足の受動的な関節運動装置
技術範囲
本発明は・整形的な矯正の見地から、足の関節を可動イヒさせるための技術に関
するものである。
当該装置は空間内の3つの互いに垂直な平面において(第1図)・足を組み立て
る整骨関節を動かすこと可能とするものである。
それは、以下の動きである。
1)軸線AA’に沿った足首の屈曲−展長2)軸線BB’に沿った距踵接続部の
内反−外反3)軸線cc’に沿った外転−内転
この装置は、特に、新生児の人足を連続的かつ受動的に可動化させるようにした
ものである。
人足は子宮内における位置異常による足の奇形である。オムブレダーネ(Omb
redanne)博士の分析によれば、人足の奇形は、第2図から第7図におい
て左足で示されているような、空間内に於ける3つの互いに垂直な平面において
生じている。
1)矢印方向(あるいは前後垂直)の平面において・足が人足(EQII I
NUS)で変形し、即ち、くるぶしから下の足の全体が脚に対して足の裏が湾曲
して、正弦鈍角で前方が開いている。(第2図)2)前面側平面からみて、足が
内反(VARLIS)で変形し、即ち、足の裏がボディの中間軸線の方に向いて
いるe(第3図)3)水平面からみて、足が内転(ADDLICTIO!i)
して変形し、即ち、その長さ方向(前後方向)の軸線がボディの中間軸線の方向
に向いている。(第5図)
上記したことから考慮して、ボートに比ゆして言えば、人足は前後方向に傾き、
回転し、かつ内外の向きが変わっている。
従来技術
所謂機械的治療のための装置として、空間内の3つの垂直軸線方向に、足を積極
的に或いは積極的−受動的に作動させるようにしたものが知られている。(所謂
、フラツフ(FRANCO)装置)上記装置は連続して受動的になされる関節運
動の概念は含まれていない。
本発明に係わる装置の創造性は、下記に列挙するような連続的かつ受動的な足の
動きを為さしめることにある。
1)独立した屈折−展伸(第1図の軸線BB’ ”)2)外転−内転を伴う外反
−内反(第1図の軸線BB’ とCC’)3)独立した足の裏側への湾曲−背面
側への湾曲(第1図の軸線4)外反−内反を伴う背面側への湾曲−足の裏側への
湾曲(第1図の軸線AA’ とBB’ )
5)外転−内転を伴う背面側への湾曲−足の裏側への湾曲(第1図の軸線AA’

れば、3つの軸線方向の変形は別個にあるいは同時に動かされる。
本発明の目的は、変形に対する矯正を為すことによって、上記した不具合の正し
L−%整形が為されることが可能な装置を提供することにある。治療の見地から
、上記不具合の正しい整形は下記の動きを為すことにより達成される。
、1)足の前部および後部の口外運動、それに上り内反と内転を生じる。
2)人足:装置は、足を回内運動させる(外反と外転を生じさせる)ことによる
第1の変形の矯正と、足を背面側へ湾曲作動させる(第り図の軸線AA’ )こ
とによる第2の矯正とを可能とする。
上記装置は、一つの変形あるいは、他の変形あるいは、2つの変形を同時に矯正
しえるものである。
発明の説明
上記目的を達成するため、本発明に係わる装置は、ベースの一方側に軸着された
足裏板システムを備え、該足裏板システムは足裏板を有し、該足裏板は湾曲運動
の軸線、外転−内転運動の軸線および内反−外反運動の軸線の直交する軸線上で
、上記足裏板に調節自在な3つの配向方向を与えるための軸着されたサポート3
.4で足裏板を支持する一方、回転タイプの駆動部材と、さらに、軸着された足
裏板システムに上記駆動部材を連結する調整ロッドを備え1;伝動システムを備
えていることを特徴とするものである。
種′々の他の特徴は、本発明が対象とする実施例(当該実施例に限定されない)
を示す添付した図面による詳細な説明から明らかになるものである。

年代的に、人足を軽減する前に、内転している足の前部および足の後部を矯正す
ることから始めることが重要であり、そのため、装置の案内軸線は外転位置に矯
正することにある。
背面側への湾曲と連系させて、或いは独立の運動によって人足の矯正することは
、上記第1の変形の矯正の後に、二次的になされるようにされている。
本発明に係わる装置は、ベース上の外反−内反の軸線BB’ により方向が設定
された第2サポート4上で、背面側への湾曲−足の裏側への湾曲の軸線AA’
によって方向が設定される第1サポート3によって支持され、外転−内転の軸線
CC”上で方向が設定さる足裏板1(第8図および第9図)を備えているアセン
ブリによって構成されていることを特徴としている。
上記装置は、特に、軸着された足裏板システムJと、固定ベースSに固定された
駆動部材■と、レバーあるいは連結アーム等の伝動システムmとを備えている。
上記軸着された足裏板システムIは、デニス・ブラウン(DenisBrovn
e)の副木タイプの公知の足裏板等からなるの適宜な足裏板1、あるいは、足の
圧力を吸収するためにコーティングしている柔軟な細胞状の発泡材を備えたプラ
スチック材料からなる足裏板、あるいは第9図に示すような、足の前部の内転を
矯正する足裏板を備えている。

足裏板ホルダー2はピボット13により軸線BB’ 上を作動可能で、内転方向
あるいは外転方向に30度まで向けることが出来、かつ、軸線AA’を中心とし
て作動されるし形状のストラップ3上でネジ32(第10図)によって固定され
ている。上記ストラップ3は背面側への湾曲−足の裏側への湾曲を発生させ、外
反−内反を生じる軸線CC° を中心とするし形状のストラップ4上に取り付け
られている。
駆動部材■はギヤ・モータ5、軸受8.9に取り付けられたガイド・ロッド6お
よびスクリユ7、可動キャリッジlO1回転計用発電機11、電気機械式若しく
は光学電気式逆転装置12、軸線BB“上に位置された続出電位差計13によっ
て構成され、モータの回転方向を逆転するシステムを備えている。
上記装置の作動は、電気制御とデスク力1らの指示?こよってなされ、デスク上
には受動的な関節運動装置の従来公知の機能(振幅、速変、力など)が表示され
ている。
連結アームあるいはレバーを備える伝動システム■は、第8図、第1O図および
第11図に示すように、U形状のストラップ4に可動キャリッジ10を連結する
主レバー14を備えている。該レノクーは外反を生じ、フォーク・ジヨイントI
5によってストラップ4に軸着および位置決めされており、左足あるいは右足の
作動のいずれが望まれているかに応じて、ストラップ4の一側あるいは他側上に
ねじ或いは指差15aを取り付けている。レバー14はピボット16により可動
キャリッジに固定されている。その動きは調整ネジ17を緩めることにより停止
される。
レバーmはまた背面側への湾曲−足の裏側への湾曲を生じさせるレバー18を備
えている。該レバーは、一方では、調整ネジ20(該ネジを緩めることによって
動きが停止される)を備えたピボット19によって可動キャリッジlOに連結さ
れ、他方では、スライド22上に位置決めされると共にネジ23によって固定さ
れるピボット21によってU形状のストラップ3に連結される。スライド22は
外反−内反との関係において、背面側への湾曲−足の裏側への湾曲の相対的なり
リアランスを調整するために用いられる。
しp<−mは第3のレバー24を備え、該レバー24は可動キャリッジ10上に
位置されると共に、外転−内転のために足裏板を駆動するシステム■を動作する
。
システム■はレバー24によって回転駆動されるピボット40を備え(第8図、
第10図および第1L図)、上記レバー24の一方側は、運動されるのが左足か
右足かに応じてピボット40の一方側のアームあるいは他方側のアーム上に軸着
され(ネジあるいは指差42aを備えたフォーク・ジヨイント42によって固定
される)、他方側には、調整ネジ43a(第1O図)を備えたピボット43によ
って可動キャリッジ10に軸着されている。このネジを緩めることにより、動き
は停止される。
システム■のピボット40はベベル・ギヤ44内に伸長し、該ベベル・ギヤ44
は、フレキシブル・ケーブル45、スライド46、足裏板ホルダーのシャフトと
固定されたユニバーサル・ジヨイント47によって構成される回転駆動用の装置
と伝動している。
フレキシブル・ケーブルは2つのユニバーサル・ジヨイントを備えた強固なアー
ムと置換しえる。
上記装置は下記の如く作動する。
物理的に、ゼロ位置(第1図および第6図)から、装置は段階的に作動する。
l)可動キャリッジによって下降されるレバー14により、ストラップ4は輪線
BB’ を中心として回転し、0°から30°の振幅で外反の足の運動を生じさ
せる。
2)可動キャリッジlOによって制御されるレバー18により、軸線AA’を中
心としてストラップ3が回転し、0°から40°振幅で背面側への湾曲の足の運
動を生じさせる。
3)システム■により軸線CC°を中心として足裏板1を回転し、0°から30
°の振幅での外転の足の運動を生じさせる。
4)レバー18により軸線AA’を中心としてストラップ3を回転すると共に、
レバー14により軸線BB’ を中心としてストラップ4を同時に回転し、外反
と背面側への湾曲の協動的な足の運動を生じさける。
5)システム■による軸線CC゛ を中心とする足裏板の回転と、レバー14に
よる軸線BB’ を中心とするストシッフ40回転で、外反と外転との協動的な
足の運動を生じさせる。
6)システム■によるのと同様に各レバー】4.18による3つの軸線AA’
、BB’ およびCC゛ を中心とするの回転で、外反と背面側への湾曲による
足の協動的な運動を生じさせる。
上記装置は軸線BB’ を中心とする外反と共に、軸線CC′ を中心とする外
転を協動的に為すシステムVも備えることが出来る。該システム■は(第12図
から第14図)軸線CC° を中心としてベース27に固定されたU形状のスト
ラップ26と、ガイド・プーリ31aに取り付けられた弾性ケーブル31の2つ
の右円環部および左円環によって足裏板ホルダー2に連結されたストラップ26
のアーム28.29を備えている。
締付ボルト32は、軸線CC′ を中心として足裏板ホルダー2が回転出来るよ
うにアンロックされ得る。
弾性ケーブル31はトラベラ−33に取り付けられる。外反は軸線BB’ を中
心とするストラップ゛4の回転により生じ、足裏板ホルダー2は軸線CClを中
心として回転する@ロッド34上の搬送器33の位置調整により、最大30°の
クリアランスで外転が、また、30°のクリアランスで外反が得られる。
その後、ネジ32は第13図に示すように、溝35の一端に当接する。

上記した装置の構造は、足裏板システムIが軸着ピボット103の貫通軸に結合
された懸架片によって支持されているため、特にコンパクトで耐久性のある軸着
された足裏板システムが利用可能となる。足裏板lは、その後、軸線CC“ と
連結が失われ、足裏板ホルダー2へ適合される足裏板は、その後、作動される対
象の形状的な形態に装置を適合する必要がある時に、非常に迅速に交換される。

ロッドを用いることにより、実際、足裏板の角度振幅をいずれであっても、回転
により変化しない。特に、作用はロッドと搬送器の間の連結ビン123から12

これに対して、トラベラ−が軸線DD’と同一線上の中立点から可能な限りの調

上記追従するトラベラ−114から116の各位置を、上記2つの位置の間で正
確に変位されるようにする調整の正確さ、および機構108に対する接近やすさ
の理由より、上記した振幅の調整は容器かつ正確に為される
有効な長さの全体に延在するネジ目を備えたネジロッド110を用いることは有
益なことであるが、ネジロッド111,112にほの全長の半分にネジ目が設け
られている。このように、上記説明および図面に示すように、同一の装置におい
て、軸線DD’の両側のネジ110の有効領域の一例あるいは他側でトラベラ−
114の変位および、モータ105の回転方向の逆転によって、簡単に左足用あ
るいは右足用として使用することが出来、そのため、右足の外反および外転の前
記した正確な移動は、左足の内反および内転の移動となる。
上記した構造の他の利点は、転位機構10Bにより制御されたロッドのアセンブ
リを介して為される移動の伝達の単純化にある。
上記し1こ手段は、結果的に、運動装置が患者の形態に対すると共に、患者に対
して為される処置に対して、運動装置の適用を容品とするものである。
本装置の特徴的な構造より、内転中足症の変形を矯正するための見地から、付加
的に足の前部の受動的な運動が出来る可能性がある。
上記目的のために、第19図に示すように、軸着された足裏板アセンブリ1はベ
ース・ストラップ3に適宜な手段によって固定された後部部分130により構成
された足裏板lを備えている。後部部分130は外方の横方向端部で位相をずら
して軸着ビン132を介して前部部分13】を支持しており、上記軸着ビン13
2は、どんな場合においても、軸線CC゛上を中心とされるピボット103と同
心に位置するようにしている。第20図に示すように、後部部分130は後部の
内側部に突出したエツジ】33を備える一方、前部部分131は前部の内側部に
突出したエツジ134および外部の中間に突出したエツジ135を備えている。

時における制御の中立化を図るために、転位機構108を調整して、プーリある
いはホイール1070回転して、軸線CC゛上で懸架片102を回転する。懸架
