Definitions

  • the invention relates to an orthopedic insole according to the preamble of claim 1. Furthermore, the invention relates to a method for producing the insole and further to an insole blank.
  • Orthopedic insoles for shoes have been known for a long time.
  • the aim of such insoles is to eliminate orthopedic defects and misalignments of the foot and the associated musculoskeletal system or to mitigate their adverse effects.
  • EP 2000039 A1 discloses insoles with cushioning inserts in the area of the forefoot, the longitudinal arch and the heel.
  • DE 3304537 A1 shows an insole that is adapted to an impression by heating and molding a blank.
  • EP 0893112 A1 discloses an insole with a pad-shaped elevation in the metatarsal area.
  • the object of the present invention is the production and use of an orthopedic insole blank and the insole formed therewith, which takes into account the defects and misalignments of the individual foot and corrects them.
  • the production of an insole blank according to the invention and the insole formed therewith is described below, for example.
  • a foam impression of the foot is made in the usual way.
  • the foam impression is then adjusted to the outer contour of the shoe according to its shape and length.
  • a cast is then made from the foam impression.
  • Plaster or plaster of Paris can be used for this, for example.
  • the casting compound should be sufficiently stable to prevent breakage. After hardening, the casting compound must be easy to work with abrasive agents or blades.
  • the cast When using plaster, the cast is then manually modelled according to the misalignments and errors to be corrected by removing material from the sole area of the cast.
  • the recess for the formation of the metatarsal elevation is brought forward into the area of the metatarsophalangeal joints. Elevations in the area of the toe prints, the ball of the foot prints and the heel are levelled, for example by planing, and connected to one another over an area along the outer edge of the footprint to form a continuous flat surface. Unevenness in the cast that should not be reflected in the insole is removed, for example by grinding or scraping.
  • an insole blank of the appropriate size is heated in hot air to about 180° C and pulled under vacuum onto the cast, which may have been post-treated.
  • the insole blank has a raised area in the metatarsal area that extends up to the metatarsophalangeal joints and has a material volume that is sufficient for the formation of the metatarsal elevation and other elevations such as the sagittal elevation and cuboid support.

Description

Schuheinlage shoe insole

The invention relates to an orthopedic insole according to the preamble of claim 1. Furthermore, the invention relates to a method for producing the insole and further to an insole blank.

Orthopedic insoles for shoes have been known for a long time. The aim of such insoles is to eliminate orthopedic defects and misalignments of the foot and the associated musculoskeletal system or to mitigate their adverse effects.

For example, EP 2000039 A1 discloses insoles with cushioning inserts in the area of the forefoot, the longitudinal arch and the heel. DE 3304537 A1 shows an insole that is adapted to an impression by heating and molding a blank. EP 0893112 A1 discloses an insole with a pad-shaped elevation in the metatarsal area.

One problem with conventional insoles is that they do not take into account the individual defects and misalignments of the foot. To make the insoles, orthotists use prefabricated blanks that in many cases cannot be fully adapted to the individual foot.

The object of the present invention is the production and use of an orthopedic insole blank and the insole formed therewith, which takes into account the defects and misalignments of the individual foot and corrects them. The production of an insole blank according to the invention and the insole formed therewith is described below, for example. First, a foam impression of the foot is made in the usual way. The foam impression is then adjusted to the outer contour of the shoe according to its shape and length.

A cast is then made from the foam impression. Plaster or plaster of Paris can be used for this, for example. The casting compound should be sufficiently stable to prevent breakage. After hardening, the casting compound must be easy to work with abrasive agents or blades.

When using plaster, the cast is then manually modelled according to the misalignments and errors to be corrected by removing material from the sole area of the cast. According to the invention, the recess for the formation of the metatarsal elevation is brought forward into the area of the metatarsophalangeal joints.

Then an insole blank of the appropriate size is heated in hot air to about 180° C and pulled under vacuum onto the cast, which may have been post-treated. According to the invention, the insole blank has a raised area in the metatarsal area that extends up to the metatarsophalangeal joints and has a material volume that is sufficient for the formation of the metatarsal elevation and other elevations such as the sagittal elevation and cuboid support.

The insole blank according to the invention in a two-layer structure is made of two plastics that are fused together in injection molding. The first plastic has a harder structure and extends from the heel area to the talus area. The second, softer plastic extends from the talus area forward to the toe area. The harder plastic extends a tab within the softer plastic in the middle of the insole blank up to the area of the metatarsal elevation.

The plastics preferably have a specific weight of around 0.3 g per cm 3. The harder structure of the first plastic is chosen so that the insole only allows a small amount of deformation when walking. In contrast, the second, softer plastic is flexible, so that the foot can roll over without any problems.

A three-layer structure can be used to complete the insole, with a third layer being glued to the entire surface of the insole on the surface facing the foot as a cushioning layer. For example, this is a soft padding material with a Shore hardness of 15 to 20. The layer thickness is, for example and preferably, around 1.6 mm. A soft padding material from the Dyafoam brand has proven to be effective.

If such a third layer is not to be used, for example because the insole thickness should be kept as low as possible, the foot-side area can also be covered with a thin fiber layer such as microfiber.

The advantage of the three-layer variant mentioned above is evident in patients who have problems with diabetes or rheumatism. The better pressure distribution resulting from the third layer prevents pressure points.

The purpose of the advanced metatarsal elevation according to the invention is to stimulate the gripping effect of the toes and thus improve blood circulation in the sole area.

In another variant of the insole design, a flat stiffening plate can be placed on the lower surface of the soft area of the insole blank, which extends from the toe area to the metatarsal area. The stiffening of the front area of the insole achieved in this way has positive effects on decompensated flat feet and splayed feet, as occurs with so-called hollow feet. If the aforementioned flat stiffening plate is not desired in order to keep the insert thickness low, a strip-shaped stiffening plate in the form of an S-shaped strip made of stiff material, preferably carbon, can be inserted instead of the flat stiffening plate, which is glued to the insert from below and has a similar effect to the flat stiffening plate.

The features of the invention are described in more detail below.

The orthopedic insole with raised and lowered sections to support the foot can have the following features:

- a supporting layer which is thermally deformed as an insole blank according to the orthopedically desired shape of the insole, whereby a metatarsal elevation is brought forward into the area of the metatarsophalangeal joints and the supporting layer has a material volume which is sufficient for the formation of the metatarsal elevation and other elevations, such as sagittal elevation and cuboid support.

Weiters kann vorgesehen sein, dass der Einlagenrohling einstückig spritzgegossen ist und zwei Arten von Kunststoff umfasst, Furthermore, it can be provided that the insert blank is injection-molded in one piece and comprises two types of plastic,

- wherein the plastics preferably each have a density of about 0.3 g/cm 3 and wherein the plastics have different stiffnesses and wherein the stiffer plastic is arranged in a rear region of the insole blank, in particular substantially in the region of the heel and the tarsal bones, and wherein the less stiff plastic is arranged in a front region of the insole blank, in particular substantially in the region of the metatarsal bones and the toes.

Furthermore, it can be provided that a fully modelled transition from the transverse metatarsal elevation to the sagittal elevation is formed. A continuous elevation can therefore extend from the sagittal area to the metatarsal area, whereby the sagittal elevation in particular can be connected to the metatarsal elevation via a bridge. Preferably, a tab of the harder plastic can extend within the softer plastic in the middle of the insole blank into the area of the metatarsal elevation. Particularly preferably, the tab of the harder plastic can extend up to about 50% of the extent of the metatarsal elevation in the direction of the metatarsophalangeal joints.

The insole blank can, for example, have a minimum thickness of 2 to 5 mm, preferably 4 mm, and the metatarsal elevation in the metatarsal area can have a thickness adapted to the foot of up to 4 times the minimum thickness, preferably 3 times.

As an alternative, a stiffening plate can be arranged on the underside of the base layer, essentially in the front, softer area.

The stiffening plate can extend over a large area across the underside of the softer, front part of the support layer, essentially in the area of the metatarsal and toes.

Alternatively, the stiffening plate may extend in a substantially S-shape from the area beneath the tongue to below the support area of the little toes.

If necessary, it can also be provided that the area of the heel impression is designed to provide moderate space for the heel bone and is preferably straight up to a base of the sagittal elevation or up to a base of the cuboid support, wherein the base of the sagittal elevation in the longitudinal extension of the insert is essentially at the same height as the base of the cuboid support.

Das Verfahren zur Herstellung orthopädischer Einlagen umfasst die Schritte: If necessary, the sagittal elevation can have an ascending branch and a descending branch. The ascending branch can run from the base of the sagittal elevation to the highest point of the sagittal elevation in the area of the talus and can be modeled, in particular reworked. The descending branch can run from the highest point of the sagittal elevation to the imprint of the ball of the big toe and follow the natural shape of the footprint. The ascending branch preferably rises more steeply on average than the descending branch descends. The process for manufacturing orthopedic insoles includes the following steps:

- Erstellen eines negativen Trittschaumabdrucks des Fußes eines Patienten, - Creating a negative foam impression of a patient’s foot,

- Erweitern einer Außenkontur des Trittschaumabdrucks auf Form und Länge entsprechend der Schuhgröße des Patienten, - Extending the outer contour of the foam impression to the shape and length according to the patient’s shoe size,

- Erstellen eines positiven Abdrucks durch Ausgießen des Trittschaumabdrucks, vorzugsweise mit Stukkaturgips, - Creating a positive impression by pouring the foam impression, preferably with plaster of Paris,

- Bearbeiten des positiven Abdrucks zur Ausbildung der für die Korrektur der Fußsohle benötigten Anhebungen und Senkungen der Sohlenfläche der Einlage, - Processing the positive impression to form the elevations and depressions of the sole surface of the insole required for the correction of the sole of the foot,

- Auswahl eines Einlagenrohlings entsprechend der Schuhgröße des Patienten,- Selection of an insole blank according to the patient’s shoe size,

- Erwärmen des Einlagenrohlings, insbesondere mittels Heißluft, auf vorzugsweise 100°C bis 250°C, besonders bevorzugt 160°C bis 200°C, gegebenenfalls 180°C,- Heating the insert blank, in particular by means of hot air, to preferably 100°C to 250°C, particularly preferably 160°C to 200°C, optionally 180°C,

- Formgeben durch Aufziehen des erwärmten Einlagenrohlings auf den bearbeiteten positiven Abdruck, bevorzugt unter Vakuum, und anschließend Abkühlen, - Shaping by pulling the heated insole blank onto the processed positive impression, preferably under vacuum, and then cooling,

- Nachbearbeitung des Einlagenrohlings nach der Formgebung - Post-processing of the insole blank after shaping

- Endfertigung der Einlage. - Final production of the insert.

When processing the positive impression, recesses can be created to model structures to treat the patient's misalignments.

The insole blank can be injection-molded in one piece and comprise two types of plastic,

- wobei die Kunststoffe bevorzugt jeweils eine Dichte von etwa 0,3 g/cm3 aufweisen und- wherein the plastics preferably each have a density of about 0.3 g/cm 3 and

- wobei die Kunststoffe unterschiedliche Steifigkeiten aufweisen und - whereby the plastics have different stiffnesses and

- wobei der steifere Kunststoff in einem hinteren Bereich, insbesondere im Wesentlichen im Bereich der Ferse und der Fußwurzelknochen, des Einlagenrohlings angeordnet ist und - wherein the stiffer plastic is arranged in a rear region, in particular essentially in the region of the heel and the tarsal bones, of the insole blank and

- wherein the less rigid plastic is arranged in a front region, in particular essentially in the region of the metatarsal bones and the toes, of the insole blank.

A tab of the harder plastic can extend within the softer plastic in the middle of the blank into the metatarsal area.

Preferably, the plastics of the insole blank are thermoplastically deformable. The insole blank is shaped to match the form of the positive impression, particularly to form a metatarsal elevation and/or a sagittal elevation and/or a cuboid support.

During the final production of the insole, a full-surface layer is applied to one side of the insole, particularly the one facing the foot.

The full-surface layer can comprise a soft padding material with a Shore hardness of 15 to 20 and preferably a thickness of 1.6 mm as a dampening layer. The full-surface layer can also be a cover comprising microfiber material.

During the final production of the insole, a stiffening plate, preferably a carbon plate, can be attached to at least a partial area of an underside of the insole, in particular the underside facing away from the foot.

The stiffening plate extends essentially over a front area of the insole, especially in the area of the metatarsal bones and the toes.

Alternatively, the stiffening plate on the underside of the insole may extend in a substantially S-shape from a central region to a region of the insole whose upper surface is intended to come into contact with the little toe.

Es zeigen: The invention is explained in more detail below in several variants with reference to the drawings. They show:

Fig. 6 is a sectional view of a longitudinal section through an insert, showing the side of the insert facing the outer edge and Fig. 7 is a sectional view of a longitudinal section through an insert, showing the side of the insert facing the inner edge.

Figure 1 shows a schematic representation of a cast 12 of a foam footprint in plan view. The foam footprint was expanded to adapt the shape of the insert to the shape of the shoe in which it is to be used. Therefore, the cast 12 is larger than the actual footprint and the edge 11 of the cast is shifted outwards, particularly in the area of the toes, compared to the area of the toe prints 16, thus expanding the area of the footprint. The areas of the foot that are particularly likely to come into contact with the ground form clear areas of the footprint. These areas are the toe print area 16, the ball print area 17 and the heel print area 18. The ball print 19 forms the largest proportion of the ball print area 17. The area in the middle of the footprint, which begins in the ball print area and extends towards the heel, is called the metatarsal area 13 and forms a transverse arch. The arch of the foot along the inner edge 21 of the foot is called the sagittal area 20.

Figures 2a and 2b show schematic representations of a reworked cast 12 of a foot foam impression. Figure 2a shows a topographical view of the reworked cast 12, Figure 2b shows the same reworked cast 12 with highlighted areas. The rework of the cast is also called modeling. During modeling, the surface of the cast 12 is planed until a continuous, flat surface is formed which connects the area of the toe impressions 16, the area of the ball impressions 17 and the area of the heel impression 18 over a narrow area along the outer edge 22. This planed, flat surface can be seen in Figures 2a and 2b as a continuous white surface without topographical contour lines. Depressions remain in the cast 12 which lie between the area of the toe impressions 16 and the area of the ball impressions 17 as well as in the sagittal area 20. In the metatarsal area 13 behind the area of the ball impressions 17 in the middle of the footprint, a metatarsal depression 23 is incorporated into the cast 12. The transition from the transverse area of the metatarsal area 13 to the sagittal area 20 is thus modelled as a metatarsal depression 23. In contrast, the depression in the cast 12 between the areas the toe impressions 16 and the ball impressions 17 and the front part of the sagittal area 20, behind the impression of the ball of the big toe 19, unprocessed. In addition, a depression 24 is modeled on the outer edge 22 of the cast 12 in front of the area of the heel impression 18, which later serves to form the cuboid support 6. The modeled areas of the metatarsal depression 23 and the depression 24 are highlighted in Figure 2b by hatching in one direction, while unprocessed areas have hatching in the opposite direction and planed areas are recognizable as a continuous white area.

Figure 3 shows a schematic representation of an insole blank 2 according to the invention. The raised so-called heel shell edge 25 runs around the area of the heel impression 18. This merges into the sagittal elevation 5 at the inner edge 21, in the illustration on the right edge. On the outer edge 22, in the illustration on the left edge, there is a pronounced elevation in front of the area of the heel impression 18, which is referred to as a cuboid support 6. In the metatarsal area 13, the insole blank 2 has a metatarsal elevation 3. The insole blank 2 has a soft front part 8 and a hard back part 7. The material of the hard back part 7, however, extends within the soft front part 8 in the form of a tab 9 into the area of the metatarsal elevation 3.

Figure 4 shows a schematic representation of the top side 14 of an insert 1. The insert 1 was produced from the insert blank 2 according to Fig. 3 by heating it, in particular using hot air, to preferably 180°C and pulling it onto the reworked cast 12 according to Fig. 2, preferably under vacuum, and then cooling it down. By pulling the insert blank 2 onto the heated state, the elevations and depressions of the reworked cast 12 are transferred to the insert blank 2 and an insert 1 adapted to the cast 12 is created. Transferred elevations and depressions thus form, among other things, the support areas of the toes in the area of the toe impressions 16, the ball of the toes in the area of the ball of the foot impressions 17 and the heel in the area of the heel impression 18, enclosed by the heel shell edge 25. In addition, the shape of the cuboid support 6, the metatarsal area 13 and the sagittal area 20 of the insole blank 2 are adapted to the shape of the modeled cast 12. This creates a fully modeled transition from the transverse metatarsal elevation 3 to the sagittal elevation 5. The heel area is preferably moderately spaced and preferably straight up to the attachment 27 of the sagittal elevation or up to the attachment of the cuboid support 6. This ensures that no undesirable pressure is exerted on the heel. The medial and lateral edges 25 are straight from the cuboid support 6 up to the attachment 27 of the sagittal elevation 5 and are provided with an integrated, exposed tendon attachment (medial Achilles tendon set). This means that there is no friction or pressure at the attachment of the sagittal elevation 5. This is a prevention against or alleviation of an existing exostosis process (medial heel spur). It also provides relief against a flat foot problem.

Derived from the sagittal elevation 5, a distinctive, well-modeled metatarsal bridge is provided in the longitudinal axis of the foot over the metatarsal to directly under the metatarsal joints of the 2nd to 4th toe rays. This ensures very good guidance of the metatarsal and prevents medial and lateral deviation of the forefoot. The end of the metatarsal elevation 3 is well raised and modeled, with the rolling moment of the anatomical elevation being such that optimal extension of the toes is activated (in the case of claw toes or hammer toes, hallux valgus). This is an important movement process in the forefoot, as it produces better muscular activity and helps to strengthen the foot muscles.

The rising branch 28 of the sagittal elevation 5 up to its highest point 29 in the area of the talus is modeled higher and the falling branch 30 in the direction of the big toe is left as it is (little to no pressure exerted) as specified by the foam imprint. Any flat feet are held in place by the rising branch 28.

The stiffening plate 10 is attached to the underside 15 of the insole 1 in order to stiffen the insole 1. However, the soft material in the front part 8 of the insole 1 allows a certain flexibility of the insole 1, particularly with regard to the feeling of walking. For maximum effectiveness, the stiffening plate 10 can extend essentially over the entire width of the insole 1 in the front part 8 and under the metatarsal area 13. For insoles 1 that are to be used in narrow shoes, the stiffening plate can, however, be made narrower, as a strip 26 (marked with dashed lines). In this case, it has a slightly S-shaped shape and runs curved from below the metatarsal area 13 to below the support area of the little toe in the area of the toe prints 16 (see Fig. 4)

Vorstehend werden einige Kernaspekte der Erfindung zusammengefasst: Some key aspects of the invention are summarized above:

In an insole according to the present invention, the rear area, i.e. the area over which the heels and tarsal bones lie, is designed to be stiff and the front area, i.e. the area over which the metatarsal bones and toes lie, is designed to be relatively softer. For this purpose, an insole blank is injection-molded in one piece from two materials with different levels of stiffness. The rear, stiff part of the blank later provides the necessary stability of the insole and stabilizes the foot on it, while the front, softer part ensures a pleasant rolling motion when walking and cushions the forefoot area.

This insole blank is an intermediate product that already has the shape of an insole, with elevations, for example a continuous elevation from the metatarsal area to the sagittal area and/or a preform of a Cuboid support. The metatarsal area and the sagittal area are connected by a kind of bridge and merge directly into one another.

When making the insole, a foam impression of a patient is taken and cast. The cast is then reworked, for example by making a depression in the metatarsal area of the cast and connecting it to the sagittal area so that the transition from the transverse area of the metatarsal area to the sagittal area is modeled as a metatarsal depression.

If a heated insole blank is then placed on this reworked cast, the fully modelled metatarsal depression of the cast is transferred to the insole blank, adapted to the patient. This means that the continuous elevation from the metatarsal area to the sagittal area of the insole blank becomes a fully modelled transition from the transverse metatarsal elevation to the sagittal elevation, adapted to the patient's needs.
