Abstract
Zmes imunoglobulinov a transferínu, určených pre intravenóznu a intraperitoneálnu aplikáciu, obsahuje imunoglobulíny v množstve 0,25 až 0,55 g a transferínu 1,0 až 4,0 g v 1 000 ml vodného roztoku albuminu o koncentrácii 4,5 až 5,5 hmotnostných °/o. Roztok albuminu sa připraví etanolovou frakcionáciou, následnou lyofilizáciou a rozpuštěním v roztoku 0,02 mól acetyl-d,l-tryptofanu a 0,02 mól kaprylanu sodného. Takto získaný roztok sa pasteurizuje pri 60 °C. pH sa upraví roztokom 0,5 M NaOH na hodnotu 6,4 až 7,4.The mixture of immunoglobulins and transferrin, intended for intravenous and intraperitoneal administration, contains immunoglobulins in an amount of 0.25 to 0.55 g and transferrin 1.0 to 4.0 g in 1,000 ml of an aqueous albumin solution with a concentration of 4.5 to 5.5 % by weight. The albumin solution is prepared by ethanol fractionation, subsequent lyophilization and dissolution in a solution of 0.02 mol of acetyl-d,l-tryptophan and 0.02 mol of sodium caprylate. The solution thus obtained is pasteurized at 60 °C. The pH is adjusted to 6.4 to 7.4 with a 0.5 M NaOH solution.
Description
E., Grabs G. H.: Infusionstherapie Θ, 301 — 304, 1980, Gofferje H. Maintz.: Infusionstherapie 5, 268 — 272, 1978, Kult J., Treutlein E., Dragoun G. P., Heiland.: Infusionstherapie 2, 313 — 318, 1975).The present invention relates to a mixture of IgG-7S and transferrin immunoglobulins for intravenous and intraperitoneal administration, wherein human albumin is added to the immunoglobulins to reduce anti-complementary activity and transferrin to increase the bacteriostatic effect at such a concentration that the percentages of the individual components correspond to a similar composition , such as in human plasma. Currently, the administration of immunoglobulins by the intravenous route encounters the nativity problem of the immunoglobulin molecule and the associated problem of anti-complementary activity. In recent years, there has been increasing reference to the bacteriostatic effect of transferrin in the world writings (Bauer HW, Mollin E. E., Grabs GH: Infusionstherapie 301, 301 - 304, 1980; , Cult J., Treutlein E., Dragoun GP, Heiland, Infusionstherapie 2, 313-318, 1975).
Podávanie uvedeného přípravku je aktuálně zvlášť v urgentných klinických situáciach u ťažkých bakteriálnych infekcií, hlavně vyvolaných gramnegatívnymi baktériami, a to v. prípadoch, kde terapia antibiotikmi a chemoterapeutikami část zlyhá.Administration of said preparation is currently particularly in urgent clinical situations in severe bacterial infections, mainly caused by gram-negative bacteria, in cases where the antibiotic and chemotherapeutic therapies fail part.
Nahrhovaný prípravok zmesi imunoglobulínov a transferínu rieši tento problém tým, že obdobným zložením jednotlivých zložiek ako v 1'udskej plazme dosahuje sa žiadaný terapeutický efekt. The proposed composition of the mixture of immunoglobulins and transferrin solves this problem by achieving the desired therapeutic effect by similar composition of the individual components as in human plasma.
Technologické zvládnutie zníženia antikomplementárnej aktivity u bielkovinných 4 roztokov makromolekulárnej povahy je větrní obťažné. Doteraz používané postupy boli orientované na gélovú filtráciu pomocou róznych nosičov· a prietokovú dialýzu. Vysoká cena dovážaných materiálov, prácnosť a energetická náročnost prietokovej dialýzy viedla k inému riešeniu podta vynálezu.Technological management of the reduction of anti-complementary activity in protein 4 solutions of macromolecular nature is windy. The procedures used so far have been directed to gel filtration using various carriers and flow dialysis. The high cost of imported materials, the laboriousness and energy demand of flow dialysis has led to another solution of the invention.
The immunoglobulin IgG-7S and transferrin mixture for intravenous and intraperitoneal use of the invention consists of 0.25 to 0.55 g of IgG-7S and 0.1 to 0.4 g of transferrin in 100 ml of an aqueous solution of 1 ' 5 to 5.5% by weight, which is stabilized by the addition of 0.02 mol of acetyl-d, 1-tryptophan and 0.02 mol of sodium caprylate and piasteurized at 60 ° C.
Příklad prevedenia 0,5 g imunoglobulínu IgG-7S pre i. v. podanie a 0,4 g transferínu sa rozpustí v 100 mililitroch vodného roztoku, ktorý obsahuje 5 % hmotnostných 1'udského albuminu, upraveného výše uvedeným spósobom, po úprave pH na hodnotu 6,5 + 0,1 sa roztok homogenizuje a filtruje cez sterilizačné vložky a rozplňuje po 20, 50 a 100 ml.The albumin solution is prepared by an ethanol fraction, followed by lyophilization and dissolution in an aqueous solution of acetyl-d, 1-tryptophan and sodium caprylate, and the pH is adjusted to 6.4 to 7.4 with a solution of 0.5 mol of sodium hydroxide. The resulting protein solution is sterilized by filtration and dispensed at 20, 50 and 100 ml. EXAMPLE 0.5 g of immunoglobulin IgG-7S for iv administration and 0.4 g of transferrin are dissolved in 100 ml of an aqueous solution containing 5% by weight of human albumin treated as above after adjusting the pH to 6.5. + 0.1, the solution is homogenized and filtered through sterilizing pads and filled with 20, 50 and 100 ml.
