８）に沿って軸線方向に移動し、スペーサステーキ１１９は、ノーズ部分９６、９４の遠位端を分離した状態に維持する。ステーキ１１９は内側ノーズ部分９６および外側ノーズ部分９４と係合してこれらをロックし、これにより、両ジョー部材８０、８２をヨーク８４上にロックする。或る場合には、ステーキ１１９は、該ステーキ１１９がストップ部材として、および／または対向ジョー部材８０、８２間の相互ギャップ距離を制御する付加ストップ部材として機能する寸法に定めるのが好ましい。この場合には、ステーキ１１９は、プラスチックのような電気的絶縁材料で形成される。ノーズ部分９４、９６は両フランジ８１、８３の側方支持体を形成し、かつデテント９０、９２がそれぞれカムスロット８６、８８内に確実に留まることを補助する。
エンド・エフェクタ組立体２２はまた、第一極と第二極との電気的絶縁を維持する内側絶縁体１０２および外側絶縁体１００を有している。外側絶縁体１００は、外側ノーズ部分９４を、内側ノーズ部分９６および電気エネルギの第二極を導く駆動ロッド７０から絶縁する。内側絶縁体１０２は、内側ノーズ部分９６を、外側ノーズ部分９４および電気エネルギの第一極を導くシャフト１２から絶縁する。この態様により、外側ノーズ部分９４は、シャフト１２とジョー部材８２との電気的連続性を与え、一方、内側ノーズ部分９６は駆動ロッド７０とジョー部材８０との電気的連続性を与える。
好ましくは、駆動ロッド７０の軸線方向移動中の駆動ロッド７０と内側ノーズ部分９６との電気的接続を維持するため、ばね接点を使用する。シールとして、ドーナツ型スペーサ１０８を用いることもできる。また、スリーブ７５は駆動ロッド７０とシャフト１２との間の絶縁体として機能し、かつ駆動濾過７０の移動中の鉗子１０の不意の短絡回路形成防止を考えることもできる。
上記のようにかつ図４に最も良く示すように、駆動ロッド組立体１１はギヤ５２を有し、該ギヤ５２はシャフト１２に取り付けられかつ軸線「Ａ」の回りでのエンド・エフェクタ組立体２２の回転運動を容易にするように設計されている。より詳しくは、ギヤ５２は上方部分５２ａおよび下方部分５２ｂを有し、各部分５２ａ、５２ｂは、それぞれ、外方に延びている１対の機械的インターフェース５４ａ、５４ｂを有している。これらのインターフェース５４ａ、５４ｂは、シャフト１２に設けられた対応対をなす機械的インターフェース３５に対して解放可能に係合する寸法を有している。好ましくは、ギヤ５２は例えばプラスチックのような電気的絶縁材料で作られ、電気外科エネルギが回転組立体２３に伝送されることを防止する。図５Ａに示すように、回転組立体２３は２つの半部２４ａ、２４ｂからなり、各半部は、ギヤ５２と係合できるようにそれぞれの半部から外方に延びているフランジ７７ａ、７７ｂを有している。回転組立体２３の回転によりシャフト１２が回転され、これにより更にエンド・エフェクタ組立体２２が軸線「Ａ」（図９および図１０参照）の回りで回転する。
再び図４に戻ると、ヨーク８４はプラスチックのような電気的絶縁材料で形成するのが好ましい。ヨーク８４の第一側面９１は第一フランジ８１に対面し、ヨーク８４の第二側面９３は第二フランジ８３に対面する。ヨーク８４が両フランジ８１、８３間に配置されると、ヨーク８４は第一ジョー部材８０を第二ジョー部材８２から電気的に絶縁しかつ隔絶する。このようにして、バイポーラ電気外科電流を、フランジ８１、８３の短絡回路を形成することなく、両ジョー８０、８２間に掴まれた組織１５０を通して流すことができる。
ヨーク８４はまた、カムスロット８６に対して可動係合できる寸法を有する、第一側面９１に設けられた第一デテント９０と、カムスロット８８と係合できる寸法を有する、第２側面９３に設けられた第二デテント９２とを有している。好ましくは、デテントとカムスロットとの組合せ９０、８６；９２、８８は、それぞれ協働して、カム／フォロワ機械的リンク機構として作動する。軸線「Ａ」に沿う駆動ロッド７０の直線運動によりヨーク８４が移動されると、デテント９０、９２がそれぞれのカムスロット８６、８８内で摺動される。一実施形態では、スロット８６、８８が両ジョー８０、８２の遠位端に対して傾斜しており、これにより、両ジョー８０、８２は互いに近付く方向および離れる方向に、ほぼ弧状態様をなして移動する。
他の実施形態では、カムスロット８６、８８の内周面は２つの傾斜を有する形状をなしており、これにより、駆動ロッド７０が完全に伸長されたときに、ジョー部材８０、８２を、互いに別々の異なる２つの態様で移動させる。例えば、カムスロット８６、８８は、両ジョー部材８０、８２を互いにほぼ弧状に移動させる第一段階すなわち近位段階と、両ジョー部材８０、８２が互いにより直線的態様で移動する第二段階すなわち遠位段階とを有する。カムスロット８６、８８は、特定目的に基いて両ジョー部材８０、８２に他の運動、例えばパラボラ運動、サイクロイド運動および／または正弦運動を引起すように寸法を定めることも考えられる。
図７および図８から最も良く理解されようが、デテント９０、９２は、それぞれ、ピボット１０３、１０５の回りのモーメントを生じさせるカムスロット８６、８８の対応内周面に対する力を付与する。好ましくは、カムスロット８６、８８は、駆動ロッド７０の遠位側への移動によりジョー部材８０、８２を一緒に移動させるように配置される。両ジョー部材８０、８２が一緒に閉じられたならば、両ジョー８０、８２が、ハンドル部材２６によりロッド７０に加えられる連続押圧力によりクランプ位置に保持されるようにすることも考えられる。上記のように、ハンドル組立体１８には、両ジョー８０、８２間で組織１５０をシールする所定のすなわち最大クランプ力を維持するためのロストモーション機構を設けることができる。
本発明の１つの長所は、通常デテント９０、９２に付随して生じる過度のクランプ力が、鉗子１０の機械的不全を防止するヨーク８４のユニークな形状により除去されることである。より詳しくは、カムスロット８６、８８は、該カムスロット８６、８８内のデテント９０、９２のカム／フォロワ運動が単に両ジョー部材８０、８２間の組織１５０をクランプするように作動しかつそれぞれデテント９０、９２とピボット１０３、１０５との間に小さいモーメントアームが形成されるように寸法を定めるのが好ましい。ヨーク８４に配置された１対のショルダ１１１、１１３は、デテント９０、９２がそれぞれカムスロット８６、８８内でそれらの最も遠位側の位置に到達する前に、フランジ８１、８３と係合して、ハンドル組立体１８により加えられるあらゆる付加クランプ力を除去するように寸法が定められる。
或る場合には、カムスロット８６、８８がそれぞれ拡大遠位端すなわち袋路（cul-de-sac）７８ａ、７８ｂを有し、これによりスロット８６、８８内の最も遠位側の位置でのデテント９０、９２のカム／フォロワ運動が袋路７８ａ、７８ｂ内に休止し、フランジ８１、８３に当接するショルダ１１１、１１３により閉鎖力が除去されるようになる。カムスロット８６、８８内に配置された袋路７８ａ、７８ｂは、ヨーク８４のショルダ部分１１１、１１３がフランジ８１、８３と係合して両ジョー部材８０、８２間に閉鎖力を付与するのとほぼ同時に、デテント９０、９２に作用する剪断応力を緩和するものと考えられる。
ショルダ１１１、１１３はフランジ８１、８３の近位端に当接して両ジョー部材８０、８２を大きい閉鎖力で一緒に閉じる。換言すると、ショルダ部分１１１、１１３はピボット１０３、１０５回りの比較的大きいモーメントを付与し、両ジョー部材８０、８２間に大きい閉鎖力を付与する。大きいクランプ力を除去するショルダ１１１、１１３と共に、カム／フォロワリンク機構のユニークな形状は、デテント９０、９２が機械的不全により破壊されることを防止する。ピボット１０３、１０５は好ましくは金属で作られ、比較的高い剪断力に耐えることができるので、ヨーク８４およびその構成部品は、組織をシールするのに必要な高いクランプ力による機械的不全の危険なくして、プラスチックのような絶縁材料から形成できる。上記のように、ヨーク８４を絶縁材料から形成することにより、ジョー部材８０、８２の短絡を防止することもできる。
上記のように、２つの機械的ファクタが、シールされる組織の得られる厚さおよびシールの有効性、すなわち、シーリング過程で対向両ジョー部材８０、８２間に加えられる圧力および対向ジョー部材間のギャップを決定する上で重要な役割を演じる。しかしながら、得られる組織シールの厚さは、力だけでは充分に制御できない。換言すれば、力が大き過ぎると、両ジョー部材８０、８２が接触して短絡が生じることがあり、このためエネルギは殆ど組織を通ることがなく、従って劣悪なシールになってしまう。また、力が小さ過ぎると、シールが厚くなり過ぎてしまう。
適正な力を加えることは、他の理由、すなわち脈管の壁を対向させること、組織のインピーダンスを、組織を通って充分な電流が流れることを可能にする充分に小さい値に低下させること、および良好なシールであることを示す必要端組織厚さの形成への寄与に加え、組織加熱中の膨張力に打ち勝つことからも重要である。
鋭い縁部での電流集中を防止しかつ突出尖点間のアーキングを防止するため、両ジョー部材８０、８２のシール表面すなわち組織接触表面１５１、２５１（図１５Ｂおよび図１６Ａ〜図１６Ｇ参照）は比較的平坦にするのが好ましい。また、ジョー部材８０、８２は、組織１５０と係合したときに組織から受ける反力により曲げられることに耐え得るように製造するのが好ましい。例えば、図２および図１６Ａ〜１６Ｇに最も良く示すように、ジョー部材８０、８２は幅「Ｗ」に沿ってテーパ状であるのが好ましい。これは、次の２つの理由、すなわち１）テーパ形状は平行で一定な組織厚さに一定の圧力を加えることができ、２）両ジョー部材８０、８２の厚い近位側部分が組織１５０の反力による曲げに耐えることができる点で有利なことによる。
図４に最も良く示すように、所望のギャップ範囲（例えば、約０．００１〜０．００５インチ、好ましくは約０．９９２〜０．００２インチ）を達成しかつ組織をシールするための所望の力を加えるため、少なくとも１つのジョー部材８０および／または８２は、２つの対向ジョー部材８０、８２の相対運動を制限するストップ部材１３９を有している。ストップ部材１３９は、シーリング表面すなわち組織接触表面１５１から、シーリング中に正確な一定のギャップ距離が形成されるようにするための特定の材料特性（例えば圧縮強度、熱膨張等）に従って定められる所定距離だけ延びるのが好ましい。
前述のように、或る場合には、ステーキ１１９がストップ部材および／または付加ストップ部材のように機能しかつ対向する両ジョー部材８０、８２の相対運動を制御／制限するように、ステーキ１１９の寸法を定めるのが好ましい。ストップ部材１３９および／またはステーキ１１９は、例えばパリレン、ナイロンおよび／またはセラミック等の絶縁材料から作られ、かつ両ジョー部材８０、８２の相対移動を上記ギャップ範囲内に制限する寸法を有することが好ましい。
図１１には使用中の本発明による内視鏡バイポーラ鉗子１０が示されており、ここでは、図１２および図１３に示すように、ハンドル組立体を移動させて管状組織１５０にクランプ力を加え、シール５２を形成する。より詳しくは、シャフト１２およびエンド・エフェクタ２２がトロカール１３０およびカニューレ１３２に挿通され、ハンドル２６を徐々に固定ハンドル２８に向けて移動して両ジョー部材８０、８２の間に管状脈管１５０を掴ませる。組織１５０の回りでジョー部材８０、８２が閉じられた後に、使用者は組織に電気外科エネルギを加える。組織１５０に加えられる電気外科エネルギの強度、周波数および時間を制御することにより、使用者は、出血を焼灼し、凝固／乾燥させ、シールしおよび／または単に低減または遅延させることができる。図１３および図１４に示すように、ひとたび管状脈管がシールされたならば、脈管１５０は、組織１５０を分離しかつこれらの間にギャップ１５４を形成するためシール１５２に沿って切断できる。
図１５Ａおよび図１５Ｂには、本発明による脈管切開バイポーラ手術鉗子（open vessel bipolar surgical forceps）が示されている。理解されようが、鉗子２００は、細長い対向シャフト２１２ａ、２１２ｂのそれぞれのそれぞれの遠位端２１４ａ、２１４ｂに取り付けられたエンド・エフェクタ組立体２２２を有している。エンド・エフェクタ組立体２２２は、遠位端２１４ａ、２１４ｂにそれぞれ配置されたジョー部材２８０、２８２を有し、これらのジョー部材は互いに対向関係をなしておりかつピボット２１９の回りで枢動する。好ましくは、各シャフト２１２ａ、２１２ｂの近位端にはそれぞれ対応するフィンガリング２３２ａ、２３２ｂが取り付けられており、これらのフィンガリングを移動させると、両ジョー部材２８０、２８２が、開位置（両ジョー部材が互いに間隔を隔てて配置される位置）から、クランプ位置すなわち閉位置（両ジョー部材が協働してこれらの間に組織１５０（図１２参照）を掴む位置）まで移動される。
図１５Ｂは、ジョー部材２８０、２８２の一実施形態を示す拡大図であり、この実施形態は、ジョー部材２８２の近位端２４３から遠位端２４５までジョー部材２８２の周縁部に沿って延びている隆起リップとして設計されたストップ部材２３９を有している。鉗子２００には更に、図１６Ａ〜図１６Ｇに関連して後述するいずれかの形状のストップ部材２３９を設けることもできる。
図１６Ａ〜図１６Ｇには、下方のジョー部材８２（２８２）上に、該ジョー部材に沿ってまたは該ジョー部材から突出して配置される非導電性材料からなるストップ部材１３９の種々の実施形態が示されている。特定目的に基いて所望の結果が得られるようにするため、１つ以上のストップ部材１３９をジョー部材８０、８２（２８０、２８２）の両方またはいずれか一方に設けることができる。本発明の開示から理解されようが、ストップ部材１３９の種々の形状は、付勢前または付勢中に組織１５０の移動を制限しかつ組織１５０が押圧されたときにジョー部材８０、８２（２８０、２８２）に短絡回路が形成されないように設計される。
図１６Ａには、ジョー部材８２の近位端１４３から遠位端１４５まで延びている長手方向隆起として形成されたストップ部材１３９が示されている。図１６Ｂには、ジョー部材８２の近位端１４３から遠位端１４５まで延びている長手方向デテントとして形成された一連のストップ部材１３９が示されている。図１６Ｃには、互いに横方向に互い違いの態様でジョー部材８２の近位端１４３から遠位端１４５まで延びている一連のサークル状ストップ部材１３９が示されている。実質的にサイズが等しい円形のストップ部材１３９を、それぞれジョー部材８２の右側および左側縁部１４７、１４９の近くに配置することを考えることもできる。しかしながら、特定目的に基いて所望の結果を得るため、１つ以上のストップ部材１３９を他のストップ部材とは異なる寸法または形状にすることができる。
図１６Ｄには、ジョー部材８２の近位端１４３から遠位端１４５まで延びている一連の円形状ストップ部材１３９であって、各々がジョー部材８２の幅「Ｗ」に沿って中心に配置されたストップ部材１３９が示されている。図１６Ｅには、ジョー部材８２の外周部から突出する隆起リップとしてとして設計された他の形態のストップ部材１３９が示されている。
図１６Ｆには、カタマラン（catamaran）状の態様でジョー部材８２の側縁部１４７、１４９から延びているＬ型のストップ部材１３９ｂを有する更に別の実施形態が示されている。好ましくは、ジョー部材８０に補完ストッパ１３９ａを配置して、両ジョーが閉位置に向かって移動されるときに両ストップ部材１３９ａ、１３９ｂが互いに当接するように構成する。ジョー部材をこのような態様で構成することにより、幅「Ｗ」（図１６Ｄ参照）に亘って、対向ジョー部材８０、８２（２８０、２８２）の両シーリング表面１５１、１５１間の全ギャップ距離（約０．００１〜０．００５インチ、好ましくは約０．００２〜０．００３インチ）の横方向一定性および安定性を得ることができる。図１６Ｇには、ストップ部材１３９ａ、１３９ｂがほぼＣ型である更に別の実施形態が示されている。
好ましくは、非導電性ストップ部材（単一または複数）１３９（２３９）は、ジョー部材８０、８２上に成形（例えば、オーバモールディング、射出成形等）し、スタンピング成形し、または堆積（例えばめっき）する。例えば、一技術として、セラミック材料を両ジョー部材８０、８２の表面上に溶射してストップ部材（単一または複数）１３９を形成する方法がある。導電性表面上に広範囲の種類の耐熱絶縁材料を堆積させる幾つかの溶射技術が考えられ、例えば、高速オキシフュエル（High velocity Oxy- fuel）蒸着、プラズマ蒸着等がある。導電性表面上にストップ部材（単一または複数）１３９を堆積させる他の技術として、例えばスライドオン成形、スナップオン成形、接着、金型成形等を考えることができる。
ストップ部材１３９（２３９）をジョー部材８２（２８２）の内向き面から約０．００１〜０．００５インチ突出させることを考えることができ、これにより、発明の開示から明らかなように、電極間の短絡回路の可能性を低下できかつ両ジョー部材８０、８２（２８０、２８２）のグリップ特性を高めることができる。ストップ部材１３９、２３９は、有効で、均一で、かつ一定の組織シールを形成する理想的ギャップ距離を生じさせると決定されている約０．００２〜０．００３インチだけ突出するのが好ましい。
別の構成として、ストップ部材１３９（２３９）はジョー部材８０、８２（２８０、２８２）の一方または両方の内向き面上に成形でき、または或る場合には、ストップ部材１３９（２３９）を任意の既知の接着方法によりジョー部材８０、８２の一方または両方の内向き面に接着するのが好ましい。本願では、スタンピングとは、例えばブランキング、剪断、熱間成形、冷間成形、引抜き、曲げおよびコイニングを含む商業的に知られている事実上全てのプレス作業を包含するが、これらに限定されるものではない。
図１６Ａ〜図１６Ｇは、ストップ部材１３９の幾つかの可能性ある形態を示すが、これらの形態は例示に過ぎず、これらに限定されるものと解釈すべきではない。他の形態を考えることもできる。例えば、図１６Ａ〜図１６Ｇの１つ以上の形態を組み合わせてジョー部材８０、８２（２８０、２８２）の一方または両方の内向き面上に異なるストップ部材１３９（２３９）の形態を形成することができる。図１６Ｃおよび図１６Ｄは、ジョー部材８２（２８２）上またはがジョー部材に沿って異なる形態に配置された円形状ストップ部材１３９（２３９）を示しているが、電極間の短絡回路の形成の可能性を低下させかつ組織グリップを向上させる上で他の形状も等しく有効であると考えられる。
また、ストップ部材１３９（２３９）はジョー部材（単一または複数）の内向き面から約０．００１〜０．００５インチ、好ましくは約０．００２〜０．００３インチ突出するのが好ましいが、或る場合には、特定目的に基いてストップ部材１３９（２３９）を上記範囲より大きくまたは小さく突出させるのが好ましいこともある。例えば、ストップ部材１３９に使用できる材料の種類およびジョー部材間の短絡回路の形成可能性を低下させると同時に両ジョー部材間の大きい押圧閉鎖力を吸収する材料の能力は変えることができ、従って、ストップ部材１３９の全体的寸法を変えて所望のギャップ距離を得ることができる。換言すれば、材料の圧縮強度は、有効なシーリングに要求される望ましい所望ギャップ距離（究極ギャップ距離）とともに、ストップ部材１３９（２３９）を形成する場合に入念に考察すべきパラメータである。
理解されようが、一方の材料の寸法を他方の材料の寸法から異ならせて、同じギャップ距離すなわち所望の結果を達成することができる。例えば、ナイロンの圧縮強度はセラミックの圧縮強度とは異なっており、従って、対向ジョー部材の閉鎖力を妨げかつ同一の所望ギャップ距離を達成するため、寸法を異ならせること、例えば厚くすることができる。
上記説明および種々の図面から、当業者ならば、本発明の範囲から逸脱することなく本発明について或る変更をなし得ることは明らかである。例えば、鉗子１０（２００）に他の特徴、例えば細長シャフト１２（２１２）に対してエンド・エフェクタ組立体２２（２２２）を軸線方向に変位させるための関節組立体を付加することは好ましいことである。
また、本発明のストップ部材の形態は、本件出願人の所有する「使い捨て電極を備えた切開脈管シーリング鉗子（OPEN VESSEL SEALING FORCEPS WITH DISPOSABLE ELECTRODES）」という名称に係るTetziaff等の係属中の１９９９年１０月２２日付米国特許出願第０９／４２５，６９６号、「使い捨て電極を備えた切開脈管シーリング鉗子（OPEN VESSEL SEALING FORCEPS WITH DISPOSABLE ELECTRODES）」という名称に係るTetziaff等の１９９８年１０月２３日付米国特許出願第０９／１７８，０２７号、および「交換可能な電極を備えたバイポーラ電気外科器具（BIPOLAR ELECTROSURGICAL INSTRUMENT WITH REPLACEABLE ELECTRODES）」という名称に係るSchmaltz等の１９９８年９月１日付米国特許出願第０９／３８７，８８３号（これらの全ての米国特許出願の内容は本願に援用する）に開示されている使い捨てまたは一部使い捨て型電気外科器具に組み込むことができる。より詳しくは、本発明の鉗子には、電気外科器具、例えばエンド・エフェクタ、シャフト（単一または複数）および／またはハンドル（単一または複数）と選択的に係合できる使い捨て型電極組立体を設けることを考えることができる。

Claims (9)

  1. 少なくとも１つの細長シャフトを有し、該シャフトの遠位端には対向ジョー部材が設けられ、該ジョー部材は、両ジョー部材が互いに間隔を隔てた関係に配置される第一位置から、両ジョー部材が協働してこれらの間で組織を掴む第二位置まで相対移動でき、
    両ジョー部材がこれらの間に保持された組織にエネルギを通すことができるように各ジョー部材に接続される電気エネルギ源と、
    前記ジョー部材の少なくとも１つに配置された中央の単一の直線的な一連の互いに間隔を隔てたストップ部材と、を更に有し、前記中央の単一の直線的な一連の互いに間隔を隔てたストップ部材は、前記ジョー部材の少なくとも１つの最近位領域および最遠位領域に配置された長手方向に配向されたデテントとして構成されていることを特徴とするバイポーラ鉗子。
  2. 前記ストップ部材の各々は、パリレン、ナイロンおよびセラミックからなる群から製造されていることを特徴とする請求項１記載のバイポーラ鉗子。
  3. 前記ストップ部材の各々は、前記ジョー部材の前記少なくとも１つの頂面から約０．００１〜０．００５インチ突出していることを特徴とする請求項１記載のバイポーラ鉗子。
  4. 前記ストップ部材は、前記ジョー部材の前記少なくとも１つの頂面からから約０．００２〜０．００３インチ突出していることを特徴とする請求項１記載のバイポーラ鉗子。
  5. 前記ストップ部材は、スタンピングにより前記ジョー部材の前記少なくとも１つに形成されていることを特徴とする請求項１記載のバイポーラ鉗子。
  6. 前記ストップ部材は、溶射により前記ジョー部材の前記少なくとも１つに取り付けられていることを特徴とする請求項１記載のバイポーラ鉗子。
  7. 前記ストップ部材は、接着により前記ジョー部材の前記少なくとも１つに取り付けられていることを特徴とする請求項１記載のバイポーラ鉗子。
  8. 前記ストップ部材は、成形により前記ジョー部材の前記少なくとも１つに取り付けられていることを特徴とする請求項１記載のバイポーラ鉗子。
  9. 駆動ロッド組立体を有し、該駆動ロッド組立体は、第一ジョー部材が第一電位を有しかつ第二ジョー部材が第二電位を有するように両ジョー部材を電気エネルギ源に接続し、
    第一および第二位置からの第一および第二ジョー部材の運動を伝達するための、駆動ロッド組立体に取り付けられたハンドルを更に有することを特徴とする請求項１記載のバイポーラ鉗子。
