【発明の詳細な説明】 (イ) 産業上の利用分野 本発明は、病院等の診療機関で使用される医療
事務用計算機に係り、特定の診療行為に付随して
発生する特殊な診療報酬の自動算定方式に関す
る。 (ロ) 従来技術 窓口業務や月次業務としての診療報酬明細書の
発行を行なう医療事務用計算機の概要は、「三洋
技報VOL.11No.1(1979年度版)」に開示されてお
り、このようなシステムでは、一般に、診療行為
はコード化されており、医療事務処理に必要な全
ての診療行為について、各々、診療コード・区分
(投薬・検査等の法定区分)・診療報酬点数又は薬
価・単位・名称で構成される診療行為データがマ
スタフアイルに記憶されている。尚、単位とは薬
剤の場合のmg・ml・g・バイアル・錠等である。
又、各患者に対しては、氏名・生年月日・住所等
の患者固有の頭書データを格納する患者頭書フア
イルと、患者が施された診療行為に対応する診療
行為データを格納する患者診療フアイルが設けら
れており、入力装置から診療コードを入力し、こ
の診療コードに対応する診療行為データをマスタ
フアイルから検索し、該当患者の患者診療フアイ
ルに検索して得た診療行為データを格納するよう
にしている。 ところで、診療行為のうち、食有入院や食無入
院等の特定の診療行為については、入院時医学管
理料という特殊な診療報酬が付随して発生する。
従つて、このような食有入院あるいは食無入院の
入院料の算定時には、食有入院料あるいは食無入
院料そのものの算定と入院時医学管理料との算定
を行なわなければならず、このため、食有入院料
あるいは食無入院料に対する診療コードの入力
と、入院時医学管理料に対する診療コードの入力
とが必要であつた。 然しながら、上記方法では、入院時医学管理料
を人力し忘れることにより、入院時医学管理料の
未算定が起こりやすく、これを防止するため、近
年、その自動算定が行なわれるようになつてき
た。 従来の自動算定方式は、入院時医学管理料が国
民健康保険や社会保険等の一般保険ではその診療
報酬点数が同一であることから、マスタフアイル
には一般保険に対する1種類の入院時医学管理料
を記憶しておき、食有入院料あるいは食無入院料
に対する診療コードの入力時に、この1種類の入
院時医学管理料を自動的に算定するようにしてい
た。ところが、老人保険や労災保険では入院時医
学管理料の診療報酬点数が一般保険とは異なるた
め、このような保険に加入している患者の食用あ
るいは食無入院料を算定する場合には、自動算定
された誤つた入院時医学管理料を入力装置から手
作業で正しい入院時医学管理料に入力し直さなけ
ればならず、操作が非常に面倒であつた。 (ハ) 発明の目的 本発明は、入院時医学管理料のように、特定の
診療行為に付随して発生する特殊な診療報酬の自
動算定を、保険の種別に関係なく、正確に行なえ
るようにすることを目的としたものである。 (ニ) 発明の構成 本発明は、医療事務処理に必要な全ての診療行
為データを記憶したマスタフアイルと、患者毎に
設けられる患者固有の頭書データを格納する患者
頭書フアイル及び各患者の診療行為データを格納
する患者診療フアイルと、入力装置とを備え、前
記入力装置から診療コードを入力し、該診療コー
ドに対応する診療行為データを前記マスタフアイ
ルから検索し該当患者の前記患者診療フアイルに
格納する医療事務用計算機において、特定の診療
行為に付随して発生する特殊診療報酬に対応する
保険毎の特殊診療行為データを、保険の種別を示
す保険識別データと共に前記マスタフアイルに記
憶せしめておくと共に、前記患者頭書フアイルに
患者が加入している保険に対応する前記保険識別
データを格納し、前記特定の診療行為に対する診
療コードの入力時、前記患者頭書フアイルの保険
識別データに基づき対応する保険の前記特殊診療
行為データを前記マスタフアイルから索出し、該
特殊診療行為データを入力された前記特定の診療
コードに対する診療行為データと共に、該当患者
の前記患者診療フアイルに書込むことにより、前
記特殊診療報酬の自動算定を行なうよう構成した
ものである。 (ホ) 実施例 第1図は、本発明の一実施例を説明するための
医療事務用計算機の構成を示すブロツク図であ
り、1は各種データを入力するためのキーボー
ド、2は各種データを表示するためのCRTデイ
スプレイより成る表示装置、3は診療報酬明細書
や集計結果を印字するためのプリンタ装置、4は
マスタフアイルと患者頭書フアイル及び患者
診療フアイルを有する磁気デイスクより成るフ
アイル装置、8はRAM9を備え各種処理を行な
う処理部であり、以下、実施例として、入院時医
学管理料の自動算定について説明する。 本実施例では、先ず、一般保険、老人保険、労
災保険等の各保険に、保険の種別を示す保険識別
データa,b,c等を定め、各保険での入院時医
学管理料に対応する複数の診療行為データHa,
Hb,Hc…を、保険識別データa,b,c…と共
に、第2図aに示すように対応づけて記憶してお
く。又、各患者頭書フアイル61,62には、第
2図bに示すように、患者コードN1,N2及び頭
書データG1,G2を各々格納すると共に、患者が
加入している保険を示す保険識別データb,cを
格納する。具体的には、各患者が新患として来院
した際、患者コードを付し、この患者コードと頭
書きデータ及び保険識別データを、表示装置2の
画面を見ながら、キーボード1から順次入力すれ
ばよい。 次に、処理部8での処理内容について、第3図
のフローチヤートを参照しながら説明する。 今、老人保険に加入している患者コードN1
患者について、当日の診療行為を入力しようとし
ているとする。このため、先ず、キーボード1か
ら患者コードN1を入力すると、処理部8は患者
コードに基づいて対応する患者頭書フアイル61
を検索し、その頭書データG1及び保険識別デー
タbをRAM9上の頭書領域9aに読出し、頭書
データG1を表示装置2の画面上に表示する。そ
こで、オペレータは表示された頭書データを確認
後、当日の診療行為に対する診療コードをキーボ
ード1から順次入力して行くと、処理部8は入力
された診療コードに対応する診療報酬点数や名称
等の診療行為データをマスタフアイルから検索
して、RAM9上の診療領域9bに記憶し、この
入力された診療コードが食用入院料あるいは食無
入院料を示す特定の診療コードであるか判別し、
特定の診療コードでなければ、マスタフアイル
から読出した診療行為データを表示し、次の診療
コードの入力に備える。次に入力された診療コー
ドが特定の診療コードでなければ同様の処理が繰
り返されるが、特定の診療コードである場合に
は、食有入院料あるいは食無入院料それ自体に対
応する診療行為データP1あるいはP2をRAM9の
診療領域9bに記憶した後、RAM9の頭書領域
9aに読出されている保険識別データbを参照

料データHbを、マスタフアイルから索出し、
このデータHbをRAM9の診療領域9bに記憶
