    ードからの患者情報を読み取る手段と、診察券カ
    ードから読み取られた患者情報を２進法コードに
    変換するＯＭＲ装置と、上記診察券カードから読
    み取られた患者情報を文字や数字に変換するＯＣ
    Ｒ装置と、上記ＯＭＲ装置及びＯＲＣ装置で変換
    された患者情報を検査依頼書の所定位置に打ち出
    すプリンターと、患者登録番号を入力する数字キ
    イーと、を有して構成されてなる転写装置。
