Description

【発明の詳細な説明】[Detailed description of the invention]

[産業上の利用分野] 本発明は生体血管に近似した応力−歪曲線およ
びコンプライアンスを有し、かつ管壁の内側から
外側まで厚さ全体にわたつて孔が存在する人工血
管とその製造方法に関する。 [従来の技術] 近年、血管外科手術の進歩とともに人工血管の
研究も進み、数多くの人工血管が開発されてきて
いる。現在、管内径約6mm以上の中口径あるいは
大口径動脈用人工血管としてはたとえば米国
USCI社製のダクロンの編物であるドベイスキー
人工血管や米国ゴア社製の延伸ポリテトラフルオ
ロエチレン(以下、EPTFEという)からなるゴ
アテツクスなどが臨床に用いられている。 これらの人工血管は、血管の内側から外側まで
連通している孔を有しており、生体に埋入後すみ
やかに仮性内皮によつて覆われ、生体組織側から
この孔を通して組織が侵入し、安定に器質化さ
れ、人工血管としての使命をはたしている。この
ように人工血管の器質化に役立つ連通孔を有する
ことを、以下有孔性を有するという。 しかし、これらの人工血管は、コンプライアン
スが生体血管と大きく異なるため、生体に埋入後
長時間経ると、吻合部にパンヌフ(pannus)の
過形成など種々の不適合に関する問題が発生す
る。また内径約6mm以下の小口径動脈人工血管と
して用いると、コンプライアンスの相違が顕著に
表われ、開存性がわるく、臨床に使用できない。
したがつて、膝から下の動脈や冠状動脈などの血
行再建手術には、自家静脈が使用されている。 以上のことから、人工血管とくに小口径動脈用
人工血管の開発にあたつては、人工血管が有孔性
を有することや、人工血管の素材の血液適合性を
向上させることに加えて、人工血管のコンプライ
アンスを生体血管に近似させることが重要である
といわれている。 しかし、現在開発されている人工血管のコンプ
ライアンスは、笹嶋らの報告(人工臓器12(1)、
179−182、1983)によれば、第1表の通りであ
る。

アンスの不一致を解決するため、米国特許第
4173689号明細書には人工血管を構成する材料と
してエラストマーを用い、管壁を多孔質とし、生
体血管に類似したコンプライアンスを有する人工
血管の製造方法に関する開示がなされている。し
かし、この人工血管は有孔性を有さない。しかも
製造された人工血管の管壁断面には非常に小さい
孔しか存在せず、比較的密な構造となつている。
このようにして製造された人工血管のコンプライ
アンスは従来の人工血管のコンプライアンスより
は大きくたるけれども、生体血管のそれと比較す
るとまだ小さい傾向にある、 本発明者は、このようなコンプライアンスの不
一致の問題を解決し、かつ器質化に最適な有孔性
を有するエラストマーからなる多孔体構造の人工
血管を開発し、特許出願をしてきている(特願昭
59−39972号、特願昭59−44396号、特願昭59−
44397号、特願昭59−44398号、特願昭59−51738
号、特願昭59−52674号明細書)。 [発明が解決しようとする問題点] 本発明者が開発してきた前記人工血管の応力−
歪曲線は、第4図の()に示したように、正常
な血圧範囲を超えた高い応力が加わると、生体血
管のそれ(第4図の()および())とは異
なつてくる。したがつて、手術時などのように異
常に高い血圧が生じたばあいには、破裂・損傷の
不安や長期間にわたる耐久性の維持に不安が残つ
ている。 本発明は前記のごとき不安を解消するためにな
されたものである。 [問題点を解決するための手段] 本発明は、有孔性を有し、生体血管に近似した
コンプライアンスを有することに加えて、生体血
管に近似した応力−歪曲線を有する人工血管をう
るためになされたものであり、エラストマーから
なり、連続的につながつている隔壁を有する多孔
体構造部分と、該多孔体構造部分の少なくとも一
部分に接触および(または)結合して存在する繊
維で構成された管状物部分とからなる有孔性を有
する人工血管であつて、コンプライアンスが0.1
〜0.8であり、かつ応力−歪曲線において、応力
0.01Kg/mm2に対応する点が歪0.2〜0.6の間に存在
し、その点の弾性率が0.1Kg/mm2以下であり、応
力0.05Kg/mm2に対応する点が歪0.5〜1.0の間に存
在し、かつ応力0.01Kg/mm2に対応する歪より大き
く、その点の弾性率が0.12Kg/mm2以上であり、応
力0.12Kg/mm2に対応する点の歪0.55〜1.2の間に存
在し、かつ応力0.05Kg/mm2に対応する歪より大き
く、その点の弾性率が0.2Kg/mm2以上であり、か
つ、応力0.05Kg/mm2における弾性率以上である人
工血管、および造孔剤および(または)曇点を有
するエラストマー溶液を心棒上にコーテイングし
たのち、凝固液に該心棒を浸漬する操作を1回ま
たは2回以上繰り返して人工血管を製造する際
に、前記操作のいずれかの段階で繊維で構成され
た管状物を前記心棒上に存在させることを特徴と
する前記人工血管の製造方法に関する。 [実施例] 本発明に用いるエラストマーとは、血液適合性
に優れた熱可塑性エラストマー、すなわち急性毒
性、炎症、溶血、発熱反応などを惹起するような
低分子溶出物を含まず、血液の生理機能に重大な
損傷を与えず、抗血栓性に優れた熱可塑性エラス
トマーである。このようなエラストマーとして
は、たとえばポリスチレン系エラストマー、ポリ
ウレタン系エラストマー、ポリオレフイン系エラ
ストマー、ポリエステル系エラストマーなどや、
これらのエラストマーにエラストマーとしての性
質を維持する範囲でエラストマー以外の高分子を
ブレンドしたものなどがあげられる。これらは単
独で用いてもよく、2種以上併用してもよい。こ
れらのうちでは臨界表面張力が35dyn/cmより小
さい疎水性のエラストマーがさらに好ましく、臨
界表面張力が30dyn/cmより小さい疎水性のエラ
ストマーがとくに好ましい。 エラストマーが疎水性になればなるほど、移植
された人工血管の外周囲を取り囲む生体軟組織と
の親和性の減少や、血液成分との相互作用の減少
や、耐水性の向上という好ましい現象が現われ
る。前記親和性が強いと、移植された人工血管を
取り囲む生体軟組織が厚く、かつ人工血管に強く
接着して生成する傾向にあり、これにともなつて
人工血管が収縮したり、変形したりする不安があ
る。 疎水性エラストマーのなかでは、強度、伸び、
耐久性、抗血栓性などに優れているなどの点か
ら、ポリエーテル型のセグメント化ポリウレタン
(セグメント化ポリウレタンウレアも含む、以下
同じ)がより好ましい。さらには、ハードセグメ
ントあるいはソフトセグメンタにフツ素を含有す
るセグメント化ポリウレタンや特開昭57−211358
号公報に開示されている主鎖中にポリジメチルシ
ロキサンを含有するセグメント化ポリウレタンが
好ましい。とくに好ましいものはソフトセグメン
トの一部にポリジメチルシロキサンを式: (式中、R1〜R6は炭素数1以上のアルキル基、
好ましくは炭素数2〜6のエチレン、プロピレ
ン、ブチレン、ヘキサメチレンなどのアルキレン
基、a、gは0〜30の整数、b、c、e、fは0
または1、dは2以上の整数を表わす)のような
形で、含有するセグメント化ポリウレタンであ
る。 本発明の人工血管の管壁は、前記エラストマー
からなる多孔体構造部分と、繊維から構成された
管状物部分とから構成されている。 該エラストマーからなる多孔体構造部分は、内
側から外側まで厚さ全体にわたつて孔が存在して
いる。該孔は少なくとも一部分が互いに連通して
おり、内側表面および外側表面には該孔の少なく
とも一部分が外部に向つて開口しており、有孔性
を有している。該孔を形成している隔壁はエラス
トマーからなり、連続的につながつている。さら
に前記隔壁自体も、その内部に微小な孔や穴を多
数含有することが疎な構造となり、生体血管に近
似したコンプライアンスや応力−歪曲線を有する
人工血管をうるために好ましい。このような疎な
構造である隔壁は、溶液のコーテイング物を乾燥
したり、溶融状態から製造したりするのは困難で
あるが、本発明の製造方法によると容易に製造し
うる。とくに好ましい多孔体構造は、管壁の内側
から外側まで厚さ全体にわたつて実質的に均一な
細孔が存在する網目状構造である。 管壁の内側表面近傍部分と外側表面近傍部分と
は、多孔体構造部分の大部分を占める両者の間の
部分に比しやや密になつていて、細孔が厚さ全体
にわたつて完全に均一でないことがある。しか
し、それが有孔性を損ねるほど極端なものでなけ
れば細孔は実質的に均一と解してよい。ここにい
う細孔の横断面の最大径にはとくに限定はない
が、1〜100μmであることが好ましく、3〜75μ
mであることがさらに好ましい。最大径が100μ
mより大きくなると強度が劣つたり、有孔性が大
きくなりすぎる傾向にあり、1μmより小さくな
ると有孔性が劣つたり、コンプライアンスが小さ
くなりすぎる傾向にある。 前記多孔体構造の内側表面と外側表面に存在す
る孔の形状にはとくに限定はないが、内側表面に
存在する孔の形状は円形あるいは楕円形であるこ
とが好ましい。この円形あるいは楕円形の最大径
は1〜100μmが好ましく、5〜50μmがさらに好
ましく、10〜30μmがとくに好ましい。最大径が
100μmより大きくなると、血液の流れがみださ
れて抗血栓性が低下する傾向にあり、1μmより
小さくなると人工血管の器質化が遅くなる傾向に
ある。 前記多孔体構造部分のコンプライアンスは生体
血管のそれに近似するか、それより大きな値を示
すものであればよい。コンプライアンスの値が生
体血管のそれより大きいばあいには、繊維で構成
された管状物との複合化により、生体血管に近似
したコンプライアンスに調節することが可能であ
る。このような多孔体構造部分のコンプライアン
スは、多孔体構造部分の中に占める孔の割合や孔
を形成する隔壁の強度や用いるエラストマーの強
度などにより調節しうる。 本発明に用いる繊維とは、糸、網、綱、織物、
編物、組物、不織物などをつくるのに使われる流
さが径の100倍以上の細くて長い物体である。前
記繊維は、生体に対して安全で、生体内での劣化
が無視でき、滅菌操作に耐え、目的の管状物に加
工できるものであれば、有機系、無機系を問わ
ず、とくに限定されることなく使用しうる。加工
性、入手の容易さ、しなやかさ、均一性などの点
からすると、再生人造繊維、半合成繊維、合成繊
維が好ましい。これらの具体例としては、セルロ
ース系、タンパス質系、ポリアミド系、ポリエス
テル系、ポリウレタン系、ポリエチレン系、ポリ
スチレン系、ポリ塩化ビニル系、ポリ塩化ビニリ
デン系、ポリフルオロエチレン系、ポリアクリル
系、ポリビニルアルコール系などの繊維があげら
れる。これらのうちでも伸縮性を有する繊維であ
ることがさらに好ましく、その具体例としては、
ゴム系、ポリウレタン弾性系、ポリエステル弾性
系などの繊維のように、繊維自体が伸縮性を有す
るものや、ウーリーナイロン、ウーリーテトロン
に代表される伸縮性かさ高加工系や、ゴムあるい
はスパンデツクスフイラメントを伸張状態にして
他の紡績糸あるいはフイラメントを巻き付けた糸
であるカバードヤーンなどがあげられる。 本発明に用いる繊維で構成された管状物とは、
前記繊維、前記繊維の少なくとも1種以上を紡績
した糸、前記繊維の少なくとも1種以上のマルチ
フイラメント、これらを組み合わせた糸などを用
いた織物、編物、組物、不織物、これらを組み合
わせたもの、あるいは発泡ポリウレタンなどから
なるスポンジ状物などからなる管状物のことであ
る。 前記管状物は繊維または繊維からつくられたも
の単独で管状構造に形成されていてもよく、エラ
ストマーからなる多孔体構造部分と組み合わされ
て人工血管になつたときに管状構造を形成するよ
うになつていてもよい。加工性、作業性、生体血
管に近似した応力−歪曲線をうるなどの点からす
ると、繊維の編物からなる管状物であることが好
ましく、伸縮性繊維をメリヤス編した管状物であ
ることがさらに好ましい。 管状物はエラストマーからなる多孔体構造部分
と組み合わせたとき、生体血管に近似したコンプ
ライアンスや応力−歪曲線を有するものとなるも
のであれば、管状物であるかぎりとくに限定はな
い。 該管状物のこのような性質は、たとえば次の2
つの方法の単独あるいは組み合わせで達成可能で
ある。 一番目は繊維の組み合わせ頻度を調節したり、
組み合わせる点をルーズに調節したりする方法で
ある。二番目は、伸縮性の繊維を使用する方法で
ある。 該管状物は、エラストマーからなる多孔体構造
部分に対して少なくとも一部分が接触および(ま
たは)結合して存在していればよく、たとえば第
2図に示したように、管状物2がエラストマーか
らなる多孔体構造部分1の外側に位置し、管状物
の一部分が多孔体構造部分1の外側表面に露出し
ている存在であつてもよく、第3図に示したよう
に、管状物2が多孔体構造部分1の内側の部分に
位置し、管状物2の一部分が多孔体構造部分1の
内側表面に露出していてもよい。さらに第1図に
示したように、管状物2が多孔体構造部分1の内
部に完全に取り込まれていてもよく、このばあい
には、管状物2を構成する繊維のほつれが阻止さ
れる、あるいは血液接触面の抗血栓性が保たれる
などの点から好ましい。 なお本発にいう接触および(または)結合と
は、エラストマーからなる多孔体構造部分と繊維
で構成された管状物部分とが、血圧や外部から加
わる応力に対してほぼ同じ歪を起こす程度に両者
の力学的相互作用があることを意味する。 生体血管のコンプライアンスは、動脈、静脈、
血管の口径などによつて異なる。したがつて、人
工血管として好ましいコンプライアンスは、人工
血管の口径や使用部位などによつて異なり、一概
には決められないが、本発明の人工血管はそれぞ
れの生体血管に近似したコンプライアンスを有す
るように製造しうる。通常の血行再建手術の行な
われる生体血管のコンプライアンスが0.1〜0.8程
度であるため、このような範囲にすることが好ま
しいと考えられる。本発明の人工血管のコンプラ
イアンスは前記のようにして調節することがで
き、0.1〜0.8の範囲で任意のものを製造すること
ができる。コンプライアンスが0.1〜0.8の人工血
管は、その太さなどにもよるが動脈用血管などの
用途に、また内径が1〜6mmであり、コンプライ
アンスが0.1〜0.5のものは小口動脈用人工血管と
して好適に使用しうる。 本明細書にいうコンプライアンスとは、式(1) C=△V/V0・△P×100 (1) (式中、Cはコンプライアンス、V0は内圧50mm
Hgのときの測定血管の内容積、△Pは内圧50mm
Hgから内圧150mmHgまでの100mmHg、△Vは内
圧50mmHgから内圧150mmHgまでの間に増加する
測定血管の内容積である)で定義されるものであ
る。実際の測定は閉鎖回路に測定血管を挿入し、
微量定量ポンプを用いてこの回路に液体を注入
し、注入液量と回路内の圧力の変化とを測定し、
(1)式からコンプライアンスを求めることができ
る。 本発明の人工血管の応力−歪曲線を第4図にも
とづいて説明する。 第4図において、()はエラストマーからな
る多孔体構造体、()は本発明の人工血管、
()は生体血管である胸部大動脈、()は生体
血管である頚動脈の各々の応力−歪曲線の一例で
ある。生体血管の応力−歪曲線は、動脈、静脈、
血管の口径、年齢、個体差などによつて異なるの
で定量化することは困難であるが、共通していえ
ることは、第4図の()、()に示したよう
に、正常な血圧範囲では小さな弾性率であり、正
常な血圧範囲をこえた応力が加わつてくると、急
激に大きな弾性率となる応力−歪曲線を有するこ
とである。第4図からわかるように、本発明の人
工血管()はこのような生体血管に近似した応
力−歪曲線を有する。 本明細書にいう応力−歪曲線が生体血管に近似
しているとは、第4図に示した()や()の
曲線に類似していることである。すなわち応力−
歪曲線において、応力0.01Kg/mm2に対応する点が
歪0.2〜0.6の間に存在し、その点の弾性率が0.1
Kg/mm2以下であり、応力0.05Kg/mm2に対応する点
が歪0.5〜1.0の間に存在しかつ応力0.01Kg/mm2に
対応する歪より大きく、その点の弾性率が0.12
Kg/mm2以上であり、応力0.12Kg/mm2に対応する点
の歪が0.55〜1.2の間に存在し、かつ応力0.05Kg/
mm2に対応する歪より大きく、その点の弾性率が
0.2Kg/mm2以上であり、応力0.05Kg/mm2における
弾性率以上であることをいう。さらには、応力
0.01Kg/mm2に対応する点が歪0.2〜0.6の間に存在
し、その点の弾性率が0.07〜0.007Kg/mm2であり、
応力0.05Kg/mm2に対応する点の歪が0.5〜1.0の間
に存在し、かつ応力0.01Kg/mm2に対応する歪より
大きく、その点の弾性率が0.2〜10Kg/mm2であり、
応力0.12Kg/mm2に対応する点の歪が0.55〜1.2の間
に存在し、かつ応力0.05Kg/mm2に対応する歪より
大きく、その点の弾性率が0.3Kg/mm2以上であり、
応力0.05Kg/mm2における弾性率以上である。 該応力−歪曲線は、高分子材料分野で通常使用
されている引張試験機(たとえば(株)島津製作所製
のオートグラフIS2000)を用いて人工血管を軸方
向に測定したものである。 応力−歪曲線の測定試料としては、人工血管を
そのまま使用することが好ましい。人工血管を軸
方向に切つて、たとえば短冊状にして測定する
と、繊維で構成された管状物の強度が変化し、異
なつた応力−歪曲線を示す傾向にある。人工血管
の円周方向の応力−歪曲線にはとくに限定はない
が、軸方向のそれに近似することが好ましい。 なお本発明にいう応力とは、引張試験中の荷重
を負荷前の試験片断面積(管壁断面積)で割つた
値であり、歪は引張試験中の伸びを負荷前の試験
片長さで割つた値である。 また本発明に用いる弾性率とは、応力−歪曲線
上の任意の位置における接線の勾配、所謂接線弾
性率である。 応力−歪曲線における小さい応力の領域は、コ
ンプライアンスを測定する正常な血圧範囲とほぼ
一致するので、コンプライアンスが生体血管に近
似していれば、応力−歪曲線のこの領域も生体血
管に近似する。一方、大きい応力の領域は、手術
時などの異常に高い血圧が生じたばあいの破裂・
損傷を防止したり、長期間にわたる耐久性の維持
をはかることがおもな役割であるから、これらの
役割を満たす範囲で近似すればよい。 本発明の人工血管では、正常な血圧範囲で測定
されるコンプライアンスや小さい応力の値は、お
もにエラストマーからなる多孔体構造部分の強度
で決まり、正常な血圧範囲をこえた応力が加わつ
た領域の大きい応力の値は、おもに繊維で構成さ
れた管状物の強度で決まる。そして、両者の組み
合わせにより、いろいろな部位の生体血管の応力
−歪曲線に近似させることが可能となる。 本発明の人工血管はこのように生体血管に近似
した応力−歪曲線を有するため、手術時などに異
常な血圧上昇が生じても、破裂したり損傷を受け
たりすることをなくすることが可能で、また移植
後の長期間にわたる耐久性の維持にも優れてい
る。 本発明の人工血管は血液適合性に優れたエラス
トマーから構成されているため、その内側、つま
り血液接触面の血液適合性は良好であるが、生体
への埋入初期の抗血栓性をさらに向上させる目的
で、表面にアルブミン、ゼラチン、コンドロイチ
ン硫酸、ヘパリン化材料などをコーテイングして
もよい。 以上説明したように、本発明の人工血管は、管
壁の内側から外側まで厚さ全体にわたつて孔が存
在するエラストマーからなる多孔体構造部分と、
該多孔体構造部分の少なくとも一部分に接触およ
び(または)結合して存在する繊維で構成された
管状物部分とから構成されて製造されているた
め、生体血管にコンプライアンスおよび応力−歪
曲線が近似している人工血管がえられる。 つぎに本発明の人工血管の製造方法について説
明する。 本発明の人工血管は造孔剤および(または)曇
点を有するエラストマー溶液を心棒上にコーテイ
ングしたのち、凝固液に該心棒を浸漬する操作を
1回または2回以上繰り返して人工血管を製造す

された管状物を前記心棒上に存在させることによ
り、製造することができる。 本発明に用いるエラストマー溶液は造孔剤を
有する溶液、曇点を有する溶液、造孔剤と曇
点を有する溶液に分類できる。 の造孔剤を含有するエラストマー溶液は、造
孔剤とエラストマーとエラストマーを溶解する溶
媒(以下、良溶媒という)を必須成分とし、造孔
剤が均一に分散されている。この溶液が凝固液に
浸漬されると良溶媒と凝固液の置換により、エラ
ストマーが析出する。えられたものから造孔剤を
溶解除去することにより、本発明の人工血管の多
孔体構造部分をうることができる。必要に応じ
て、良溶媒とはよく混和するがエラストマーを溶
解しない溶媒(以下、貧溶媒という)を、エラス
トマー溶液の凝固速度、多孔体構造などを調節す
る目的で添加してもよい。 曇点を有するエラストマー溶液はエラストマ
ーと良溶媒と曇点を形成するために必要な量の貧
溶媒とを必須成分とする。曇点とは高分子が溶解
している状態からコロイド状に析出する、つまり
相変化を起こす温度である。この溶液は曇点温度
以下で取り扱うと均一なコーテイングが難かし
く、多孔体構造をえがたい傾向にあるので、該溶
液を曇点を超える温度に保つて心棒上にコーテイ
ンズしたのち、ただちにあるいは相変化を生じさ
せたのち曇点温度以下の凝固液に該心棒を浸漬す
ることが好ましい。この操作により、コーテイン
グ層でのエラストマー溶液の相変化と凝固液中で
のエラストマーの析出とを同時に、あるいは順々
に生じさせることができ、多孔体構造部分をうる
ことができる。 の造孔剤と曇点を有するエラストマー溶液
は、造孔剤とエラストマーと良溶媒と曇点を形成
するために必要な量な貧溶媒とを必須成分とす
る。この溶液はの溶媒と同様にしてエラストマ
ー溶液中のエラストマー濃度は、エラストマーを
析出させたのち、造孔剤を溶解除去することによ
り本発明の多孔体構造部分をうることができる。 前記エラストマーの種類や溶液の組成によつて
異なるので一概に限定することはできないが、通
常は5〜35%(重量%、以下同様)であることが
好ましく、10〜30%であることがさらに好まし
く、12.5〜25%であることがとくに好ましい。エ
ラスマー濃度が5%より低いと均一な成形が困難
になる傾向にある。またエラストマー濃度が35%
をこえると、溶液の粘度が高いため、均一なコー
テイングなどが困難になる傾向にある。 本発明に用いる造孔剤は、良溶媒に不溶なもの
であり、製造した人工血管から除去できるもので
ある。生体内に埋入する人工血管に用いることを
考えると、造孔剤としても生体に対して安全なも
のを用いることが好ましい。それゆえ造孔剤とし
ては、食塩や炭酸カルシウムのような無機塩類、
グリコースやデンプンのような水溶性糖類あるい
は蛋白質などが好ましい。しかし、食塩のような
無機塩類や溶性糖類などは本質的に吸湿性である
ため、微細粒径にすると表面積が増大し、空気中
の湿気などで二次凝集をおこしやすい傾向にあ
り、取り扱いに十分注意をはらう必要がある。一
方、蛋白質な微粒子にしても空気中の湿気などで
二次凝集をおこすこともなく、均一に安定した分
散が可能である。それゆえに造孔剤としては、蛋
白質がとくに好ましい。 また蛋白質を用いると、成形した人工血管か
ら、アルカリ液、酸液あるいは酵素を含む液など
によつて、該造孔剤を容易に溶解除去しうる。こ
のように好ましい蛋白質の具体例としては、カゼ
イン、コラーゲン、ゼラチン、アルブミンなどが
あげられ、これらのうちではカゼインがとくに好
ましい。 前記造孔剤の粒径は74μm以下が好ましく、
50μm以下さらに好ましく、30μm以下であらこ
とがとくに好ましい。ここでいう粒径とは篩の目
の一辺の長さを表わし、これらの篩で分級された
粒子のことである。粒径が74μmより大きくなる
と多孔体構造の孔が大きくなり過ぎる傾向にあ
る。 本発明に使用する造孔剤の量は、必要とされる
有孔性、造孔剤の粒径およびエラストマー溶液の
組成(とくに曇点の有無)とによつて変化するの
で一概に決定することはできないが、好ましくは
20〜500%(造孔剤/エラストマーの容量%、以
下同様)、さらに好ましくは50〜350%、とくに好
ましくは100〜300%である。造孔剤の量が500%
をこえると有孔性が大きくなりすぎたりエラスト
マー溶液の粘度が高くなりすぎる傾向にある。一
方、造孔剤の量が20%未満になると有孔性が乏し
くなる傾向にある。 本発明に用いる良溶媒は、エラストマーの種類
によつて変化するので一概には決定できないが、
たとえばN,N−ジメチルアセトアミド、N,N
−ホルムアミド、N−メチル−2−ピロリドン、
ジオキサン、テトラヒドロフランなどの溶媒あげ
られ、これらを単独で用いてもよく、2種以上併
用してもよい。 本発明に用いる貧溶媒は、良溶媒とはよく混和
するがエラストマーを溶解しない溶媒であるばよ
く、たとえば水、低級アルコール類、エチレング
リコール、プロピレングリコール、1,4−ブタ
ジエンオール、グリセリンなどがあげられ、これ
らを単独で用いてもよく、2種以上併用してもよ
い。 本発明に用いる凝固液は、実質的に貧溶媒であ
ればよい。たとえば水、低級アルコール類、エチ
レングリコール、プロピレングリコール、1,4
−ブタンジオール、グリセリンなどがあげられ、
これらを単独で用いてもよく、2種以上併用して
もよい。これらのうちで好ましいものとしては、
エチレングリコール、プロピレングリコールある
いはこれらを主成分とする貧溶媒があげられ、さ
らに好ましいものとしては前記貧溶媒99〜50容量
%に1〜50容量%の良溶媒を添加した混合溶媒で
ある。なぜなら凝固液に良溶媒を含ませることに
より、凝固液中でのエラストマー溶液の凝固速度
が遅くなることに起因するためか、有孔性が容易
にえられるようになる。 本発明に用いる心棒は、エラストマー溶液に溶
解しない限りとくに限定されるものではなく、た
とえば表面が滑らかなガラス棒、テフロン棒ある
いはステンレス棒などが好適に使用される。また
心棒のかわりに任意の形の型を用いることによ
り、管状の成型物以外の各種の医療成形体が本発
明の方法によつて製造しうることはいうまでもな
い。たとえば型として平板を用いれば膜状の成形
体がえられ、これは人工皮膚などに利用しうる。 繊維で構成された管状物を心棒上に存在させる
方法としては、繊維で構成された管状物で心棒を
覆う方法や、繊維や繊維からなる帯状物などを管
状構造になるように心棒上に巻き付ける方法が最
も代表的である。繊維で構成された管状物は、心
棒上に直接存在させてもよいし、エラストマーが
析出した心棒上に存在させてもよいが、エラスト
マーが析出した心棒上に存在させたのち、エラス
トマー溶液のコーテイングとエラストマーの析出
を1回または2回以上行なう方法が好ましい。 このようにしてえられた本発明の人工血管は、
つぎのような優れた性質を有している。 人工血管の器質化に役立つ有孔性を有する。 コンプライアンスが生体血管に近似してい
る。 応力−歪曲線が生体血管に近似している。 さらに、本発明の人工血管の管壁が実質的に
エラストマーの連続した多孔体構造に、繊維で
構成された管状物の少なくとも一部分が接触お
よび(または)結合している構造物であるた
め、つぎに示すような有用な性質をも併用す
る。 縫合針の貫通性がよく、縫合が容易である。 縫合針の貫通孔が自己閉塞する。 血圧のかかつた実際の使用状態では結節を生
じ難い。 このような性質を有する本発明の人工血管は、
生体に移植した際の開存性に優れており、吻合部
におけるパンヌフの過形成などの問題もほとんど
発生しない。また手術時などに異常に高い血圧が
生じたばあいの破裂・損傷の不安もほとんどな
く、長期間にわたる耐久性の維持にも優れてい
る。さらには血行再建手術時の作業性に優れてい
る。 したがつて本発明の人工血管は、動脈や静脈な
どの血行再建手術にあたつて、人工血管、バイパ
ス用人工血管、パツチ用材料に使用できるととも
に、ブラツドアクセスなどにも使用することがで
きる。とくに0.1〜0.8のコンプライアンスを有す
る動脈用人工血管として用いることが好ましい。
とくにコンプライアンスと応力−歪曲線が生体血
管に近似しているため、現在臨床に使用する人工
血管が存在しない0.1〜0.5のコンプライアンスを
有し、内径約1〜6mmの小口径動脈用人工血管と
しても使用できる。それゆえ膝から下の動脈の血
行再建や、大動脈−冠状動脈バイパス用人工血管
として好適に使用しうる。また本発明の人工血管
は、尿管などの生体の柔らかい管状物の代替えと
しての使用も可能である。 つぎに実施例にもとづいて本発明の人工血管お
よびその製造方法を説明する。 実施例 1 4,4′−ジフエニルメタンジイソシアネート
27.35部(重量部、以下同様)とポリオキシテト
ラメチレングリコール(分子量2000)54.7部を用
いてプレポリマーを合成したのち、エチレングリ
コール4.75部と両末端ポリエチレングリコールポ
ルジメチルシロキサン(両末端のポリエチレング
リコールの平均分子量681、ポリジメチルシロキ
サンの平均分子量1040)13.2部を用いて鎖延長を
行ない、主鎖にポリジメチルシロキサンを含有す
るセグメント化ポリウレタンを合成した。 えられたポリウレタンの抗張力は350Kg/cm2、
伸びは670%であり、ジスマンプロツトから求め
た臨界表面張力は28dyn/cmであつた。 ジオキサン50mlとN,N−ジメチルアセトアミ
ド30mlとの混合溶媒に、粒径30μm以下のカゼイ
ン22.5gをホモジナイザーで撹拌分散させ、つい
で前記ポリウレタン15gを加えて撹拌溶解させ
た。この分散溶液に直径3mmのガラス棒を浸漬し
たのち取り出し、ガラス棒上に前記分散溶液を均
一にコーテイングした。この心棒をエチレングリ
コールに浸漬し、エラストマーを析出させた。 つぎに20デニールのスパンデツクスに70デニー
ルのナイロン繊維を巻き付けたカバードヤーンを
用いて12針のヘツドを有するリボン編機で編んだ
直径約3mmの管状物を前記エラストマーが表面に
析出しているガラス棒にかぶせた。該ガラス棒を
前記分散溶液に浸漬させたのち取り出して、表面
に前記分散溶液を付与させたのち、前記凝固液に
浸漬させた。充分にエラストマーを析出させたの
ち該心棒を取り出し、表面の凝固液を除去した。
さらに前記と同様の方法で心棒上に分散溶液を付
着させたのち、凝固液中でエラストマーを析出さ
せた。そののち充分水洗を行ない、ガラス棒を抜
き取り、複合管成物をえた。この複合管状物をPH
13.5の水酸化ナトリウム水溶液に浸漬し、カゼイ
ンを抽出除去し、最後に充分水洗を行ない、本発
明による人工血管をえた。 えられた人工血管は内径約3mm、外径約4.5mm
であつた。この人工血管を走査型電子顕微鏡で観
察したところ、内側表面には約20〜30μmの円形
〜楕円形の孔が存在し、外側表面には約1〜10μ
mの円形ないし不定形の孔が存在した。管壁断面
には中央部に繊維で構成された管状物が取り込ま
れており、残りのエラストマーの部分は孔の隔壁
を構成する連結したエラストマーからなる網目状
構造となつていた。 つぎにこの人工血管に120mmHgの水圧で水をと
おしたところ、人工血管の内側表面1cm2当り1分
間に約50mlの水が人工血管の外側表面に浸透し、
有孔性を有することがわかつた。 つぎにこの人工血管を牛血でプレクロツトリン
グしたのち、長さ8cmに切つて、閉鎖回路に挿入
し、1ストローク0.05ml送液する定量ポンプで牛
のACD血液をこの閉鎖回路に送液して、内圧の
変化を測定した。定量ポンプのストローク数と内
圧の変化とから、(1)式よりコンプライアンスを測
定したところ、0.30であつた。 この人工血管の応力−歪曲線を島津オートグラ
フIS2000を用いて測定したところ、生体血管に近
似している第4図に示した()のグラフがえら
れた。 つぎにこの人工血管を雑種成犬の大腿動脈に約
7cmの長さで移植したところ、2カ月以上の開存
性を示した。 またこの人工血管は、任意の箇所で切断しても
切断部分がほつれることなく、縫合性にも優れ、
縫合針の貫通孔は針を除くと自己閉塞した。また
該人工血管は、内圧が50〜150mmHg存在する状態
で結節を生じ難い傾向にある。 以上のことからこの人工血管は優れたものであ
り、とくに小口径動脈用人工血管として優れてい
ることがわかる。 実施例 2 粒径30μm以下のカゼイン22.5gを、プロピレ
ングリコール45ml、ジオキサン57.8ml、N,N−
ジメチルアセトアミド24.8mlの混合溶媒に、ホモ
ジナイザーを用いて撹拌分散させた。この分散液
を実施例1で用いたポリウレタン22.5gに添加し
て、80℃でポリウレタンを溶解した。えられた溶
液の曇点は約45℃であつた。80℃に保つたこの溶
液に、直径3mmのガラス棒を浸漬したのち取り出
し、ガラス棒上に前記溶液を均一にコーテイング
した。ついで18℃の水にこのガラス棒を浸漬し、
エラストマーを析出させた。 つぎに2デニールのウーリーテトロンを約48本
合わせた糸を用いて12針のヘツドを有するリボン
編機で非常に疎な構造に編んだ直径約3mmの管状
物を前記エラストマーが表面に析出しているガラ
ス棒にかぶせた。該ガラス棒を前記分散溶液に浸
漬させたのち取り出して、表面に前記分散溶液を
付着させたのち、凝固液に浸漬させた。そののち
充分水洗を行ない、ガラス棒を抜き取り複合管状
物をえた。この複合管状物をPH13.5の水酸化ナト
リウム水溶液に浸漬し、カゼインを溶解除去し、
最後に充分水洗を行ない、本発明の人工血管をえ
た。えられた人工血管は内径約3mm、外径約4.5
mmであつた。この人工血管の内面、外面、管壁断
面を走査型電子顕微鏡で観察したところ、内面に
は約15〜20μmの楕円形の孔が存在し、外面には
約5μmの不定形の孔が存在した。管壁断面には、
中央部に繊維で構成された管状物があり、残りの
エラストマーの部分は均一な網目状構造となつて
いた。 実施例1と同じ方法で透水量、コンプライアン
スおよび応力−歪曲線を測定したところ、透水量
は約110ml、コンプライアンスは0.25であつた。
この人工血管の応力−歪曲線は応力0.01Kg/mm2に
対応する歪が0.35であり、その点の弾性率が0.06
Kg/mm2であり、応力0.05Kg/mm2に対応する歪が
0.65であり、その点の弾性率が0.3Kg/mm2であり、
応力0.12Kg/mm2に対応する歪が0.75であり、その
点の弾性率は1.0Kg/mm2であつた。 実施例 3 実施例1記載のポリウレタン15gをN,N−ジ
メチルアセドアミド55mlとプロピレングルコール
30mlとの混合溶媒に70℃で撹拌溶解した。この溶
液の曇点は約40℃であつた。 20デニールのスパンデツクスに30デニールのダ
クロン繊維を巻き付けたカバードヤーンを用いて
実施例1の方法で編んだ直径約3mmの管状物をか
ぶせたガラス棒を、70℃に保つた前記溶液に浸漬
したのち取り出し、ガラス棒上に前記溶液を均一
にコーテイングした。コーテイング層が白濁する
まで空気中に放置したのち、15℃の水にガラス棒
を浸漬し、混合溶媒を水で充分置換した。そのの
ちこの操作をもう一度繰り返し、充分水洗を行な
つたのち、ガラス棒を抜き取り人工血管をえた。 えられた人工血管は内径約3mm、外径約4.5mm
であつた。この人工血管の内面、外面、管壁断面
を走査型電子顕微鏡で観察したところ、内面には
最大約10μmの楕円形の孔がほぼ均一に存在し、
部分的に管状物の繊維が観察された。外面には内
面より最大径の小さい孔が存在していた。管壁断
面には繊維からなる管状物部分と孔を形成する隔
壁が連即的につながつた多孔体構造部分とが存在
した。 実施例1と同じ方法で透水量、コンプライアン
スおよび応力−歪曲線を測定したところ、透水量
は約20ml、コンプライアンスは0.35であつた。こ
の人工血管の応力−歪曲線は応力0.01Kg/mm2に対
応する歪が0.45であり、その点の弾性率が0.045
Kg/mm2であつた。また応力0.05Kg/mm2に対応する
歪が0.8であり、その点の弾性率が0.21Kg/mm2で
あり、応力0.12Kg/mm2に対応する歪が1.0であり、
その点の弾性率が0.5Kg/mm2であつた。

After the coating layer was left in the air until it became cloudy, the glass rod was immersed in water at 15°C, and the mixed solvent was sufficiently replaced with water. Thereafter, this operation was repeated once more, and after thorough washing with water, the glass rod was removed and an artificial blood vessel was obtained. The resulting artificial blood vessel has an inner diameter of approximately 3 mm and an outer diameter of approximately 4.5 mm.
It was hot. When the inner surface, outer surface, and cross section of the tube wall of this artificial blood vessel were observed using a scanning electron microscope, oval pores with a maximum size of about 10 μm were almost uniformly present on the inner surface.
Tubular fibers were observed in some parts. There were pores with a smaller maximum diameter on the outer surface than on the inner surface. In the cross section of the tube wall, there existed a tubular part made of fibers and a porous structure part in which partition walls forming pores were continuously connected. When the water permeation amount, compliance and stress-strain curve were measured in the same manner as in Example 1, the water permeation amount was about 20 ml and the compliance was 0.35. In the stress-strain curve of this artificial blood vessel, the strain corresponding to a stress of 0.01Kg/ mm2 is 0.45, and the elastic modulus at that point is 0.045.
It was Kg/ mm2 . Also, the strain corresponding to a stress of 0.05Kg/mm 2 is 0.8, the elastic modulus at that point is 0.21Kg/mm 2 , and the strain corresponding to a stress of 0.12Kg/mm 2 is 1.0.
The elastic modulus at that point was 0.5 Kg/mm 2 .

造部分の少なくとも一部分に接触および(また

分とからなる有孔性を有する人工血管であつて、
コンプライアンスが0.1〜0.8であり、かつ応力−
歪曲線において、応力0.01Kg/mm2に対応する点が
歪0.2〜0.6の間に存在し、その点の弾性率が0.1
Kg/mm2以下であり、応力0.05Kg/mm2に対応する点
が歪0.5〜1.0の間に存在し、かつ応力0.01Kg/mm2
に対応する歪より大きく、その点の弾性率が0.12
Kg/mm2以上であり、応力0.12Kg/mm2に対応する点
の歪が0.55〜1.2の間に存在し、かつ応力0.05Kg/
mm2に対応する歪より大きく、その点の弾性率が

れた管状物部分とからなる有孔性を有する人工血
管であつて、コンプライアンスが0.1〜0.8であ
り、かつ応力−歪曲線において、応力0.01Kg/mm2
に対応する点が歪0.2〜0.6の間に存在し、その点
の弾性率が0.1Kg/mm2以下であり、応力0.05Kg/
mm2に対応する点が歪0.5〜1.0の間に存在し、かつ
応力0.01Kg/mm2に対応する歪より大きく、その点
の弾性率が0.12Kg/mm2以上であり、応力0.12Kg/
mm2に対応する点の歪が0.55〜1.2の間に存在し、
かつ応力0.05Kg/mm2に対応する歪より大きく、そ
の点の弾性率が0.2Kg/mm2以上であり、かつ、応
