Description

【発明の詳細な説明】 本発明は、血管系統特に人間の心臓血管系内に
カテーテルを導入する際に用いるためのガイドワ
イヤに関する。
心臓血管系統の中へのカテーテルの挿入を容易
にするため、従来ガイドワイヤが提供されてき
た。このガイドワイヤの1つが、1983年7月13日
に提出された米国特許出願第513222号に開示され
ている。このようなガイドワイヤにあつては、非
常に細い導管特にこの導管の部分的に閉じられた
区分の中へこのガイドワイヤを導入することが困
難であることが判明した。それ故、心臓血管系の
中の細い導管の中へうまく導入しうる改良された
ガイドワイヤの必要がある。
本発明の一般的な目的は、血管系統特に人間の
心臓血管系内の細い導管の中へ導入しうるカテー
テル用のガイドワイヤを提供することである。
本発明のもう1つの目的は、高いトルク性能を
有するカテーテル用のガイドワイヤを提供するこ
とである。
本発明のもう1つの目的は、非常にフロツピー
な即ちばたばた動き易い可撓性のカテーテル用の
ガイドワイヤを提供することである。
本発明のもう1つの目的は、ガイドワイヤの遠
位端に種々の程度のフロツピー度即ち可撓性を設
けることのできるカテーテル用のガイドワイヤを
提供することである。
本発明の他の目的と特徴は、添付図面と組合せ
て以下に詳細に述べる好ましい実施態様の説明か
ら明らかとなるであろう。
カテーテルと共に用いるためのガイドワイヤ
は、0.008〜0.020インチ(0.2032〜0.508mm)の範
囲の直径をもつ近位部分と0.007インチ(0.1778
mm)未満の直径をもつ遠位部分とを有するステン
レス鋼製の可撓性で円筒形の細長い要素からな
る。細長い要素の永久変形を防ぐためステンレス
鋼製のコイルが細長い要素の遠位部分に固定さ
れ、実質的にラジオパクの即ち放射性不透性の材
料から形成された追加のコイルがステンレス鋼コ
イルに固定されている。
第1図と第2図に示す本発明によるガイドワイ
ヤ11は、ステンレス鋼のような高い捩り強度を
有する適当な金属材料から形成された可撓性で円
筒形の細長い要素12からなる。所望により、ス
テンレス鋼以外の他の材料、例えばある炭素鋼、
チタン及びベリリム銅等を利用しうることが理解
されるべきである。細長い円筒形要素は、ワイヤ
状で中空の円筒形要素の形でもよく、又ワイヤ状
でむくの円筒形コアの形でもよい。
細長い円筒形要素12を形成するために利用す
るワイヤは、例えば20%クロムと10%ニツケルを
含有し最小引張強度が240KsiのNo.304ステンレス
鋼から適当な方法で形成されるのがよい。それ
は、0.0150ステンレス鋼ワイヤ材料を0.016材料
に引いて細くすることにより加工硬化されるのが
よい。ワイヤは、次に真直ぐにされ、所望の長さ
に切断される。その後、ワイヤが焼鈍され、焼鈍
の後芯なし研削されて第1図に示す可撓性の細長
い要素が得られる。
第1図に示すように、可撓性の細長い円筒形要
素12は、0.008〜0.020インチ(0.2032〜0.508
mm)のような適当な直径好ましくは約0.016イン
チ(0.4mm)の直径を有する円筒形部分12aを
備えている。細長い要素12は、円筒形部分12
aの一端に接続するテーパ付き部分12bを備
え、テーパ付き部分12bは、もう1つの円筒形
部分12cに接続し、この円筒形部分12cは、
0.005〜0.010インチ(0.127〜0.254mm)のような
適当な寸法好ましくは約0.008インチ(0.2032mm)
の太さを有する。円筒形部分12cに接続するも
う1つのテーパ付き部分12dが設けられてい
る。もう1つの円筒形部分12eがテーパ付き部
分12dに接続している。円筒形部分12eは、
0.007インチ(0.1778mm)未満の適当な寸法例え
ば0.003インチ(0.0762mm)±0.0005インチ
(0.0127mm)の寸法を有する。
可撓性の細長い要素全体の長さは、150〜250cm
の範囲内の適当な長さでよいが、約175cmの長さ
であるのが好ましい。円筒形部分12eは、2〜
6cmの範囲内の長さでよいが、約4cmの長さをも
つのが好ましい。円筒形部分12cは、25〜30cm
の範囲内の長さをもつことができるが、約26.5cm
の長さを有するのが好ましい。テーパ付き部分1
2dの長さは約1/2cmがよいが、テーパ付き部分
12bの長さは約1cmとするのがよい。
細長い可撓性要素12の近位端は、冠状導管を
通してガイドワイヤを移動させるのを容易にする
ように適当な材料で被覆されている。例えば、円
筒形部分12aの相当部分を適当な厚さ例えば
0.001インチ(0.0254mm)の厚さにテフロン被覆
15を施すのがよい。
ステンレス鋼のような適当な材料の細長いコイ
ル13が、第2図に示すように半田14の使用の
ような適当な手段により、円筒形要素12の遠位
端に固定されている。図示のように、コイル13
は円筒形部分12cの上に延び、可撓性要素12
と細長いコイル13との間の半田結合部14が、
テーパ付き部分12bの近くに形成されている。
コイル13は、約0.003インチ(0.0762mm)の
直径をもつステンレス鋼ワイヤから形成され、コ
イルの外径が約0.017インチ(0.4318mm)となる
ように巻かれている。コイル13は、きつくぎつ
しり詰め込まれるような方法で、即ち換言すれば
“ボトム アウト(bottomed out)”されるよう
な方法で巻かれる。半田14は、コイルの間の隙
間を満すがコイルの外径を著しくは増大させない
ような量でコイルに施される。コイル13は、細
長い要素12の遠位端の方へ円筒形部分12eに
隣接する領域へ延びている。
実質的にX線を透さない材料から形成されたも
う1つの細長いコイル16が設けられている。こ
のコイル16は、少くとも13gm/cm3の密度を有
する材料から形成されるべきである。この要件に
適合する適当な材料には、金、タンタル、タング
ステン、プラチナ、イリジウム、レニウム及びこ
れらの材料の合金が含まれる。細長いコイル16
用に利用されるワイヤは、プラチナ合金から形成
され、適当な寸法を有するが、ステンレス鋼コイ
ル13の寸法に実質的に等しい寸法を有するのが
好ましい。この理由で、ワイヤは、0.003インチ
(0.0762mm)の直径を有し、コイルの外径が約
0.017インチ(0.4318mm)となるように巻かれる。
細長いプラチナコイル16の1つおきの巻きが細
長いステンレス鋼コイル13の1つおきの巻きの
間に配置されるような方法で、第2図に詳細に示
すように、コイル16の一端がコイル13の遠位
端の中へねじ込まれている。このねじ込み結合
は、第2図に断面斜線で示してあり、ステンレス
鋼コイル13の断面斜線が1つの方向に延び、プ
ラチナコイル16の断面斜線は前記1つの方向か
ら90度変位した方向に延びている。プラチナコイ
ル16に付加的な可撓性を与えるために、コイル
の巻きをぎつしり詰め即ち“ボトム アウト”す
るよりもむしろ適当な距離例えば0.005〜0.0015
インチ(0.127〜0.0381mm)だけ離間させる。別
態様として、2つのコイル13と16を一緒に突
き当てることができる。
0.003インチ(0.0762mm)の幅と0.001インチ
(0.0254mm)の厚さのような適当な寸法のタング
ステンのような適当な材料から形成された安全リ
ボン17が、細長い円筒形要素12の端からコイ
ル16の最外端即ち遠位端へ延びている。安全リ
ボン17の近位端と、ねじ込まれ又は一緒に突き
合せられたコイル13と16の2つの端部とは、
ろう付け18のような適当な手段により細長い要
素12と結合されて単一組立体を形成している。
半田結合部14と同様に、ろう付け結合部18
は、材料がコイル16の間の隙間を満たすような
方法で形成される。ろう付け結合部18が、安全
リボン17の近位端を細長い要素12の円筒形部
分12eに固定している。
第2図に示すように、コイル16は、要素12
の遠位端を越えて適当な距離だけ、例えば1〜2
cmの長さ、好ましくは1.5cmだけ延びている。コ
イル16の遠位端は、この遠位端に丸味を付ける
と共に安全リボン17の遠位端を固定するための
適当な装置を備え、プラチナコイル16の遠位端
とリボン17の遠位端とに接着される金のような
適当な材料から形成された球又はプラグ19から
成る。
第1図と第2図に示されているガイドワイヤの
製造と関連して利用される半田材料とろう付け材
料は、従来の種類のものである。例えば、半田
は、従来の銅、銀合金又は錫銀合金がよく、ろう
付け材料は、銀、銅、錫とニツケルの合金とする
のがよい。
前述のガイドワイヤは、当業者に周知の方法で
バルン型カテーテルの導入を容易にするため心臓
血管系内の導管を探り針で探るために利用しうる
非常に可撓性の遠位端を備えているのでフロツピ
ーワイヤ(floppy wire)として特徴づけること
ができる。ガイドワイヤの形状は、心臓血管系内
の導管に容易に従うことができる。従来の蛍光透
視鏡を用いて移動を観察することもできる。
第1図と第2図に示すガイドワイヤよりも僅か
に小さい可撓性即ちフロツピー度(floppiness)
をもつ本発明によるもう1つのガイドワイヤが第
3図に示されている。第3図に示すガイドワイヤ
26は、第1図と第2図に関連して上述した形式
の細長い円筒形要素12からなる。同様に、ステ
ンレス鋼製のコイル13が、第1図と第2図の実
施態様において説明した方法で利用され、半田1
4により可撓性の細長い要素12に結合される。
コイル16を形成した方法と同じ方法でプラチナ
製コイル27が設けられるが、このコイル27
は、コイル16よりも長さが短かく、細長い可撓
性要素12の遠位端まで延びているにすぎない。
コイル27は、コイル13の遠位端の中へねじ込
まれ、ろう付け18により細長い要素12に接着
されている。コイル27は、可撓性の細長い要素
12の端を超えて延びていないので、第1図と第
2図の実施態様において設けられた安全ワイヤ即
ち安全リボン17を設けることは必要でない。金
のような適当な材料から形成されたプラグ又は球
28が、コイル17の遠位端に形成され、可撓性
の細長い要素12の遠位端に結合されている。前
の実施態様のように、追加の可撓性を与えるめ
に、コイル27の巻きを外端で例えば0.0005〜
0.0015インチ(0.0127〜0.0381mm)だけ離間させ
るのがよい。
前述の説明から、心臓血管系内で遭遇する種々
のタイプの導管特に部分的に閉塞した導管を通り
抜ける種々の異なる性能のガイドワイヤを提供し
うるように、種々の変化する程度の可撓性をもつ
ガイドワイヤが提供されたことがわかる。この種
のガイドワイヤは、この閉塞部を通り抜けること
を容易にし、当業者に周知の方法でバルン型カテ
ーテルの導入を容易にする。設けられたコイル
は、ガイドワイヤの永久変形を妨止する。
【図面の簡単な説明】
第1図は、本発明によるガイドワイヤの構成と
関連して用いられる芯ワイヤの側面図である。第
2図は、本発明によるガイドワイヤの部分断面側
面図で、フロツピーな遠位端を備えている。第3
図は、本発明によるガイドワイヤのもう1つの実
施態様の一部断面の側面図で、フロツピー度がよ
り少ない遠位端を有する。 11,26……ガイドワイヤ、12……可撓性

Claims (1)

  1. 【特許請求の範囲】 1 ガイドワイヤにおいて: 高い捩り能力を有する金属材料から形成され、
    0.008〜0.020インチ(0.2032〜0.508mm)の範囲内
    の直径をもつ近位部分0.007インチ(0.1778mm)
    より小さい直径をもつ遠位部分とを有する細長い
    可撓性の円筒形要素と; 金属材料から形成され且つ前記遠位部分に固定
    された第1コイルと; 第1コイルの構成材料と異なり且つ実質的に放
    射線を透過しない材料から形成され、第1コイル
    の遠位端に固定された第2コイルと; 第2コイルの遠位端に固定された丸味のある構
    造体を有する装置と; を具備するガイドワイヤ。 2 第2コイルの外端が、第2コイルの外端に固
    定され且つ可撓性の細長い円筒形要素にも固定さ
    れた安全ワイヤと共に、可撓性の細長い円筒形要
    素の遠位端を越えて延びている特許請求の範囲第
    1項に記載のガイドワイヤ。 3 第2コイルの外端に丸味のある構造体を形成
    する前記装置が、可撓性の細長い円筒形要素の遠
    位端に接着されている特許請求の範囲第2項に記
    載のガイドワイヤ。 4 前記第1コイルと第2コイルとの少くとも一
    部が有する個々の互違いの巻きが相互の間に配置
    されるように、前記第1コイルと第2コイルとが
    一緒にねじ込まれている特許請求の範囲第1項に
    記載のガイドワイヤ。 5 一緒にねじ込まれている部分が、一緒にろう
    付けされ且つ可撓性の細長い要素にろう付けされ
    ている特許請求の範囲第4項に記載のガイドワイ
    ヤ。 6 第2コイルが、安全ワイヤと共に可撓性の細
    長い要素の遠位端を越えて延びており、安全ワイ
    ヤの一端が可撓性の細長い要素にろう付けされて
    おり、安全ワイヤの他端が、第2コイルの外端に
    丸味のある構造体を形成する装置に接着されてい
    る特許請求の範囲第1項に記載のガイドワイヤ。 7 第1コイルがステンレス鋼から形成され、第
    2コイルがプラチナ合金から形成されている特許
    請求の範囲第1項に記載のガイドワイヤ。
