Foretrukne forbindelser med formlen I er endvidere sådanne, hvori R·*" betegner en forgrenet alkylgruppe, f.eks. tert.-butyl eller neopentyl. Indolizinderivanterne fremstillet ifølge opfindelsen har vist sig at have nyttige farmakologiske 5 egenskaber, og nærmere betegnet er de i stand til at inhibere calciumvandring på cellemembranniveauet. Disse egenskaber giver de omhandlede forbindelser betydelig værdi til behandling af visse patologiske hjertesygdomme og, nærmere betegnet, til behandling af angina pectoris, forhøjet blodtryk, hjertearytmi 10 og cerebral kredsløbsinsufficiens. På ant isvu1stområdet kan forbindelserne fremstillet ifølge opfindelsen være nyttige som middel til styrkelse af anticancermidlers virkning.
De ifølge opfindelsen fremstillede forbindelser kan indgå i 15 farmaceutiske eller veterinære midler, der som aktivt stof indeholder midst ét indolizinderivat med formlen I eller et farmaceutisk acceptabelt syreadditionssalt deraf sammen med en farmaceutisk bærer eller ekscipiens derfor. Afhængigt af dén valgte administrationsvej vil den daglige dosering for et men-20 neske være en vægt på 60 kg være fra 2 til 500 mg aktivt stof.

Forbindelserne med formlen II er kendte forbindelser, der 10 er beskrevet i de britiske patentskrifter nr. 1.518.443 og nr. 2.064.536.
Den nuværende lægelige opinion er af den opfattelse, at pato-fysiologien af angina pectoris er et spektrum gående fra 15 ren stationær aterosclerotisk læsion til ren koronar arteriespasme.
Mange patienter antages imidlertid at have en kombination af både læsion og spasme.

Angina som følge af ren stationær læsion kan forhindres ved formindskelse af oxygenbehovet. Angina som følge af koronar arteriespasme kan behandles ved forhindring af selve spasmen, hvorved oxygentilførslen forøges.

Med denne nye forståelse af angina skulle optimal anti-anginal behandling tilvejebringe denne dobbelte virkning: Forøgelse af oxygentilførslen samtidig med formindskelse af oxygenbehovet.

Det er kendt, at calciumioner, der arbejder på det cellulare plan, regulerer vasokonstriktionsgraden og derved spiller en afgørende rolle i det anginale angreb.
35 Calciumantagonistiske forbindelser påvirker cellemembranen til selektivt at blokere for calciums adgang til den kontraktile proces i arteriecellen. Via denne virkning frem-

4 bringer sådanne forbindelser følgende patologiske fænomener: - Vasospastisk angina ved udvidelse af koronararterier 5 til forøgelse af myokardial oxygentilførsel og hindring af koronar arteriespasme.
- Præstationsforbunden angina ved udvidelse af perifere arterier til formindskelse af efterbelastning og myo-kardialt oxygenbehov.
- Blandet angina, der involverer elementer af vasospastisk og præstationsforbunden angina.
15 Blandt produkterne benyttet til den langvarige forhindring af angina kan nævnes de β-receptorblokerende midler. Disse β-blokkere forhindrer angina primært ved formindskelse af myokardialt oxygenbehov, ligesom disse midler formindsker calciumantagonisterne, såsom nifedipin, det myokardi-20 ale oxygenbehov, men tilvejebringer en yderligere dimen- * sion af anginabekæmpelse ved forøgelse af myokardial oxygentilførsel, når koronar arteriespasme foreligger.
Endvidere: 25 - kan β-blokkerne i virkeligheden forårsage spasme, medens calciumantagonisterne hindrer spasme, - formindsker β-blokkerne myokardial perfusion i post- 30 stenotiske områder, medens calciumantagonister forøger myokardial perfusion til både normale og poststenotiske områder, - er β-blokkerbehandling begrænset hos patienter med myo- 35 kardial dysfunktion, astma, bundtforgreningsblokke (bundle branch blocks) og diabetes. Calciumantagonisterne kan administreres sikkert til alle disse patientgrupper.

Endvidere er nogle forskere af den opfattelse,.at den kar-diodepressive virkning af calciumantagonistiske forbindelser på det myokardiale niveau "in situ" i et vist omfang reguleres af en refleksmekanisme. Denne refleksmekanisme, hvoraf 5 hovedvirkningsretningen går gennem det Ø-adrenerge system, medfører en stigning i styrken og hurtigheden af hjertekontraktionen. Når der således sker et for kraftigt fald i det arterielle tryk, udløses en refleksvirkning, der frigør endogene sympatiske transmitstoffer. Når dette system 10 er blokeret, forværres en tilstand af hjerteinsufficiens.
Denne frigørelse af catecholaminer kan delvis neutralisere den kardiodepressive virkning af calciumantagonisterne. I modsætning til de β-adrenerge inhibitorer inhiberer calciumantagonisterne i virkeligheden ikke den myokardiale reaktion 15 på Ø-adrenerge catecholaminer.
Via denne adrenerge kompensationsmekanisme kunne calciumantagonisterne derfor i modsætning til Ø-adrenerge midler i nogen grad modificere visse uønskede perifere fænomener, 20 såsom et fald i det arterielle tryk.
Der kendes allerede et indolizinderivat, nemlig butoprozin eller 2-ethyl-3[4-(3-di-n-butylamino-l-propyl)oxy-benzoyl]-indolizin, der udviser både a- og Ø-antiadrenerge egenskaber, 25 d.v.s. delvis inhiberende egenskaber over for de a- og 0-adrenerge reaktioner (britisk patentskrift nr. 1.518.443), samt calciumantagonistiske egenskaber (Biochemical Pharmacology, bind 30, nr. 8, side 897-901, 1981).
30 Det har imidlertid vist sig, at forbindelserne fremstillet ifølge opfindelsen med formlen I og de farmaceutisk acceptable syreadditionssalte deraf er i besiddelse af calciumantagonistiske egenskaber samtidig med, at de mangler signifikante an-tiadrenerge egenskaber i tilfælde af doser, ved hvilke sådanne 35 egenskaber iagttages i forbindelse med butoprozin.

Eftersom den inhiberende virkning af forbindelserne fremstillet ifølge opfindelsen på det adrenerge system kun er meget svag eller ikke-eksisterende, er det rimeligt at antage, at i overensstemmelse med den ovenfor anførte mekanisme vil anven-5 delsen af disse forbindelser i mange tilfælde være mere fordelagtig end anvendelsen af butoprozin.
Det fremgår således, at caliumantagonisterne fremstillet ifølge den foreliggende opfindelse, repræsenterer en ny løsning med hensyn til behandling af angina pectoris, især i de tilfælde, hvor det foretrækkes ikke at inhibere det adrenerge systems reaktion.
Resultaterne af farmakologiske forsøg udført med henblik på 15 bestemmelse af kardiovaskulære egenskaber ved forbindelserne fremstillet ifølge opfindelsen er anført nedenfor.
I. Calciumantagonistiske egenskaber.
20 De inhiberende egenskaber med hensyn til calciumvandring på cellemembranniveauet, der udvises af forbindelserne fremstillet ifølge opfindelsen, er blevet bestemt ved måling af deres antagonistiske virkning med hensyn til den kontraktile reaktion på kaliuminduceret depolarisering i rotters aorta. Det er 25 velkendt, at depolarisering ved hjælp af kalium i glatmuskel-membranen gør denne gennemtrænge!ig for ekstracellul art calcium og fremkalder muskulær kontraktion.
Måling af inhiberingen af den kontraktile reaktion på kalium-30 induceret depolarisering eller den toniske kontraktion som følge af kaliuminduceret depolarisering kan derfor repræsentere et middel til bedømmelse af en forbindelses evne til at inhibere den cellulare membrangennemtrængelighed for Ca -ioner.

Hanrotter af Wistar-arten med en vægt på ca. 300 g fik fjernet deres brystaorta, som blev spiralskåret i strimler med en længde på ca. 40 mm og en bredde på ca. 3 mm. Disse stykker blev anbragt i en 25 ml beholder indeholdende en modificeret 5 Krebs-bicarbonatopløsning (NaCl 112 millimol, KC1 5 millimol, NaHC03 25 millimol, 1 millimol, MgSO^ 1,2 millimol,
CaCl2 2,5 millimol, glucose 11,5 millimol, destilleret vand indtil 1.000 ml). Denne opløsning blev oxygeneret, medens den blev holdt ved 37°C.

Præparatet blev tilknyttet en transducer for isometrisk kraft, og den kontraktile reaktion blev registreret efter at være blevet forstærket.

En trækpåvirkning på 2 g blev udøvet på organet, som i 60 minutter blev holdt i den modificerede Krebs-bicarbonatopløsning. Kontraktioner blev derpå fremkaldt ved at erstatte Krebs-bicarbonatopløsningen med en Krebs-kalium- opløsning (NaCl 17 millimol, KCl 100 millimol, NaHCO-, 25 mil-

limol, K^PO^ 1 millimol, MgSO^ 1,2 millimol, CaC^ 2,5 millimol, glucose 11,5 millimol, destilleret vand indtil 1.000 ml). Da den kontraktile reaktion viste sig at være reproducerbar, blev en given dosis af forbindelsen fremstillet ifølge opfindelsen indført i badet. 60 minutter senere blev en 25 ny spasme fremkaldt ved hjælp af kaliumdepolarisering. Resultaterne opnået i hver undersøgt aorta blev derpå udtrykt i procent af den maksimale kontraktive effekt, der blev målt før inkubering med forsøgsforbindelsen.

Ved intravenøs administration af forbindelserne fremstillet 20 ifølge opfindelsen til en hund i en dosis på 5 mg - 10 mg/kg reduceres hjertefrekvensen med 15% til 40%, og der forårsages et langsomt progressivt fald i det arterielle tryk. Endvidere har forsøg udført også med en hund vist, at ved en dosis på 5 mg/kg administreret intravenøst i overensstemmelse med frem^ 25 gangsmåden beskrevet i britisk patentskrift nr. 1.518.443 har forbindelserne fremstillet ifølge opfindelsen som helhed kun svage α-antiadrenerge egenskaber, generelt mindre end 50%, og mangler eller næsten mangler /3-antiadrenerge egenskaber. Buto-prozin derimod udviser under de samme betingelser en a-anti-30 adrenergisk virkning, der er langt større end 50%, og en /3-an-tiadrenergisk virkning på ca. 50%. Sammenlignet med butoprozin udviser forbindelserne fremstillet ifølge opfindelsen derfor et interval for antiadrenergiske virkninger, der er meget mere begrænset, og nogle af forbindelserne har slet ikke sådanne 35 virkninger. Samtidig har forbindelserne fremstillet ifølge opfindelsen vist sig at håve calcium-antagonistiske egenskaber svarende til butoprozins egenskaber.

De antiadrenerge egenskaber hos forbindelserne fremstillet ifølge opfindelsen er blevet sammenlignet med butoprozins anti-adrenerge egenskaber ved de nedenfor omtalte forsøg.
5 De nedenfor omtalte forsøg blev udført med henblik på bestemmelse af evnen hos forbindelserne fremstillet ifølge opfindelsen til at reducere den epinephr i ni nducerede stigsnnng i blodtrykket (anti-a-virkning) og den isoprenalin-inducerede acceleration af hjerteslaget (anti-Ø-virkning) hos hunde, der for-10 inden var blevet bedøvet med pentobarbital og atropiniseret.
For hver hund bestemtes først den dosis epinephrin (mellem 3 og 10 pg/kg), der fremkaldte en reproducerbar stigning i blodtrykket på ca. 133 x 102 Pa, og den dosis isoprenalin (1 til 2 15 pg/kg), der fremkaldte en reproducerbar stigning i hjerteslaget på ca. 70 slag/minut. Epinephri ndosen og isoprenalindosen, der blev bestemt på denne måde, blev injiceret skiftevis hvert 10. minut, og efter opnåelse af to successive referencereaktioner administreredes en mængde af forsøgsforbindelsen intra- 20 venøst.
Anti-a-virkninq
Den procentiske reduktion af det ved hjælp af forsøgsforbindelsen fremkaldte forhøjede blodtryk sammenlignet med det som 25 reference forinden opnåede forhøjede blodtryk (ca. 100 mm Hg) blev registreret.

Den procentiske reduktion af den ved hjælp af forsøgsforbin-30 delsen fremkaldte acceleration af hjerteslaget sammenlignet med den forinden målte referencetachykardia (ca. Det vil fremgå, at til terapeutisk anvendelse vil forbindel-5 serne fremstillet ifølge opfindelsen normalt blive administreret i form af et farmaceutisk eller veterinært præparat, som kan foreligge i en enhedsdosisform, der er hensigtsmæssig for den ønskede administrationsmåde.
10 Det farmaceutiske eller veterinære præparat kan således foreligge i en enhedsdosisform, der er egnet til oral administration, f.eks. en overtrukken eller uovertrukken tablet, en hård eller blød gelatinekapsel, et emballeret pulver eller en suspension eller sirup. Præparatet eller midlet kan al-15 ternativt foreligge i form af et suppositorium til rektal administration eller som en opløsning eller suspension til parenteral administration.
Når det foreligger i en enhedsdosisform, kan midlet eksempel-20 vis indeholde fra 15 til 50 vægt% aktiv bestanddel til oral administration, fra 3 til 15% aktiv bestanddel til rektal administration og fra 3 til 5% aktiv bestanddel til parenteral administration.
25 Uanset den form, de kan indtage, vil de farmaceutiske eller veterinære midler normalt blive fremstillet ved at kombinere mindst én af forbindelserne med formlen I eller et farmaceutisk acceptablet syreadditionssalt deraf med en passende farmaceutisk bærer eller ekscipiens derfor, f.eks. et eller flere 30 af følgende materialer: mælk, sukker, stivelser, talk, magne-siumstearat, polyvinylpyrrolidon, alginsyre, kolloidal silica, destilleret vand, benzyl al kohol eller aromatiserende stoffer.
De følgende eksempler illustrerer opfindelsen.

EKSEMPEL 1.
Fremstilling af 2-ethyl~3-[4(3-tert,-butylamino-propyl)- c oxy-benzoyl]-indolizinsyreoxalat.
En opløsning indeholdende 4,2 g (0,11 mol) 2-ethyl-3-[4-(3-brom-propyl)oxybenzoyl]-indolizin og 2,4 g (0,033 mol) tert.-butylamin i 40 ml toluen blev opvarmet til 50°C i 50 timer i en kolbe. Ved reaktionens afslutning lod man blandingen køle, og den blev udhældt i 40 ml vand og gjort alkalisk med en vandig opløsning af 10% natriumhydroxid.
Den organiske fase blev fraskilt, og den vandige opløsning ekstraheredes med toluen. De organiske faser blev kombine- 1 ζ ret og vasket med vand indtil neutralitet. Den resulterende opløsning blev inddampet til tørhed, og den olieagtige rest rensedes ved hjælp af elueringskromatografi over aluminiumoxid under anvendelse af 1,2-dichlorethan som elu-eringsmiddel.

Det rensede olieagtige produkt blev opløst i ethylether, og en etheropløsning af vandfri oxalsyre blev tilsat, hvorpå det opnåede faste produkt blev rekrystalliseret i metha= nol.

Herved opnåedes 3,9 g 2-ethyl-3-[4-(3-tert.-butylamino-propyl)-oxo-benzoyl]indolizinsyreoxalat i et udbytte på 75% med smeltepunkt 207 - 208°C.
