を放出しうるステントを提供する。 【構成】 多層ラミネート構造のステントであって、ひ
とつの層がステントの構造要件を担い、付加的な層が予
測されうる速度で薬剤を放出する。構造層及び薬剤放出
層はどちらも最終的には完全に生体により分解吸収され
る。

Description

【発明の詳細な説明】
【0001】
【産業上の利用分野】本発明は、一般にステント(sten
t )と呼ばれる膨張可能な管腔内の導管の移植片に関す
る。特に本発明は、生分解性で治療薬剤を放出しうるス
テントに関する。
【従来の技術】ステントは生体の導管の管腔中に内植さ
れ、該導管の開通性を維持する。ステントの布置及び配
置を容易にする様々なデリバリー・システムが考案され
ている。ステントは始めは収縮状態で扱われるが、該状
態では直径が小さくなっていることから、ステントが比
較的容易に管腔中に導入され、所定の場所に誘導されう
る。一旦配置されるとステントの直径は管腔の直径と等
しいかそれ以上まで拡大し、よってそこを流動体が自由
に流れることができるようになる。特に冠状の用途に適
応するシステムでは、ステントを膨張時の拡大形態に自
動的に固定することを可能にする相互作用要素の組合わ
せを導入したステント設計が用いられる。ステントが配
置されるべき管腔に予め布置したガイドワイヤーに沿っ
てステントを所定の場所に移動する。ステントの内側に
あるバルーンの膨張によりステントは膨張し固定され
る。続いてバルーンを収縮させカテーテル及びガイドワ
イヤーを回収してステントをその場所に残留させる。ス
テント表面から伸びる要素が導管壁に付着して、ステン
トを管腔中の所定の位置にしっかりと固定する。
【0002】従来のステントは、一般にステンレス鋼、
タンタル、又は金等の毒性のない、実質上生体適合性の
金属で形成されている。しかし、一般に約7〜21日以
内に動脈又は導管の内皮層がステント壁の内部及び全体
に成長し、その時点でステントの有用性は実質的に減少
し、ステントが管腔中に存在し続けることがいかなる様
々な問題又は合併症の原因にもなりうることが確認され
ている。従って、一定期間内に完全に分解吸収されうる
生分解性又は生体吸収性材料のステントを形成すること
が提案されている。導管の継続的な薬理学的治療又はス
テントの内植を必要とする状況が、しばしば要求され、
又は望まれる。そのような治療方法は一般に局所的に施
される場合に最も有効であり、従って、薬剤の放出をス
テントに依存して治療を与えることが提案されている。
特定の薬剤を吸収し、その後特定の環境条件の下に実質
上予測可能な速度で処方した期間中該薬剤を放出しうる
材料は公知である。そのような薬剤含浸材料のステント
を形成するか、又はそのような材料をステントと結合す
ることにより、ステントは開通性の維持と薬剤の施与の
2つの目的を達成することができる。先行技術では、配
置後に生体に完全に吸収され、内植を容易にするのに必
要な物性を有し、導管の開通性を一定期間維持するとい
う主機能を果たし、分解吸収される前に薬剤を徐々に放
出するステントを提供することは不可能であった。必要
な物性を薬剤放出材料に付与する従来の試み、あるいは
薬剤放出特性又は吸収される薬剤の効能に関して妥協す
ることなくそれらの材料にそのような特性を付与する試
みは成功していない。

【発明が解決しようとする課題】本発明は、生体に完全
に分解吸収され、かつ特定の薬剤を放出しうるステント
を提供する。更に該ステントは、その内植を容易にする
と同時にその構造的機能を果たすのに必要な全ての物性
を有する。これは、多層のラミネート構造を用いること
で達成される。生体吸収性の第1層を物性により選ぶ。
ひとつ又は複数の分解吸収されうる付加層を、該層が様
々な薬剤を保持する能力と、その後ステントが内植によ
り曝される特定の環境に該層が曝される際に該薬剤を徐
々に放出する能力により選ぶ。

【課題を解決するための手段】ラミネート構造によりひ
とつのステント中に複数の異なる薬剤含有材料を組合わ
せることが可能となる。層を適切な形態とすることで、
薬剤をステントの配置と同時に放出することも、予定し
制御された順序で放出することもできる。更に、異なる
薬剤を用いて、生体の異なる部分を治療対象とすること
ができる。即ち、ステントの外表面にのみ関係する薬剤
含有層は薬剤を導管壁に直接放出するのに対し、ステン
トの内表面にのみ関係する薬剤含有層は薬剤を管腔内及
び管腔中の自由流に放出する。ステントのラミネート構
造により、構造層をラミネート前に形成することが可能
となる。従って、層に十分な強度を付与するために有効
であると考えられる加工及び処理の間の過酷な条件に該
構造層をかけうる。従来技術の場合、この過酷な条件
が、いかなる薬剤又は薬剤放出材料の効果をも低下さ
せ、又は低下の原因となる効果を有することがあった。
本発明では、構造層の形成が完成した後でのみ薬剤含浸
層を構造層と結合させる。本発明のステントの構造層用
に選ばれる材料は、必要な物性を与える一方で、分解吸
収可能でなければならない。最大の引張強さ及び最適な
柔軟性を得るように押出成形し、延伸したポリ−L−乳
酸又はポリグリコール酸等のポリマーを用いることで、
これらの要件は満足されうる。十分な量の特定の薬剤を
保持し、内植後に遭遇する環境に曝された場合に一定速
度又は少なくとも予測しうる速度で該薬剤を放出し、更
に最終的に生体に完全に吸収されるように用いられる材
料の特性に基づいて、薬剤放出層を選ぶ。そのような機
能を果たしうるポリマーには、ポリ−DL−乳酸又はポ
リカプロラクトンが含まれる。そのようなポリマーをま
ずひとつ又は複数の放出すべき薬剤と混合し、次に押出
成形又は溶媒キャストする。次に単層又は多層の薬剤含
有層及び構造層を熱又は溶媒を用いて相互にラミネート
する。本発明は、血管形成処置が施された後にそこで再
狭窄を阻害し、また管腔を通過する流動体の凝塊を阻害
するのに役立つ薬剤をステントの外表面から放出する、
冠状動脈中に内植可能なステントに有益に適用しうる。
あるいは、本発明によるステントを前立腺癌の治療に利
用してもよい。この用途では、化学療法薬剤が内植され
たステントを経て直接尿道へ放出される。本発明のこれ
ら及びその他の特徴及び長所は、添付した図面と共に例
として本発明の本質を説明する次の好ましい態様の詳細
な説明から、明らかなものとなるであろう。

【実施例】図に本発明の好ましい態様を示す。概説する
と、図1は内植前のステント12を示している。該ステ
ントは、配置されるべき管腔を通って移動しうる程度に
十分小さな外径を有し、また内部にバルーンカテーテル
を受け入れる程度に十分大きな内径を有する、巻形の円
筒として形成される。カテーテルの外表面から伸びるつ
まみ14は、バルーンが膨張した後に孔16とかみ合う
大きさとされる。バルーンの膨張過程により円筒は広が
り、よって膨張する。要素14と16が一旦かみ合う
と、該ステントは膨張状態に完全に固定され、決して弛
緩しない。図2は、本発明によるステント12の拡大断
面図である。該ステントは多層を有するラミネート構造
を含む。図示した特定の態様は層18、20、及び22
の3層を有する。中間の、比較的厚い層18は、ステン
トが管腔の開通性を維持することを可能とするために必
要な物性を付与する、ステントの構造成分を含む。この
中間層はまた望ましい柔軟性をステントに付与し、ステ
ントが配置されること、及びその後膨張することを可能
にする。構造層18の両側の比較的薄い層は薬理学物質
を放出する。比較的薄い層を形成する材料は、該材料が
薬剤を吸収する能力と、その後ステントが内植後に遭遇
する環境に曝された場合に該薬剤を予測しうる速度で放
出する能力により選ばれる。図示した態様では、該層の
各々が中間層18の表面に接合するように配置されてい
る。これらの層は同種の薬剤を含んでいても異なる薬剤
を含んでいてもよい。あるいは、ステントの一方の表面
にひとつの薬剤放出層のみをラミネートしてもよい。更
に他の態様として、付加的な薬剤放出層を重ね合わせて
設け、様々な薬剤を連続的に放出することもできる。
【0006】構造層に用いられる材料は、該材料がステ
ントに必要な物性を付与する能力に基づくとともに、生
体に完全に分解吸収されうる能力によって選ばれる。
「分解吸収性」とは、生分解性、生体浸蝕性、又は生体
吸収性のいずれかであるような全ての材料を包含するこ
とを意味し、経時的に分解し徐々に吸収される材料、又
は分解が主として加水分解によるか又は代謝過程により
仲介されるかによらず生体によって分解される材料を含
む。前述のように、そのような材料の強度は、ステント
が一旦膨張し固定した形態で、ステントが内植された導
管の開通性を維持しうる程度でなければならない。これ
に加えて、構造層材料の物性は、例えば内蔵するバルー
ンの膨張によりステントが膨張しうる十分な柔軟性を与
えるものでなければならない。更に、管腔を経由し目標
とする内植サイトに至る蛇行性である可能性のある経路
を通ってステントが移動することを容易にするために、
ある程度の長さ方向の柔軟性が望ましい。要求される構
造的完全性及び分解吸収性の両方を与える能力を伴う材
料は、一般には天然ポリマーである。公知の方法で押出
成形し、延伸して最大の引張強さ及び最適な柔軟性を得
たポリ−L−乳酸又はポリグリコール酸等のポリマー
は、このような用途に好適である。ポリオルトエステル
又はポリ酸無水物を用いることもできる。本発明では、
ステントのラミネート構造により、従来なら薬剤又は薬
剤含有材料が受けた可能性のある過酷な条件の影響に配
慮することなく、構造層を加工及び処理してその物性を
改良しうる。薬剤放出層20及び22に用いられる材料
は、吸収される能力、及び様々な薬剤を保持し、その後
ステントの内植後に予測しうる速度で該薬剤を放出する
能力により選ばれる。このような目的に特に有効である
ことが見出されている材料にはポリ−DL−乳酸又はポ
リカプロラクトン等のポリマーが含まれる。これらのポ
リマーを放出されるべきひとつ又は複数の薬剤と混合
し、その後周知の方法で押出成形又は溶媒キャストする
ことが可能である。中間構造層及びいくつかの薬剤放出
層を形成した後に、熱又は溶媒を用いてそれらの層を相
互にラミネートする。例えば、ポリ−L−乳酸とポリ−
DL−乳酸の層を密着して配置し、それらの層を温度約
55℃にかけることで、2つの層を結合しうる。次に完
成したラミネートを適切な寸法に打抜くかレーザー切断
する。ステントを始めは巻形状態に維持し、その後膨張
状態に固定することを可能にするために必要な要素及び
特徴を、同様に周知の方法で形成する。巻形とし、成形
する最終的な操作により、ステントは実質上いつでも使
用しうるようになる。
【0007】ステントの寸法、ステントの強度要件及び
物性、及び選ばれる特定の薬剤及び薬剤放出速度は、ス
テントの最終目的により決定される。例えば、最終目標
が冠状動脈の開通性の維持である場合、本発明によるス
テントは、管腔に曝される最も内側の層が血栓症の可能
性を減少させる薬剤を放出するように形成されるという
特徴を含む。薬剤の中でヘパリン又はプロスタサイクリ
ンがこの目的に適する。そのような用途において、外側
の層20は再狭窄を処置する薬剤を有効に放出しうる。
この目的に有効であることが見出されている薬剤にはア
ンギオペプチン、メトトレキセート、及びヘパリンが含
まれる。本発明の特定の形態について説明し記述した
が、本発明の精神及び範囲を逸脱することなく様々な改
良がなされうることは当業者に明らかとなるであろう。
様々なステントの設計及び用途を本発明から得ることが
できる。従って本発明は、添付した請求項以外によって
制限されることを意図するものではない。

Claims (25)

    【特許請求の範囲】
  1. 【請求項1】 導管中に内植するための膨張可能な管腔
    内ステントであって、(1)該ステントに構造剛性を付
    与するように選ばれる分解吸収されうる材料により形成
    される第1層、及び(2)該第1層に結合され、治療薬
    剤を選択された速度で放出するように選ばれる分解吸収
    されうる材料による第2層、を含み、内植により該ステ
    ントがまず該導管に対し望ましい程度の構造支持を与
    え、該薬剤を選択された速度で放出し、最終的に完全に
    分解吸収されることを特徴とする、前記ステント。
  2. 【請求項2】 前記第1層が更に該ステントに付加的な
    望ましい物性を付与するように選ばれる、請求項1記載
    のステント。
  3. 【請求項3】 前記第1層がポリ−L−乳酸を含む請求
    項1記載のステント。
  4. 【請求項4】 前記第1層がポリグリコール酸を含む請
    求項1記載のステント。
  5. 【請求項5】 前記第2層がポリ−DL−乳酸を含む請

  9. 【請求項9】 請求項1記載のステントであって、分解
    吸収されうる材料による第3層が前記第1層に結合して

    れ、内植により該ステントがまず該導管に対し望ましい
    程度の構造支持を与え、該薬剤を選択された速度で放出
    し、最終的に完全に分解吸収されることを特徴とする、
    前記ステント。
  10. 【請求項10】 前記第1及び第2層から放出される薬剤
    が同一である請求項9記載のステント。

    る薬剤を放出し、前記第3層が該ステントの第1層の内
    側に結合していて血栓症を処置する薬剤を放出すること
    を特徴とする、前記ステント。
  13. 【請求項13】 前記第2層がアンギオペプチンを放出す
    るように選ばれ、前記第3層がヘパリンを放出するよう
    に選ばれる、請求項9記載のステント。
  14. 【請求項14】 管腔中に内植するための生分解性の薬剤
    放出ステントの製造方法であって、(1)ステントが該
    管腔中でその構造的機能を果たすことを可能にするため
    に要求される物性を有する生分解性材料による第1層を
    形成する工程、(2)該管腔中で遭遇するような環境に
    曝されたときに選択された薬剤を放出する生分解性材料
    による第2層を形成する工程、及び(3)前記第2層を
    前記第1層と結合して、生分解性で、薬剤を放出し、該
    ステントが該管腔中でその構造的機能を果たすために要
    求される物性を有するラミネート構造を与える工程、を
    含む、前記製造方法。
  15. 【請求項15】 請求項14記載の方法であって、更に
    (4)該管腔中で遭遇するような環境に曝されたときに
    選択された薬剤を放出する生分解性材料による第3層を
    形成する工程、及び(5)前記第3層を前記第1層と結
    合する工程、を含む、前記方法。
  16. 【請求項16】 前記第1層をポリマー材料の押出及び延
    伸によって形成する請求項14記載の方法。
  17. 【請求項17】 前記ポリマー材料がポリ−L−乳酸を含
    む請求項16記載の方法。
  18. 【請求項18】 前記ポリマー材料がポリグリコール酸を
    含む請求項16記載の方法。
  19. 【請求項19】 前記第2層が前記薬剤とポリマー材料の
    混合物を含む請求項14記載の方法。
  20. 【請求項20】 前記ポリマー材料がポリ−DL−乳酸を
    含む請求項19記載の方法。
  21. 【請求項21】 前記ポリマー材料がポリカプロラクトン
    を含む請求項19記載の方法。
  22. 【請求項22】 熱を印加して前記第2層を前記第1層と
    結合する請求項14記載の方法。
  23. 【請求項23】 溶媒を用いて前記第2層を前記第1層と
    結合する請求項14記載の方法。
