Description

【発明の詳細な説明】 本発明は腔内吻合用外科用ステープリング装置に関する
ものである。特に、１回使用、使い捨て形に簡単に製造
することができ、かつ、作動ギャップ設定／表示手段と
作動ギャップの範囲全体にわたってステープルを適正に
彎曲するように、また吻合された筒状身体器官から容易
に除去するように形成されたアンビルとを備えた外科用
ステーブリンク装置に関する。
過去１０年間において、食道から直腸に至るまで多くの
内臓及び腸の手術において通常の縫合力の代わりに外科
用ステープルを使用する例が増大している。米国特許第
３，１９３，１６５号、第３．３３８，８４７号、第３
，５５２，６２６号、第３，５９３，９０３号及び第３
，６３８，６５２号は同時出願の特願第８９０，２６２
号と共に、この種の手術について従来専門家によって考
案された外科用ステーブリング装置の代表例を記載して
いる。外科用ステープルとこの種のステーブリング装置
の使用は多くの困難な手術をはるかに簡単にした。しか
し、さらに重要なことは、ステープルの使用がこの種の
手術の所要時間を大巾に短縮させ、患者が麻酔状態に保
持されなければならない時間を短縮させた事実である。
米国特許第３，１９３，１６５号と第３，３８８．８４
７号は、ステープルを単一のリングカートリッジの中の
手で装入する必要のある永久型、再使用可能装置の例を
示している。この方の装置は二、三の欠点をもっている
。一般に、これらの装置は構造が複雑、製造費が高く、
かつ、大型で重い。これらの装置は絶えず保守されなけ
ればならず、使用後に分解し洗浄しなければならず、ま
た手術のために滅菌しなければならない。また、各手術
のために、ステープルを１つづつ手でカートリッジの中
に装入しなければならない。ステーブリング装置に装着
したメスを定期的に研摩しなければならず、また各部品
の組立、分解、保守及び取替のために特殊の工具を必要
とする場合が多い。人間の消化器系を構成する管状構造
、即ち、食道、小腸及び大腸は同一患者についても相異
なる径と壁圧を有することは周知である。このような消
化器系の筒状構造の中に円形吻合部を配置するには、前
記した型の装置が吻合される器官の内部スペースの中ま
で導入されなければならない。
この装置のステープル収容部の径は、ステーブリングさ
れる器官の内径に対応しなければならない。
前記した米国特許第３，５５２．６２６号は、相異なる
径の交換可能ステープル担持ヘッドを記載している。し
かし、この種の装置は前記のような欠点があるだけでな
く、その取替え可能ヘッドの設置と除去が困難である。
この種のヘッドを装置に対して取付けるために必要とさ
れる取付部材が大型であるので、このヘッドの最小サイ
ズには限界があるからである。さらに、その除去に際し
て腸の内壁面を損傷する可能性のあるローレット付きノ
ーズナツトを備えている。前記の同時出願特許の装置は
完全に使い捨て型とすることができ、あるいは使い捨て
ステープルカートリッジを備えた永久型装置とすること
もできる。しかし、永久型装置としては前述のような欠
点をまぬがれない。
ステープル及びステーブリング装置の使用に伴う利点と
共に、二、三の個有の欠点がある。第１に、吻合される
組織厚さに対して適正なステープルサイズを選定する必
要がある。第２に、適正なステープルサイズが選定され
たとしても、ステープルはステープルの形成がきつ過ぎ
たり、また、ゆる過ぎたりしてはならない。もし、ステ
ープルが過度にきつく形成されると、治癒過程に必要と
される血液の循環が遮断され、組織が壊死する。
一方ステープルの形成がゆる過ぎれば、出血及び漏洩が
生じる場合がある。故にきつ過ぎるステープルも、また
、ゆる過ぎるステープルも重大な問題と合併症を生じる
可能性がある。
本発明は、極めて簡単な設計の腔内吻合用ステーブリン
グ装置に関するものである。この装置は永久型、再使用
可能装置の形に製造することができる。これは１回使用
、使い捨て型ステーブリング装置として製造するのに特
に適当である。本装置は、“末端−末端°　　“末端−
側面“、側面−側面”及び“側面−末端°吻合部を配置
することができる。本発明の装置は、１回使用、使い捨
て型装置として製造するのに適しているので、１種また
は複数の長さに簡単に製造することができ、また消化器
系の中の内径の大部分をカバーする種々の径のステープ
ル収容ケーシングを備えることができる。
また、本発明のステーブリング装置は、軽量で、かつ操
作が容易である。ステープルを収容するケーシングは単
一環状列にステープルを装着することもできるが、同心
バタンの環状２列に互い違いにステープルを配置するの
が好ましく、これによって漏洩と出血を止めるための過
度縫合の必要を除くことができる。
本発明の装置は、アンビルとステープルケーシングとの
間隔が装置の“作動ギャップ”の範囲内にあることを視
見的にまた触覚的に表示する手段と、“作動ギャップ”
の範囲内において、実際ギャップを、縫合される組織の
厚さに適した距離に設定するため、目盛を備えた可調整
手段とを含む改良型ギャップ設定手段を備えている。
本発明の装置は、不慮のステープル突出を防止するため
のスライド安全ロックアウトを備えることができる。ま
た本装置は、後述するように、吻合部の両側の圧力を均
等化にするために通気することができる。
さらに、前記の従来型の装置では、ステープルが彎曲さ
れ組織が接合された後に、その装置を吻合部位から除去
するのが困難である。しかし、本発明の装置は、吻合部
位から装置を簡単に除去するための独特のプロフィルの
改良型アンビルを備えている。
これまで、外科用ステープルをＢ形に形成する形状のア
ンビル手段を０１えた外科用ステープリング装置が考案
されている。これは、例えば米国特許節３，４９０，６
７５号及び第３，４９４．５３３号に記載されている。
本発明のアンビルは、種々の脚長のステープルを使用す
る必要がなく、“作動ギャップ“の範囲内全体にわたっ
て、ステープルをＢ形に適当に彎曲させる形状のアンビ
ルポケットを備えている。
本発明によれば、消化器系の器官等の筒状器官を相互に
接合するための使い捨て外科用ステーブリング装置が提
供される。この装置は、２つの半休部を永久的に相互接
合してなる細長い本体からなり、この本体は筒状ハウジ
ングと、大径の同軸円筒形ハンドル部とを有する。この
筒状ハウジングは、その自由端に、同軸円筒形ケーシン
グを支持し、このステープルケーシングは、少なくとも
１環状列の外科用ステープルを収容している。
装置本体内部に調整ロッドが同軸的かっ、非回転的に装
着され、このロッドはステープルケーシングを越えて突
出した前端部ををする。この調整ロッドの前端部上に、
ステルプルケーシングに対向してアンビルがアンビルナ
ツトによって着脱自在に装着される。このアンビルはス
テープルケーシングに対向する環状平面を備え、その外
周部は前方内側に傾斜した薄いフランジを備える。装置
本体のハンドル部の自由端に備えられた：Ａ整ナツトに
より、アンビルと調整ロッドは、アンビルがステープル
ケーシングに隣接した位置と、アンビルがステープルケ
ーシングから離間した位置との間を、装置本体に対して
軸方向に移動自在である。
アンビルは、その環状平面上に複数のアンビルポケット
を有し、アンビルとステープルケーシングとの間の、本
装置“作動ギャップ”をなす距離の範囲内において、ス
テープルを打ち曲げる。
調整ロッドの後端は、調整ノブと協働するインジケータ
を備え、このインジケータは、アンビルが前記作動ギャ
ップの範囲内の距離をもってステープルケーシングから
離間していることを視覚的にまたは触覚的に表示する。
また、接合される筒状身体器官の組織厚さに応じて、ア
ンビルとステープルケーシングとの距離を作動ギャップ
の範囲内に正確に設定するためのリングスケールが調整
ノブ上に設けられている。
また、本発明の装置は、接合される組織の中にステープ
ルを刺し通しアンビルと当接させるための、また余分の
組織を除去するためのレバー作動型ステープルドライバ
ーとメス組立体とを備える。
さらに、この装置は、レバーをその不作動位置に固定す
るためのスライダー安全ロックアウト手段を備えること
ができる。
まず第１図と第２図を参照して本発明の外科用ステープ
リング装置を説明する。これらの図において同一部品は
同一番号で示す。本発明の装置を全体として１で示し、
この装置１は、筒状ハウジング部分３とこれよりも少し
大径の同軸円筒形ハンドル部４とからなる細長い本体２
からなる。
本発明の外科用ステーブリング装置は、再使用可能の滅
菌可能装置として製造することもできるが、また１回使
用、使い捨て装置として製造するのにも適当である。例
示した目的に沿って、本装置の使い捨ての形について説
明する。この場合、その各部品は、外科環境において使
用するに適し、またオートクレーブ、酸化エチレン、照
射またはその他の標準的方法によって滅菌することので
きる金属またはプラスチックからなる。
細長い本体２は、プラスチック素材から成形された２個
の鏡像（対称な）半体を公知の方法によって相互に固く
接着してなる。一方の半体は第１図において２ａて示さ
れ、他方の半体は第２図において２ｂで示される。これ
ら２つの半体２ａ。
２ｂは、相互に鏡像ではなくても、その他の形で実質的
に同形とすることもできる。
本体２の筒状ハウジング３は、その前端部に円筒形ステ
ープルケーシング５を備える。これについては後述する
。
本体２の内部に調整ロッド６が装着され、その軸方向に
延びる。この調整ロッド６の前端はケーシング５を超え
て突出し、アンビル組立体７を保持している。このアン
ビル組立体７については後述する。
ハンドル部４の後端に調整ノブ８が回転自在に装着され
ている。この調整ノブ８は中空円筒形部材であって、そ
の第１部分８ａはハンドル部４の後端を超えて突出して
いる。調整ノブ８は、第１部分８ａより小さい外径の第
２部分８ｂを有する。
この第２部分８ｂの外径は、ハンドル部４の内部に正確
に受は入れられ、その内部を回転できるように定められ
る。調整ノブ８は、さらに小径の第３部分８Ｃを有する
。この第３部分８Ｃの中にネジ穴９が貫通している。
前述したように、調整ノブ８は本体１のハンドル部４の
内部に回転自在に装着されている。ハンドル部４の内部
において調整ノブ８を定位置に保持するため、本体部の
各半体２ａと２ｂは環状内向き協働フランジを備える。
半体２ｂの内側フランジを１０で示す。他方の半体２ａ
もこれと同様の、整列した環状内側フランジ（図示され
ず）を有することは理解されよう。この半体２ｂのフラ
ンジ１０と半体２ａに協働するフランジが、調整ノブ８
の部分８ｂと８Ｃとの間に形成された前向きショルダ１
１に当接する。同じく、調整ノブの部分８ａと８ｂとの
間に形成されたショルダにはハンドル部４の端部が当接
する。最後に、調整ノブ８は環状ノツチ１３を備えるが
、ハンドル部４の内側面にはこれに対応するノツチ１４
が備えられている。調整ノブ８の軸方向移動を防止する
ため、これらのノツチ１３と１４の中にワッシャ１５が
配置されている。
下記の理由から、調整ノブ８は透明プラスチックで成形
されることが好ましい。ハンドル部４の後端に隣接して
その外面にリングスケール１６が備えられる。
調整ロッド６の後端はネジ山１８を有する。このネジ部
分１８は調整ノブ８のネジ穴９に係合する。また各半体
２ａと２ｂは、その内面上に細長いチャンネルまたはみ
ぞ穴を形成されている。半体２ｂの中のチャンネルまた
はみぞ穴は点線１９で示されている。半体２ａ上の同形
のみぞ穴は図示されていないが、これらの半体が接合さ
れたとき、みぞ穴と半体２ａの対応のみぞ穴が直径方向
に対向する。調整ロッド６は横方向穴２０をその内部に
有している。この横方向穴２０の中にビン２１が挿通さ
れ、このビン２１の両端が半体２ｂのみぞ穴１９と半体
２ａの同形のみぞ穴の中に摺動自在に係合する。その結
果、ビン２１は調整ロッド６の回転を防止する。しかし
、調整ノブ８が一方向に回転させられるとき、このノブ
８が調整ロッド６とネジ係合しているので、ロッド６は
本体１に対して軸方向に移動させられることは明白であ
ろう。その結果、アンビル組立体７はステープルケーシ
ング５から離れる方向に移動させられる。逆方向への調
整ノブ８の回転は調整ロッド６の連軸方向の移動を生じ
、その結果、アンビル組立体７はステープルケーシング
５に向かって移動する。
調整ロッド６上に、円筒形の中空駆動管２２が摺動自在
に同軸的に装着されている。この駆動管２２はその端部
にステープルドライバ５６と、後述するメス５９を装着
している。また駆動管２２は、その前端に内側に突出し
た複数突起の環状列を備えている。ここで、突起の１つ
を２３で示す。
同様に、駆動管２２はその後端に内側突起の環状列を備
える。その１つを２４で示す。駆動管２２は、装置本体
１と調整ロッド６に対して軸方向に移動自在であり、ま
た突起列２３と２４は、調整ロッド６に対する駆動管２
２の低摩擦案内手段をなす。
駆動管２２の前端に円筒形スプール２５が固着され、こ
のスプール２５はその両端に環状フランジ２５ａと２５
ｂを備える。本体の半体部２ａと２ｂは垂下した延長部
２６（第１図）と２７（第２図）をそれぞれ備える。レ
バー２８がピン２９上に枢着され、このビン２９の両端
はそれぞれ延長部２６と２７の中に装着される。レバー
２８の上部は二股をなし、一対のローブに終わり、その
一方を第２図において３０で示す。これらのローブ３０
は、スプール２５の環状フランジ２５ａと２５ｂの中間
の両側に配設される。その結果、レバー２８を本体２の
ハンドル部４に向かって移動させた場合、レバー２８の
ローブ３０はスプール２８と協働して、駆動管２２を調
節ロッド２６に対して軸方向に移動させる。
本体２の環状ハウジング部３の前端において、半体部２
ａと２ｂは相互に係合する内側環状フランジを備えてい
る。半体部２ｂの内側環状フランジを第２図において３
１で示す。ここで半休部２ａに対応する環状フランジは
図示されていない。
駆動管２２は一対の緩く装着されたワッシャ３２と３３
を保持している。これらのワッシャ３２゜３３は、駆動
管２２上に取付けられた圧縮バネ３４の両端の止め部材
として役立つ。ワッシャ３２は常態においてフランジ３
１およびその半体部２ａ上の対応するフランジと当接し
、他方のワッシャ３３は常にスプール２５のフランジ２
５ａに当接している。このようにして、圧縮バネ３４は
第１図と第２図に図示するように、駆動管２２とレバー
２８をその正規不作動位置に向かって弾発する戻しバネ
として作用する。
装置１は、レバー２８と駆動管２２の不慮の作動を防止
するため、スライダ安全ロックアウト３５を備えている
。このため、半体部２ｂは、下垂延長部２７のすぐ背後
にみぞ穴３６を備えている。半体部２ａも対応のみぞ穴
（図示されず）を備える。ロックアウト３５は、このみ
ぞ穴の中に摺動自在に係合している。ロックアウト３５
は、みぞ穴３６の上において半体部２ｂと係合するショ
ルダ３７を備える。ロックアウト３５は、その半体側に
おいても、半体部２ａと係合するための同様のショルダ
（図示されず）を備える。
ロックアウト３５は、その後端に係合部３８を備え、こ
の係合部３８は外科医の親指または人差指に係合されて
、ロックアウト３５をその鎖錠位置にまたは鎖錠位置か
ら移動させることができる。
また、ロックアウト３５は、その前端にノーズ部３９を
備え、このノーズ部３９は、レバー２８の二股部上に形
成されたショルダに係合してレバーの作動を防止する。
このショルダの一方を第２図において４０で示す。第２
図においては、ロックアウト３５は、レバー２８の作動
を防止する鎖錠位置に示されている。第２図から明白な
ように、ロックアウト３５は後方に移動させることがで
き、その場合、ノーズ３９はレバーのショルダ（その一
方を４０で示す）から離脱してレバー２８は自由に作動
される。
ここで、第２図、第３図及び第５図について述べる。こ
れらの部品において同一部品を同一番号で示す。本体１
の円筒形ハウジング部３の最先端部４１は小径を有する
。ハウジング部３は、その小径部４１に隣接して環状フ
ランジ４２を備えてショルダ４３を限定している。
円筒形ケーシング５は中空部材からなり、その後端に軸
方向孔４４を備え、この軸方向孔４４は、ハウジング部
３の前記小径部分をぴっちりと受け、ケーシング５の後
端がショルダ４３に当接するようになされる。さらに、
ケーシング５が正確な回転位置に配置されるように（従
ってその内部のステープルがアンビル組立体７のアンビ
ルポケットと正確に整列するように）、ハウジング３の
小径部４１は複数の周方向に等間隔のキー（その２つを
４５と４６で示す）を備え、これに対してケーシング５
の孔４４は対応のキーみぞを備える。その２つを４７と
４８で示す。ケーシング５をプラスチックで成形し、小
径部４１ならびにフランジ４２に対して接着または溶接
することができる。
ステープルケーシング５の内側面に対してステープル案
内部材４９が固着されている（接着、溶接または類似の
方法により）。このステープル案内部材４９はプラスチ
ック材から成形することができる。
第３図から明らかなように、このステープル案内部材４
９はチャンネル５０の第１環状列を有し、各チャンネル
はステープル５１を含み、またチャンネル５２の第２環
状列を含む。各チャンネル５２の中にステープル５３が
収容される。第２環状列のチャンネル５２は第１環状列
５０の内側に配置され、また、これに対して互い違いに
配置される。このため、本装置は荷重の互い違いのステ
ープル列を同心バタンで植込むことができる。
駆動管２２の最先端部は、ショルダ５４をなす小径部２
２ａを有する。この小径部２２ａの上に、ワッシャ５５
と環状ステープルドライバー５６が装着される。このス
テープルドライバー５６は歯５７の第１環状列を有し、
各＠５７は、ステープル案内部材４つの各チャンネル５
０に整列しその中に入る。同様に、ステープルドライバ
ー５６は歯５８の第２環状列を有し、各歯５８はステー
プル案内部材４９の各チャンネル５２の中に入る。
駆動管２２がレバー２８によって前方に移動させられる
ときに、ドライバー５６の歯５７と５８がステープル５
１と５３をそれぞれのチャンネル５０と５２から押し出
し、縫合される組織を刺し通して後述のようにアンビル
組立体７に当接させる。また、駆動管２２の小径部分２
２ａの上に円筒形メスが装着される。小径部２２ａの最
先端は据えこみされていて、ワッシャ５５と、ドライバ
ー５６とメス５９をその上に捕捉装着する。メス５９の
最先端環状縁は内側ベベル５９ａを形成されて切断縁を
なしている。ワッシャ５５は、ドライバー２２がステー
プルドライバー５６と円筒形メス５９に対して均一力を
加えるようにする。さらに、ワッシャ５５が本体２の最
先端部４１の末端に当接して、駆動管２２とレバー２８
の正規の不作動位置を決定する。
ここで、第４図と第５図について述べる。これらの図に
おいてアンビル組立体７が最も明瞭に図示されている。
アンビル組立体７はアンビル６０とアンビルナツト７３
とを含む。アンビル６０は円筒形部材であって、その後
端は開き、かつその前端は、軸方向孔６３以外は、閉じ
ている。第４図において最も明瞭に見られるように、こ
の軸方向孔６３は円形であるが、平面部６４を備える。
この平面部６４の目的については後述する。アンビル６
０の後端には薄い環状フランジ６５を備えられ、このフ
ランジ６５は、ステープルケーシング５の方に向いた環
状平面部（アンビル面）６６を示す。フランジ６５の外
周縁６７は丸く形成され、またフランジ６５の前面６８
は前方内側に傾斜している。その結果、アンビル６０は
ボタン状断面を有するが、その重要性については後述す
る。
アンビルフランジ６５の後側平面部６６はアンビルポケ
ット対６９ａ、６９ｂの第１環状列を備え、６対のポケ
ット６９ａ、６９ｂはそれぞれのステープル５１の両脚
と協働する。同様にアンビル面６６はアンビルポケット
対７０ａ−７０ｂの第１環状列を備え、各ポケット対７
０ａ−７０ｂは対応のステープル５３の両脚と協働する
。ドライバー５６とその歯５７．５８が、ステープル５
１．５３をチャンネル５０．５２から、接合される組織
を通して駆動し、アンビル６０に当接させる際に、これ
らのポケット対６９ａ−６９ｂと７０ａ−７０ｂは、後
述するように、ステープル５１と５３の両脚を適当な彎
曲させる。半剛性ワッシャ７１がアンビル６０の内部に
配置され、円筒形メス５９のバックアップ構造として作
用する。
調整ロッド６の最先端部７２は小径を有し、かつ、その
外側にネジ山を有する。このネジ部分７２はその上に平
面部７２ａを有し、この平面部７２ａはアンビル孔６３
の平面部６４に対応している。そのため、アンビル６０
が調整ロッド６のネジ山部７２上に装着されたときに、
これらの平面部６４と７２ａが協働して、アンビルポケ
ット６９ａ−６９ｂと７０ａ−７０ｂはステープル案内
部材４９のステープルチャンネル５０と５２と正しく整
列される。これは、キー４５．４６とキーみぞ４７と４
８がステープルケーシング５とこれに固着されたステー
プル案内部材４９０回転位置を決定し、またビン２１が
調整ロッド６の回転を防止するが達成される。
アンビル６０は、調整ロッド６のネジ山部７２に対して
アンビルナツト７３によって着脱自在に固定される。こ
のナツト７３はプラスチックまたは類似物から形成され
、軸方向金属インサート７４を備える。金属インサート
７４は軸方向孔７５を備え、この孔７５は内側にネジ山
を有して、ロッド６のネジ山部上に係合する。アンビル
ナツト７３が配置されたとき、アンビル６０は、ロッド
６の小径ネジ山部７２によって形成されたショルダ７６
に当接する。
アンビルナツト７３は、その接合される筒状体部材の中
に挿入され易いように切頭円錐形をなすのが好ましい。
またアンビルナツト７３の外側面には複数の縦方向グル
ープまたは溝７７を備え、外科医がこれをつかみ易くな
している。これによってアンビルナツト７３とアンビル
６０は、公知のように末端−側面ならびに側面−末端吻
合手術等の二、三の外科手術に際して着脱自在となる。
調整ロッド６の先端部は軸方向孔７８を備え、この孔７
８は側面孔または横方向孔７９に達する。
アンビルナツト７３の金属インサート７４の孔７５はこ
の金属インサートの内部を貫通し、また孔７８と同軸で
ある。同様にアジピルナツト７３のプラスチック部の最
前端は孔８０を備え、この孔は金属インサート７４の孔
７５及び調整ロッド６の孔７８．７９と連通ずる。これ
ら一連の孔は通気路をなす。その目的については後述す
る。
この構造を完成するため、ロッド６の後端の最後端部８
１はネジ山を有せず、キャップ状インジケータ８２が備
えられている。このインジケータ８２は、アンビル６０
とステープルケーシング６０との間隔が後述するように
装置の作動ギャップまたは操作ギャップの範囲内にある
ことを触覚的にまたは視覚的に表示するものである。
本発明の装置の構造について説明したので、下記におい
てその用法及び操作について説明する。
例示の目的から、本装置の操作を簡単な末端−末端吻合
手術に関して説明し、第２図、第５図及び第６図を参照
する。まず、アンビル６０は、その環状平面部６６がス
テープルケーシング５ならびにステープル案内部材４９
に当接または当接に近い状態となるように配置する。ま
た、レバー２８の不慮の作動を防止するため、スライダ
安全ロックアウト３５はその鎖錠位置になければならな
い。
第６図には、接合されるべき２個の管状身体器官または
内腔８３と８４が図示されている。末端−末端吻合手術
の際には、装置の前端は、もし可能ならば患者の体の自
然開口を通して、あるいは吻合側と反対の内腔側の切開
部を通して内腔８３の中に挿入される。ケーシング５が
第６図の内腔８３に対して図示の位置に達したとき、外
科医が調整ノブ８を回転させてアンビル６０をケーシン
グ５から離れる方向に移動させる。そこで第６図に図示
のように、内腔８３の末端を、調整ロッド６の露出部の
周囲において、簡単な締めヒモ縫合８５によって結紮す
る。そののち、アンビル組立体７を内腔８４の中に挿入
し、この内腔８４を、ロッド６の露出部の周囲に、締め
ヒモ縫合８６によって図示のように結紮する。
第６図のように内腔８３と８４が装置に対して結紮され
たときに、次の段階では、外科ステープル５１．５３に
よってこれらの内腔を接合するため、ケーシング５とア
ンビル６０の側面６６との間に適正なギャップを設定す
る。吻合行程に先立って、外科医は前記の同時特許出願
に記載された型の組織厚さＤＪ定器を用いて、内腔８３
と８４の組織の厚さを測定しておく。勿論、吻合される
組織の厚さは内腔８３と８４の組織厚さの合計である。
この値を考慮に入れて、外科医はステープルケーシング
５とアンビル６０の側面６６との適正ギャップを下記の
ようにして設定することができる。まず、アンビル６０
の側面６６をケーシング５の前端に近づける方向に調整
ノブ８を回転させる。インジケータ８２の後端が調整ノ
ブ８の後端と実質的に共面となるまで、調整ノブ８の回
転を継続する。これは触覚と視覚の両方によって確認さ
れる。もし調整ノブ８が透明プラスチックで作られてい
れば、外科医はインジケータ８２がこの共面位置に近づ
くのを容易に検知することができる。
インジケータ８２の後端が調整ノブ８の後端と共面にな
ったとき、外科医は装置１の作動ギャップの範囲内でケ
ーシング５とアンビル６０の側面６６との最大間隔が達
成されたと確信できるように、調整ロッド６の長さが定
められる。この時点において、外科医は先に測定された
組織厚さに対応して最終ギャップを設定するため、リン
グスケール１６とインデックスマーク１７とを使用する
。
最終ギャップが達成されたとき、スライダ安全ロックア
ウト３５をその後方位置、即ち、非鎖錠位置まで移動さ
せ、外科医はレバー２８を本体２のハンドル部４に向か
って絞る。レバー２８のローブ（第２図においてその一
方３０を示す）がスプール２５と係合して、駆動管２２
は調整ロッド６に対して前方に移動させられる。また、
これによって、ドライバー５６とその歯５７．５８はス
テープル５１と５３をそれぞれのステープル案内チャン
ネル５０と５２から押出す。これらのステープル５１．
５３は内腔８３，８４の組織を貫通し、各ステープル５
１．５３の脚は、それぞれのアンビルポケット対６９ａ
−６９ｂネジ７０ａ−７０ｂの中に入って打ち曲げられ
る。外科医は装置１の作動ギャップ内の一部ギャップで
操作しているのであるから、ステープル５１．５３の満
足な曲げが保証される。またワッシャ５４により、ドラ
イバー５６に加えられる曲げ力がすべてのステープル５
１．５３に対して均一となるように保証される。
第２図と第５図とから明らかなように、円筒形メス５９
はステープル５１と５３に対して後方に片寄らされてい
る。その結果、メス５９は、これらのステープル５１．
５３の十分な曲げが完了するまでは、メス５９は内腔８
３と８４の組織内部に切込み始めない。円筒形メス５９
の目的は、調整ロッド６に結紮された内腔８３と８４の
組織部分を切除するにある。内側ベベル５９ａは、より
平行な切断をなし、また切除された組織部分をメスの中
に押込む。またワッシャ５４は、メス５９の円筒形切断
縁に沿って均一な力を加えることを保証する。
アンビル６０の中に配置された半剛性ワッシャ７１は円
筒形メス５９のマナ板として作用し、切断される組織を
支持する。このワッシャ７１は、切断される組織に対す
る引延力または引張り力を最小限になすように、アンビ
ル面６６と同水準に配置される。
ステーブリング段階と切除段階が終了したのち、吻合部
位から装置を除去する必要がある。このために、アンビ
ル６０をケーシング５から引出す方向に調整ノブ８を回
転させる。ギャップを開こうとするアンビル６０の運動
は、ピストン作用で部分的真空を生じ、接合された両方
の内腔をギャップの中に圧潰させようとする。これは、
第５図について説明した孔８０，７５．７８及び７９か
らなる通気系統によって防止される。
アンビル組立体７がケーシング５から離間されたとき、
装置１を除去することができる。この装置１の除去のた
めには、円筒形メス５９によって組織中に残された孔の
中をアンビル組立体７を通過させる必要がある。この孔
は最初の２器官の内径より小である。前述したようなア
ンビル６０のボタン形状の故に、ステーブリングされた
吻合部の一部がフランジ６５の上を滑らされる間に、吻
合部の残余の部分が調整ロッド６に近接する。吻合され
た組織をボタン状フランジ６５に沿って丸く進行させる
ことにより、このアンビル組立体７は、組織またはステ
ープルラインに対して過度のひずみを与えることなく、
吻合部位を通して引出すことができる。その後、装置は
内腔８３から簡単に除去され、吻合手術は完了する。完
了した吻合部を第７図に示す。
前述したように、人間の消化器系の各器官は同一患者に
おいてさえも相異なる径と壁圧を有する。
故に、本発明の装置１は、相異なる外径のステープルケ
ーシング５を使用できるものでなければならない。これ
に限定されるものではないが、もし本装置１が２１ｍｍ
、２５ｍ＋ｓ、２８ｍｍ及び３３■の外径のステープル
ケーシング５を用いることができるならば、消化器系の
各器官の内径の大部分がカバーされる。もちろん、本発
明の装置は、これにより小径または大径のステープルケ
ーシング５を備えるようにすることができる。
ステープルケーシング５中のステープル５１゜５３の数
はそのケーシングの外径に依存している。
研究結果によれば、止血及び漏洩の防止閉鎖のため、外
側環状列のステープル５１と内側環状列ステープル５３
は、第３図に図示のように、少し重なり合わなければな
らない。しかしながら、もしステープル５１と５３が過
度に相互近接していれば、血液の循環が影響され、治療
にも悪影響を与える。さらに、もしステープル５１と５
３が過度に相互接近していれば、ステーブリング吻合さ
れた部位の弾性が少なくなり、その結果、装置を吻合部
位から除去することがさらに困難になる。また、すべて
の生組織は切断されたときに収縮するのであるから、ス
テープル５３の内側環状列と円筒形メス５９との中間に
残さなければならない最適組織縁部が存在する。
本発明の装置１は、その最終用途に応じて任意適当な操
作ギャップを備えるように設計できることは理解されよ
う。これに限定されるものではないが、もし装置１が１
〜３順の作動ギャップを備えているならば、この装置１
は消化器系の実質的にすべての器官について使用するこ
とができることが見い出された。１〜３＋ｎｎの作動ギ
ャップを有するように設計された装置１の実施例におい
ては、リングスケール１６は０．５ｎｏｓ増分で０〜３
ｍ１１の目盛を備えていた。リングスケール１６の３關
と表示されたマークがハンドル部４のインデックスマー
ク１７と整列した場合、インジケータ８２の後端面が調
整ノブの後端と共面であって、作動ギャップの最大値が
得られていることを示している。
本発明の装置１は、その作動ギャップ全体にわたって同
一サイズの外科ステープル５１．５２を使用することが
できる。これは、外科用ステープル５１と５３を′Ｂ°
形に形成しようとするアンビルポケット６９ａ−６９ｂ
及び７０ａ−７０ｂの形状によって可能となる。外科用
ステープルの一例５１を第８図に示す。前述したように
、２環状列のステープル５１と５３は同形である。ステ
ープル５１は、その最初の形態において、実質水平のク
ラウン部５１ａと下垂脚部５１ｂ、５１ｃからなるＵ形
をなす。脚５１ｂと５１ｃの自由端は、組織進入点５１
ｄと５１ｅをなすように、下向向き傾斜角をもって切断
されている。外科用ステープル５１が、吻合される組織
内部をステープルドライバー５６によって前進させられ
、一対のアンビルポケット６９ａと６９ｂの中に入るに
従って、脚５１ｂと５１ｃは彎曲形に相互の方に曲げら
れる。第９図は、第８図のステープルが最大ギャップに
おいて完全に形成された状態を示す。
第１０図は、装置１の作動ギャップの最小値において形
成された最終形状のステープル５１を示す。
装置１が１〜３止の作動ギャップを有する前記の実施例
において、約５．５ｍｍまたは約０．２１６インチの脚
長を備えてい、るならば、ステープル５１と５３は第９
図と第１０図に図示のように形成されることが分った。
第１１図、第１２図及び第１３図は、アンビルポケット
対６９ａと６９ｂの一例を示す。すべてのポケット対６
９ａ−６９ｂは同形であり、また、すべてのポケット対
７０ａと７０ｂはポケット対６９ａ及び６９ｂと同形で
あることが理解されよう。
第１１図〜第１３図及び第４図から明白なようニ、ポケ
ット対６９ａと６９ｂはアンビル６０の後向き面６６の
中に形成されている。アンとルボケット６９ａと６９ｂ
は相互に鏡面（対称）をなし、他の点では同形である。
適当な余裕をもってステープルの打ち曲げを実施できる
ように、アンビルポケット対６９ａと６９ｂの長さ合計
はステープルクラウン部の長さ（即ちステープルの巾）
よりも少し長い。第１２図において、これはステープル
５１の脚５１ｂと５１０（第８図）が丁度ポケットの中
に入ろうとするその切位置を点線で示している。第１３
図から明らかなように、ポケット５９ａ、６９ｂの側面
は内向き下方に傾斜し、放射方向余裕を与え、ステープ
ル脚５１ａと５１ｃをそれぞれのポケットの最深部の中
心に向かって押圧する。
第１２図について述べれば、ポケット６９ａと６９ｂは
、それぞれまず縦方向に延びた大径部分６９ｃと６９ｄ
をそれぞ有する。これらの大径部分６９ｃ、６９ｄは、
それぞれステープルの脚部５１ｂ、５１ｃの先端５１ｄ
、５１ｅと協働して、これらの先端を相互の方に彎曲さ
せる。ポケット６９ａと６９ｂは、またそれぞれ縦方向
に延びた小径部分６９ｅと６９ｆを有する。ステープル
脚５１ｂと５１ｃがさらにアンビル６０の方に押される
にしたがって、ポケットの小径部分６９ｅと６９ｆはス
テーブ１１脚５１ｂと５１ｃに、それ自体の曲率半径を
形成させる。装置１のギャップ設定によって、上述のよ
うにステープル５１の第９図の形から第１０図の形まで
の最終形状が決定される。装置１の作動ギャップの範囲
内のすべてのギャップ設定値において、ステープル５１
はＢ形状に形成される。
本発明の装置１がその作動ギャップの範囲全体にわたっ
て単一の脚長のステープルを使用できるようにするため
、最も重要な要因は、ステープル５１の脚長、ワイヤ径
、引張り強さ、ならびにポケット６９ａ、６９ｂの輪郭
である。操作例においては、ステープル５１は約０．０
１１インチのワイヤ径と、約１５０，０００〜１６０，
０００ｐｓｉの引張り強さと、約０．２１６インチの脚
長とを有する不銹鋼で作られていた。アンとルポケット
６９ａと６９ｂは、その間隔を含めて約０．１８２イン
チの全体長を有し、各ポケット６９ａと６９ｂは約０．
０２８インチの最大中、及び約０．０１５インチの最大
深さを有していた。
大径部６９ｃと６９ｄは約０．０８１インチの径を有し
ていた。小径部６９ｅと６９ｆは、それぞれ約０．０３
５インチの径を有していた。この操作例において、ステ
ープル５１は、１〜３ｍｍの作動ギャップ全体にわたっ
て、満足にまた確実に、許容可能のＢ形に形成された。
本発明は前記の実施例に限定されるものでなくその主旨
の範囲内において変更実施できる。例えば、リングスケ
ール１６はへ本体２より調整ノブ８に隣接して装着し、
調節ノブ８がインジケーターマーク１７を担持すること
ができる。

Claims (1)

  1. 【特許請求の範囲】 １、筒状身体器官を接合するのに用いられ、少くとも環
    状例の外科用ステープルを収容する円筒形ステープルケ
    ーシングを有する管内吻合外科用ステープリング装置用
    アンビルであって、前記アンビル（６０）はアンビル面
    （６６）を有し、このアンビル面（６６）は前記ケーシ
    ング（５）に対向し、前記アンビル（６０）は前記アン
    ビル面（６６）が前記ケーシング（５）に隣接する位置
    と前記ケーシング（５）から離間した位置との間を移動
    自在であり、前記ケーシング（５）内部に配置されたス
    テープルドライバー（５６）及び円筒形メス（５９）と
    前記ステープルドライバー（５６）及び前記円筒形メス
    （５９）は接合される円筒状身体器官を通してステープ
    ル（５１、５３）を刺し通し、このステープル（５１、
    ５３）を前記アンゼル面身体に当接させて湾曲させ、ま
    た前記筒状身体器官の余分の部分を切断するために前記
    ステープルドライバー（５６）と前記メス（５９）を前
    記ケーシング（５）内部の後退位置から前記アンビル（
    ６０）に向って移動自在であるアンビルにおいて、前記
    アンビル（６０）は前記ケーシング（５）の径より小さ
    い横方向寸法の本体部を備え、この本体部は外側に拡大
    して前記ケーシング（５）の径とほぼ同一の最大径を有
    する環状フランジ（６５）を形成し、前記フランジ（６
    ５）を有する前記アンビル（６０）の前記端部は前記ア
    ンビル面（６６）を有し、前記装置。（１）の一部分は
    前記本体部を含む前記ケーシング（５）から離れる方向
    へ前記フランジ（６５）を越えて延び、かつ、前記アン
    ビル（６０）の前記フランジ（６５）の最大横方向寸法
    より小さい最大横方向寸法を有することを特徴とするア
    ンビル。 ２、前記アンビル（６０）を支持し移動させる手段を有
    し、この手段はロッド状部材（６）からなり、前記アン
    ビル（６０）は、前記ロッド状部材（６）を受ける軸方
    向孔（６３）と、前記ロッド状部材（６）と協働して前
    記アンビル（６０）を前記ロッド状部材（６）上に位置
    させ前記アンビル（６０）が前記ロッド状部材（６０）
    に対して回転するのを防止する前記アンビル（６０）に
    関連した平面部（６４）と、前記ロッド状部材（６）に
    ねじ係合して前記アンビル（６０）を前記ロッド状部材
    （６）上に保持するアンビルナット（７３）と、前記ケ
    ーシング（５）から離れる方向へ前記アンビル（６０）
    の前記フランジ（６５）を越えて延び、前記アンビル（
    ６０）の前記本体部と前記アンビルナット（７３）を含
    み、前記フランジ（６５）の横方向寸法より小さい横方
    向寸法を有する前記装置（１）の部分とを有する特許請
    求の範囲第１項記載のアンビル。 ３、前記フランジ（６５）の外円端縁は円形である特許
    請求の範囲第１項記載のアンビル。 ４、前記フランジ（６５）を備えた前記アンビル（６０
    ）の端部は、その中に中心くぼみを有し、このくぼみの
    中に半剛性物質のワッシャ状部材（７１）が装着され、
    かつ、前記円筒形メス（５９）のバックアップ手段をな
    すことを特徴とする特許請求の範囲第１項に記載のアン
    ビル。 ５、前記装置（１）の作動ギャップをなす前記アンビル
    面（６６）と前記ケーシング（５）間隔の範囲と、前記
    アンビル面（６６）がこのアンビル面（６６）に形成さ
    れた少くとも環状１列のアンビルポケット対（６９ａ、
    ６９ｂ）と、前記アンビルポケット対（６９ａ、６９ｂ
    ）の数は前記ケーシング（５）に収容された前記外科用
    ステープル（５１）の数に等しく、かつ、前記外科用ス
    テープル（５１）は頂部（５１ａ）により接続された一
    対の脚（５１ｂ、５１ｃ）を有するＵ形要素からなり、
    前記アンビルポケット対（６９ａ、６９ｂ）の各々のア
    ンビルポケットは互いに鏡像関係を有し互いに離間して
    同じ外科用ステープル（５１）の脚（５１ｂ、５１ｃ）
    の一方をそれぞれが受け、各前記ポケット対の各ポケッ
    ト（６９ａ、６９ｂ）は細長いくぼみからなり、前記く
    ぼみは第１長手方向湾曲大径底面部分（６９ｃ、６９ｄ
    ）と第２長手方向湾曲小径底面部分（６９ｂ、６９ｆ）
    とを有し、前記第１長手方向湾曲大径底面部分（６９ｃ
    、６９ｄ）は対応するステープル脚（５１ｂ、５１ｃ）
    と最初に接して前記ステープル脚を互いの方へ打ち曲げ
    、前記第２長手方向湾曲小径底面部分（６９ｂ、６９ｆ
    ）は前記ステープル脚（５１ｂ、５１ｃ）を互いに反対
    方向に湾曲させて、前記ステープル（５１）をＢ字形と
    し、これによって単一の脚長のステープル（５１）が前
    記アンビル面（６６）を前記ケーシング（５）間の前記
    差動ギャップ内のいかなる間隔においても十分に折り曲
    げられることができる特許請求の範囲第１項記載のアン
    ビル。 ６、前記ケーシング（５）内の第１及び第２同心環状列
    の外科用ステープル（５１、５３）と前記環状列の外科
    用ステープル（５１、５３）は互いに周方向に片寄らさ
    れ、前記アンビル（６０）は第１及び第２同心環状列の
    アンビルポケット対（６９ａ−６９ｂ、７０ａ−７０ｂ
    ）とを有し、前記環状列のアンビルポケット対（６９ａ
    −６９ｂ、７０ａ−７０ｂ）は互いに周方向に片寄らさ
    れて各前記アンビルポケット対（６９ａ−６９ｂ、７０
    ａ−７０ｂ）は前記ケーシング（１５）内の前記外科用
    ステープル（５１、５３）の一方と整合されることを特
    徴とする特許請求の範囲第１項記載のアンビル。
