- 【特許請求の範囲】 1.患者身体内の構造に対し電気的エネルギーを与える方法であって、この方 法は、複数の分離された電極端子を含む電極配列構体が前記身体内構造と少なく とも部分的に接触するようにして電気外科プローブを前記身体内構造に隣接させ て位置決めする過程と、患者の身体上又は身体内に於いて前記電極配列構体と共 通電極との間に高周波電圧を印加する過程とからなることを特徴とする患者の身 体内構造に対し電気的エネルギーを与える方法。 2.少なくとも二つの前記電極端子から流れる電流は、電極端子と前記共通端 子との間のインピーダンスに基づいて、独立に制御されることを特徴とする請求 の範囲1に記載の方法。 3.前記電極端子と前記共通電極の間に低インピーダンスの電路が存在する場 合には、電流が制限されることを特徴とする請求の範囲2に記載の方法。 4.前記電極端子と前記共通電極の間に高インピーダンスの電路が存在する場 合には、電流が制限されることを特徴とする請求の範囲2に記載の方法。 5.高周波電圧は、周波数に於いては20KHz乃至は20MHzの範囲にあり、電圧に 於いては5V乃至は300V(実効値)の範囲にあるとを特徴とする請求の範囲1 に記載の方法。 6.請求の範囲1に記載の方法は、前記電極配列構体と身体構造との界面に於 ける温度を計測し、事前に設定した前記界面の温度を維持するようにする電圧、 電流、デューティーサイクルを制御する過程を更に含むことを特徴とする方法。 7.前記電極配列構体は導電性の環境に置かれることを特徴とする請求の範囲 1に記載の方法。 8.前記導電性環境は塩性溶液を具備することを特徴とする請求の範囲7に記 載の方法。 9.前記電極配列構体は繊維軟骨組織又は関節軟骨組織方面の切除のために、 それら組織に対向して位置決めされることを特徴とする請求の範囲1に記載の方 法。 10.電気外科プローブはアクセスカニューレを介して経皮的に体内に導入され ることを特徴とする請求の範囲1に記載の方法。 11.互いに離間した電極端子は15mm2以下の接触面積を有することを特徴とす る請求の範囲1に記載の方法。 12.互いに離間した電極端子は0.001mm2から2mm2の範囲にある接触面積を有 することを特徴とする請求の範囲11に記載の方法。 13.前記面積は0.01mm2から1mm2の範囲であることを特徴とする請求の範囲12 に記載の方法。 14.身体構造に接した電極表面を介して電源から高周波電圧を掛ける改良され た電気外科施術方法であって、この方法は電極配列構体を身体構造に接触させる と共に、この電極配列構体の個々の電極端子を流れる電流を独立に制限し、低い インピーダンスを介して電源に接続された電流帰路をなす電極端子は過剰な加熱 を起こさないようにしたことを特徴とする改良された電気外科的施術方法。 15.高周波電流は電流制限された電源を介して個々の電極端子に独立に供給さ れることを特徴とする請求の範囲14に記載の改良された方法。 16.電極表面と身体構造の界面に於ける事前に設定された温度を維持するため に、全ての電極端子に対する電圧、電流、又はデューティーサイクルは共通に制 御されることを特徴とする請求の範囲15に記載の改良された方法。 17.操作側端部と施術側端部を有するシャフトと、このシャフト の施術側端部に設けられ、接触表面に配置された複数の電気的に絶縁された電極 端子を含む電極配列構体と、シャフトの操作側端部近くに設けられ、電極端子を 高周波電圧電源に結合するコネクタとを具備することを特徴とする電気外科プロ ーブ。 18.前記シャフトは少なくともその長さの一部が全体として剛性を備えている ことを特徴とする請求の範囲17に記載の電気外科プローブ。 19.前記シャフトは少なくともその長さの一部が全体として柔軟性を備えてい ることを特徴とする請求の範囲17に記載の電気外科プローブ。 20.前記シャフトは少なくともその施術側端部に於いて柔軟性を備え、その柔 軟な施術側端部を選択的に偏向する手段を更に含んでいることを特徴とする請求 の範囲19に記載の電気外科プローブ。 21.請求の範囲17に記載の電気外科プローブは、更に前記シャフト上に設けた 共通電極を含み、前記コネクタはこの共通電極を高周波電圧電源に電気的に結合 することを特徴とする電気外科プローブ。 22.請求の範囲21に記載の電気外科プローブは、前記共通電極上に設けた電気 的に非導電性の有孔性シールドを更に含んでいることを特徴とする電気外科プロ ーブ。 23.前記シールドは前記共通電極の半径方向に離間して、両者の間に毛管並み の空隙を形成していることを特徴とする請求の範囲22に記載の電気外科プローブ 。 24.前記電極配列構体は前記シャフトの施術側端部に配設されていることを特 徴とする請求の範囲21に記載の電気外科プローブ。 25.前記共通電極は0.5mmから2mmの距離の範囲で電極配列構体から離れてい る帯状電極であることを特徴とする請求の範囲21に記 載の電気外科プローブ。 26.前記電極配列構体は0.01mm2から2.5cm2の範囲の面積を備えていることを 特徴とする請求の範囲25に記載の電気外科プローブ。 27.互いに離間した各電極端子は5mm2以下の接触面積を有することを特徴と する請求の範囲19に記載の電気外科プローブ。 28.互いに離間した電極端子は0.001mm2から2mm2の範囲の面積を有すること を特徴とする請求の範囲27に記載の電気外科プローブ。 29.電極端子の面積は0.01mm2から1mm2の範囲であることを特徴とする請求の 範囲28に記載の電気外科プローブ。 30.前記電極配列構体は前記シャフトの側面に配置されていることを特徴とす る請求の範囲17に記載の電気外科プローブ。 31.電極端子を直線配列した前記電極配列構体は切開用の刃として作用するこ とを特徴とする請求の範囲17に記載の電気外科プローブ。 32.前記電極配列構体は少なくとも4個の電極端子を含むことを特徴とする請 求の範囲17に記載の電気外科プローブ。 33.請求の範囲17に記載の電気外科プローブは前記電極配列構体に設けた温度 センサを更に備え、このセンサは前記コネクタによって高周波電圧源に電気的に 結合されていることを特徴とする電気外科プローブ。 34.電極端子は電極端子一個の直径の約10分の1からその1直径の範囲にある 距離間隔で互いに他と離間されていることを特徴とする請求の範囲17に記載の電 気外科プローブ。 35.電極端子は距離にして0.05mmから1mmだけシャフトの施術側端部方向に延 在していることを特徴とする請求の範囲17に記載の電気外科プローブ。 36.複数の独立した限流型電流電源と、共通電極接続体と、前記電流源の個々 の電極端子を前記電気外科プローブ内の個々の電極端子に電気的に結合する電気 外科プローブ上に設けたコネクタに接続するコネクタとを備えたことを特徴とす る電気外科用電源。 37.前記独立の電流源は互いに他と並列に接続され、かつ共通電圧源には直列 に接続する電流制限用誘導素子からなることを特徴とする請求の範囲36に記載の 電気外科用電源。 38.前記独立の電流源は互いに他と並列に接続され、かつ共通電圧源には直列 に接続する電流制限用容量素子と、互いに他と並列に接続され、かつ共通電圧源 には直列に接続する抵抗素子とからなることを特徴とする請求の範囲36に記載の 電気外科用電源。 39.請求の範囲36に記載の電気外科用電源は、温度設定点と前記コネクタを介 して電気外科プローブから受ける温度計測値とに応答して前記電圧源の出力、又 はデューティーサイクルを調整する温度制御装置を更に含んでいることを特徴と する電気外科用電源。 40.電気外科プローブと電気外科用電源とからなる電気外科システムであって 、前記電気外科プローブは操作側端部と施術側端部を有するシャフトと、このシ ャフトの施術側端部近傍に設けられ、接触表面に配置された複数の電気的に絶縁 された電極端子を含む電極配列構体と、シャフトの操作側端部近傍に設けられ、 電極端子を個々に電気外科用電源に結合するコネクタとからなると共に、前記外 科用電源は複数の独立した電流電源と、前記電流源の個々の電極端子を前記電極 配列構体の個々の電極端子に電気的に結合するシャフトコネクタに接続するコネ クタとを具備することを特徴とする電気外科システム。 41.前記シャフトは少なくともその長さの一部が全体として剛性を備えている ことを特徴とする請求の範囲40に記載の電気外科シス テム。 42.前記シャフトは少なくともその長さの一部が全体として柔軟を備えている ことを特徴とする請求の範囲41に記載の電気外科システム。 43.請求の範囲41に記載の電気外科システムは、前記コネクタによって高周波 電源に電気的に結合される前記シャフト上に設けた共通電極をさらに含むことを 特徴とする電気外科システム。 44.前記共通電極はシャフトの外側であって、かつ電極配列構体から操作側端 部方向に離間して設けられることを特徴とする請求の範囲43に記載の電気外科シ ステム。 45.前記電極配列構体はシャフトの施術側端部に配置されることを特徴とする 請求の範囲40に記載の電気外科システム。 46.前記電極配列構体はシャフトの外側部に配置されることを特徴とする請求 の範囲40に記載の電気外科システム。 47.前記電極配列構体は直線状に配列され、切開用の刃として作用することを 特徴とする請求の範囲40に記載の電気外科システム。 48.前記電極配列構体は少なくとも6個の電極端子を含んでいることを特徴と する請求の範囲40に記載の電気外科システム。 49.請求の範囲40に記載の電気外科システムは、前記電極配列構体に設けられ た温度センサを更に含み、この温度センサは前記コネクタによって高周波電圧源 に電気的に結合されていることを特徴とする電気外科システム。 50.前記限流型電圧源は互いに他と並列に接続されると共に、共通電圧源には 直列に接続された電流制限抵抗素子を含んでいることを特徴とする請求の範囲40 に記載の電気外科システム。 51.請求の範囲40に記載の電気外科システムは、温度設定点と前記コネクタを 介して電気外科プローブから受ける温度計測値とに応 答して前記電圧源の出力を調整する温度制御装置を更に含んでいることを特徴と する電気外科システム。
