Abstract
Ovaj pronalazak se odnosi na korišćenje inhibitora IL-18 za pripremu medikamenta za lečenje i/ili prevenciju povreda centralnog nervnog sistema a pre svega traumatske povrede glave.
Description
OBLAST TEHNIKE
Ovaj pronalazak je na polju patoloških stanja mozga. Još specifičnije, on se odnosi na korišćenje inhibitora IL-18 za lečenje i/ili prevenciju povreda centralnog nervnog sistema (CNS) , pre svega traumatskih povreda mozga.
STANJE TEHNIKE
Godine 1989, opisana je aktivnost seruma izazvana endotoksinom koju je izazvao interferon-γ (IFN-γ) dobijen iz slezine miša (Nakamura et al., 1989). Ova serumska aktivnost je funkcionisala ne kao direktna indukcija IFN-γ, već pre kao ko -stimulant zajedno sa IL-2 ili mitogenima. Pokušaj da se purifikuje aktivacija iz postendotoksinskog mišjeg seruma otkrio je naizgled homogeni 50-55 kDa protein. Budući da drugi citokini mogu da deluju kao ko-stimulanti za produkciju IFN-γ, neuspeh da se neutralizuju antitela na IL-1 , IL-4, IL-5, IL-6, ili TNF da se neutralizuje serumska aktivnost ukazuje da je to bio nedvosmisleni faktor. Godine 1995, isti naučnici su pokazali da je endotoksinom-indukovani ko-stimulant za produkciju IFN-γ prisutan u ekstraktima jetre miševa prethodno kondicioniranih sa P. acnes (Okamura et al., 1995). U ovom modelu, populacija makrofaga jetre (Kupfferove ćelije) su se širile, pa kod tih miševa niska doza bakterijskog lipopolisaharida (LPS), koja kod prethodno kondicioniranih miševa nije smrtonosna, postaje smrtonosna. Ovaj faktor, nazvan faktor kojhi indukuje IFN-γ (IGIF) i kasnije prepoznat kao interleukin-18 (IL-18), prodšćen je do homogenosti od 1.200 grama jetre miševa tretiranih sa P. acnes. Degenerisani oligonukleotidi izvedeni iz aminokiselinskih sekvenci prečišćene IL-18 korišćeni su za kloniranje mišje IL-18 cDNK. IL-18 je jedan 18-19 kDa protein od 157 amino kiselina, koja nema očiglednih sličnosti sa ma kojim peptidom u bazama podataka. Glasnička RNK za IL-18 i interleukin-12 (IL-12) već su detektovane u Kupfferovim ćelijama i aktiviranim makrofagima. Rekombinantni IL-18 indukuje IFN-gama jače od IL-12, očigledno potpuno odvojenim putem (Micallef et al., 1996). Slično endotoksinomindukovanoj serumskoj aktivnosti, IL-18 ne indukuje IFN-γ po sebi, već prvenstveno deluje kao ko-stimulant sa mitogenima ili IL-2. IL-18 pojačava proliferaciju T ćelija, očigledno preko puta koji je zavisan od IL-2, i pokačava produkciju Th1 citokina in vitro i ispoljava sinergiju kada se kombinuje sa IL-12 u smislu pojačane produkcije IFN-γ (Maliszewski et al., 1990).
Pošto je kloniran mišji oblik, godine 1996 zabeležena je i humana cDNK za IL-18 (Ushio et al., 1996).
Kloniranjem IL-18 Iz zahvaćenih tkiva i ispitivanjem IL-18 genske ekspresije, utvrđena je tesna veza ovog citokina sa autoimunim bolestima. Negojazni dijabetični miševi (NOD) spontano razvijaju autoimuni insulitis i dijabetes, što se može ubrzati i sinhronizovati samo jednom injekcijom ciklofosfamida. IL-18 mRNK je pokazana pomoću PCR reverzne transkriptase kod pankreasa NOD miša tokom rane faze insulitisa. Nivoi IL-18 mRNK povećavaju se brzo posle tretmana ciklofosfamidom i prethode porastu IFN-γ mRNK, i potom dijabetesu. Zanimljivo je da ova kinetika podražava kinetiku IL-12-p40 mRNK, što dovodi do bliske korelacije individualnih nivoa mRNK. Kloniranje IL-18 cDNK iz pankreasne RNK praćeno sekvenciranjem otkriva identičnost sa IL-18 sekvencom kloniranom iz Kupfferovih ćelija i in vivo prethodno aktiviranih makrofaga. Isto tako, makrofagi NOD miša su reagovali na ciklofosfamid IL-18 genskom ekspresijom dok makrofagi Balb/c miševa paralelno nisu. Prema tome, IL-18 ekspresija je abnormalno regulisana kod autoimunih NOD miševa i tesno povezana sa razvojem dijabetesa (Rothe et al., 1997).
IL-18 igra potencijalnu ulogu u imunoregulaciji ili inflamaciji povećavanjem funkcionalne aktivnost Fas liganda na Th1 ćelije (Conti et al., 1997). IL-18 je takođe eksprimiran u adrenalnom korteksu pa stoga može sekretovati neuro-imunomodulator, koji igra važnu ulogu u orkestraciji imunološkog sistema po stresogenom iskustvu (Chater, 1986).
In vivo, IL-18 se formira cepanjem pro-IL-18, i njegova endogena aktivnost izgleda objašnjava produkciju IFN-γ u P. acnes i smrti posredovanoj LPS-om. Zreli IL-18 se proizvodi od svoga prekursora IL-Ιβ konvertujućeg enzima (IL-1beta-konvertujući enzim, ICE, kaspaze-1).
IL-18 receptor se sastoji od najmanje dve komponente, koje sarađuju u vezivanju liganda. Mesta vezivanja visokog i niskog afiniteta za IL-18 nađena su u mišjim IL-12 stimulisanim T ćelijama (Yoshimoto et al., 1998), ukazuje na multipli lančani receptorski kompleks. Do sada su identifikovane dve receptorske podjedinice gde obe pripadaju porodici IL-1 receptora (Parnet et al., 1996). Transdukcija signala IL-18 uključuje aktivaciju NF-kΒ (DiDonato et al., 1997).
Claims (12)
1. Upotreba IL-18 inhibitora, pri čemu je IL-18 inhibitor IL-18 vezujući protein (IL-18 BP), za proizvodnju leka za lečenje traumatske povrede mozga (zatvorene povrede glave), gde se 3 dana nakon povrede daje jedna doza IL-18 BP.
2. Upotreba prema bilo kojem zahtevu pod 1, naznačena time što je protein koji vezuje IL-18 glikozilovan na jednom ili vise položaja.
3. Upotreba prema bilo kom zahtevu pod 1, naznačena time što je IL-18 BP fuzionisan za sav ili za deo imunoglobulina (Ig).
4. Upotreba prema bilo kojem od prethodnih zahteva, naznačena time što lek sadrži i interferon.
5. Upotreba prema zahtevu 4, naznačena time što interferon je interferon-α ili inreferon-β.
6. Upotreba prema bilo kojem od prethodnih zahteva, naznačena time što lek obuhvata i faktor nekroze tumora (TNF).
7. Upotreba prema zahtevu 6, naznačena time što TNF je TNF alfa.
8. Upotreba prema bilo kojem od prethodnih zahteva, naznačena time što lek sadrži i anti-inflamatorni agens.
9. Upotreba prema zahtevu 8, naznačena time što je anti-inflamatorni agens COX-inhibitor, tačnije COX-2 inhibitor.
10. Upotreba prema bilo kojem od prethodnih zahteva, naznačena time što lek sadrži i antioksidant.
11. Upotreba prema bilo kojem od prethodnih zahteva, naznačena time što se inhibitor IL-18 koristi u količini od oko 0.001 do 100 mg/kg telesne mase, ili oko 0.01 do 10 mg/kg telesne mase, ili oko 0.1 do 5 mg/kg telesne mase, ili oko 1 do 3 mg/kg telesne mase.
12. Upotreba prema bilo kojem od prethodnih zahteva, naznačena time što se IL-18 inhibitor daje potkožno.
