Abstract

(57)【要約】 【課題】 末梢血管微小循環障害に関連する末梢血管疾
患、蜂巣炎または過剰脂肪蓄積に関連した反美容症候の
治療のための局所用調製物における、エスクロシドの単
独使用またはフォルスコリンまたはサルビア・ミルチオ
リザジテルペンなどのアデニル酸シクラーゼ刺激剤およ
び／またはアピゲニン骨格を有する２量体フラボンなど
のホスホジエステラーゼインヒビターとの配合使用を提
供する。 【解決手段】 あらゆるオリジンの過剰脂肪蓄積を軽減
するために、組成物にはカフェイン、テオフィリンおよ
びそれらの誘導体を有利に組み合わせる。

【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は、エスクロシドを単
独でまたはフォルスコリンまたはサルビア・ミルチオリ
ザ（Salvia miltiorrhiza）ジテルペンなどのアデニル
酸シクラーゼ刺激剤および／またはアピゲニン骨格構造
を有する２量体フラボンなどのホスホジエステラーゼイ
ンヒビターと組み合わせて含む末梢血管微小循環障害に
関係する末梢血管疾患の治療における局所的使用のため
の調製物に関し、さらに、本発明は、蜂巣炎または過剰
脂肪沈着と関連した反美容症候（unesthetisms）の治療
におけるエスクロシドの単独使用または上記のアデニル
酸シクラーゼ刺激剤および／または上記のホスホジエス
テラーゼインヒビターとの配合使用に関する。後者に関
して、上記化合物にはカフェイン、テオフィリン、ペン
トキシフィリンを加えることができる。
【０００２】本発明はまた、上記の調製物の調製のため
のエスクロシドの単独使用またはフォルスコリンまたは
サルビア・ミルチオリザジテルペンなどのアデニル酸シ

ビターとの配合使用に関する。
【０００３】
【発明が解決しようとする課題】エスクロシドは、果実
の果皮およびAesculus hippocastanum（トチノキ）の樹
皮から抽出されるクマリン配糖体であって、そのような
化合物は浸透性障害および毛細血管脆弱に関連した病理
学的状態の治療において長年使用されており、これは今
も静脈鬱血性血管疾患、痔疾の治療における局所的また
は全身的適用のため、および眼科学における鬱血除去薬
として使用されている。今、驚いたことに、エスクロシ
ドの単独適用または上記の化合物との配合適用を局所的
に行った場合に適用領域における動脈血流の増加（動脈
の伸縮性細胞および前毛細血管小動脈への活性がその脈
拍動を増加する）および灌流毛細血管数の増加の双方が
もたらされ、それによって表面部および深部の循環が改
善されることが見出された。この現象は、赤外線光パル
ス血量計、レーザードップラー流量計およびコンピュー
タ化されたビデオ毛細管顕微鏡などの非侵襲的方法によ
って測定された。最初の二つの方法によって動脈および
前毛細血管小動脈の血管運動性が明示され、後者によっ
て毛細血管床および脈管形成域における変化を評価する
ことができる。基礎データを記録した後、プラセボ調製
物または活性組成物または活性主成分を含む調製物を治
療すべき生体領域に適用する。通常、二つの対称の生体
部分を用いて、試験をランダム化する。治療処理後直ち
に、処理領域を、ハロゲン光プローブを供えた５０〜４
００倍の倍率のビデオ毛細管顕微鏡（Scopeman-Moritex
Video Imaging System, Alfa Strumenti、ミラノ）を
用いて、毛細管密度（単位表面当たりの血液灌流された
毛細血管数）を測定して、毛細血管の空間配置およびそ
れらの形態を評価する。この機器は生物顕微鏡画像を連
続的に記録するためのビデオカメラを備えており、処理
段階中の変化を定量することができる。エスクロシドの
単独または最も重要な化合物例としてGinkgo biloba２
量体フラボンまたはアメントフラボンなどの上記の化合
物との併用による処理の２０分後、基礎的およびプラセ
ボ処理の対照と比較して２００〜３００％までの毛細血
管密度の増加が起きた。上記のようにして評価された
０．５〜３％の範囲の濃度のエスクロシドの作用は、１
〜３時間持続する。上記したようにビデオ毛細管顕微鏡
によって、皮膚微細脈管形成を評価し、灌流された毛細
血管数および空間配置およびその層形成を明らかにする
ことが可能である。
【０００４】
【課題を解決するための手段】エスクロシドを単独でま
たはアデニル酸シクラーゼ刺激活性または抗ホスホジエ
ステラーゼ活性を有する化合物および適宜また、カフェ
イン、テオフィリン、ペントキシフィリンなどの他の脂
肪分解剤と併用して含む調製物を、蜂巣炎などの慢性静
脈欠陥による疾患を有する領域または強制食餌に続く反
美容症候などの過剰脂肪蓄積に適用した場合、エスクロ
シドおよび他の化合物による脂肪分解作用による領域微
小循環の改善によって症状の顕著な軽減が驚いたことに
認められ、これが本発明の一部をなす。適用は病理症状
または反美容症候の軽重によるが、２、３日間から数カ
月、通常は３カ月間の範囲で行われる。
【０００５】血管系に関しては、エスクロシドは単独で
または抗ホスホジエステラーゼ剤またはアデニル酸シク
ラーゼ刺激剤との配合で、慢性静脈機能不全、レイノー
病および肢端チアノーゼにおいておよび風邪誘導性血管
痙攣、とくに手指および足指の微小循環における血管痙
攣において使用することができる。該活性成分を用いて
処理した領域の高められた血流はまた、老化初期の真
皮、とくに顔および首の皮膚、蜂巣炎様の皮膚、皮下組
織層疾患および妊娠線に好ましく作用する。さらに、高
められた血液供給はまた、頭皮などの滑らかではない皮
膚に好ましく作用し、一次および二次脱毛症の治療に有
用である。
【０００６】美容分野では、本発明の目的化合物の主た
る使用は、先進工業国人口に大きな割合で影響を及ぼし
ている皮膚の老化および蜂巣炎に関する。
【０００７】上記の使用例として、慢性静脈機能不全
（段階Ｉ）の患者２０人を二つのグループに分けて、
１．５％エスクロシドを含む調製物またはプラセボを用
いて大腿内転筋から足首までに１日に２回適用して、３
カ月間治療した。
【０００８】患者は、長期治療の前に皮膚微小循環にお
ける増加のビデオ毛細管顕微鏡測定による急性試験によ
って応答能を評価した。処理後２０分の測定によると、
プラセボで治療したグループの毛細血管密度は８．６±
２．２％であり、エスクロシド治療を受けたグループで
は１８．６±３．１％であった（ステュデントのｔテス
トによる計算でｐ＜０．０１）。
【０００９】観察された症状およびその程度は、０〜４
の範囲のスコアによって評価され、次表のように報告さ
れる。

て、２０人の対象グループを大腿内転筋領域から足首ま
でを１．５％のエスクロシド、０．３％のサルビア・ミ
ルチオリザ抽出物（この抽出物は１５％のタンシノンＡ
２を含む）、０．４％Ginkgo biloba２量体フラボン（イ
ンデナ(INDENA)特許ＥＰ０３６０５５６に従って調製）
および０．２％テオフィリンを含む調製物で３カ月間治
療した。これらの対象において、第１図のデータ報告に
示されるように観察されたパラメターは大きく変化し
た。
【００１３】別の試験において、過剰脂肪蓄積を伴う大
腿内転筋領域の線維硬化性皮下組織層疾患２０人の対象
グループを、１．５％のエスクロシド、０．３％のサル
ビア・ミルチオリザ抽出物および１％のペントキシフィ
リンを含む調製物で２カ月間治療した。主なコントロー
ルパラメターは、過剰脂肪蓄積の減少だったので、大腿
内転筋領域の直径を測定したが、これは２．６±０．２
ｃｍという驚異的な減少を示した。
【００１４】上記の病理的症状および反美容症候を治療
するために組成物構成および組成物の割合または組成物
の性質を変えて他の多くの試験が行われた。本発明によ
る調製物は、上記の活性成分の他に、従来のキャリア、

１００ｇのゲル化エマルジョンは以下の組成を有する。
【００２０】 エスクロシド １．００ｇ ミリスチン酸イソプロピル ５．００ｇ 保存剤 ０．４０ｇ 香料 ０．１０ｇ ポリアクリルアミド、 Ｃ13-14 イソパラフィンおよびラウリン酸アルコール ７（ＯＥ） （セピゲル３０５−セピック） ３．００ｇ 精製水 全量を１００ｇにする。

    【特許請求の範囲】
  1. 【請求項１】 活性成分としてエスクロシドをアデニル
    酸シクラーゼ刺激剤および／またはホスホジエステラー
    ゼインヒビターおよび／または脂肪分解剤と組み合わせ
    て含む局所的使用のための薬剤および／または美容用調
    製物。
  2. 【請求項２】 アデニル酸シクラーゼ刺激剤としてフォ
    ルスコリンまたはサルビア・ミルチオリザジテルペンま
    たはこれらの混合物を含む請求項１に記載の薬剤および
    ／または美容用調製物。
  3. 【請求項３】 ホスホジエステラーゼインヒビターとし
    てアピゲニン骨格を有する２量体フラボンを含む請求項
    １または２に記載の薬剤および／または美容用調製物。
  4. 【請求項４】 フラボンをGinkgo biloba２量体フラボ
    ンおよびアメントフラボンから選択することを特徴とす
    る請求項３に記載の薬剤および／または美容用調製物。
  5. 【請求項５】 脂肪分解剤としてカフェイン、テオフィ
    リン、ペントキシフィリンまたはこれらの混合物を含む
    請求項１〜４のいずれか１項に記載の薬剤および／また
    は美容用調製物。
  6. 【請求項６】 末梢血管微小循環障害に関連する末梢血
    管疾患、蜂巣炎および過剰脂肪蓄積に関連した反美容症
    候の治療のための請求項１〜５のいずれか１項に記載の
    薬剤および／または美容用調製物。
  7. 【請求項７】 末梢微小循環障害に関連する末梢血管疾
    患、蜂巣炎および過剰脂肪蓄積に関連した反美容症候の
    治療のための局所用の医薬品製造のためのエスクロシド
    の単独使用またはアデニル酸シクラーゼ刺激剤および／
    またはホスホジエステラーゼインヒビターおよび／また
    は脂肪分解剤との配合使用。
