Abstract
Description
The invention relates to medicine, namely to neurology.
- 2005. Vol. - 4. - P.752-759).It is known that the incidence of dementia in elderly people is high and tends to further increase due to stroke, which increases the risk of dementia by 4-12 times. So, the frequency of dementia in the first 3-6 months after a stroke ranges from 5 to 32%, and after 12 months - from 8 to 26%. Among the total number of people, the proportion of patients with dementia who had a stroke was 7%, after 3 years - 10%, after 25 years - 48% (Leys D. et al. Poststroke dementia // Lancet Neyrology. - 2005. Vol. - 4. - P.752-759).
Х., Albert M.S. et al. Predicting time to nursing home care and death in individuals with Alzheimer disease // Journal of the American Medical Association, - 1997. - #277. - P.806-812). С этим исходом связаны следующие прогностические факторы: наличие экстрапирамидных симптомов - относительный риск (ОР) равнялся 2,56 при 95% доверительном интервале (ДИ) от 1,49 до 4,41; наличие психотических симптомов (ОР=1,50 при ДИ от 1,04 до 2,15); появление первых признаков заболевания в возрасте до 65 лет (ОР=1,62 при ДИ от 0,99 до 2,67).There is a method for predicting dementia in Alzheimer's disease, which allows to predict the length of the period from the appearance of symptoms of dementia (dementia) to the development of addiction in patients (Stern Y., Tang M.Kh., Albert MS et al. Predicting time to nursing home care and death in individuals with Alzheimer disease // Journal of the American Medical Association, - 1997. - # 277. - P.806-812). In this work, the authors refer to prognostic risk factors: age, neurological symptoms of the disease, level of cognitive decline. At the start of the study, all patients were already diagnosed with mild dementia. Patient follow-up continued for 7 years. In 164 (69%) patients there was an addiction to outside help and the need for placement in a nursing home or similar institution providing care and treatment. The following prognostic factors are associated with this outcome: the presence of extrapyramidal symptoms - the relative risk (RR) was 2.56 with a 95% confidence interval (CI) of 1.49 to 4.41; the presence of psychotic symptoms (RR = 1.50 with CI from 1.04 to 2.15); the appearance of the first signs of the disease under the age of 65 (RR = 1.62 with a CI from 0.99 to 2.67).
Фактором, связанным с низкой вероятностью этого исхода, был более высокий показатель по краткой шкале оценки психического статуса (MMSE) в начале заболевания (ОР=0,93 при ДИ от 0,90 до 0,96).A factor associated with the low likelihood of this outcome was a higher rate on a brief scale of assessment of mental status (MMSE) at the onset of the disease (RR = 0.93 for CI from 0.90 to 0.96).
The disadvantages of this method include the fact that the forecast relates to the period of onset of addiction in assistance, i.e. the patient already has symptoms of dementia. The known method is not intended and cannot be used to predict the onset of dementia, since the authors did not consider pre-dementive cognitive impairment or moderate cognitive impairment, which is the "preclinical" stage of dementia.
(MMSE).The closest in technical essence to the proposed one is a method for predicting the development of post-stroke dementia, including the determination of risk factors - indicators of neuropsychological testing (S.P. Markin, V.A. Markina. Neuropsychological study of patients who have had a stroke. // Applied informational aspects of medicine. - Volume 10. - No. 1. - S.64-69). To implement the known method, neuropsychological testing is carried out: a “5 words” test, a Schulte test, a “clock drawing” test, an assessment of mental status on the MMSE scale and a battery for frontal dysfunction (FAB). The authors of the known method found that the combination of the “5 words” test, Schulte’s test and the “clock drawing” test allows to identify patients with cognitive impairment, and for a more accurate diagnosis, namely the identification of pre-dementia disorders, the mental status assessment scale (MMSE) can be used )
The disadvantages of this method include the fact that it is intended for the diagnosis of cognitive impairment, while the leading criterion is the assessment of mental status on the MMSE scale. This scale can only be used to identify severe cognitive impairment. Post-stroke cognitive impairment is a heterogeneous state, for the precise diagnosis of which additional diagnostic studies are necessary. Those. the known method does not allow to predict, as well as to identify less pronounced cognitive impairment.
The objective of the invention is to develop a method for predicting the development of post-stroke dementia in patients with post-stroke moderate cognitive impairment (UKI), based on the assessment of risk factors affecting its development.
The technical result of the proposed method is to ensure the accuracy of the prognosis of the development of post-stroke dementia in the next 3-year period in a patient with the consequences of acute cerebrovascular accidents (cerebrovascular accidents) and cognitive impairments that do not reach the degree of dementia.
The technical result of the proposed method is achieved by the fact that to predict the development of post-stroke dementia, risk factors are determined from the anamnesis of life and clinical and laboratory parameters.
The difference of the method lies in the fact that magnetic resonance imaging of the brain is additionally carried out, focal and diffuse changes in the brain are determined in the frontoparietal and temporal-occipital regions, both in the right and left hemispheres, as well as in the region of the optic tubercle .
Distinctive techniques of the proposed method is also the fact that from the anamnesis of life determine: age, coronary heart disease (CHD) and diabetes.
The difference of the proposed method lies in the fact that the established risk factors are assigned gradations (a 1 ... 10 ) and numerical values, where
а1 - возраст: 50-59 лет - 0; 60-69 лет - 1; 70-79 лет - 2;and 1 - age: 50-59 years - 0; 60-69 years - 1; 70-79 years - 2;
а2- ИБС: нет - 0; есть - 1;and 2 - coronary heart disease: no - 0; there is - 1;

а6 - постинсультные очаговые изменения в левом полушарии, лобно-теменная область: нет - 0; есть - 1;and 6 - post-stroke focal changes in the left hemisphere, fronto-parietal region: no - 0; there is - 1;
а7 - постинсультные очаговые изменения в левом полушарии, височно-затылочная область: нет - 0; есть - 1;and 7 - post-stroke focal changes in the left hemisphere, temporal-occipital region: no - 0; there is - 1;

The difference between the method also lies in the fact that the prognostic factors F 1 and F 2 are determined by the formulas:
The authors of the proposed method found that with F 2 greater than or equal to F 1, a high risk of developing dementia is predicted, and with F 1 greater than F 2 , the risk of developing dementia is low.
A comparative analysis with the prototype showed that the proposed method differs from the known above methods and, therefore, meets the criteria of the invention of "novelty."
From the analysis of patent and specialized literature, the authors found that the proposed method has features that distinguish it not only from the prototype, but also other technical solutions in this and related fields of medicine. The authors of the proposed method did not identify technical solutions that allow to carry out a forecast for the development of post-stroke dementia. Clinical observations of the authors of the proposed method indicate that the use of the proposed technical solution allows us to predict the development of post-stroke dementia over the next 3 years in a particular patient with post-stroke UKN, not reaching the degree of dementia. According to the authors of the proposed method, the accuracy of the forecast for the development of post-stroke dementia is 86.6%. The above allows us to conclude that the technical solution meets the criterion of "inventive step".
A method for predicting the development of post-stroke dementia, comprising the claimed invention, is intended for use in healthcare. The implementation of its capabilities is confirmed by the clinical examples described in the application. From the foregoing, it follows that the claimed invention meets the condition of patentability "industrial applicability".
The proposed method is as follows.
A clinical neuropsychological and neuroimaging examination using magnetic resonance imaging (MRI) of the brain is performed on a patient with the consequences of a stroke and cognitive impairment that does not reach the degree of dementia.
Based on the history of diseases and the results of the examination, risk factors are identified to which appropriate gradations and numerical values are assigned.
The values of prognostic factors F 1 and F 2 are determined by the above formulas, after which their numerical characteristics are compared. If F 1 is greater than F 2, a low risk of developing post-stroke dementia is predicted. If the value of F 2 is greater than or equal to F 1 , then the patient is at risk of developing post-stroke dementia for 3 years. On this basis, the patient should be recommended to conduct appropriate therapeutic measures, consult specialists, eliminate and correct “managed” risk factors.
The proposed method for predicting the development of post-stroke dementia is confirmed by examples of specific performance.

After determining and establishing gradations of risk factors, the values of prognostic factors F 1 and F 2 were calculated using the above formulas:
After the calculation, the values of the evaluation functions were obtained: F 1 = -0.7; F 2 = 1.31. Comparing their numerical characteristics, we find that F 2 is greater than F 1 , therefore, the patient is at risk of developing dementia.

1. Возраст - 65 лет, a1=1;1. Age - 65 years, a 1 = 1;
2. ИБС - нет, а2=0;2. IHD - no, but 2 = 0;
3. Сахарный диабет 2 типа - нет, а3=0;3. Type 2 diabetes mellitus - no, but 3 = 0;
4. Очаговые постинсультные изменения в правом полушарии в лобно-теменной области - нет, а4=0;4. Focal post-stroke changes in the right hemisphere in the frontoparietal region - no, but 4 = 0;
5. Очаговые постинсультные изменения в правом полушарии в височно-затылочной области - нет, а5=0;5. Focal post-stroke changes in the right hemisphere in the temporal-occipital region - no, but 5 = 0;
6. Очаговые постинсультные изменения в левом полушарии в лобно-теменной области - есть, а5=1;6. Focal post-stroke changes in the left hemisphere in the frontoparietal region - there are, and 5 = 1;
7. Очаговые постинсультные изменения в левом полушарии в височно-затылочной области - нет, а7=0;7. Focal post-stroke changes in the left hemisphere in the temporal-occipital region - no, but 7 = 0;
8. Постинсультные изменения в области зрительного бугра - нет, а8=0;8. Post-stroke changes in the area of the optic tubercle - no, but 8 = 0;
9. Субкортикальные очаги лейкоареоза в базальных ядрах (бледный шар) - нет, а9=0;9. Subcortical foci of leukoaraiosis in the basal nuclei (pale ball) - no, but 9 = 0;
10=1.10. Subcortical foci of leukoaraiosis in the white matter of the frontal lobes - is, and 10 = 1.
F2.After determining and establishing gradations of risk factors, the values of prognostic factors F 1 and F 2 were calculated.
F1=-0,17; F2=-6,15.Values of the evaluation functions: F 1 = -0.17; F 2 = -6.15.
A comparison of their numerical characteristics showed that F 1 is greater than F 2 therefore, the patient is not threatened by the development of dementia.
Conclusion: the prognosis was carried out according to the condition of the patient in 2006.
In 2009, the patient retained VCH.
86,6%.Evaluation of the effectiveness of the proposed forecasting method was carried out in the training and control groups. In the training group of 128 patients, the prediction accuracy of post-stroke dementia was 77.4%. In the control group, 30 patients with post-stroke dementia were included; the diagnosis was accurate in 26 patients; the accuracy of prediction was 86.6%.
Thus, the use of the proposed method for predicting the development of post-stroke dementia in patients with post-stroke VHF allows identifying patients with an increased risk of dementia and timely treatment and preventive measures, which will prevent the progression of the course of cognitive impairment and the development of dementia in this group of patients.
