Description
Die vorliegende Erfindung bezieht sich auf ein Mittel zur Prophylaxe und Therapie der Prostatahyperplasie, das als Wirkstoffkomponente mindestens einen Aromatasehemmer und mindestens ein Antiandrogen enthält.
Bei der Prostatahyperplasie handelt es sich um eine gutartige Vergrösserung der Prostata, die von der sogenannten «inneren» Prostata ihren Ausgang nimmt. Die Beschwerden sind vor allem auf die auftretenden Obstruktionen der Harnröhre zurückzuführen. Die Blasenentleerung ist erschwert, und es kommt zu Restharnretentionen. Ohne operativen Eingriff kann es zu Harnstoffvergiftungen kommen.
Bislang ist eine medikamentöse Behandlung dieser sehr häufigen Erkrankung bei älteren Männern kaum möglich. Die für diesen Zweck benutzten Phyto-Präparate, wie zum Beispiel ß-Sitosterin, Mischungen aus verschiedenen Pflanzenextrakten und Kombinationen aus Pflanzenextrakten mit dem neurotropen Spasmolytikum Azoniaspirochlorid erwiesen sich in einer einjährigen Studie als unwirksam. Zwar empfanden die Patienten unter der Therapie eine Besserung der Miktionssymptomatik, eine Rückbildung der hyperplastischen Prostata wurde jedoch nicht erzielt.
Auch Hormone finden Anwendung bei der Behandlung der Prostatahyperplasie; von diesen Substanzen ist insbesondere das Depot-Gestagen Gestonoroncapronat zu nennen. Im Vergleich mit den Phyto-Präparaten wird mit Gestonoroncapronat eine verbesserte Wirkung erzielt. Die vor der Behandlung verlängerte Miktionsdauer wird deutlich verkürzt und der maximale Flowwert (Urinfluss in der Zeiteinheit) verbessert. Eine deutliche Reduktion der Adenomgrösse ist aber auch hier nicht festzustellen.
In der US-Patentschrift 3 423 507 wird die Behandlung der Prostatahypertrophie mit den gestagen und anti-andro-gen wirkenden Estern des 6-Chlor-17-hydroxy-la,2a-methy-len-pregna-4,6-dien-3,20-dions (Cyproteronestern) beschrieben. Es hat sich jedoch gezeigt, dass auch unter dieser Behandlung nur eine teilweise Rückbildung der Hyperplasie eintritt.
In der DE-OS 2 817 157 wird zur Prophylaxe und Therapie der Prostatahyperplasie die Verwendung von AntiÖstrogenen in Kombination mit antiandrogen wirkenden Substanzen vorgeschlagen. Doch die Anwendung von AntiÖstrogenen ist nur in einem bestimmten Dosisbereich möglich, da bei höheren Dosen auch AntiÖstrogene agonistisch (als Östrogene) wirken.
Bei der Prostatahyperplasie handelt es sich um eine gutartige Vergrösserung der Prostata, wobei in wechselndem Ausmass Interstitium (Stroma) und Epithel beteiligt sind. Als Ursache kann u.a. eine Verschiebung des Östrogen/ Androgen-Verhältnisses zugunsten der Östrogene angesehen werden. In verschiedenen Untersuchungen hat sich gezeigt, dass bei älteren Männern die Serumtestosteronkonzentratio-nen abfallen; gleichzeitig nimmt der Anteil an SHBG (sex hormone binding globuline, spezifisches Transportprotein für Steroide) zu, so dass die biologische Verfügbarkeit an Androgenen noch weiter abnimmt.^
Die beim Manne vorhandenen Östrogene stammen vorwiegend aus der peripheren Aromatisierung von androgenen Hormonen und nicht aus der testikulären Biosynthese (H.U. Ausserdem wurde gefunden, dass Östrogene auch in der Prostata selbst durch Aromatisierung von Androgenen entstehen, wobei festgestellt wurde, dass Fibroblasten-Kulturen aus menschlichem Prostatahyperplasiegewebe Testosteron stärker zu Östrogenen aromatisieren als Fibroblasten-Kultu-ren, die aus gesundem Prostatagewebe stammen (H.U. Da die Prostata des Mannes Östrogenrezeptoren besitzt und das Interstitium (Stroma) Zielorgan für Östrogene ist, kommt es durch Östrogene zur Stimulierung des fribromus-kulären Gewebes. Daraus folgt, dass beim Menschen die Prostatahyperplasie vorwiegend eine durch Östrogene stimulierte Erkrankung des fribromuskulären Interstitium (Stroma) ist.
Diese Befunde werden auch gestützt durch Untersuchungen am Hund. Es konnte gezeigt werden, dass Östrogenbehandlung zu einer Stimulation des Interstitiums, Androgen-behandlung aber zu einer Stimulation des Drüsenpithels (Pa-renchym) führt (Tunn, U. W., Schüring, B., Senge, Th., Neumann, F., Schweikert, H.U. und H.P. Rohr (1981): «Mor-phometric analysis of prostates in castrated dogs after treatment with androstanediol, estradiol und cyproterone aceta-te», Invest. Urol. 18: 289-292).
Auch in autoradiographischen Studien an menschlichem Prostatahyperplasiegewebe konnte gezeigt werden, dass nur das Drüsenepithel (Parenchym) ein Zielorgan für Androgene ist, nicht aber das Interstitium. Ausserdem wurde gefunden, dass Fibroblasten-Kulturen aus menschlichem Prostatahyperplasiegewebe Testosteron stärker zu Östrogenen aromati-

sieren als Fibroblasten-Kulturen, die aus gesundem Prostatagewebe stammen.
Es ist die Aufgabe der Erfindung, ein Mittel für den Mann zu entwickeln, das zu einer Rückbildung sowohl des epithelialen als auch des fibromuskulären Anteils der Prostata führt.
Das erfindungsgemässe Mittel ist dadurch gekennzeichnet, dass es als Wirkstoffe mindestens einen Aromatasehemmer und mindestens ein Antiandrogen enthält, wobei das Gewichtsverhältnis von Aromatasehemmer zum Antiandrogen in einem Bereich von 20 : 1 bis 1 : 1 liegt. Vorzugsweise liegt das Verhältnis von Aromatasehemmer zum Antiandrogen in einem Bereich von 10 : 1 bis 4:1.
Aromatasehemmer und Antiandrogen können sowohl kombiniert in einer einheitlichen Applikationsform als auch getrennt in zwei verschiedenen Applikationsformen angewendet werden. Durch die Anwendung eines Aromatase-hemmers wird die Bildung von biologisch wirksamen Östrogenen verhindert. Damit wird die Stimulation des fribromus-kulären Gewebes unterbunden, und es kommt zu einer Rückbildung mit Grössenabnahme und damit zur erwünschten Besserung der klinischen Symptomatik (Miktionsbe-schwerden).
Darüber hinaus führt das Antiandrogen zu einer Rückbildung des epithelialen Anteils der Prostata. Durch die Behandlung mit einer Kombination eines Aromatasehemmers und eines Antiandrogens werden somit beide Gewebearten der Prostata, die für die Entstehung des Krankheitsbildes verantwortlich sind, im Wachstum gehemmt.
Für die Verwendung im erfindungsgemässen Mittel sind alle Stoffe geeignet, die als Aromatasehemmer wirken, d.h. die als Substrat für die Aromatase infrage kommen, ohne nach der Aromatisierung Östrogene oder andere hormonelle .- s Wirkungen zu besitzen; bevorzugt ist Testolacton (17a-Oxa-D-homoandrost-1,4-dien-3,17-dion).
Neben dem genannten Testolacton wurden auch die nachfolgend genannten Verbindungen als Aromatasehemmer untersucht, wobei man zu ähnlichen Resultaten kam: io Androsta-4,6-dien-3,l 7-dion

25 Als Antiandrogene können sowohl Steroidverbindungen als auch nichtsteroidale Verbindungen eingesetzt werden. Als Steroide kommen insbesondere die Verbindungen der allgemeinen Formel I und II infrage:

R3 und R4 den Acylrest einer in der Steroidchemie gebräuchlichen Säure darstellen, bedeuten.
Unter der Bezeichnung Acylrest sollen insbesondere die Reste der in der Steroidchemie zur Veresterung sekundärer und tertiärer Hydroxygruppen gebräuchlichen Säuren verstanden werden. Bevorzugt sind aliphatische Carbonsäuren mit 1-8 Kohlenstoffatomen, wie zum Beispiel Essigsäure, Propionsäure, Buttersäure, Valeriansäure, Isovaleriansäure, Capronsäure, Önanthsäure usw. Die Ester der Essigsäure sind besonders bevorzugt.
Unter Alkyl sollen vor allem niedere Alkylgruppen mit 1-5 Kohlenstoffatomen verstanden werden, wobei die Methylgruppe bevorzugt ist.
Typische Verbindungen der allgemeinen Formel I sind die 17-Ester von zum Beispiel:

25 Die Antiandrogene können in Mengen von etwa 1 bis 100 mg, vorzugsweise 5 bis 40 mg, pro Tag verabreicht werden.
Die Wirkstoffe können mit den in der galenischen Pharmazie üblichen Zusätze, Trägersubstanzen und/oder Ge-schmackskorrigentien nach an sich bekannten Methoden zu den üblichen Applikationsformen verarbeitet werden.
Für die bevorzugte orale Applikation kommen insbesondere Tabletten, Dragees, Kapseln, Pillen, Suspensionen oder Lösungen infrage.
Für die parenterale, insbesondere intramuskuläre Applikation sind vor allem ölige Lösungen, wie zum Beispiel Sesamöl- oder Rizinusöllösungen, geeignet. Zur Erhöhung der Löslichkeit können Lösungsvermittler, wie zum Beispiel Benzylbenzoat oder Benzylalkohol, zugesetzt werden.
Die wie oben angegeben formulierten Arzneimittel enthalten für die orale Applikation vorzugsweise 10 mg bis 250 mg Testolacton und 1 mg bis 50 mg Cyproteronacetat und für die parenterale Applikation 20 mg bis 200 mg Test-45 olacton und 5 mg bis 100 mg Cyproteronacetat bzw. jeweils die biologisch äquivalenten Dosen eines anderen Aromatase-hemmers und Antiandrogens.

Claims (5)
1. Mittel zur Prophylaxe und Therapie der Prostatahyperplasie, dadurch gekennzeichnet, dass es als Wirkstoffe mindestens einen Aromatasehemmer und mindestens ein Antiandrogen enthält, wobei das Gewichtsverhältnis von Aromatasehemmer zum Antiandrogen in einem Bereich von 20 :1 bis 1 :1 liegt.
2. Mittel nach Anspruch 1, dadurch gekennzeichnet, dass das Gewichtsverhältnis von Aromatasehemmer zum Antiandrogen in einem Bereich von 10 :1 bis 4:1 liegt.

PATENTANSPRÜCHE
3. Mittel nach Anspruch 1, dadurch gekennzeichnet, dass man als Aromatasehemmer Testolacton und als Antiandrogen Cyproteronacetat einsetzt.
4. Mittel nach Anspruch 1, dadurch gekennzeichnet, dass eine orale Darreichungsformdosis 10 mg bis 250 mg Testolacton und 1 mg bis 50 mg Cyproteronacetat enthält.
5. Mittel nach Anspruch 1, dadurch gekennzeichnet, dass eine parenteral applizierbare Dosis 20 mg bis 200 mg Testolacton und 5 mg bis 100 mg Cyproteronacetat enthält.
