Claims (1)
- 【特許請求の範囲】 1.組織に逆行性穴を形成する装置10にして、 その軸線に関して細長であり且つその断面寸法よりも著しく長い部材11であ って、配置過程中、人間又は動物の身体の外部組織を通じて略その軸線に沿った 方向に挿入可能な寸法とされた前記部材11と、 該部材11における終端12及び基端13であって、配置中、最初に組織に入 る前記終端12と、組織の外側に留まる間に制御装置26を作用させる前記基端 13と、 人間又は動物の外部組織を通じて配置し得るように前記部材11の前記終端1 2と関係付けられた先端14と、 一又は複数の組織切除要素17及び引き裂き器18を有する、展開可能な組織 分離器16とを備え、 前記組織切除要素17が前記部材11の断面寸法に沿って全体として持ち運ぶ ときの位置であり且つ該断面寸法内にある格納位置内に配置され、前記組織切除 要素17の各々が、格納位置から移動したとき、先端14に関して露出位置に配 置され得るように前記軸線に関して可動であり、その引き裂き器18が、前記部 材11が軸線に沿って逆行動作する間に組織に接触することにより、逆行させて 引き抜く間に組織を引き裂き、 前記基端13と前記展開可能な組織分離器16との間に配置された連結機構1 9であって、基端13にて該連結機構19を作動させることにより、選択的に配 置するために使用されて、組織切除要素17の各々を配置し且つ保持し、その引 き裂き器18が露出位置にある状態にし、外部組織を通じて配置した後に、部材 11を引き出したとき前記組織切除要素17が、前記組織を通じて逆行するだけ で最初の穿剌部又は切開部が拡張されるようにする前記連結機構とを備えること を特徴とする装置。 2.組織に逆行性穴を形成する、請求の範囲第1項に記載の装置10にして、 前記先端14が、軸線に対してある角度で、テーパー付き、斜角状又は面取り加 工されて、前記部材11を外部組織を通じて挿入するのに必要な挿入力を軽減す るようにしたことを特徴とする装置。 3.組織に逆行性穴を形成する、請求の範囲第1項に記載の装置10にして、 前記展開可能な組織分離器16が、前記基端13から受け取った無線周波数エネ ルギを少なくとも各々の引き裂き器18に伝達し得るように前記部材11と絶縁 状態に関係付けられた電極23と、 前記基端13における前記電極23と関係付けられて、前記基端13と各引き 裂き器18との間に無線周波数エネルギを供給するエネルギ供給線24と、 無線周波数エネルギを提供する電気外科手術用発電機25であって、前記エネ ルギの大きさ及び周波数を調節する制御装置26を備える前記電気外科手術用発 電機25と、 前記組織及び前記エネルギ供給線24に結合されて回路を接続し、これにより 、人間又は動物の身体の組織を通じて逆行させて引き抜く間に電気外科手術的効 果を提供する戻り経路27とを備えることを特徴とする装置。 4.組織に逆行性穴を形成する、請求の範囲第1項に記載の装置10にして、 前記部材11が、前記部材11の略軸線に沿った位置に配置された貫通路22を 備え、前記基端13と終端12とを流体連通させる一方、流体の流動源と流体連 通するように接続して、流体を前記終端12に向け又は該終端12から離れる方 向の何れかに動かし且つ身体に対して出入りさせるようにしたことを特徴とする 装置。 5.組織に逆行性穴を形成する、請求の範囲第4項に記載の装置10にして、 前記部材11が各種の断面形状を有することを特徴とする装置。 6.組織に逆行性穴を形成する、請求の範囲第5項に記載の装置10にして、 前記部材11が略湾曲した断面形状を有することを特徴とする装置。 7.組織に逆行性穴を形成する、請求の範囲第5項に記載の装置10にして、 前記部材11が略三角形の形状を有することを特徴とする装置。 8.組織に逆行性穴を形成する、請求の範囲第3項に記載の装置10にして、 前記部材11が非導電性の電気絶縁体であり、前記戻り経路27が各引き裂き器 18とエネルギ供給線24との間にあって回路を接続し、導体28が各引き裂き 器18の僅かに基端方向で前記部材18上に配置され、その間を双極式に引き裂 くようにしたことを特徴とする装置。 9.組織に逆行性穴を形成する、請求の範囲第1項に記載の装置10にして、 前記引き裂き器18の各々は、逆行させて引き抜く間に組織を切除する鋭利な切 刃を有することを特徴とする装置。 10.組織に逆行性穴を形成する、請求の範囲第1項に記載の装置10にして、 前記連結機構19が前記ハンドル15と前記要素17との間に接続されて、該要 素17を直接、動かして、逆行させて引き抜く間に、組織を切除することを特徴 とする装置。 11.組織に逆行性穴を形成する、請求の範囲第9項に記載の装置10にして、 ハンドル15が前記基端13と関係付けられて制御装置26を作用させ且つ終端 12を操作し、ハンドル15には、前記連結機構19を作用させるトリガー31 が保持されていることを特徴とする装置。 12.組織に逆行性穴を形成する、請求の範囲第1項に記載の装置10にして、 振動装置が前記ハンドル15内に保持され且つ前記連結機構19と作用可能に関 係付けられ、逆行させて引き抜く間に、各引き裂き器18に動作を付与して、軸 線に沿って引き出すのに必要な力を軽減することを特徴とする装置。 13.組織に逆行性穴を形成する、請求の範囲第3項に記載の装置10にして、 前記戻り経路27が、人間又は動物の外部組織に固定された電極23のパッド1 9であり、電流が各引き裂き器18から組織を通じて流れる単極性の電気外科用 回路を形成することを特徴とする装置。 14.組織に逆行性穴を形成する、請求の範囲第1項に記載の装置10にして、 弾性的負荷手段35が前記展開可能な組織分離器16に加えられて、一又は複数 の組織切除要素17を格納位置から付勢して、露出した引き裂き器18の形態と なるようにし、ロックが前記連結機構19と関係付けられて、組織切除要素17 を格納位置に保持し、ロックを解放したとき、組織切除要素17が前記部材11 の軸線に関して動く動作が、基端13にて連結機構19に加えられた力により開 始する、前記連結機構19の軸方向への動きにより行われることを特徴とする装 置。 15.組織に逆行性穴を形成する、請求の範囲第1項に記載の装置10にして、 前記先端14が尖っておらず、丸味が付けられ又は柔軟な材料から成り、身体の 内部にアクセスし得るように切開小刀で切開した後に、組織に非外傷的に挿入し 得るようにしたことを特徴とする装置。 16.人間又は動物の腹壁に逆行性穴を形成する装置10を配置する方法にして 、 終端12及び基端13を有する細長の部材11の軸線を人間又は動物の腹の外 皮に対して略直角に整合させることと、 前記終端12を組織を通じて配置し且つ前記基端13が組織の外側に残るよう にすることと、 最初に格納位置にて断面寸法内に配置された一又は複数の組織切除要素17を 有する組織分離器16を露出位置に展開させ、該露出位置にて、前記基端13と 前記展開可能な組織分離器16との間に配置された連結機構19を使用すること により、前記組織切除要素17の各々の引き裂き器18が組織を引き裂く位置に 配置され、組織切除要素17の基端13にて操作して露出位置にすることにより 、前記組織切除要素17を選択的に配置し且つ保持するようにすることと、 前記連結機構19によりその格納位置から移動された後に、前記部材11に関 して露出位置に固定される間に、前記部材11と共に前記組織切除要素17の各 々を逆行するように動かすことと、 体腔から引き抜く間に組織を引き裂き、前記引き裂き器18が、軸線に沿って 前記部材11と共に逆行するように動く間に組織に接触することにより、組織を 切除し、外部組織を通じて配置した後に、部材11を引き出したとき、前記組織 切除要素17が、組織の開口部を拡張するようにすることとを含むことを特徴と する方法。 17.請求の範囲第16項に記載の方法にして、前記引き裂き段階が電気外科的 に行われることを特徴とする方法。 18.請求の範囲第16項に記載の方法にして、前記組織切除要素17を展開さ せる前記段階が、前記組織切除要素17を前記部材11から外方に動かすことに より行われることを特徴とする方法。 19.請求の範囲第16項に記載の方法にして、前記連結機構19を使用する前 記段階が、基端13にてトリガー31を引いて、前記組織切除要素17を配置す ることを含むことを特徴とする方法。 20.請求の範囲第17項に記載の方法にして、電気外科的に引き裂く前記段階 が、人間又は動物の組織に対し戻り経路27を付与することにより行われること を特徴とする方法。 21.請求の範囲第16項に記載の方法にして、前記引き裂き段階が、より柔軟 な内部組織にアクセスし得るように小刀で切開部を形成した後、該組織に非外傷 的に入り得るように尖っておらず、丸味を付け又は柔軟な材料から成る先端14 を有する逆行性穴形成装置10を作用させることにより行われることを特徴とす る方法。 22.組織に逆行性穴を形成する装置10にして、 その軸線に関して細長であり且つその断面寸法よりも著しく長い部材11であ って、配置過程中、人間又は動物の身体の外部組織を通じて略その軸線に沿った 方向に挿入可能な寸法とされた前記部材11を備え、前記部材11が、前記部材 11の略軸線に沿った位置に配置された貫通路22を備え、前記基端13と終端 12とを流体連通させる一方、流体の流動源と流体連通するように接続して、流 体を前記終端12に向け又は該終端12から離れる方向の何れかに動かし且つ身 体から出入りさせ、 該部材11における終端12及び基端13であって、配置中、最初に組織に入 る前記終端12と、組織の外側に留まる間に制御装置26を作用させる前記基端 13と、 人間又は動物の外部組織を通じて配置し得るように前記部材11の前記終端1 2と関係付けられた先端14と、 一又は複数の組織切除要素17及び引き裂き器18を有する、展開可能な組織 分離器16とを備え、 前記組織切除要素17が、前記部材11の断面寸法に沿って全体として持ち運 ぶときの位置であり移動し且つ該断面寸法内にある格納位置内に配置され、前記 組織切除要素17の各々が、格納位置から移動したとき、前記部材11に関して 露出位置に配置され得るように前記軸線に関して可動であり、 その引き裂き器18が、前記部材11が軸線に沿って逆行動作する間に組織に 接触することにより、逆行させて引き抜かれる間に組織を引き裂き、 前記展開可能な組織分離器16が、前記基端13から受け取った無線周波数エ ネルギを少なくとも各々の引き裂き器18に伝達し得るように前記部材11と絶 縁状態に関係付けられた電極23を備え、 前記部材11が、非導電性の電気絶縁体であり、 前記戻り経路27が、各引き裂き器18とエネルギ供給線24との間にあって 回路を接続し、導体28が各引き裂き器18の僅かに基端方向で前記部材18上 に配置され、その間を双極式に引き裂くようにし、 前記基端13における前記電極23と関係付けられて、前記基端13と各引き 裂き器18との間に無線周波数エネルギを供給するエネルギ供給線24と、 無線周波数エネルギを提供する電気外科手術用発電機25であって、前記エネ ルギの大きさ及び周波数を調節する制御装置26を備える前記電気外科手術用発 電機25と、 前記基端13と前記展開可能な組織分離器16との間に配置された連結機構1 9であって、基端13にて該連結機構19を作動させることにより、その選択的 に配置するときに使用されて、組織切除要素17の各々を配置し且つ保持し、そ の引き裂き器18が露出位置にある状態にし、外部組織を通じて配置した後に、 部材11を引き出したとき、前記組織切除要素17が、前記組織を通じて逆行す るだけで、単に最初の穿剌部又は切開部を拡張するようにする前記連結機構とを 備え、 ハンドル15が、前記基端13と関係付けられて制御装置26を作用させ且つ 終端12を操作し、ハンドル15には、前記連結機構19を作用させるトリガー 31が保持されて、 前記弾性的負荷手段35が、前記展開可能な組織分離器16に加えられて、一 又は複数の組織切除要素17を格納位置から付勢して、露出した引き裂き器18 となるようにし、ロックが前記連結機構19と関係付けられて、組織切除要素1 7を格納位置に保持し、 ロックを解放したとき、組織切除要素17が前記部材11の軸線に関して動く 動作が、基端13にて連結機構19に加えられた力により開始される、前記連結 機構19の軸方向への動きにより行われることを特徴とする装置。 23.組織に逆行性穴を形成する、請求の範囲第22項に記載の10にして、単 極性回路を接続し得るように前記組織及びエネルギ供給線24に接続された戻り 経路27を備え、これにより、人間又は動物の身体の組織を通じて逆行させて引 き抜く間に、電気外科手術的作用が提供されるようにしたことを特徴とする装置 。 24.組織に逆行性穴を形成する、請求の範囲第22項に記載の装置10にして 、前記先端14が、予め形成した切開部又は穿剌部を通じて容易に挿入し易いよ うに柔軟であり、尖っておらず、又は丸味を付けられたことを特徴とする装置。 25.組織に逆行性穴を形成する、請求の範囲第22項に記載の装置10にして 、前記先端14が、前記部材11を外部組織を通じて挿入するのに必要な挿入力 を軽減すべく、軸線に対してある角度で、テーパー付き、斜角状に形成され、又 は面取り加工されることを特徴とする装置。
