Claims (1)
- 【特許請求の範囲】 1. 補てつ装置であって、 中心軸と自身内に画された流路とを有しターゲット内腔への挿入に対してサイ ズが与えられた略管状の近位ステントであって、前記近位ステントは、前記流路 と流通するよう、自身の側壁から伸びる近位部材を有する、前記近位ステントと 、 中心軸と自身内に画された流路とを有しターゲット内腔への挿入に対してサイ ズが与えられた略管状の遠位ステントであって、前記遠位ステントは、該遠位ス テントの前記流路と流通するよう、自身の側壁から伸びる遠位部材を有する、前 記遠位ステントと、 第1の端部と第2の端部とを有し、前記近位部材と前記遠位部材との間に伸び 、前記近位ステントと前記遠位ステントとの間に流通を与えるよう、これらと流 通している、略管状の中央部材と、 を備え、前記近位ステントと遠位ステントとは、該ターゲット内腔の壁と係合す るよう、膨張可能に構成され配置される。 2. 請求項1で述べられている装置であって、前記近位部材と遠位部材とのそ れぞれが、略「Y」の接合部を画するように、前記近位ステントと前記遠位ステ ントとから伸びる。 3. 請求項1で述べられている装置であって、前記近位ステントと前記遠位ス テントとのそれぞれがメタルメッシュを備え、前記中央部材の前記第1の端部と 前記第2の端部がそれぞれ、前記近位ステントと前記遠位ステントとの間に実質 的にシールされた流通を与えるような方法で、前記近位部材の上を覆う。 4. 請求項1で述べられている装置であって、近位ステントと遠位ステントと が膨張可能なステントである。 5. 請求項1で述べられている装置であって、中央部材がポリテトラフルオロ エチレン製である。 6. 請求項3で述べられている装置であって、中央部材の前記第1の端部と前 記第2の端部とがそれぞれ、前記近位部材と前記遠位部材とに縫合される。 7. 請求項3で述べられている装置であって、中央部材の前記第1の端部と前 記第2の端部とがそれぞれ、前記近位部材と前記遠位部材とに接着剤で取り付け られる。 8. 請求項3で述べられている装置であって、中央部材の端部がフレア状であ る。 9. 請求項3で述べられている装置であって、近位部材及び遠位部材の端部が 、シール材料でコーティングされている。 10. 請求項1で述べられている装置であって、遠位部材は遠位ステントと一 体で作られ、近位部材は近位ステントと一体で作られている。 11. 請求項1で述べられている装置であって、近位部材が、近位ステントの 中心軸に対して自身の開端部が遠位部材の方を向くような傾斜で配置され、遠位 部材が、遠位部材の中心軸に対して遠位部材の開端部が近位部材の該開端部の方 を向くような傾斜で配置される。 12. 請求項1で述べられている装置であって、近位ステントと遠位ステント とのそれぞれが、略円筒状であり少なくとも開端部を1つ有し、近位部材と遠位 部材とのそれぞれが、略円筒管状部材であり開端部を有する。 13. 請求項1で述べられている装置であって、近位ステントと遠位ステント とのそれぞれが、ターゲット内腔の内壁と接触することに適合する補強材を有し ている。 14. 内腔の中の閉塞をバイパスさせるための補てつ装置であって、該装置は : 中心軸と自身内に画された流路とを有し閉塞の上流の内腔の中への挿入に対し てサイズが与えられた略管状の近位ステントであって、前記近位ステントは、前 記流路と流通するよう、自身の側壁から伸びる近位部材を有する、前記近位ステ ントと、 中心軸と自身内に画された流路とを有し閉塞の下流の内腔の中への挿入に対し てサイズが与えられた略管状の遠位ステントであって、前記遠位ステントは、該 遠位ステントの前記流路と流通するよう、自身の側壁から伸びる遠位部材を有す る、前記遠位ステントと、 第1の端部と第2の端部とを有し、前記近位部材と前記遠位部材との間に伸び 、閉塞をバイパスさせるよう前記近位部材と前記遠位部材との間に流通を与える よう、これらと流通している、略管状の中央部材と、 を備え、前記近位ステントと遠位ステントとは、該ターゲット内腔の壁と係合す るよう、膨張可能に構成され配置される。 15. 請求項14で述べられている装置であって、前記近位部材と遠位部材と のそれぞれが、略「Y」の構成を画するように、前記近位ステントと前記遠位ス テントとから伸びる。 16. 請求項14で述べられている装置であって、前記近位ステントと前記遠 位ステントとのそれぞれがメタルメッシュを備え、前記中央部材の前記第1の端 部と前記第2の端部がそれぞれ、前記近位ステントと前記遠位ステントとの間に 実質的にシールされた流通を与えるような方法で、前記近位部材の上を覆う。 17. 請求項14で述べられている装置であって、近位部材が、近位ステント の中心軸に対して自身の開端部が遠位部材の方を向くような傾斜で配置され、遠 位部材が、遠位部材の中心軸に対して遠位部材の開端部が近位部材の該開端部の 方を向くような傾斜で配置される。 18. 請求項14で述べられている装置であって、近位ステントと遠位ステン トとのそれぞれが、略円筒状であり少なくとも開端部を1つ有し、近位部材と遠 位部材とのそれぞれが、略円筒管状部材であり開端部を有する。 19. 請求項14で述べられている装置であって、近位ステントと遠位ステン トとが膨張可能なステントである。 20. 閉塞した内腔の中に、閉塞部をバイパスするように装置を設置する方法 であって、該装置は、中心軸と自身内に画された流路とを有し閉塞した内腔への 挿入に対してサイズが与えられた略管状の第1のステントであって、前記第1の ステントは、該流路と流通するよう、自身の側壁から伸びる第1の部材を有する 、前記第1のステントと;中心軸と自身内に画された流路とを有し閉塞した内腔 への挿入に対してサイズが与えられた略管状の第2のステントであって、前記第 2のステントは、該第2のステントの前記流路と流通するよう、自身の側壁から 伸びる第2の部材を有する、前記第2のステントと;前記第1のステントと前記 第2のステントとの間に連通を与えるよう、前記第1の部材に結合される第1の 端部と前記第2の部材に結合される第2の端部とを有する、略管状の中央部材と 、を備え、該方法は、以下のステップを備える: 該閉塞部の上流及び下流の一方の入口位置の或る位置に、閉塞した内腔の壁を 貫く第1の穿刺を形成するステップと; 虚脱状態において第1のステントを第1の穿刺を介して閉塞した内腔の中に挿 入するステップと; 第1のステントを閉塞した内腔の内壁に係合するように膨張させるステップと ; 第1の部材を第1の穿刺を通るように延長させるステップと; 別のステント入口位置に、閉塞した内腔の壁を貫く第2の穿刺を形成して、閉 塞部が第1の穿刺と第2の穿刺の間に配置されるステップと; 虚脱状態において第2のステントを第2の穿刺を介して閉塞した内腔の中に挿 入するステップと; 第2のステントを該内腔の内壁に係合するように膨張させるステップと; 第2の部材を第2の穿刺を通るように延長させるステップと; 閉塞部をバイパスさせるように、流体が、第1のステント及び第1の部材の中 を通り、中央部材及び第2の部材の中を通り、そして第2のステントの中を通っ て出るように流れることができるよう、皮下の組織又は筋膜の内部にある第1の 部材と第2の部材との間に中央部材を配置するステップと。 21. 請求項20で述べられている方法であって、該装置の該中央部材が合成 材料製である。 22. 第1の内腔と第2の内腔とを装置により接合する方法であって、該装置 は、自身内に画された流路とを有し第1の内腔への挿入に対してサイズが与えら れた略管状の第1のステントであって、前記第1のステントは、該流路と流通す るよう、自身の側壁から伸びる第1の部材を有する、前記第1のステントと;自 身内に画された流路とを有し第2の内腔への挿入に対してサイズが与えられた略 管状の第2のステントであって、前記第2のステントは、該第2のステントの前 記流路と流通するよう、自身の側壁から伸びる第2の部材を有する、前記第2の ステントと;前記第1のステント及び前記第2のステントと連通するように、前 記第1の部材に結合される第1の端部と前記第2の部材に結合される第2の端部 とを有する、略管状の中央部材と、を備え、該方法は、以下のステップを備える : 第1の内腔の壁を貫く第1の穿刺を形成するステップと; 虚脱状態において第1のステントを第1の穿刺を通って第1の内腔の中へと挿 入するステップと; 第1のステントを第1の内腔の内壁に係合するように膨張させるステップと; 第1の部材を第1の穿刺を通るように延長させるステップと; 別のステント入口位置に、第2の内腔の壁を貫く第2の穿刺を形成するステッ プと; 虚脱状態において第2のステントを第2の穿刺を通って第2の内腔の中へと挿 入するステップと; 第2のステントを第2の内腔の内壁に係合するように膨張させるステップと; 第2の部材を第2の穿刺を通るように延長させるステップと; 閉塞部をバイパスさせるように、流体が、第1のステント及び第1の部材の中 を通り、中央部材及び第2の部材の中を通り、そして第2のステントの中を通っ て出るように流れることができるよう、皮下の組織又は筋膜の内部にある第1の 部材と第2の部材との間に中央部材を配置するステップと。 23. 第1の内腔と第2の内腔との間に流通を形成するための補てつ装置であ って、 中心軸と自身内に画された流路とを有し第1の内腔への挿入に対してサイズが 与えられた略管状の近位ステントであって、前記近位ステントは、該流路と流通 するよう、自身の側壁から伸びる近位部材を有する、前記近位ステントと、 中心軸と自身内に画された流路とを有し第2の内腔への挿入に対してサイズが 与えられた略管状の遠位ステントであって、前記遠位ステントは、該遠位ステン トの前記流路と流通するよう、自身の側壁から伸びる遠位を有する、前記遠位ス テントと、 第1の端部と第2の端部とを有し、前記近位部材と前記遠位部材との間に伸び 、前記近位ステントと前記遠位ステントとの間に、従って前記第1の内腔と前記 第2の内腔との間に流通を与えるよう、これらと流通している、略管状の中央部 材と、 を備え、前記近位ステントと遠位ステントとは、該ターゲット内腔の壁と係合す るよう、膨張可能に構成され配置される。 24. 閉塞した内腔をバイパスするために装置を設置する方法であって、該装 置は、中心軸と自身内に画された流路とを有し閉塞した内腔への挿入に対してサ イズが与えられた略管状の第1のステントであって、前記第1のステントは、該 流路と流通するよう、自身の側壁から伸びる第1の部材を有する、前記第1のス テントと;中心軸と自身内に画された流路とを有し閉塞した内腔への挿入に対し てサイズが与えられた略管状の第2のステントであって、前記第2のステントは 、該第2のステントの前記流路と流通するよう、自身の側壁から伸びる第2の部 材を有する、前記第2のステントと;前記第1のステントと前記第2のステント との間に連通を与えるよう、第1の端部と第2の端部とを有し、前記第1の部材 と前記第2の部材との間に延長することに適合する、略管状の中央部材と、を備 え、該方法は、以下のステップを備える: 前記閉塞部に略近接した皮膚及び筋膜層の中に切開を形成するステップと; 皮膚及び筋膜層の中に中央部材をトンネリングし、第1のステントと第2のス テントとがこれらのほぼ間に該閉塞部が配置されるような方法で向きが与えられ るように、第1のステントと第2のステントとがそれに固定される、ステップと ; 該閉塞部の上流及び下流の一方の或る位置に、内腔壁を貫く第1の穿刺を形成 するステップと; 虚脱状態において第1のステントを第1の穿刺を介して内腔の中に挿入するス テップと; 第1のステントを内腔の内壁に係合するように膨張させるステップと; 第1の部材を第1の穿刺を通るように延長させるステップと; 前記第1の穿刺に近接した位置に、内腔壁を貫く第2の穿刺を形成して、閉塞 部が第1の穿刺と第2の穿刺の間に配置されるステップと; 虚脱状態において第2のステントを第2の穿刺を介して内腔の中に挿入するス テップと; 第2のステントを該内腔の内壁に係合するように膨張させるステップと; 閉塞部をバイパスさせるように、流体が、第1のステント及び第1の部材の中 を通り、中央部材及び第2の部材の中を通り、そして第2のステントの中を通っ て出るように流れることができるよう、第2の部材を第2の穿刺を通るように延 長させるステップと。
