- 【特許請求の範囲】 1.ヒトまたは動物組織で用いるための外科移植可能な装置と、付随的ポリマ ー系とからなる医療装置であって、該付随的ポリマー系が: (a)実質的に水またはヒトもしくは動物の体液に不溶性である医薬上許容さ れる、生物分解性の熱可塑性ポリマー、および該ポリマーを可溶化し、水または ヒトもしくは動物の体液と混和しまたは分散しうる生体適合性の有機溶媒;また は (b)医薬上許容される、生物分解性のインシトゥ反応性プレポリマー; からなることを特徴とする医療装置。 2.外科移植可能な装置が、縫合材、ステープル、クランプ、織物、メッシュ 、クロス、網、膜、炭素繊維、人工骨、スクリュ、骨プレート、整形外科用ロッ ド、骨ステント、爪、シリコーン弁、臀部インプラント、膝部インプラント、人 工心臓、義歯、歯科インプラントおよびカテーテルからなる群より選択される請 求項1記載の医療装置。 3.ポリマーが熱可塑性であって、ポリラクチド、ポリグリコリド、ポリカプ ロラクトン、ポリ無水物、ポリアミド、ポリウレタン、ポリエステルアミド、ポ リオルトエステル、ポリジオキサノン、ポリアセタール、ポリケタール、ポリカ ルボネート、ポリオルトカルボネート、ポリホスファゼン、ポリヒドロキシブチ レート、ポリヒドロキシバレレート、ポリアルキレンオキサレート、ポリアルキ レンスクシネート、ポリ(リンゴ酸)、ポリ(アミノ酸)、ポリ(メチルビニル エーテル)、キチン、キトサン、およびコポリマー、ターポリマー、およびその いずれかの組み合わせからなる群より選択される請求項1記載の医療装置。 4.熱可塑性ポリマーが、ポリラクチド、ポリグリコリド、ポリカプロラクト ンならびにそのコポリマーおよびターポリマーからなる群より選択される請求項 3記載の医療装置。 5.さらに、医薬上許容される、生物分解性インシトゥ反応性プレポリマーの 架橋反応を促進することのできる生体適合性硬化剤を含有する請求項1記載の医 療装置。 6.インシトゥ反応性プレポリマーがアクリル酸エステル末端の生物分解性プ レポリマーである請求項1記載の医療装置。 7.さらに、生体適合性硬化剤として過酸化ベンゾイルを配合してなる請求項 6記載の医療装置。 8.水および体液と混和しまたは分散しうる医薬上許容される有機溶媒を、医 薬上許容される、生物分解性のインシトゥ反応性プレポリマーと組み合わせてな る請求項1記載の医療装置。 9.生体適合性有機溶媒が、N−メチル−2−ピロリドン、2−ピロリドン、 C2−C6アルカノール、2−エトキシエタノール、アルキルエステル、カルボネ ート、エチレングリコール、ジメチルエーテル、プロピレングリコール、1,3 ーブチレングリコール、ε−カプロラクトン、γ−ブチロラクトン、ジメチルラ クトン、ジメチルホルムアミド、ジメチルアセトアミド、ジメチルスルホキシド 、ジメチルスルホン、環状アルキルアミド、デシルメチルスルホキシド、オレイ ン酸、N,N−ジメチル−m−トルアミド、ソルケタール、1−ドデシルアザシ クロヘプタン−2−オン、およびそのいずれかの組み合わせからなる群より選択 される請求項1記載の医療装置。 10.生体適合性有機溶媒が、N−メチル−2−ピロリドン、2−ピロリドン、 ジメチルスルホキシド、プロピレンカルボネート、エチルラクテートおよびその いずれかの組み合わせからなる群より選択される請求項9記載の医療装置。 11.付随的ポリマー系がさらに生物学的に活性な物質を含有する請求項1記載 の医療装置。 12.生物学的に活性な物質が、抗生物質、鎮痛剤、成長促進剤、凝塊剤、麻酔 剤、防腐剤、酸化防止剤およびその混合物からなる群より選択される請求項11 記載の医療装置。 13.外科移植可能な装置が、生組織に移植された時に該装置からの放出能を有 する生物学的に活性な物質をその中に組み入れる請求項1記載の医療装置。 14.付随的ポリマー系が水またはヒトもしくは動物の体液の存在にて凝集能ま たは硬化能を有する請求項1記載の医療装置。 15.外科移植可能な装置の体における適合性の亢進方法であって、 次の工程: (a)付随的ポリマー系を外科移植可能な装置および体組織に適用する工程、 ここに該付随的ポリマー系が: (i)実質的に水またはヒトもしくは動物の体液に不溶性である医薬上許容 される、生物分解性の熱可塑性ポリマー、および該ポリマーを可溶化し、水また はヒトもしくは動物の体液と混和しまたは分散しうる生体適合性有機溶媒;また は (ii)医薬上許容される、生物分解性のインシトゥ反応性プレポリマー; とからなり、 (b)該外科移植可能な装置を体組織に移植する工程 からなることを特徴とする方法。 16.さらに、付随的ポリマー系を水またはヒトもしくは動物の体液の存在下で 凝集または硬化させることからなる請求項15記載の方法。 17.外科移植可能な装置を移植する前に、付随的ポリマー系を該外科移植可能 な装置に適用する請求項15記載の方法。 18.外科移植可能な装置を移植する前に、付随的ポリマー系を組織に適用する 請求項15記載の方法。 19.外科移植可能な医療装置が、縫合材、ステープル、クランプ、織物、メッ シュ、クロス、網、膜、炭素繊維、人工骨、スクリュ、骨プレート、整形外科用 ロッド、骨ステント、爪、シリコーン弁、臀部インプラント、膝部インプラント 、人工心臓、義歯、歯科インプラント、カテーテルおよびポリマービーズからな る群より選択される請求項15記載の方法。 20.付随的ポリマー系をカテーテルの回りに適用する請求項15記載の方法。 21.医療装置が、縫合材、ステープル、クリップ、クランプ、織物、メッシュ 、クロス、網および膜からなる群より選択される請求項15記載の方法。 22.付随的ポリマー系またはその誘導体でコートされている包帯であって、該 付随的ポリマー系が: (a)実質的に水またはヒトもしくは動物の体液に不溶性である医薬上許容さ れる、生物分解性の熱可塑性ポリマー、および該ポリマーを可溶化し、水または ヒトもしくは動物の体液と混和しまたは分散しうる生体適合性の有機溶媒;また は (b)医薬上許容される、生物分解性のインシトゥ反応性プレポリマー; からなることを特徴とする包帯。 23.包帯が生物分解性であって、織物、メッシュ、膜、クロス、網およびその 組み合わせからなる群より選択される請求項22記載の包帯。 24.生物学的に活性な物質を包帯、付随的ポリマー系またはその両方に組み入 れる請求項22記載の包帯。 25.体組織の傷口をシールする方法であって、 次の工程: (a)付随的ポリマー系を包帯および体組織に適用する工程、ここに該付随的 ポリマー系が: (i)実質的に水またはヒトもしくは動物の体液に不溶性である医薬上許容 される、生物分解性の熱可塑性ポリマー、および該ポリマーを可溶化し、水また はヒトもしくは動物の体液と混和しまたは分散しうる生体適合性有機溶媒;また は (ii)医薬上許容される、生物分解性のインシトゥ反応性プレポリマー; とからなり、 (b)該付随的ポリマー系を体液の存在下で凝集または硬化させる工程 からなることを特徴とする方法。 26.外科移植用装置と、固体マトリックスとからなる医療装置であって、該固 体マトリックスが: (a)水またはヒトもしくは動物の体液を、実質的に水または動物の体液に不 溶性である医薬上許容される、生物分解性の熱可塑性ポリマー、および該ポリマ ーを可溶化し、水またはヒトもしくは動物の体液と混和したは分散しうる生体適 合性有機溶媒に加え;または (b)触媒または開始剤を、移植前に、医薬上許容される、生物分解性のイン シトゥ反応性プレポリマーに添加する ことによって付随的ポリマー系より調製される医療装置。 27.固体マトリックスが微孔を有する請求項26記載の医療装置。 28.微孔を有する固体マトリックスが生物学的に活性な物質を放出する請求項 27記載の医療装置。 29.移植装置が、縫合材、ステープル、クランプ、織物、メッシュ、クロス、 網、膜、炭素繊維、人工骨、スクリュ、骨プレート、整形外科用ロッド、爪、シ リコーン弁、臀部インプラント、膝部インプラント、人工心臓、義歯、歯科イン プラントおよびカテーテルからなる群より選択される請求項26記載の医療装置 。 30.骨障害の治療法であって、 (a)付随的ポリマー系を生骨髄における骨ステントに置き、ここに該付随的 ポリマー系が、医薬的に有効な量の生物学的に活性な薬剤と、 (i)実質的に水またはヒトもしくは動物の体液に不溶性である医薬上許容 される、生物分解性の熱可塑性ポリマー、および該ポリマーを可溶化し、水また はヒトもしくは動物の体液と混和しまたは分散しうる生体適合性有機溶媒;また は (ii)医薬上許容される、生物分解姓のインシトゥ反応性プレポリマー; とからなり、 (b)該付随的ポリマー系を凝集または硬化させる ことからなる方法。 31.付随的ポリマー系を生骨髄における骨ステントまたは骨ポートに注入する 請求項30記載の骨障害の治療法。 32.生物学的に活性な薬剤の医薬上有効量が、慢性貧血症を患っている患者に て赤血球生成を刺激するのにエリトロポイエチンが十分な量である請求項30記 載の骨障害の治療法。
