Abstract

(57)【要約】 （修正有） 【構成】活性成分として溶液の１０〜２０％（ｐ／容
量）のケトプロフェンリジン塩を含む泡沫の形体の親水
性医薬組成物。 【効果】この泡沫は関節、筋肉、腱及び靭帯のリューマ
チ性又は外傷性の痛みがあり且つ炎症の状態の局部治療
に適用される。

Description

【発明の詳細な説明】
【０００１】本発明の目的は活性成分としてアリールプ
ロピオン酸の類に属する抗炎症活性を有する化合物を含
む泡沫の形体の局所用医薬組成物に関する。
【０００２】より詳しくは、本発明の目的の組成物に含
まれるアリールプロピオン酸はケトプロフェンリジン塩
としても知られる２−（３−ベンゾイルフェニル）プロ
ピオン酸のリジン塩として表され、この化合物は１９８
６年１０月２１日に発行された本出願人名義のイタリア
特許第１,１０４,５５６号の目的物を構成するものであ
る。
【０００３】大多数の非ステロイド性抗炎症剤又はＦＡ
ＮＳとして知られる２−（３−ベンゾイルフェニル）プ
ロピオン酸又はケトプロフェンはその作用機序が本質的
にプロスタグランジン、プロスタサイクリン及びトロン
ボキサンの生合成の阻害に由来する。ケトプロフェンに
比べてケトプロフェンリジン塩は同様の薬学的プロフィ
ールと類似の消炎鎮痛効果を示すにもかかわらず相当に
高い水中の溶解性と大きな胃の耐性という有利な条件を
提供する。筋肉骨格器官は特に起こり得る体の外傷の結
果として局部炎症過程が形成されやすく、そのため自由
に使える局所非ステロイド性抗炎症（ＦＡＮＳ）薬を含
む組成物は便利である。局所投与経路は確かに局部炎症
過程の場合において非ステロイド性抗炎症薬のために選
択される一つの投与経路であり、なぜならそれは大多数
のそのような医薬を用いる治療の間に現れる付帯的な影
響特に胃と腎臓におけるその出現を最小にするからであ
る。
【０００４】その表面局所効果のために使用する薬物を
別にして、皮膚に適用される薬物は適当な皮膚層を浸透
して所望の治療効果を産生しなければならない。従って
ＦＡＮＳはその作用が効果を現すには角質層と表皮の生
きている部分を通過しなければならない。角質化された
組織である角質層は人工的な半浸透膜のような挙動を示
し、従って薬物分子は濃度の勾配による受動拡散により
浸透する。従ってこの角質層を通過する浸透速度は担体
中の薬物の濃度、その水中の溶解性及びその角質層と担
体との間の分配係数に依存する。
【０００５】そこから抗炎症薬が所望の効果を発揮し得
るためにはそれを高められた濃度で投与すべきこと、従
ってそれを局所投与用医薬形体の中に高濃度で含ませな
ければならないという結論に導かれる。
【０００６】本発明の泡沫の形体の局所用医薬組成物は
ケトプロフェンリジン塩の１０〜２０％の濃度を得るこ
とが可能であり、そのような濃度は他の剤形例えばゲル
では得ることができない。本発明の局所用医薬組成物に
より与えられる別の有利な条件はそれが皮膚とある程度
相互作用することのあり得る賦形剤を極めて低い量含む
ことにより示される。特に泡沫の形体の医薬組成物にお
ける前記賦形剤の量は全部で溶液の５％〜１０％（ｐ／
容量）の百分率であり、更にアルコール賦形剤は極めて
低い含有量すなわち５％より低いそれであるが、一方伝
統的な局所用医薬形体例えばＦＡＮＳを含むゲル又はク
リームにおいてはアルコール賦形剤の百分率は遥かに高
く、それらは溶液の４０％（ｐ／容量）に達する場合も
ある。
【０００７】泡沫の形体の医薬組成物は１０〜２０％の
ケトプロフェンリジン塩溶液、好ましくは１５％のケト
プロフェンリジン塩溶液を含み、そしてエロゲート（er
ogate）される場合に適当なガス例えば一般にブタン−
プロパンと呼ばれるブタン、プロパンおよびイソブタン
ガスの混合物、好ましくはブタン：プロパン：イソブタ
ン＝４：４：２の比率で混合したものを溶液の３〜７％
（ｐ／ｐ）量で用いて乳化される。
【０００８】溶液の５％（ｐ／ｐ）に相当するガス混合
物の量が特に好ましい。泡沫の形体の医薬組成物には他
の成分例えば界面活性剤又はイオン性又は非イオン性乳
化剤、皮膚軟化剤、粘稠剤及び場合により少量の香り及
び静菌物質も含まれる。イオン性乳化剤としてはラウリ
ル硫酸カリウムを使用することができ、そして非イオン
性乳化剤としては非エトキシル化誘導体例えばソルビタ
ンエステル又はエーテル（ＳＰＡＮ）又はエトキシル化
誘導体例えばポリオキシエチレンソルビタンエーテル又
はエステル及びポリオキシメチレンエーテル及びエステ
ル（Cremophor,Myri）を使用することができる。乳化剤
の量は溶液の２〜８％(ｐ／容量）である。イオン性乳
化剤の特に好ましい量は溶液の２〜３％（ｐ／容量）で
あり、一方非イオン性乳化剤についてはその量は溶液の
３〜５％（ｐ／容量）である。
【０００９】皮膚軟化剤としてはプロピレングリコー
ル、グリセリン又は３００〜３,０００の平均分子量を
もつポリエチレングリコール（Carbowax of Polywax 30
0-1,500-3,000）を使用することができる。皮膚軟化剤
の量は溶液の２〜６％（ｐ／容量）の間で変動し、溶液
の３〜４％（ｐ／容量）の量が特に好ましい。
【００１０】粘稠化物質としては植物起源の飽和又は不
飽和脂肪酸の置換されたアミド（Comprelan-Henkel）；
置換された及び／又は塩化セルロース例えばメチルエチ
ルセルロース、ナトリウム・カルボキシメチルセルロー
ス、メチルセルロース、イソプロピルメチルセルロー
ス；種々の分子量の置換されたビニルピロリドンポリマ
ー例えばポリビニルピロリドンＫ３０；カルボキシポリ
メチレン（Carbopol）を使用することができる。粘稠剤
の量は溶液の０.２〜３％（ｐ／容量）の間で変動す
る。場合により添加する香りは溶液の０.０５〜０.２％
（ｐ／容量）の量のネロリンラベンダーから構成させる
ことができる。
【００１１】厳密には必要でないが、時間が経過した場
合微生物学的純度を保つために添加することができる静
菌物質は安息香酸とそのエステル（Nipagin-Nipasol）
又はベンジルアルコールから構成させることができ、そ
して溶液の０.１〜０.５％（ｐ／容量）の量を添加す
る。本発明の目的の泡沫形体の局所用抗炎症医薬組成物
は適用が簡単であり、そして関節、筋肉、腱及び靭帯の
リューマチ性又は外傷性の痛みがあり且つ炎症の状態の
局部治療に適している。適当な薬量検討は治療するべき
面積と炎症の実態により泡沫を治療する部位に１日２〜
３回適用し、軽く摩擦して浸透させることを予想する。
【００１２】次の実施例は本発明の目的を限定するので
はなく例証するためにのみ提示するものである。
【００１３】実施例 １ ケトプロフェンリジン塩（１.５kg）を撹拌しながら浄
化した水（６.５リットル）に溶解する。
【００１４】別の容器にPolysorbate 80（０.４kg）；
プロピレングリコール（０.４kg）；ＰＶＰ Ｋ２５(ポ
リビニルピロリドン)(０.３kg）；ネロリンラベンダー
（０.０２kg）；ベンジルアルコール(０.０３kg）を仕
込み、十分量の水で１リットルにする。
【００１５】第二の溶液を撹拌しながら第一の溶液に添
加し、次いで水で所定量（１０リットル）にする。
【００１６】pHを調節する(範囲限界：７〜８.０）。加
圧濾過し小型ビンに分注する。バルブを小型ビンに取り
付け固定する。ブタン−プロパンガス（５％ｐ／ｐ）を
装填して加圧状態にする。
【００１７】１００mlの溶液の最終組成は次の通りであ
る。
【００１８】 ケトプロフェンリジン塩 １５.０ｇ Ｐｏｌｙｓｏｒｂａｔｅ ８０ ４.０ｇ プロピレングリコール ４.０ｇ ポリビニルピロリドン(ＰＶＰ Ｋ２５) ３.０ｇ ネロリンラベンダー ０.２ｇ ベンジルアルコール ０.３ｇ 十分量の浄化した水で１００mlにする。
【００１９】実施例 ２ 適当に佐薬を変更して実施例１に記述したのと同様に操
作を行い、次の最終組成が得られる。
【００２０】 ケトプロフェンリジン塩 １５.００ｇ ラウリル硫酸ナトリウム ２.００ｇ ポリエチレングリコール(Carbowax 300) ５.００ｇ ナトリウム・カルボキシメチルセルロース b.v. ０.２５ｇ ネロリンラベンダー ０.２０ｇ Ｎｉｐａｇｉｎ ０.１０ｇ Ｎｉｐａｓｏｌ ０.０２ｇ 十分量の浄化した水で１００mlにする。
【００２１】実施例 ３ 適当に佐薬を変更して実施例１に記述したのと同様に操
作を行い、次の最終組成が得られる。
【００２２】 ケトプロフェンリジン塩 １５.００ｇ Ｓｐａｎ ２０ ５.５０ｇ グリセリン ３.００ｇ ヒドロキシプロピルメチルセルロースＥ ３ ０.３５ｇ ネロリンラベンダー ０.２０ｇ 安息香酸 ０.１２ｇ 十分量の浄化した水で１００mlにする。
【００２３】実施例 ４ 適当に佐薬を変更して実施例１に記述したのと同様に操
作を行い、次の最終組成が得られる。
【００２４】 ケトプロフェンリジン塩 １５.００ｇ Ｃｒｅｍｏｐｈｏｒ ＥＬ ４.００ｇ プロピレングリコール ４.００ｇ メチルエチルセルロース ０.３０ｇ ネロリンラベンダー ０.２０ｇ Ｎｉｐａｇｉｎ ０.１２ｇ 十分量の浄化した水で１００mlにする。
【００２５】実施例 ５ 適当に佐薬を変更して実施例１に記述したのと同様に操
作を行い、次の最終組成が得られる。
【００２６】 ケトプロフェンリジン塩 １５.００ｇ Ｍｙｒｊ ５２ ５.００ｇ グリセリン ３.００ｇ ポリビニルピロリドンＫ ３０ ４.００ｇ ネロリンラベンダー ０.１５ｇ ベンジルアルコール ０.３０ｇ 十分量の浄化した水で１００mlにする。
【００２７】ケトプロフェンリジン塩の親水性溶液の吸
収を雌ウサギ（ニュージーランド、９〜１１週令）を使
用し、実施例１に記述したすべての賦形剤組成を持つ種
々の濃度（５％、１０％、１５％及び２０％）の泡沫の
形体の組成物を局所投与して研究した。
【００２８】この泡沫を予め除毛した（処理前２４時
間）４群のウサギ（ｎ＝３）の背中の１００cm2(１０cm
×１０cm）の面積に１０mg／kg（動物の平均体重２００
０ｇ±２００ｇ）を投与した。
【００２９】動物は処理直後メタボリックケージに入れ
た。２mlの血液を投与後０（投与前）、１５、３０、６
０、１２０、２４０及び３６０分に採取し、そして尿を
０〜６時間、６〜１２時間及び１２〜２４時間の間に集
めた。
【００３０】血漿及び尿中のケトプロフェン酸の水準の
測定をスプロフェン酸を内部標準（ＩＳ）として使用す
るＨＰＬＣ法により行った。
【００３１】血漿の試料をＩＳ添加後１Ｍリン酸緩衝液
でpH＝２.０に酸性化し、エチルエーテルで抽出した。
尿はＩＳ添加後１Ｎ ＮａＯＨで３０分間アルカリ加水
分解し、次いで１Ｎ ＨＣｌを添加して中和し、最後に
１Ｍリン酸緩衝液でpH＝２.０の酸性にし、次いでエチ
ルエーテルで抽出した。血漿と尿のエーテル層を窒素の
もとで蒸発乾燥した。血漿と尿抽出液の残留物を０.２
Ｍギ酸アンモニウム（pH＝６.５）に取り上げ、ＨＰＬ
Ｃに注入した（注入量２０μｌ）。
【００３２】クロマトグラフィーの条件は次の通りであ
る。 カラム：Merck Lichrospher 10RP-18 ５μｍ １２５×
４mm、移動相プレカラム 付き：０.２Ｍギ酸アンモニウム pH（３５％ＣＨ3Ｃ
Ｎ） 流速：１ml／分 検出：ＵＶ（２５８nm）
【００３３】適用した方法は血漿については０.０２０
〜２μg／ml(ｒ2 ０.９９）、そして尿については０.０
５〜２５μg／ml（ｒ2 ０.９９）の測定範囲で線型であ
る。この方法の限界は０.０１μg／mlであり、そして検
出可能な最小濃度は血漿と尿のいずれについても０.０
５μg／mlである。
【００３４】ウサギに５％、１０％、１５％及び２０％
の泡沫形体のケトプロフェンリジン塩の親水性溶液を１
０mg／kgの用量で投与した場合血漿水準では方法の検出
限界より高い値が測定されないが、一方尿中には投薬可
能な水準のケトプロフェンが認められた。
【００３５】表１は各動物の全排泄量と関連する平均
(±ＳＤ)の値を示す。表１は５％、１０％、１５％及び
２０％溶液を適用後尿中ケトプロフェンの水準の増加が
認められ、その量が組成物中に存在する活性素の濃度に
比例する状態を示している。

【００３７】尿採取の各期間に認められるケトプロフェ
ンの濃度と組成物中の活性素の濃度との直接比較をダン
カン試験（Duncan test）により行った。得られた結果
はケトプロフェンの５％、１０％及び１５％を含む製剤
は相互に統計学的に差異がある（ｐ ０.０５）が、一方
１５％製剤と２０％のそれとの間には統計学的差異が存
在しないことを示している。

Claims (4)

    【特許請求の範囲】
  1. 【請求項１】 活性成分として溶液の１０〜２０％（ｐ
    ／容量）の濃度のケトプロフェンリジン塩、溶液の２〜
    ８％（ｐ／容量）の濃度の適当なイオン性又は非イオン
    性乳化剤、溶液の３〜４％（ｐ／容量）の濃度の適当な
    皮膚軟化剤、溶液の０.２〜３％（ｐ／容量）の濃度の
    適当な粘稠剤、溶液の０.０５〜０.０２％（ｐ／容量）
    の濃度の場合によって加えられる適当な香り及び溶液の
    ０.１〜０.５％（ｐ／容量）の濃度の適当な静菌性化合
    物を含み、そして使用する噴射ガスが３〜７％（ｐ／
    ｐ）の濃度のブタン：プロパン：イソブタンの混合物か
    らなることを特徴とする泡沫の形体の局所用抗炎症医薬
    組成物。
  2. 【請求項２】 イオン性乳化剤がラウリルサルフェート
    から構成され、そして非イオン性乳化剤がソルビタンエ
    ステルとエーテル、ポリオキシメチレンエステルとエー
    テル、ポリオキシエチレンソルビタンエステルとエーテ
    ルからなる群より選ばれ、そして皮膚軟化剤がグリセリ
    ン、プロピレングリコール及びポリエチレングリコール
    からなる群より選ばれ；粘稠剤がアミド、セルロース、
    ビニルピロリドンポリマー及びカルボキシポリメチレン
    からなる群より選ばれ；場合によって加えられる香りが
    ネロリンラベンダーからなり、そして静菌物質が安息香
    酸とそのエステル及びベンジルアルコールからなる群よ
    り選ばれる請求項１記載の泡沫の形体の局所用抗炎症医
    薬組成物。
  3. 【請求項３】 ケトプロフェンリジン塩が溶液の１５％
    （ｐ／容量）の濃度であることを特徴とする請求項１及
    び２のいずれか一項記載の泡沫の形体の局所用抗炎症医
    薬組成物。
  4. 【請求項４】 噴射ガスが５％（ｐ／ｐ）の濃度のブタ
    ン：プロパン：イソブタン＝４：４：２の混合物からな
    ることを特徴とする請求項３記載の泡沫の形体の局所用
    抗炎症医薬組成物。
