すことのできる形状および大きさを有している
細長形のハウジングと; b 前記ハウジングを経て長手方向に血液を送り
出す前記ハウジング内の回転ポンプ手段と; c 前記ハウジングから延在し、かつ前記血液内
に該血管を閉塞することなく入れられる導管手
段と; d 前記ポンプ手段に動力を供給するための動力
手段と; e 前記動力手段から前記ポンプ手段に駆動力を
伝達するための前記導管内の駆動手段; とを具えることを特徴とする脈管内血液ポンプ。
2 前記駆動手段が前記導管手段を経て延在する
可撓性のケーブルを含むようにしたことを特徴と
する請求の範囲第1項に記載のポンプ。
3 前記回転ポンプ手段が、固定子および回転子
手段と、これらの固定子および回転子手段を相対
的に回転させるように取付ける軸受手段とを具
え;前記軸受手段をパージシールし;かつパージ
流体を前記導管手段を経て前記軸受手段に十分な
速度で放出させて、血液要素が前記パージシール
に入らないようにしたことを特徴とする請求の範
囲第1項に記載のポンプ。
4 前記回転ポンプ手段をほぼ10000〜20000rpm
の回転速度で作動する軸流ポンプとしたことを特
徴とする請求の範囲第1項に記載のポンプ。
5 前記回転ポンプ手段の血流速度を毎分当たり
ほぼ4リツトルとしたことを特徴とする請求の範
囲第1項に記載のポンプ。
技術分野 本発明は血液ポンプ、特に大手術をすることな
く脈管系内に経皮挿入して右または左心室の血液
流動を十分にすることにより一時的に心臓機能を
助けることのできる小形血液ポンプに関するもの
である。
背景技術 医者は心臓性シヨツクの症例(すなわち、心臓
はまだ機能しているも、患者を生存させるのには
十分な血液を送り出していない状態)に頻繁に直
面する。このような状態の緊急処置には、患者を
生存させるのに十分な程度に血流を維持する何等
かの形態の心臓機能救援手段がしばしば必要とさ
れるも、他の手段を着手して心臓そのものを一層
正常に機能すべく回復させる必要がある。
現在斯種の心臓機能救援手段に対する有効な方
法は、大手術をして脈管系に接続するポンプ装置
によるものか、または適当な動脈内に挿入するバ
ルーンカテーテルによるものだけである。遺憾な
がら、大手術は心臓性シヨツクの患者には時間が
かかり過ぎ、また危険を伴ない、しかもバルーン
カテーテルは心臓からの血液の送り出しを十分に
向上させることができない。
非開胸脈管接近法を用いて血液の左側循環を支
えることが提案されている。斯種の方式では2つ
の動脈部位を同時に用いて血液を心臓から動脈系
へと送り出すようにしている。しかし、この技法
には長くて孔の小さいカニユーレを使用する必要
があり、斯様なカニユーレでは圧力降下が大きく
なる。カニユーレの導管が負圧を受けると、空気
塞栓症および重い合併症をまねくことになる。さ
らに、ポンプを心臓に接近させるためには鎖骨下
動脈を用いてポンプを挿入する必要がある。斯か
る動脈への外科的接近法は大腿部動脈への場合よ
りもずつと困難である。
右心機能を支えるため外部ポンプを用いるに
は、2つの静脈部位にて同時に接近させる必要が
ある。この場合にはポンプ出力を長くて孔の小さ
いカニユーレによつて肺動脈に戻す必要がある。
この場合にも負圧による圧力降下が大きくて同様
な合併症や、場合によつては空気塞栓症が生じた
りする。従つて従来技術では心臓の血流を迅速に
十分なものして、重症患者を助けることのできる
簡単な構成で、危険度の低い血液循環救援装置を
提供することはできなかつた。
発明の開示 本発明は、末梢静脈または動脈系のいずれかを
介して右または左心室に直接接近させて、血液循
環を助けるのに十分な程度に小さい軸方向タイプ
を可とする高速小形血液ポンプを提供することに
より従来の諸問題を解決することにある。斯かる
装置は末梢経皮挿入するものであり、しかもその
装置の流量は毎分当り3〜4程度とすることが
できる。
右心室の機能を支えるにはポンプを三尖弁およ
び肺動脈弁を経て肺動脈内へと前向きに挿入す
る。斯様な処置は末梢血管から大腿部静脈,外頚
静脈または頭部付近の静脈を経て行うことができ
る。
左心室の機能を支えるにはポンプを大動脈弁を
経て左心室へと逆向きに挿入して行う。これは鎖
骨下動脈または大腿部静脈のいずれかから行うこ
とができる。
ポンプを周囲から接近させるのが困難な場合に
は、少し胸壁切開して脈管接近させることにより
ポンプを心室先端部へと導入させることができ
る。
本発明によるポンプは全身の循環通路の部位に
小直径のカニユーレを経て延在させる可撓性ケー
ブルまたは水圧管路を介して人体の外側における
小形動力ユニツトによつて駆動させる。
必要な血液を放出させるためには、本発明のポ
ンプを10000〜20000rpmの軸速度で作動させる必
要がある。このような高速回転ポンプは以前は血
液ポンプの用途には適さないと考えられていた。
その理由は、羽根車チヤンバの固定壁部付近を通
る羽根車の羽根の先端部がゼブリ溶血を生ぜしめ
る程に大きい剪断力を生ずるであろうと一般に考
えられていたからである。本発明は、羽根の先端
部の線速度がポンプの直径に依存し、このために
本発明によるような小形ポンプでは斯かる線速度
が十分に小さくなり、ポンプ動作に必要な速度で
もさほど溶血を生じないと云う認識に基づいて成
したものである。
本発明によるポンプでは、その回転部と固定部
との間にパージシールを用いることとによつて血
栓形成を回避させる。さらに、軸受潤滑剤として
作用し得るエタノールまたは同様な静菌剤をパー
ジ液体として用いることによつて静菌効果を達成
することもできる。
従つて、本発明の目的は心臓の機能を助ける目
的のために大手術をすることなく脈管系を経て心
臓に迅速に接近させることができ、しかも患者へ
の危険度が低い高容量小形血液ポンプを提供する
ことにある。
本発明の他の目的は血液凝固を阻止する上述し
たタイプのポンプを提供することにある。

発明を実施するための最良の形態 本発明の脈管内血液ポンプ10は、適当に短く
て、可撓性の送り込みカニユーレ14を逆向きに
挿入することによつて大動脈弁15を経て心臓1
1の左心室12(第1a図)に接近させることが
できる。カニユーレ14は、それを有利に挿入さ
せることのできる大腿部18における大腿部動脈
17(第1b図)から大動脈16を経て案内させ
る。ポンプ10の作動時には血液が左心室12か
らカニユーレ14の入口素子19およびカニユー
レ14の内部を経てポンプ10内に矢印(第1a
図)の方向に送り出される。ポンプ10の出口に
は送り出された血液が出口素子20を経て大動脈
16へと放出される。
本発明ポンプを、ここでは大腿部動脈を経て逆
向きに挿入して左心室の機能を助けることについ
て述べたが、本発明ポンプはさほど変更しなくて
も所要に応じ他の挿入技法により上述したように
して左心室または右心室の機能を助けるように使
用することもできることは明らかである。
ポンプ10は大腿部動脈17および大動脈16
内に入れたカテーテル26を経て延在させる可撓
性の軸24によつて駆動させるのが好適である。
カテーテル26は下記に述べるような目的および
機能をするパージ流体用の導管としても作用す
る。ポンプ10に動力を与えるのには軸24代り
にカテーテル26を用いる水圧系を同等に用いる
ことができる。
第1a図および第1b図の配置では、患者の身
体の外側に置いた適当な支持ユニツト28によつ
て軸24に対する駆動力を与えると共に、カテー
テル26を経てポンプ10にパージ流体も与え
る。カテーテル26の直径は十分小さくして、そ
れを動脈に挿入する際に血液がそのまわりを比較
的支障なく流れるようにする。
次に第2図に転ずるに、ポンプ10はそのポン
プ本体に沿つてほぼ円筒形をしているハウジング
30を有している。入口素子19および出口素子
20にはテーパーを付けて、患者の脈管系を経て
のポンプ10の挿入および引出しをし易くする。
支持支柱38(これは固定子羽根としても作用す
る)はハウジング30を固定子支持軸受40およ
びチユーブ取付け円錘部42に接続する。
円筒状固定子ハブ40はケーブル24によつて
駆動されるような回転運動に対して回転子44を
支持する。回転子44の軸方向運動は軸受フラン
ジ46によつて防止される。回転子44は一連の
回転子羽根を支承し、これらの羽根の湾曲断面
(第4図に明示してある)は固定子羽根36のそ
れに似ている。かし、各羽根リングにおける回転
子羽根の数は、第3図に明示したように、各リン
グにおける固定子羽根の数とは相違させる(例え
ば、回転子羽根はリング当り7個とし、固定子羽
根はリング当り11個とすることができる)。
ジヤーナル軸受41はカテーテル26を経てポ
ンプ10内に圧力下で導入させるパージ流体を絶
えず流すことによつて滑らかにする。パージ流体
50はフエースシール52を経て流れ、最終的に
は界面56の個所にて血流54に放出される。
1983年9月28日出願の凝血抑制血液ポンプなる名
称の同時係属米国特許出願第537243号に記載した
ように、ポンプ10内での血栓形成は、環状界面
56に沿つてパージ流体を十分に流出し続けて界
面56の個所にて血液要素が回転子および固定子
表面に接触しないようにすることによつて防止さ
れる。
本発明のポンプは極めて小さくする必要があ
る。その外径は理想的には7〜9mm以下とすべき
である。ほぼ4/分の流量で必要な血液を流す
ためには、本発明のポンプをほぼ10000〜
20000rpmの速度で作動させる必要がある。ポン
プの作動時には、羽根36および48の先端部が
絶えず回転子44およびハウジング30にそれぞ
れ接近して動く第2図の58および60の個所に
おける羽根先の環状領域にて剪断作用が生ずる。
しかし、ポンプの寸法が極めて小さいため、領域
58にて生ずる最悪のケースでのハウジング30
に対する羽根48の先端部の相対運動は5m/秒
以下に留まる。この相対速度は、羽根先端部の適
当な隙間(ほぼ0.1mm)と相俟つてスペース58
内の剪断力を十分低いレベルに維持して、不当な
溶血を生ぜしめなくするに十分な低い値である。
スペース60の方がスペース58よりもポンプ1
0の軸線に近いために、固定子羽根36と回転子
44との間の相対速度が小さくなるので、スペー
ス60内での赤血球細胞膜の破壊は一層少なくな
る。
本発明は手術することなく、しかも患者への危
険も最小にして迅速にあてがうことができ、かつ
取外すことのできる有効な脈管心臓機能救援方法
および装置を提供すること明らかである。
