- 【特許請求の範囲】 1.移植片送達システムであって、 a)細長いシャフトと該細長いシャフトの遠位部上の送達ベースとを有するカテ ーテルと、 b)近位端と遠位端とを有し、該細長いシャフト上に圧縮状態で設けられる移植 片と、 c)該送達ベース上に設けられ、近位端および遠位端と、第1および第2の拡張 部と、取付手段とを有するアンカー部材であって、該第2の拡張部が該第1の拡 張部と該アンカー部材の該近位端との間に位置し、該取付手段が該移植片を該第 1の拡張部に連結して、それによって該第1の拡張部の拡張により該取付手段が 該移植片の該遠位端を引き寄せて該第2の拡張部を覆うようにする、アンカー部 材と、 d)該アンカー部材を拡張させる、該送達ベース上の手段と、 を含む移植片送達システム。 2.前記取付手段が、前記移植片を係合する手段を有する前記アンカー部材の 周囲に半径方向に配置された複数のステープルを含む、請求項1に記載の移植片 送達システム。 3.前記移植片を係合する手段がフックを含む、請求項2に記載の移植片送達 システム。 4.前記移植片を係合する手段がTバーを含む、請求項2に記載の移植片送達 システム。 5.前記取付手段が、前記移植片の前記遠位端から延びて前記アンカー部材を 介して巻回する複数の移植片リボンを有する、請求項1に記載の移植片送達シス テム。 6.前記取付手段が、前記移植片に織り込まれて前記アンカー部材に確実に取 り付けられた複数のワイヤを含む、請求項1に記載の移植片送達システム。 7.前記第1の拡張部が、複数の正弦曲線形状拡張部材を含む、請求項1に記 載の移植片送達システム。 8.前記アンカー部材が、前記近位端の前に前記遠位端が拡張するように構成 されている、請求項1に記載の移植片送達システム。 9.前記アンカー部材を拡張する手段が、該アンカー部材の前記近位端の前に 前記遠位端を拡張させる、請求項1に記載の移植片送達システム。 10.前記アンカー部材が自己拡張性であり形状記憶材料から形成されている、 請求項1に記載の移植片送達システム。 11.前記アンカー部材が、NiTi合金から形成されている、請求項10に記載の移 植片送達システム。 12.前記細長いシャフトが第1の外径を有し、前記送達ベースが該第1の直径 よりも大きい第2の直径を有する、請求項1に記載の移植片送達システム。 13.前記拡張する手段が、膨張可能バルーンを含む、請求項12に記載の移植片 送達システム。 14.前記膨張可能バルーンの内部と流体連通して前記細長いシャフトを介して 延びる膨張管腔を含む、請求項13に記載の移植片送達システム。 15.前記送達ベースの直径が、前記細長いシャフトの直径の約1.1〜約4.0倍で ある、請求項12に記載の移植片送達システム。 16.前記送達ベースの直径が、前記細長いシャフトの直径の約2.0〜約2.5倍で ある、請求項15に記載の移植片送達システム。 17.人工アセンブリであって、近位端と遠位端とを備えて圧縮された状態にあ る大動脈移植片と、突合わせ継ぎ手式取付手段により該移植片の該遠位端に確実 に取り付けられた拡張可能アンカー部材とを含み、該突合わせ継ぎ手式取付手段 が、該移植片の該遠位端から延びて該アンカー部材を介して巻回する複数の移植 片リボンを含む、人工アセンブリ。 18.前記アンカー部材が自己拡張性である、請求項17に記載の人工アセンブリ 。 19.前記アンカー部材が、形状記憶材料から形成される、請求項18に記載の人 工アセンブリ。 20.前記アンカー部材が、NiTi合金から形成される、請求項19に記載の人工ア センブリ。 21.前記アンカー部材が、近位端と遠位端とを有するステントと、前記移植片 に対して第2の正弦曲線形状拡張部材よりも近位側の第1の正弦曲線形状円形拡 張部材と、該第2の正弦曲線形状拡張部材に確実に取り付けられた突合わせ継ぎ 手式取付手段とを有し、該第2の正弦曲線形状拡張部材が拡張したときに、該移 植片の長手方向移動を引き起こして、該移植片の前記遠位端を引き寄せて該第1 の正弦曲線形状拡張部材を覆うようにする、請求項17に記載の人工アセンブリ。 22.前記アンカー部材が、拡張したときに、前記近位端よりも前に前記遠位端 が拡張するように構成されている、請求項21に記載の人工アセンブリ。 23.患者の大動脈の一部を治療する移植片を配置するキットであって、 a)細長いシャフトを有するカテーテルと、該細長いシャフト上の遠位部の送達 ベースと、該大動脈内に該移植片を配置する手段とを有する送達カテーテルと、 b)近位端と遠位端とを有し、圧縮状態にあるときに該細長いシャフト回りに位 置するように構成されている、大動脈移植片と、 c)該送達ベース上に位置するように構成されているアンカー部材であって、該 アンカー部材が、近位端および遠位端と、第1および第2の拡張部と、取付手段 とを有し、該第2の拡張部が該第1の拡張部と該アンカー部材の該近位端との間 に位置し、該取付手段が該大動脈移植片を該第1の拡張部に連結して、該第1の 拡張部の拡張により該取付手段が該大動脈移植片の該遠位端を引き寄せて該第2 の拡張部を覆うようにするアンカー部材と、 を含むキット。 24.前記配置手段が膨張可能バルーンを含む、請求項23に記載のキット。 25.前記送達カテーテルが、前記膨張可能バルーンの内部と流体連通して前記 細長いシャフトを介して延びる膨張管腔を含む、請求項24に記載のキット。 26.前記細長いシャフトが第1の外径を有し、前記送達ベースが該第1の外径 よりも大きい第2の外径を有し、該第2の外径が該第1の外径の約1.1〜約4.0倍 である、請求項23に記載のキット。 27.前記送達カテーテル上に位置する前記移植片および前記アンカー手段が、 約7mm未満の挿入直径を有する、請求項23に記載のキット。 28.前記取付手段が、前記移植片を係合する手段を有する前記アンカー部材の 周囲に半径方向に配置された複数のステープルを含む、請求項23に記載のキット 。 29.前記移植片を係合する手段がフックを含む、請求項28に記載のキット。 30.前記移植片を係合する手段がTバーを含む、請求項28に記載のキット。 31.前記取付手段が、前記移植片の前記遠位端から延びて前記アンカー部材を 介して巻回する複数の移植片リボンを含む、請求項23に記載のキット。 32.前記取付手段が、前記移植片に織り込まれて前記アンカー部材に確実に取 り付けられた複数のワイヤを含む、請求項23に記載のキット。 33.前記アンカー部材が、複数の正弦曲線形状円形拡張部材を含む、請求項23 に記載のキット。 34.前記アンカー部材が、拡張したときに、前記近位端より前に前記遠位端が 拡張するように構成されている、請求項23に記載のキット。 35.前記アンカー部材を拡張させる手段が、前記近位端より前に前記遠位端を 拡張させる、請求項23に記載のキット。 36.前記アンカー部材が、自己拡張性であり形状記憶材料から形成される、請 求項23に記載のキット。 37.前記アンカー部材がNiTi合金から形成される、請求項23に記載のキット。
