Abstract

A ledger panel form comprising a generally rectangular body having a frame member, plurality of elongated horizontal brackets spacedly disposed on said frame member, vertical bracket perpendicularly disposed on the lower portion of said frame member and a surface panel disposed on said frame member; said frame member comprises a pair of elongated vertical side frame members, a bottom pivot plate disposed at the bottom portion of said elongated vertical side frame members and a ledger panel member disposed at the upper portion of said elongated vertical side frame members. Said elongated vertical side frame members are provided with holes where guide bushings are disposed.
